Purpose: The aim of this study was to investigate the available alveolar bone height between the maxillary molars and the sinus floor according to their anatomical relationship using cone-beam computed tomographic (CBCT) images. Methods: A total of 752 maxillary first (M1) and second molars (M2) on CBCT scans of 188 patients were selected. First, each maxillary molar was categorized as type 1, 2, 3, or 4 according to the relationship of the molar root with the maxillary sinus floor. The frequency distribution of each type was analyzed. Second, the shortest vertical distance (VD) of each molar was measured from the furcation midpoints of the roots to the lowest point of the sinus floor by 2 observers. Intraclass correlation coefficients and the t-test were calculated for the VD measurements. Results: For M1, type 3 was the most frequent, followed by type 2. For M2, type 3 was the most common, followed by type 1. The VD measurements of type 1 were 9.51±3.68 mm and 8.07±2.73 mm for M1 and M2, and those of type 3 were 3.70±1.52 mm and 4.03±1.53 mm for M1 and M2, respectively. The VD measurements of M2 were significantly higher in female patients than in male patients. Conclusions: Type 3 was the most frequent anatomical relationship in the maxillary molars, and showed the lowest alveolar bone height. This information will help clinicians to prevent complications related to the maxillary sinus during maxillary molar treatment and to predict the available bone height for immediate implant planning.

Background Ultrasonic dissection devices cause less thermal damage to the surrounding tissue than monopolar electrosurgical devices. We compared the effects of using an ultrasonic dissection device or an electrocautery device during prosthetic breast reconstruction on seroma development and short-term postoperative complications. Methods We retrospectively reviewed the medical records of patients who underwent implant-based reconstruction following mastectomy between March 2017 and September 2018. Mastectomy was performed by general surgeons and reconstruction by plastic surgeons. From March 2017 to January 2018, a monopolar electrosurgical device was used, and an ultrasonic dissection device was used thereafter. The other surgical methods were the same in both groups. Results The incidence of seroma was lower in the ultrasonic dissection device group than in the electrocautery group (11 [17.2%] vs. 18 [31.0%]; P=0.090). The duration of surgery, total drainage volume, duration of drainage, overall complication rate, surgical site infection rate, and flap necrosis rate were comparable between the groups. Multivariate analysis revealed that the risk of seroma development was significantly lower in the ultrasonic dissection device group than in the electrocautery group (odds ratio for electrocautery, 3.252; 95% confidence interval, 1.242-8.516; P=0.016). Conclusions The findings of this study suggest that the incidence of seroma can be reduced slightly by using an ultrasonic dissection device for prosthesis-based breast reconstruction. However, further randomized controlled studies are required to verify our results and to assess the cost-effectiveness of this technique.
Study Design: Prospective observational study. Purpose: This prospective analysis aimed to evaluate the efficacy and bone-bonding rate of hybrid hydroxyapatite (HA) spacers in expansive laminoplasty. Overview of Literature: Various types of spacers or plates have been developed for expansive laminoplasty. Methods: Expansive open-door laminoplasty was performed in 146 patients with cervical myelopathy; 450 hybrid HA spacers and 41 autogenous bone spacers harvested from the spinous processes were grafted into the opened side of each lamina. The patients were followed up using computed tomography (CT), and their bone-bonding rates for hybrid HA and autogenous spacers, bone-fusion rates of the hinges of the laminae, and complications associated with the implants were then examined. Results: Clinical symptoms significantly improved in all patients, and no major complications related to the procedure were noted. The hybrid HA spacers exhibited sufficient bone bonding on postoperative CT. The hinges completely fused in over 95% patients within 1 year of the procedure. Only 4 spacers (0.9%) developed lamina sinking, and most expanded laminae maintained their positions without sinking or floating throughout the follow-up period. Conclusions: Hybrid HA spacers contributed to high bone-fusion rates of the spacers and hinges of the laminae, and no complications were associated with their use. Cervical laminoplasty with these spacers is safe and simple, and it yields sufficient fixation strength while ensuring sufficient bone bonding during the immediate postoperative period.
Nowadays computer-guided "flapless" surgery for implant placement using templates is gaining popularity among clinicians and patients. The advantages of this surgical protocol are its minimally invasive nature, accuracy of implant placement, predictability, less post-surgical discomfort and reduced time required for definitive rehabilitation. Aim of this study is to describe the digital implant protocol, thanks to which is now possible to do a mini-invasive static guided implant surgery. This is possible thanks to a procedure named surface mapping based on the matching between numerous points on the surface of patient's dental casts and the corresponding anatomical surface points in the CBCT data. With some critical points and needing an adequate learning curve, this protocol allows to select the ideal implant position in depth, inclination and mesio-distal distance between natural teeth and or other implants enabling a very safe and predictable rehabilitation compared with conventional surgery. It represents a good tool for the best compromise between anatomy, function and aesthetic, able to guarantee better results in all clinical situations.
구치부 부분 결손 환자에서 즉시하중군과 지연하중군의 임상적 성공률을 측정하기 위한 전향적 연구가 계획되었다. 즉시하중군에서는 17명의 환자에 42개의 GSIII 임플란트(Osstem, Korea)를 식립하고 48시간 안에 하중을 가하였다. 대조군인 지연하중군에서는 15명의 환자에 27개의 GSIII 임플란트를 식립하고 평균 $2.6{\pm}1.7$개월 후에 하중을 가하였다. 하중 전 1차안정성을 ISQ로 측정하였고 임상증상, 동요도, 연조직 반응을 식립 직후, 3개월 후, 6개월 후, 12개월 후에 평가하였다. 변연골 흡수량은 치근단 방사선 영상으로 측정하였고 Mann-Whitney test (${\alpha}$=0.05)와 repeated measured ANOVA (${\alpha}$=0.05)로 두 군의 차이를 검정하였다. 식립 직후 실험군과 대조군의 ISQ는 각각 $80.3{\pm}7.1$과 $69{\pm}17$이었다. 실험군은 95.23%의 성공률을, 대조군은 100% 성공률을 나타냈다. 3개월과 6개월 후 실험군의 변연골 흡수량은 대조군에 비해 유의성 있게 컸다(p<0.05). 하지만 12개월 후 두 군의 차이는 관찰되지 않았다 (p>0.05). 변연골 흡수량은 성별과 식립위치에 따른 차이가 없었다 (p>0.05). 실험의 한계 내에서, 1차안정성이 획득된다면 구치부 식립 GSIII 임플란트의 즉시하중은 예지성 있는 치료법이라고 여겨진다.
심미적으로 중요한 전치부에서 단일치아 임플란트를 이용한 보철수복은 임상의에게 새로운 도전과제중의 하나가 되었다. 그러나 전치부에서 single-unit implant는 식립할 부위의 치조골 상태가 양호해야 하는데 치은연하 파절로 인해 치아 주위에 골 손실이 있는 경우 immediate implant는 심미적으로 문제를 야기 할 수 있다. 따라서 본 증례는 "orthodontic extrusive remodeling"을 통해 파절치와 주위 조직을 교정적으로 정출시켜 임플란트 식립을 위한 연조직과 경조직을 증대시킨 후 임플란트 식립을 통해 상악측절치에서 성공적 인 심미수복을 이룬 증례에 관한 것이다.
목적: 삼차원 유한요소분석을 이용하여 상악 전치부 즉시하중 임플란트의 식립 깊이가 주위 골의 응력 분포와 임플란트의 미세움직임에 미치는 영향을 알아보고자 하였다. 재료 및 방법: 임플란트 상단이 치조정 피질골 상연을 기준으로 0.00 mm, 0.25 mm, 0.50 mm, 0.75 mm, 1.00 mm 깊게 위치되도록 총 5개 골모형을 제작하였다. 고정체와 주위 골 계면에 마찰접촉과 35 Ncm의 식립 토크를 재현하였다. 임시 보철물에 178 N의 정하중을 고정체의 중심축에 대하여 축방향의 수직하중과 $30^{\circ}$의 경사하중으로 가하고 유한요소 분석을 시행하였다. 결과: 임플란트의 식립 깊이가 주위 골의 응력 분포에 상당한 영향을 주었다. 임플란트의 가장 큰 미세움직임이 $39.34{\mu}m$로 나타났다. 경사하중이 수직하중에 비해서 응력 분포와 미세움직임의 변화에 크게 기여하였다. 결론: 임플란트 식립 깊이의 증가는 피질골에 집중된 응력을 분산시키는데 유리하였으며, 초기 골유착 실패와 관련이 있는 미세움직임에는 크게 영향을 주지 않았다.
Purpose: This research sought to determine the resorption rate of bone grafted to the maxillary sinus according to the grafted material's type, patient's age, systemic disease, implant size, site of implant placement, and residual ridge height. Materials and Methods: This research targeted 24 patients who had immediate Osstem$^{(R)}$ implant (US Plus$^{(R)}$) placement after bone graft. The panorama was taken before the surgery, after the surgery, and 6 months after the surgery. Vertical height change and resorption rate of the grafted bone were measured with the same X-rays and compared. The influence of the following factors on the grafted bone material's resorption rate was evaluated: grafted material type, patient's age, systemic disease, implant size, site of implant placement, and residual ridge height. Results: Patients in their 40s had $34.0{\pm}21.1%$ resorption rate, which was significantly higher compared to the other age groups (P<0.05). There was no significant relationship between systemic disease and grafted bone resorption. There was no significant relationship between implant size (diameter, length) and grafted bone resorption. There was no significant relationship between the site of implant placement and grafted bone resorption. The ramal bone-grafted site was significantly more resorbed than the ramal bone/Bio-Oss$^{(R)}$-grafted site, maxillary tuberosity bone/Bio-Oss$^{(R)}$-grafted site, and ramal bone/maxillary tuberosity bone/Bio-Oss$^{(R)}$-grafted site (P<0.05). There was no significant difference in the grafted bone resorption rate in the sinus between more than 4 mm and less than 4 mm residual ridge heights. After an average of 6 months, a second surgery was done; given an average follow-up of 1.9 years, the success rate and survival rate of the implant were 96.9% and 98.4%, respectively. Conclusion: These results indicate that the bone resorption rate of grafted bone among patients in their 40s is higher compared to patients in their 50s and over, and that only autogenous bone (ramus) shows higher resorption rate than the mixed graft of autogenous bone and xenogenous graft (Bio-oss) after maxillary sinus graft.
Purpose: Rehabilitation of the edentulous posterior maxilla with dental implants often poses difficulty because of insufficient bone volume caused by pneumatization of the maxillary sinus and by crestal bone resorption. Sinus grafting technique was developed to increase the vertical height to overcome this problem. The present study was designed to evaluate the sinus floor augmentation with anorganic bovine bone (Bio-$cera^{TM}$) using histomorphometric and clinical measures. Patients and methods: Thirteen patients were involved in this study and underwent total 14 sinus lift procedures. Residual bone height was ${\geq}2mm$ and ${\leq}6mm$. Lateral window approach was used, with grafting using Bio-$cera^{TM}$ only(n=1) or mixed with autogenous bone from ramus and/or maxillary tuberosity(n=13). After 6 months of healing, implant sites were created with 3mm diameter trephine and biopsies taken for histomorphometric analysis. The parameters assessed were area fraction of new bone, graft material and connective tissue. Immediate and 6 months after grafting surgery, and 6 months after implantation, computed tomography (CT) was taken and the sinus graft was evaluated morphometric analysis. After implant installation at the grafted area, the clinical outcome was checked. Results: Histomorphometry was done in ten patients.Bio-$cera^{TM}$ particles were surrounded by newly formed bone. The graft particles and newly formed bone were surrounded by connective tissue including small capillaries in some fields. Imaging processing revealed $24.86{\pm}7.59%$ of new bone, $38.20{\pm}13.19%$ connective tissue, and $36.92{\pm}14.51%$ of remaining Bio-$cera^{TM}$ particles. All grafted sites received an implant, and in all cases sufficient bone height was achieved to install implants. The increase in ridge height was about $15.9{\pm}1.8mm$ immediately after operation (from 13mm to 19mm). After 6 months operation, ridge height was reduced about $11.5{\pm}13.5%$. After implant installation, average marginal bone loss after 6 months was $0.3{\pm}0.15mm$. Conclusion: Bio-$cera^{TM}$ showed new bone formation similar with Bio-$Oss^{(R)}$ histomorphometrically and appeared to be an effective bone substitute in maxillary sinus augmentation procedure with the residual bone height from 2 to 6mm.
목적 : 악성 골종양의 종양 대치물을 이용한 사지 구제술에서 방사선학적 변화를 평가하는 데 목적이 있다. 대상 및 방법 : 종양 대치물로써 사지 구제술을 받은 69례를 대상으로 하였고 평균 추시 기간은 4년 9개월이었다. International Symposium On Limb Salvage(ISOLS)에서 제창한 방사선학적 대치물 평가 체계(Radiological Implants Evaluation System)를 사용하여 평가하였으며 술후 1년 및 최종 추시로 나누어 시행하였다. 우수, 양호, 보통, 불량을 각각 통계분석을 위해 4, 3, 2, 1로 변환하여 분석하였고 평가 결과를 시멘트 사용 여부, 병변의 위치, 종양의 종류, 종양 대치물의 종류 및 나이(<20세 vs ${\geq}$20세)에 따라 나누어 T-test 및 ANOVA를 사용하여 비교 분석하였다. 결과 : 최종 추시에서 시멘트군이 무시멘트군보다 골 재형성 및 접촉면 항목에서 우수했으나 고정 항목(anchorage)에서는 무시멘트군이 우수했다. 병변의 위치 상 근위 대퇴부 및 원위 경골부가 다른 부위보다 골 재형성면에서 불리하였으며 골육종군이 비골육종군보다 고정 및 관절면에서 불리하였다. 대치물의 종류 및 연령에 따른 유의한 차이는 없었다. 결론 : 방사선학적 중기 추시상, 시멘트를 이용한 재건술이 골 흡수 및 해리에서 더 우수하였고 무시멘트 술식은 고정(anchorage) 면에서 우수하였다. 원위 대퇴골 및 근위 경골이 골 흡수면에서 유리한 해부학적 위치였으며 골육종군은 화학요법으로 인해 고정 및 관절면에서 불리하였다고 생각한다.
