Purpose: The purpose of this study is a finite element analysis of supporting bone according to custom abutment angle. Methods: Implant fixture was selected with a diameter of 4 mm and the length of 13 mm. The fixture and abutment was designed by a combination of the abutment screw clamping force to produce a custom abutment model of $0^{\circ}$, $15^{\circ}$, $25^{\circ}$ and $35^{\circ}$. The loading condition of 176 N was applied to the lingual surface of the crown, near to the incisor edge, and horizontal load. An oblique load of $90^{\circ}$ was applied long axis of the implant fixture analyze the stress of supporting bone. Results: The result of mechanical analysis was observed that the supporting bone stress analysis of the horizontal load, the von Mises stress values (MPa) are given in the order of TH00 (432.6) > TH25 (418.0) > TH15 (417.4) > TH35 (415.8), the oblique load, the von Mises stress values are given in the order of TO00 (459.3) > TO15 (399.6) > TO25 (374.8) > TO35 (343.4) Conclusion: The $35^{\circ}$ abutment over the current clinical tolerance limits will be available for clinical application.
When chewing forces are repetitively applied to a dental implant, gaps are often generated at the interface surfaces between the abutment and the fixture, which results in some deterioration, such as loosening of the fastening screw, dental retraction, and fixture fractures. To cope with such problems, a sealing-type abutment having a number of grooves along the conical-surface circumference was previously developed, and it showed better sealing performance than conventional ones. To enhance the sealing performance, a new model in which a gold ring is inserted into the top groove of the sealing-type abutment is developed in this study. In addition, a manufacturing process is developed, and performance evaluations, such as fatigue and sealing tests, are carried out. The evaluation results show that the gold-ring-inserted sealing abutment yields better performance than conventional ones.
연구 목적: 내부연결형 임플란트 고정체에 연결 방식이 다른 4종류의 지대주를 연결하여 교합력을 가하였을 때 각 구조에 발생하는 응력의 차이를 비교하고, 이들 연결방식이 임플란트 주위조직의 응력분포에 미치는 영향을 3차원 유한요소법을 통하여 알아보고, 지대주의 선택 기준을 마련하고자 하였다. 연구 재료 및 방법: 고정체로는 직경 4 mm, 길이 11.5 mm의 SS-$III^{(R)}$ (Osstem, Korea)를 사용하였으며, 사용된 지대주는 각각, 모델 1에서 Solid abutment, 모델 2에서 Com-Octa abutment, 모델 3에서 ComOcta Gold abutment, 모델 4에서 Octa abutment를 사용하였고, 네 가지 하중조건으로 치관중심와 (central fossa)에 fixture 장축에 평행하게 점하중으로100N의 수직하중, 협측교두에 대하여 fixture 장축에 평행하게 점하중으로 100N 수직하중, 치관중심와에서 설측으로 100N의 $30^{\circ}$ 경사하중, 협측교두 (buccal cusp)에 치아의 바깥쪽에서 내측으로 100N의 $30^{\circ}$ 경사하중을 주었고 3G.Author (Plasso Tech, USA)를 이용하여 분석하였다. 결과: 1. 골조직에서는 모든 모델에서 하중조건에 관계없이 가장 큰 응력이 고정체 상부에 집중되었고, 고정체 하부에서는 근단 부위에서 응력 집중을 보였으며, 그 외의 부위에서는 큰 응력 집중은 보이지 않았다. 2. 고정체에서의 응력은 모든 모델에서 하중조건에 관계없이 neck 부위에서 최대의 양상을 보였다. 3. 응력은 골에서보다 임플란트 내부에서 훨씬 높았다. 4. 중심와에 수직하중인 하중조건 1에서 가장 낮은 응력이 관찰되었으며, 이 때의 응력집중 현상도 가장 적게 나타났다. 서로 다른 지대주에서 응력분포 양상을 살펴본 결과, 같은 고정체를 사용한 경우에 지대주의 연결 형태에 따른 골조직에서의 응력 분포의 차이는 없었다.
Fiber reinforced materials have favorable mechanical properties. Moreover, the strength to weight ratios of this material is superior to those of most alloys. Comparing to the metals, it showed many other advantages as well, including non-corrosiveness, translucency and easy repair characteristic. Since, it has the potential for the chair-side and laboratory fabrication, it is not surprising that fiber reinforced composites offer the potential for use in various applications in dentistry. To make the well-fitted restorations, Fiber reinforced composite (FRC) has been suggested as an alternative framework material for the implant supported fixed prosthesis. Two fixed partial denture fabrication procedures were tried. Vectris fiber was pressed to the EsthetiCone gold cylinder on the implant positioned cast. And then, Targis were added on it. In the other method, we used the customized component using UCLA abutment. The beads for retaining the Vectris fiber were added on the abutment. If careful laboratory and clinical techniques were done, these two techniques would fulfill the demands of the esthetics and strength.
안착형 외부 연결 임플란트를 적절히 사용하기 위해서는 식립 시에 임플란트 간의 간격에 주의해야 하며 2차 수술 시에 임플란트 상부로 올라간 골을 다듬어야 한다. 육각형태를 가진 지대주가 원형 지대주에 비해 유용하다. 외부 연결형 임플란트는 악간간격이 제한된 경우에 적용할 수 있다. 안착형 외부연결(조직수준) 임플란트는 상부 직경의 두께로 인한 생역학적 장점과 내재된 점막관통부로 인한 생물학적 장점을 가지고 있다. 조직수준 임플란트를 수복할 때 지대주 수준과 임플란트 수준의 변연 중 악간공간에 따라 선택한다. 어떤 한 가지 임플란트 종류로는 모든 증례를 만족시키기 어려우므로 교합력 및 골조건을 고려하여 적절한 임플란트를 선택하는 것이 필요하다.
목적: 이 연구의 목적은 타이타늄 구성요소를 가진 지르코니아 지대주가 내부연결 임플란트에 사용되었을 때 나사 풀림에 대해 알아보는 것이다. 열순환 후 타이타늄 소켓을 가진 지르코니아 지대주와 타이타늄 지대주의 나사 풀림과 제거력을 비교 분석하였다. 연구 재료 및 방법: 내측연결 임플란트와 내측연결 타이타늄 지대주, 타이타늄 소켓을 가진 외측연결 지르코니아 지대주를 준비하였다. 내부연결형 임플란트에 5개의 타이타늄 지대주를(대조군), 타이타늄 소켓을 가진 5개의 지르코니아 지대주(실험군)를 각각 연결하고 30 Ncm의 토크로 조인 후 열순환 처리 전후의 지대주 나사 제거력을 측정하였다. 각 시편들은 열순환 장치의 수조에서 $5^{\circ}C$와 $55^{\circ}C$의 물에 60초씩 교대로 2,000회의 열순환을 시행하였다. 각 시편의 나사 풀림을 조사하고 열순환 전후에 나사 제거력을 통계학적으로 분석하였다. 결과: 두 그룹 모두에서 열순환 후 지대주 나사 풀림을 보이지 않았다. 열순환 전후의 나사 제거력 차이는 대조군에서 $-1.34{\pm}2.53Ncm$, 실험군에서 $-1.26{\pm}2.06Ncm$ 로 나타났다. 독립표본 T 통계분석 결과 두 군간에 유의한 차이를 보이지 않았다(P > 0.05). 결론: 이 실험의 결과 내에서 내부연결 임플란트를 위한 지르코니아 지대주 내 타이타늄 소켓이 열순환 후 지대주 나사풀림에 타이타늄 지대주와 비교하여 유의한 영향을 나타내지 않았다.
연구 목적: 본 연구의 목적은 임플란트 지대주 연결에 사용되는 기성품 나사와 맞춤형 나사의 풀림 토크를 비교하는 것이다. 연구 재료 및 방법: 세가지 임플란트 시스템에(Osstem, Astra, Zimmer) 대해 고정체와 지대주의 연결에 기성품 나사군(대조군)과 맞춤형 나사군(실험군)으로 총 6군(n = 6)으로 나누었다. 조임 토크조절은 디지털 토크 측정기를 이용하였으며 각 임플란트 제조사가 추천한 조임 토크 값을 적용하였다. 체결 10분 후 동일한 조임 토크를 다시 적용하고 5분 후에 풀림 토크력을 측정하였다. 이 과정을 10회 반복 측정하였다. 반복 하중 실험에서는 6개 군(n = 3)에 대해 체결 10분 후 2차 조임 토크를 적용하고 $30^{\circ}$ 경사로 50 N 하중으로 총 1,000,000번 반복 하중을 가하였다. 반복 하중 적용 이후 풀림 토크값을 측정하였다. 통계 방법으로는 10회 반복 측정에서 풀림 토크값의 차이를 비교하기 위해 repeated measures of ANOVA test (${\alpha}$=.05)를 사용하였고, 반복 하중 후 풀림 토크값의 차이를 비교하기 위해서 각 시스템별 Independant t-test로 통계 처리하였다. 결과: 모든 군에서 반복 횟수가 증가할수록 풀림 토크값이 유의성 있게 감소하는 것으로 나타났다(P<.05). 10회 반복 측정 실험에서는 세 종류의 임플란트에서 대조군(기성품나사)과 실험군(맞춤형 나사) 간에 풀림 토크값은 유의차가 없었다(P>.05). 반복 하중 실험에서 세가지 시스템에서 대조군과 실험군 간의 풀림 토크력은 유의한 차이가 없었다(P>.05). 결론: 반복 측정된 풀림력 검사와 반복 하중을 적용 후 풀림력 검사에서 맞춤형 나사와 기성품 나사의 풀림력은 유의한 차이가 없었다.
Statement of problem: In order to increase preload with reducing the friction coefficient, abutment screws coated with pure gold and Teflon as dry lubricant coatings have been introduced. But the reported data indicate that if screw repeated tightening and loosening cycle, an efficiency of increasing preload was decreased by screw surface wearing off. Purpose: This study was to evaluate the influence of tungsten carbide/carbon coating, which has superior hardness and frictional wear resistance, on the preload of abutment screws and the stability of coating surface after repeated closures. Material and method: The rotational values of abutment screws and the compressive forces between abutment and fixture were measured in implant systems with three different joint connections, one external butt joint and two internal cones. Moreover the stability and the alteration of coating surface were examined by comparison of the compressive force and the removable torque values during 10 consecutive trials, observation with scanning electron microscope and analyzed the elemental composition with energy dispersive x-ray spectroscopy Results and conclusion: 1. Application of coating resulted in significant increase of compressive force in all implant systems(P<.05). The increasing rate of compressive force by coating in external butt joint was gloater than those in internal cones (P<.05). 2. Coated screw showed the significant additional rotation compared to non-coated screw in all implant systems (P<.05). There were no significant differences in the increasing rate of rotation among implant systems (P>.05). 3. Removable torque values were greater with non-coated screw than that with coated screw (P<.05). 4. Coated screw showed insignificant variations in the compressive forces during 10 consecutive trials(P>.05) 5. After repeated trials, the surface layer of coated screw was maintained relatively well. However surface wearing and irregular titanium fragments were found in non-coated screw.

Purpose: To evaluate the effects of intra-alveolar socket grafting, subepithelial connective tissue grafts, and individualized abutments on peri-implant hard and soft tissue outcomes following immediate implant placement. Methods: This randomized experimental study employed 5 mongrel dogs, with 4 sites per dog (total of 20 sites). The mesial roots of P3 and P4 were extracted in each hemimandible and immediate dental implants were placed. Each site was randomly assigned to 1 of 4 different treatment groups: standardized healing abutment (control group), alloplastic bone substitute material (BSS) + standardized healing abutment (SA group), BSS + individualized healing abutment (IA group), and BSS + individualized healing abutment + a subepithelial connective tissue graft (IAG group). Clinical, histological, and profilometric analyses were performed. The intergroup differences were calculated using the Bonferroni test, setting statistical significance at P<0.05. Results: Clinically, the control and SA groups demonstrated a coronal shift in the buccal height of the mucosa ($0.88{\pm}0.48mm$ and $0.37{\pm}1.1mm$, respectively). The IA and IAG groups exhibited an apical shift of the mucosa ($-0.7{\pm}1.15mm$ and $-1.1{\pm}0.96mm$, respectively). Histologically, the SA and control groups demonstrated marginal mucosa heights of $4.1{\pm}0.28mm$ and $4.0{\pm}0.53mm$ relative to the implant shoulder, respectively. The IA and IAG groups, in contrast, only showed a height of 2.6mm. In addition, the height of the mucosa in relation to the most coronal buccal bone crest or bone substitute particles was not significantly different among the groups. Volumetrically, the IA group ($-0.73{\pm}0.46mm$) lost less volume on the buccal side than the control ($-0.93{\pm}0.44mm$), SA ($-0.97{\pm}0.73mm$), and IAG ($-0.88{\pm}0.45mm$) groups. Conclusions: The control group demonstrated the most favorable change of height of the margo mucosae and the largest dimensions of the peri-implant soft tissues. However, the addition of a bone substitute material and an individualized healing abutment resulted in slightly better preservation of the peri-implant soft tissue contour.

Purpose: To retrospectively evaluate the relationship between the vertical position of the implant-abutment interface and marginal bone loss over 3 years using radiological analysis. Methods: In total, 286 implant surfaces of 143 implants from 61 patients were analyzed. Panoramic radiographic images were taken immediately after implant installation and at 6, 12, and 36 months after loading. The implants were classified into 3 groups based on the vertical position of the implant-abutment interface: group A (above bone level), group B (at bone level), and group C (below bone level). The radiographs were analyzed by a single examiner. Results: Changes in marginal bone levels of $0.99{\pm}1.45$, $1.13{\pm}0.91$, and $1.76{\pm}0.78mm$ were observed at 36 months after loading in groups A, B, and C, respectively, and bone loss was significantly greater in group C than in groups A and B. Conclusions: The vertical position of the implant-abutment interface may affect marginal bone level change. Marginal bone loss was significantly greater in cases where the implantabutment interface was positioned below the marginal bone. Further long-term study is required to validate our results.
