Electrophysiological and radiological evidence for the multifocal nature of a case of multifocal acquired demyelinating sensory and motor neuropathy

  • Multifocal acquired demyelinating sensory and motor (MADSAM) neuropathy is a variant of chronic acquired demyelinating polyneuropathy. A 65-year-old women presented with upper arm weakness. A nerve conduction study showed conduction blocks over intermediate segments with sparing of distal compound action potentials. Magnetic resonance imaging revealed asymmetric hypertrophy of the brachial plexus on the affected side. These findings represent important electrophysiological and radiological evidence of MADSAM neuropathy. The condition of the patient began to improve after starting intravenous immunoglobulin administration.

  • After a detailed review of the literature availabe on the problems of immunoglobulin level in colostrum and blood serum of calves and after a discussion of its impact on health condition, the following investigations on this subject are presented. 1. Calves to fail ill in the rearing period show significantly lower serum Ig levels even during the first two days of life, when they are still clinically healthy, as well as up to the end of the first week of life than those of calves remaining in good health. 2. Ig values sharply rise during the disease period, IgA and IgM responding more rapidly than IgG. 3. In calves suffering from a disease of the gastrointestinal tract serum IgM titer is lowest, while in calves suffering from a disease of the respiratory duct serum IgA titer is lowest and in those suffering or dying from both disease complexes all Ig titers are reduced. 4. Calves showing high serum Ig levels during the first three days of life reveal higher daily gains in weight than those with lower serum Ig levels during that period.

Immunomodulatory Effects by Bifidobacterium longum KACC 91563 in Mouse Splenocytes and Macrophages

  • The present study evaluates the immunomodulatory effect of Bifidobacterium longum KACC 91563 in murine primary splenocytes and macrophages. B. longum KACC 91563 regulated T- and B-cell proliferation and inhibited the Th1 (IL-2, IFN-γ)/Th2 (IL-4, IL-10) cytokine imbalance and immune cytokine production. Moreover, immunoglobulin E (IgE) levels were significantly lower after treatment with B. longum KACC 91563. These findings suggest that B. longum KACC 91563 could modulate the systemic immune system toward both IgE production and regulation of the Th1/Th2 balance.

  • Intercellular adhesion molecule-1 (ICAM-1) is a member of the immunoglobulin superfamily, its main function being to participate in recognition and adhesion between cells. ICAM-1 is considered closely related to occurrence, development, metastasis and invasion process of hepatocellular carcinoma (HCC). A variety of inflammatory cytokines and stimulus affect its expression through the nuclear factor-kappa B (NF-${\kappa}B$) signal transduction pathway. In the initial stage of inflammation, hepatocirrhosis and tumor development, ICAM-1 is expressed differently, and has varied effects on different cells to promote occurrence of malignancy and metastasis. ICAM-1 has diagnostic significance for AFP-negative or suspected HCC, and may be a prognositic significance. It is thus widely used in studies as a biomarker which reflects cancer cells metastasis as well as curative effect of drugs. Many new treatments of HCC may be based on the effects of ICAM-1 on different levels of function.

Magnoliae Cortex inhibits immediate-type allergic reactions

  • The effect of aqueous extract of Magnoliae Cortex (Magnoliaceae) (MCAE) on the immediate-type allergic reaction was investigated. MCAE inhibited compound 48/80 induced systemic anaphylactic reaction in rats. MCAE (0.1 and 1 g/kg) also significantly inhibited local immunoglobulin E (lgE)-mediated passive cutaneous anaphylactic (PCA) reaction. MCAE (0.001 to 1 mg/ml) dose-dependently inhibited the histamine release from rat peritoneal mast cells (RPMC) activated by compound 48/80 or anti-dinitrophenyl (DNP) 1gE. Moreover, MCAE (0.01 to 1 mg/ml) had a significant inhibitory effect on anti-DNP IgE-mediated tumor necrosis $factor-{\alpha}$ $(TNF-{\alpha})$ production. These results indicate that MCAE inhibits immediate-type allergic reaction in vivo and in vitro.

  • Common variable immune deficiency (CVID) is the most common primary immune deficiency, which is manifested as chronic recurrent respiratory infections and hypoglobulinemia. CVID usually presents in the second or third decade of life. A 33-year-old woman was admitted with recurrent pneumococcal pneumonia with bacteremia and had very low levels of serum immunoglobulin G, M and A. This case emphasized a high index of suspiciousness for diagnosis of CVID in a mid-adulthood patient presenting with recurrent pneumonia with hypoglobulinemia.

Activation-induced Cytidine Deaminase in B Cell Immunity and Cancers

  • Activation-induced cytidine deaminase (AID) is an enzyme that is predominantly expressed in germinal center B cells and plays a pivotal role in immunoglobulin class switch recombination and somatic hypermutation for antibody (Ab) maturation. These two genetic processes endow Abs with protective functions against a multitude of antigens (pathogens) during humoral immune responses. In B cells, AID expression is regulated at the level of either transcriptional activation on AID gene loci or post-transcriptional suppression of AID mRNA. Furthermore, AID stabilization and targeting are determined by post-translational modifications and interactions with other cellular/nuclear factors. On the other hand, aberrant expression of AID causes B cell leukemias and lymphomas, including Burkitt's lymphoma caused by c-myc/IgH translocation. AID is also ectopically expressed in T cells and non-immune cells, and triggers point mutations in relevant DNA loci, resulting in tumorigenesis. Here, I review the recent literatures on the function of AID, regulation of AID expression, stability and targeting in B cells, and AID-related tumor formation.

Literature Review on Biological Effects of Gyejibokryeong-hwan against Gynaecological Diseases

  • Objectives: To investigate therapeutic mechanisms of Gyejibokryeong-hwan (GJBRH) against gynaecological diseases, articles on biological assay were gathered and analyzed. Methods: The articles were classified as being from domestic or international journals, and by their year of publication. The mechanisms of the biological effects against gynaecological diseases were noted. Results: Of the 14 articles analyzed, 13 were published in China and 1 was from Japan. GJBRH showed therapeutic effect against uterine and mammary gland diseases. Uterine-related diseases such as endometriosis, hysteromyoma, adenomyosis, cancer, and inflammation can be improved by the administration of GJBRH through anti-angiogenesis, anti-inflammation, the modulation of immune cell and immunoglobulin, and the regulation of hormone secretion. GJBRH also reduced mammary hyperplasia by regulating hormone and cytokine release. Conclusions: We speculate that the inhibitory effect against uterine and mammary gland diseases could be related to the therapeutic efficacy of GJBRH in improving gynaecological diseases.

가와사끼병 급성기 치료의 최신 지견 (Update on treatment in acute stage of Kawasaki disease)

  • Kawasaki disease (KD) was first described by Dr. Tomisaku Kawasaki in his 1975 study, published in Pediatrics. Its pathogenesis is still not clearly understood. Early diagnosis and treatment are very important to preventing concomitant coronary artery complications. Most KD patients respond well to the standard treatment of aspirin and intravenous immunoglobulin; however, some of them are refractory to the standard treatment, and so adjuvant therapies with corticosteroids and anti-tumor necrosis $factor-{\alpha}$ ($TNF-{\alpha}$) antibody are necessary. In this article, the author reviews and summarizes the most recent literature on the treatment of refractory KD.
