Objectives : The aim of this study was to investigate the effect of Chunghwatang (CHT) for atopic dermatitis. Methods : CHT, a verified anti-oxidant and anti-inflammatory effect, was treated in atopic dermatitis animal model to investigate cytokine levels and immunoglobulin production. Results : Clinical skin index was 47.1%, suggesting significant efficacy of CHT in atopic dermatitis treatment. Serum IL-4, IL-6, IL-13, TNF-${\alpha}$ and histamine productions were significantly decreased to 52.3%, 61.8%, 68.0%, 37.4%, and 46.6%, respectively. The production of IL-5 was decreased to 33.3%. The increase of Immunoglobulin IgG1 production along with IgE through the interaction of IgE and IL-4 induced by IL-4 and IL-13 were measured as 20.7% and 23.4%, respectively. Conclusions : The results above, along with the in vitro test results, strongly supports the CHT samples as effective immunomodulator in AD treatments, suggesting its use in clinical practice and basic information for EBM.
This study was carried out to investigate the effect of dietary supplementation of red ginseng by-product (RGB) on the laying performance, blood biochemistry, and microbial population in laying hens. A total of 120 Hy-Line Brown laying hens (75 weeks old) were randomly allotted to 1 of 3 dietary treatments with 4 replicates per treatment. A commercial-type basal diet was prepared, and 2 additional diets were prepared by supplementing 5.0 or 10.0 g/kg of RGB to the basal diet at the expense of corn. The diets were fed to hens on an ad libitum basis for 4 weeks. There were no differences in feed intake, egg weight, and feed conversion ratio during 4 weeks of the feeding trial. However, hen-day egg production was significantly greater (p<0.05) for the RGB treatment groups than that for the basal treatment group. There were no differences in triglyceride, aspartate aminotransferase, and alanine aminotransferase during the 4-week feeding trial. However, RGB supplementation increased (p<0.05) the serum immunoglobulin G (IgG) and IgM content compared with basal treatment group. The total cholesterol was lower (p<0.05) in the RGB treatments groups than that in the basal treatment group. The intestinal Lactobacillus population was greater (p<0.05) for the RGB treatments groups than that for the basal treatment group. However, the numbers of Salmonella and Escherichia coli were not different among dietary treatments. During the entire experiment, there was no significant difference in egg quality among all the treatments. In conclusion, in addition to improving hen-day production, there were positive effects of dietary RGB supplementation on serum immunoglobulin and cholesterol levels in laying hens.
Immunoglobulin G4 (IgG4) related autoimmune diseases are characterized by high serum IgG4 concentrations, sclerosing inflammation of numerous IgG4-positive lymphoplasma cells of varying origin, and a positive response to steroid treatment. Autoimmune pancreatitis, sclerosing cholangitis, and retroperitoneal fibrosis are representative presentations of IgG4 related autoimmune disease. Herein, we describe 2 patients (40-years-old woman and 47-years-old man) diagnosed with pulmonary involvement of IgG4-related autoimmune disease. The patients were admitted for an evaluation of the lung mass or multiple lung nodules found on chest radiography. Surgical lung biopsies were performed and pathologic finding revealed lymphoplasmacytic sclerosing inflammation with numerous IgG4 positive cells. The patients had elevated serum total IgG and IgG4 levels. Treatment consisted of high dose methylpredinisolone (1 mg/kg/day) and demonstrated good responsiveness. However, one patient experienced 2 relapses while being tapered off of steroid treatment.

T cell immunoglobulin mucin domain (TIM)-3 is an immunomodulatory molecule and upregulated in T cells by several cytokines. TIM-3 also influences mast cell function but its transcriptional regulation in mast cells has not been clarified. Therefore, we examined the transcript level and the promoter activity of TIM-3 in mast cells. The TIM-3 transcript level was assessed by real-time RT-PCR and promoter activity by luciferase reporter assay. TIM-3 mRNA levels were increased in HMC-1, a human mast cell line by TGF-${\beta}1$ stimulation but not by stimulation with interferon (IFN)-${\alpha}$, IFN-${\lambda}$, TNF-${\alpha}$, or IL-10. TIM-3 promoter -349~+144 bp region relative to the transcription start site was crucial for the basal and TGF-${\beta}1$-induced TIM-3 promoter activities in HMC-1 cells. TIM-3 promoter activity was increased by over-expression of Smad2 and Smad4, downstream molecules of TGF-${\beta}1$ signaling. Our results localize TIM-3 promoter activity to the region spanning -349 to +144 bp in resting and TGF-${\beta}1$ stimulated mast cells.
목 적 : IVIG은 다양한 면역 질환에서 면역 조절제로 사용되고 있다. 가와사끼병에서 고용량의 IVIG 투여가 단백과 지질을 포함한 다양한 생화학적 지표에 미치는 영향을 알아보고자 하였다. 방 법 : IVIG(2 g/kg)을 투여받은 12 명의 가와사끼병 환아에 대해 IVIG 투여 전, 2시간, 24시간 및 7일 후에 혈청을 분리하고 $-20^{\circ}C$에 냉동보관하였다. 대조군으로 20명의 같은 연령군의 건강한 아동 혈청을 이용하였다. 결 과 : 알부민은 IVIG 투여 2시간 및 24시간 후 유의한 감소를 보였으며, 7일 후에는 투여 전 수준으로 회복되었다. 총 콜레스테롤과 트리글리세라이드는 7일 후 증가하는 경향을 보였다. HDL-콜레스테롤과 CRP 치는 IVIG 투여 2시간과 24시간 후 유의한 감소를 보였다. 결 론: 가와사끼병 환아에 투여된 고용량의 IVIG는 단시간 내에 단백 지표들과 HDL-콜레스테롤 치의 변화를 가져온다. HDL-콜레스테롤의 변화는 전신적인 단백 대사의 변화에 의할 것으로 보인다.
The object of this trial was to investigate the effect of dietary ${\beta}$-1,3/1,6-glucan supplementation on the performance and immunological response of broiler chickens. Two hundred and forty 1-day old male broilers ($39{\pm}1g$) were separated into six treatments which were given six different feeds containing 0 (control), 25, 50, 75, 100 and 125 mg/kg dietary ${\beta}$-1,3/1,6-glucan supplementation. On days 21 and 42, body weight gain, feed consumption and feed conversation rate were recorded as measures of growth performance. The levels of key cytokines in the immuno-regulating pathway: interleukin-1 (IL-1), interleukin-2 (IL-2), interferon $\gamma$(IFN-$\gamma$, tumor necrosis factor $\alpha$(TNF-$\alpha$, and the concentrations of signal molecules: peripheral blood plasma globulin, serum Immunoglobulin G (IgG) and intestinal secretary Immunoglobulin A (sIgA), were measured as indices of the immune response to determine suitable levels of dietary ${\beta}$-1,3/1,6-glucan supplementation. The results indicated that performance was elevated quadratically with dietary ${\beta}$-1,3/1,6-glucan supplementation. Maximal growth performance and an enhanced immunological response were obtained at a supplemented level of 50 mg/kg.
Purpose: We conducted a study to determine which factors may be useful as predictive markers in identifying Kawasaki disease (KD) patients with a high risk of resistance to intravenous immunoglobulin (IVIG) and developing coronary artery lesions (CAL). Methods: We enrolled 287 patients in acute phase of KD at a single center. The demographic, clinical and laboratory data were collected retrospectively. Results: There were 34 patients in the IVIG resistant group. The IVIG resistant group had significantly higher serum N-terminal-pro-brain natriuretic protein (NT-proBNP) levels (P<0.01) and polymorphonuclear neutrophil (PMN) percentage (P<0.01) in comparison to the IVIG responders. The results yielded sensitivity (78.8%, 60.6%), specificity (58.2%, 90%) and cutoff value (628.6 pg/mL, 80.3%) of NT-proBNP and PMN respectively, in predicting IVIG resistance. Despite IVIG administration, 13 of the 287 patients developed CAL. The patients in the CAL group had higher NT-proBNP levels (P<0.01) and higher PMN percentage (P<0.01). In these patients, the results yielded sensitivity (73.3%, 56.7%), specificity (67.9%, 88.9%) and cutoff value (853.4 pg/mL, 80.3%) of NT-proBNP and PMN respectively, for predicting CAL. The area under the curve (AUC) for predicting resistance to IVIG was NT-proBNP 0.712, PMN 0.802. The AUC for predicting CAL was NT-proBNP 0.739, and PMN 0.773. Conclusion: Serum NT-proBNP levels and PMN percentage were significantly elevated in patients with KD with IVIG resistance and CAL. Thus, they may be useful predicting markers for IVIG resistance and development of CAL in KD patients.

Immunoglobulin G4 (IgG4)-related disease is a newly recognized condition characterized by fibroinflammatory lesions with dense lymphoplasmacytic infiltration, storiform-type fibrosis and obliterative phlebitis. The pathogenesis is not fully understood but multiple immune-mediated mechanisms are believed to contribute. This rare disease can involve various organs and pleural involvement is even rarer. We report a case of IgG4-related disease involving pleura. A 66-year-old man presented with cough and sputum production for a week. Chest radiography revealed consolidation and a pleural mass at right hemithorax. Treatment with antibiotics resolved the consolidation and respiratory symptoms disappeared, but the pleural mass was unchanged. Video-assisted thoracoscopic surgery was performed. Histopathology revealed dense lymphoplasmacytic infiltration and storiform fibrosis with numerous IgG4-bearing plasma cells. The serum IgG4 level was also elevated. Further examination ruled out the involvement of any other organ. The patient was discharged without further treatment and there is no evidence of recurrence to date.
저자들은 가와사끼병으로 IVIG 치료로 해열을 보인 후, 급격히 발병한 관절염을 보인 5명의 환아에 대해 임상적 경과, 실험실 소견 등을 조사하였다. IVIG 치료에 반응 후 평균 5.8일 후에 관절염 증상이 나타났으며, 임상적 분류상 소수 관절형 형이 3례, 다수관절형이 2례로 나타났다. 검사실 소견에서 류마토이드 인자는 1례에서 양성을 보였으나 이 후 음성으로 전환되었다. 3명에서 조사된 HLA B27은 모두 음성을 보였다. 치료로는 고용량 아스피린(2례), 비스테로이드성 소염제(이부프로펜, 3례), 스테로이드제(메틸프레드니솔론, 1례)를 사용하였다. 가와사끼병의 경과 중 정맥용 면역글로불린 치료 후 관절염이 드물게 관찰된다. 이러한 관절염은 스테로이드제를 비롯한 항염증제에 양호한 반응을 보였고 재발은 관찰되지 않았다.
