작은 직경 임플란트를 이용한 임플란트 지지 피개의치에 대한 환자 만족도 조사 (Patients satisfaction for implant supported overdenture with small diameter implant)

  • 연구 목적: 양악 총의치를 사용 중인 환자를 대상으로 하악에 4개의 작은 직경의 임플란트를 식립하여 implant supported overdenture로 전환 시 환자의 만족도 (저작, 유지, 발음, 심미, 사회적 문제, 심리적 문제)를 평가하였다. 연구 재료 및 방법: 60세 이상의 완전 무치악 환자 중 상하악 총의치를 장착한 환자를 대상으로 하였다. 수술 전에 진단 스텐트를 이용한 방사선 사진 (CT, panorama) 촬영을 시행하고 현재 사용하고 있는 의치에 대한 만족도에 대해 조사하였다. 그 후 하악 이공 전방부에 4개의 mini dental implant 식립 및 immediate loading 후 방사선 사진 촬영을 하였다. 식립 1달 후와 3달 후에 방사선 사진을 촬영하였으며 수술 전과 동일한 설문지를 통한 overdenture 사용 시 만족도 조사하였다. 결과: 현재 9명의 환자를 대상으로 시행하였다. 1. 총 36개의 임플란트를 식립하였으며 이중 1개의 임플란트가 실패하여 97.2%의 생존률을 보였다. 2. 임플란트 식립 전, 후 만족도 비교는 oral health impact profile 49를 이용한 설문 조사를 통해 이루어 졌다. 저작 시 불편감, 유지, 발음, 심미, 사회적 활동시 불편감, 심리적 불편감 등으로 비교 분석하였으며 5점 기준으로 불편감이 심할 수록 낮은 점수를 부여하였다. 그 결과 전반적으로 만족도가 상승하였다. 그 중 유지력 면에서 가장 크게 만족도가 상승하였으며 사회활동시 불편감, 심리적 불편감, 저작시 불편감, 심미, 발음 순으로 개선되었다. 결론: 이번 연구를 통하여 mini dental implant를 이용한 의치를 사용시 환자의 만족도가 높아진 것을 볼 수 있었다. 추 후 좀 더 많은 환자를 통해 장기간 연구를 할 예정이며 추가적인 방사선 사진을 촬영하여 임플란트 주위 골 소실 여부를 관찰할 예정이다.

Considerations for patient selection: Prepectoral versus subpectoral implant-based breast reconstruction

  • Background In recent years, breast implants have been frequently placed in the subcutaneous pocket, in the so-called prepectoral approach. We report our technique of prepectoral implant-based breast reconstruction (IBR), as well as its surgical and aesthetic outcomes, in comparison with subpectoral IBR. We also discuss relevant considerations and pitfalls in prepectoral IBR and suggest an algorithm for the selection of patients for IBR based on our experiences. Methods We performed 79 immediate breast reconstructions with a breast implant and an acellular dermal matrix (ADM) sling, of which 47 were subpectoral IBRs and 32 were prepectoral IBRs. Two-stage IBR was performed in 36 cases (20 subpectoral, 16 prepectoral), and direct-to-implant IBR in 43 cases (27 prepectoral, 16 subpectoral). The ADM sling supplemented the inferolateral side of the breast prosthesis in the subpectoral group and covered the entire anterior surface of the breast prosthesis in the prepectoral group. Results The postoperative pain score was much lower in the prepectoral group than in the subpectoral group (1.78 vs. 7.17). The incidence of seroma was higher in the prepectoral group (31.3% vs. 6.4%). Other postoperative complications, such as surgical site infection, flap necrosis, implant failure, and wound dehiscence, occurred at similar rates in both groups. Animation deformities developed in 8.5% of patients in the subpectoral group and rippling deformities were more common in the prepectoral group (21.9% vs. 12.8%). Conclusions The indications for prepectoral IBR include moderately-sized breasts with a thick well-vascularized mastectomy flap and concomitant bilateral breast reconstruction with prophylactic mastectomy.

  • Adequate bone quantity is one of the important factor to obtain osseointegration after implantation. Guided bone regeneration (GBR) has widely used in implantation for reconstruction of bony defects. Since introducing this procedure, there are many studies about survival rate of implants, changing in surrounding bone volume after function. The purpose of this study was to evaluate the amount of resorption according to placement timing and survival rate after function. The subjects were patients who had been operated with GBR from Jun 2003 to Jun 2004 in Seoul National University Bundang Hospital. They were divided into simultaneous and delayed placement group. The follow up had been performed at the time of just after GBR, 1, 3, 6, 12, 24-month later and standard periapical radiographs were taken to estimate the bone level at the time. The total average of bone level change in radiographs was 1.94mm(${\pm}0.25$), and 1.92mm(${\pm}0.72$) in simultaneous installation, 2.03mm(${\pm}0.25$) in delayed installation. In this report, the survival rates were 92.2% in simulataneous group and 92.3% in delayed group. Insufficient primary stability, early contamination of wound, overloading, poor oral hygiene, and infection were thought to be associated factors in the failed cases.

A Comparison of Implants Used in Double Door Laminoplasty : Allogeneic Bone Spacer versus Hydroxyapatite Spacer

  • Objective : The aim of this study was to compare the clinical and radiological outcomes associated with the use of hydroxyapatite (HA) spacer and allogeneic bone (AB) spacer in laminoplasty. Methods : From January 2006 to July 2014, 79 patients with cervical spondylotic myelopathy or ossification of the posterior longitudinal ligament underwent cervical laminoplasty. The radiologic parameters were obtained from plain radiography and three-dimensional computed tomography. All images were taken before and after surgery. Cervical lordosis, spinal canal dimension, fusion between lamina and spacer, and resorption of spacer were checked. Clinical outcomes were assessed using visual analog scale and Japanese Orthopedic Association. Results : Double-door laminoplasty was performed on 280 levels : 182 in the HA group and 98 in the AB group. The mean follow-up was 23.1 months (range : 4-69 months). Similar fusion rates were found in these groups (p=0.3). The resorption rate between lamina and spacer was lower in the HA group (p<0.001). During the immediate postoperative period, the canal dimension of both groups increased compared with the results in the preoperative period. However, the canal dimension of the AB group decreased over time compared with that of the HA group (p<0.001). Conclusion : Double-door laminoplasty improved the clinical outcomes of both groups. However, the spinal canal dimension in the AB group showed a greater degree of reduction than in the HA group at the final postoperative follow-up. Therefore, we suggest that surgeons consider the use of larger-sized AB spacers in double-door laminoplasties.

두개안면골 결손부 재건에 있어서 티타늄 그물판의 임상적 유용성 (Clinical Usefulness of Titanium Mesh in Reconstruction of the Craniofacial Bone Defects)

  • Purpose: Reconstruction of the craniofacial defects can be carried out with autogenous tissues, allogenic implants, or alloplastic materials. Titanium mesh systems have been used for bony reconstruction in non load-bearing areas. They offer several advantages: immediate availibility without any donor site morbidity, easy handling, stable 3-D reconstruction, and low susceptibility to infection. The aim of this study is to evaluate the usefulness and complications of titanium mesh system in the reconstruction of the craniofacial defects. Methods: From Jan. 2000, to Dec. 2004, we performed reconstruction of craniofacial bone defects in 21 patients who had benign or malignant tumor and fracture events in the cranium, orbit, nasal bone, maxilla, zygoma and the mandible. The size of the defects ranged from $1.0{\times}1.5cm$ to $12{\times}10cm$. Two different mesh systems, micro-titanium augmentation mesh and dynamic mesh was used for bony reconstruction in non load-bearing areas. The patients were evaluated from 1 to 4 yrs clinically and radiographically with a mean follow up period of 1.5 yrs. Results: There were no serious complications, including wound infection, foreign body reaction, exposures or loos of the mesh, central infection and pathologic findings of bone around mesh exception of one patient, who had expired of skull base tumor recurrence. Long-term stability of the reconstructions and the overall functional and aesthetic outcome was excellent. Conclusion: Our experiences demonstrate that the Titanium mesh system is a relatively safe and efficient method in the craniofacial reconstruction and have broadens our choices of therapeutic procedures in the craniomaxillofacial surgery.

The Absence of Atrial Contraction as a Predictor of Permanent Pacemaker Implantation after Maze Procedure with Cryoablation

  • Background: The absence of atrial contraction (AC) after the maze procedure has been reported to cause subsequent annular dilatation and to increase the risk of embolic stroke. We hypothesized that the lack of AC could increase the risk of permanent pacemaker (PPM) implantation in patients undergoing the maze procedure. Methods: In 376 consecutive patients who had undergone a cryo-maze procedure and combined valve operation, recovery of AC was assessed at baseline and at immediate (${\leq}2$ weeks), early (${\leq}1$ year, $4.6{\pm}3.8$ months), and late (>1 year, $3.5{\pm}1.1$ years) postoperative stages. Results: With a median follow-up of 53 months, 10 patients underwent PPM implantation. Seven PPM implants were for sinus node dysfunction (pauses of $9.6{\pm}2.4$ seconds), one was for marked sinus bradycardia, and two were for advanced/complete atrioventricular block. The median (interquartile range) time to PPM implantation was 13.8 (0.5-68.2) months. Our time-varying covariate Cox models showed that the absence of AC was a risk factor for PPM implantation (hazard ratio, 11.92; 95% confidence interval, 2.52 to 56.45; p=0.002). Conclusion: The absence of AC may be associated with a subsequent risk of PPM implantation.

Treatment of Nonsyndromic Craniosynostosis Using Multi-Split Osteotomy and Rigid Fixation with Absorbable Plates

  • Background: Nonsyndromic craniosynostosis is a relatively common craniofacial anomaly and various techniques were introduced to achieve its operative goals. Authors found that by using smaller bone fragments than that used in conventional cranioplasty, sufficiently rigid bone union and effective regeneration capacity could be achieved with better postoperative outcome, only if their stable fixation was ensured. Methods: Through bicoronal incisional approach, involved synostotic cranial bone together with its surrounding areas were removed. The resected bone flap was split into as many pieces as possible. The extent of this 'multi-split osteotomy' depends on the degree of dysmorphology, expectative volume increment after surgery and probable dead space caused by bony gap between bone segments. Rigid interosseous fixation was performed with variable types of absorbable plate and screw. In all cases, the pre-operational three-dimensional computed tomography (3D CT) was checked and brain CT was taken immediately after the surgery. Also about 12 months after the operation, 3D CT was checked again to see postoperative morphology improvement, bone union, regeneration and intracranial volume change. Results: The bony gaps seen in the immediate postoperative brain CT were all improved as seen in the 3D CT after 12 months from the surgery. No small bone fragment resorption was observed. Brain volume increase was found to be made gradually, leaving no case of remaining epidural dead space. Conclusion: We conclude that it is meaningful in presenting a new possibility to be applied to not only nonsyndromic craniosynostosis but also other reconstructive cranial vault surgeries.

완전 무치악 환자에서 디지털 시스템을 이용한 임플란트 즉시 보철수복 증례 (Immediate restorations in a fully edentulous patient utilizing digital system: A case report)

  • 본 증례는 하악 완전 무치악 환자에서 인상채득과 석고모형 제작과정 없이 CBCT와 구강스캐너를 이용하여 수술 전에 한 번의 치과방문으로 임플란트 식립과 동시에 즉시 보철수복을 시행하는 방법을 제시하였다. 무치악 부위 연조직 형태, 수직교합고경, 상하악 악궁관계에 대한 정보는 환자가 사용하고 있는 의치를 스캔하여 얻었다. 이 방법은 무치악 환자에서 임플란트를 이용한 보철수복 과정을 기존의 방법보다 내원 횟수를 단축시켰다.

BONE RESPONSE OF THREE DIFFERENT SURFACE IMPLANTS: HISTOMORPHOMETRIC AND RESONANCE FREQUENCY ANALYSIS IN DOGS

  • Statement of problem. Reducing treatment time in implant dentistry is a matter of main concern. There are so many factors affecting the success rate of immediate or early loaded implant for the initial bone response. The especially microscopic properties of implant surfaces playa major role in the osseous healing of dental implant. Purpose. The aims of this study were to perform a histologic and histomorphometric comparison of the healing characteristics anodically roughened surface, HA coated surface and RBM surface implant, and to compare of ISQ values measured by $Osstell^{TM}$ for resonance frequency analysis in dogs mandible during 2 weeks. Material and method. Bone blocks from 2 dogs were caught after covered healing for 0 day(2 h); Group I, 1 week; Group II and 2 weeks; Group III. One longitudinal section was obtained for each implant and stained with hematoxylin and eosin. Histomorphometric analysis was done with Kappa Imagebase system to calculate bone-to-implant contact and bone volumes inside the threads. ISQ values were measured in every time of surgery schedule. Conclusion. The experiment revealed that: 1. The percentages of bone-to-implant contact on the fixture in each group were not significantly different(P > 0.05). 2. The percentages of bone area inside the threads on the fixture in each group were not significantly different(P > 0.05). 3. The ISQ level showed clinical stability of each fixture during 2 weeks(all ISQ level ${\geq}71$ ).

A Prospective Analysis of Dynamic Loss of Breast Projection in Tissue Expander-Implant Reconstruction

  • Background Breast projection is a critical element of breast reconstruction aesthetics, but little has been published regarding breast projection as the firm expander is changed to a softer implant. Quantitative data representing this loss in projection may enhance patient education and improve our management of patient expectations. Methods Female patients who were undergoing immediate tissue-expander breast reconstruction with the senior author were enrolled in this prospective study. Three-dimensional camera software was used for all patient photographs and data analysis. Projection was calculated as the distance between the chest wall and the point of maximal projection of the breast form. Values were calculated for final tissue expander expansion and at varying intervals 3, 6, and 12 months after implant placement. Results Fourteen breasts from 12 patients were included in the final analysis. Twelve of the 14 breasts had a loss of projection at three months following the implant placement or beyond. The percentage of projection lost in these 12 breasts ranged from 6.30% to 43.4%, with an average loss of projection of 21.05%. Conclusions This study is the first prospective quantitative analysis of temporal changes in breast projection after expander-implant reconstruction. By prospectively capturing projection data with three-dimensional photographic software, we reveal a loss of projection in this population by three months post-implant exchange. These findings will not only aid in managing patient expectations, but our methodology provides a foundation for future objective studies of the breast form.
