Effectiveness of Iliopsoas Self-Stretching on Hip Extension Angle, Gluteus Maximus Activity, and Pelvic Compensations during Prone Hip Extension in Subjects with Iliopsoas Shortness

  • Purpose: This study examined the effectiveness of iliopsoas self-stretching on the hip extension angle, gluteus maximus (GM) activity, and pelvic compensated angle during prone hip extension (PHE) in subjects with iliopsoas shortness. Methods: Twenty-healthy subjects with iliopsoas shortness were recruited. Electromyography (EMG) was used to examine erector spinae (ES), multifidus (MF), GM, and biceps femoris (BF) while performing PHE. An electromagnetic tracking motion analysis device was used to measure the pelvic compensations. The pelvic compensations while performing PHE were considered to be anterior tilting and rotation. A modified Thomas test was used to monitor the hip extension angle before and after iliopsoas self-stretching. A paired t-test was used to investigate the significant difference after iliopsoas self-stretching during PHE. The level of statistical significance was set to ${\alpha}=0.05$. Results: Muscle activity of GM and hip extension angle were significantly greater after iliopsoas self-stretching compared to that before iliopsoas self-stretching during PHE (p<0.05). BF and pelvic rotation angle were significantly lower after iliopsoas self-stretching compared to that before iliopsoas self-stretching during PHE (p<0.05). The muscle activity of ES was not significantly different between PHE before and after iliopsoas self-stretching (p>0.05). Conclusion: Iliopsoas self-stretching can be effective in selectively strengthening the GM muscles with minimized pelvic compensation in subjects with iliopsoas shortness.

만성 허리통증 환자의 엉덩허리근 단축과 허리 불안정성 검사 간에 관련성 연구 (A Study on the Relationship Between the Results of Shortening of the Iliopsoas Muscles and the Lumbar Instability Tests in Patients with Chronic Low Back Pain)

  • PURPOSE: This study examined the relationship between lumbar instability and shortening of the iliopsoas muscles in patients with chronic low back pain. METHODS: Forty-nine patients with chronic low back pain participated in this study. The modified Thomas test was used to examine the shortening state of the iliopsoas muscle. The length of the iliopsoas muscle was measured using the hip flexion angle, and those with a flexion angle of 10° or more were classified as positive. Five subtests were used for the lumbar instability test: testing for prone lumbar instability, passive lumbar extension, anterior-posterior mobility, passive straight leg raise, and age. Those who tested positive for at least three of these tests were classified as positive for the lumbar segment instability test. RESULTS: There was a significant association between the results of the lumbar instability test and the shortening of the iliopsoas test (p < .05). After analyzing the association between the iliopsoas length test and the five lumbar instability subtests, the results of the prone lumbar instability test (p < .001) and the anterior-posterior mobility test (p < .05) showed a significant association with the iliopsoas length test. CONCLUSION: The association between lumbar instability and shortening of the iliopsoas muscles was examined in 49 patients with chronic low back pain. Patients with shortened iliopsoas muscles tested positive in the lumbar instability tests more often. Hence, the length test of the iliopsoas muscle can be used to determine lumbar instability in patients with chronic low back pain.

  • 장요 점액낭은 장요근건과 고관절 막 사이에 존재하는 구조물로 정상적인 경우 초음파로 확인하기 힘들지만 일부에서 고관절과 연결되어 있어 관절내 병변에 동반되어 장요 점액낭염이 나타나기도 한다. 저자들은 만성 고관절 동통을 호소하는 환자들에서 장요 점액낭염과 동반된 비구순 파열 2예를 초음파를 통해 진단하고 경험하였기에 이를 보고하고자 한다.

엉덩허리근에 대한 수동 신장이 폐 기능에 미치는 영향 (The Effects of Passive Stretching of the Iliopsoas Muscles on Pulmonary Function)

  • PURPOSE: By virtue of its location, the iliopsoas muscle has the potential to affect the function of the diaphragm. Therefore, the purpose of this study was to investigate the effects of passive stretching of the iliopsoas muscles on pulmonary function. METHODS: Twenty male university students participated in this study. Subjects with low back pain, negative results on the modified Thomas test, and chest breathing patterns not directly related to the function of the iliopsoas muscles were excluded from this study. A digital pulmonary function measuring device (Pony FX, COSMED Inc, Italy) was used to test pulmonary function. The test was performed three times: the first test before passive stretching, the second test 10 minutes after the first test, and the third after passive stretching. Passive stretching was performed 5 times for each of the left and right iliopsoas muscles. Passive stretching was carried out for 20 seconds followed by a 10-second break. The SPSSWIN (ver. 27.0) statistics program was used for data analysis, and the significance level was α =.05. RESULTS: Among slow vital capacity (SVC) parameters, tidal volume (VT), inspiratory reserve volume (IRV), inspiratory capacity (IC) and vital capacity (VC) improved significantly after passive stretching. Also, among the maximal effort vital capacity (MEVC) parameters, forced vital capacity(FVC) and forced expiratory volume in one second(FEV1) improved significantly after passive stretching. CONCLUSION: The results showed that among the various pulmonary function parameters measured, passive stretching of the iliopsoas muscles can improve the SVC and MEVC. Therefore, passive stretching of the iliopsoas muscles may be considered for use as a treatment method to improve pulmonary function.

장요근의 신장운동과 근력운동이 만성요통환자의 요부 정적유연성과 통증강도에 미치는 영향 (The effects of the iliopsoas muscle stretching and strengthening exercise on the static flexibility and pain intensity for the chronic low back pain patients)

  • Background: The purpose of this study was to investigate the effect of stretching and strengthening exercise on the static flexibility and pain intensity for the iliopsoas muscle, which is one of the main reasons for the chronic low back pain. Methods: The subjects of this study were 15 male adult patients with showed 6 score or higher in the visual analogue scale(VAS) and complained of low back pain over three months who visited department of the physical therapy, KIA motors Industrial Health Center, from October, 2008 through December, 2008. Fifteen subjects were trained stretching, mat exercises and sling exercises for iliopsoas muscle at 4-5 times a week for 4 weeks. I measured the changes on the extensibility of iliopsoas muscle, static flexibility of low back and VAS between pre- and post exercise treatment. Date were analyzed using the Wilcoxon's signed rank test considering the size of the samples. Results: 1. The angle of the hip joint that showed the extensibility of iliopsoas muscle was relieved, which was significant statistically (p<.05). 2. The static flexibility was statistically significantly improved in the trunk flexion test, trunk extension test and Schober-Test (p<.05). 3. The VAS showed decrease, which is significant statistically (p<.05). Conclusion: It is believed that the exercise treatment of iliopsoas muscle has the significant effects on the improvement of static flexibility and decrease of pain intensity for the chronic low back pain patients.

장요근 압통을 동반한 요추 추간판 탈출증 환자에 대한 장요근 MST 호전도 비교 연구: 무작위 대조 연구 (The Comparative Study of Improvement of Patients Who Were Diagnosed with HIVD of L-Spine with Iliopsoas Muscles Tenderness by MST(Motion Style Treatment) on Iliopsoas Muscles: A Randomized, Controlled, Trial)

  • Objectives : This research was to evaluate the MST effectiveness on iliopsoas muscle of herniated inter-vertebral disc in lumbar spine. Methods : This research was carried out with the 58 inpatients who received treatment for their HIVD in lumbar spine from January 1 to April 12, 2014 in Daejeon Jaseng hospital of oriental medicine. We randomly divided into two groups: Group A=common treatment on HIVD of L-spine without MST(acupuncture, pharmacopuncture, herb medication, chuna treating and physiotherapy) and Group B=common treatment on HIVD of L-spine with MST. To verify the MST effect, tenderness(checked by algometer pressure), NRS(numeric rating scale), and ODI(Oswestry disability index) were measured before and after treatment. The statistically significance was evaluated by SPSS 18.0 for Windows. Results : In the Group B, the tenderness was significantly decreased compared with Group A. However, on the NRS and ODI, it did not produce the meaningful results as these two values decreased in all groups. Conclusions : From this research, when patients who were diagnosed HIVD of L-spine with iliopsoas muscles tenderness receive MST additionally, the satisfaction of patients as well as physician confidence will increase.

장요근 아시혈의 침치료 및 추나치료를 통해 요추협착증 및 하지길이의 차이가 호전된 1례에 대한 임상 보고 (A Clinical Study of Patient with Lumbar Stenosis and Leg Length Inequality by Acupuncture Treating and Chuna Treating to A-Shi Point of Iliopsoas Muscle)

  • Objectives : The purpose of this report is to examine the effects of acupuncture treating and chuna treating to A-Shi point of iliopsoas muscle on lumbar stenosis and leg length inequality. Methods : We investigated one patient suffering from lumbar stenosis, which came to Sung-Min Oriental Medicine Clinic from December 4, 2006 to March 3, 2007. And we operated acupuncture treating and chuna treating to A-Shi point of iliopsoas muscle. Results : That patient's subjective symptoms such as lumbago, right leg weakness and right leg numbness have improved. Conclusions : Acupuncture treating and chuna treating to A-Shi point of iliopsoas muscle were associated with improvement of lumbar stenosis and leg leng inequality.

장요근 침치료와 마사지로 호전된 하지불안증후군 환자 치험례 (Effect of Acupuncture and Massage on Iliopsoas Muscle in Patients with Restless Leg Syndrome : Case Report)

  • Restless legs syndrome(RLS) is a neurological disorder characterized by an irresistible urge to move legs to stop uncomfortable or odd sensations. We report a new approach to improve RLS using acupuncture and massage on iliopsoas Muscle. The patients who have RLS were treated by acupuncture and massage on iliopsoas Muscle. We measured 3~6 times IRLS(Korean Versoin of International Restless Legs Scale), VAS(visual analogue scale), and angle(between the legs and the table) during the treatment period. After treatment, RLS symptoms decreased, In case 1, IRLS changed 34 to 10, VAS changed 8 to 2, angle changed 20 to 5. In case 2, IRLS changed 22 to 12, VAS changed 10 to 5, angle changed 15 to 10. In case 3, IRLS changed 30 to 9, VAS changed 8 to 1, angle changed 25 to 5. Acupuncture and massage on iliopsoas muscle for RLS was effective. To effective acupuncture treatment for RLS, more research is needed.

장요근을 대상으로 한의학적 치료를 시행한 국내외 임상연구 동향 분석 (An Analysis of Domestic and International Clinical Research Trends on Korean Medicine Treatments for Iliopsoas Muscle)

  • Objectives The objective of this study was to investigate clinical the trends of Korean medicine treatments for iliopsoas muscle. Methods 6 Korean and foreign on-line databases were researched for articles discussing Korean medicine treatments for iliopsoas muscle. The included studies were classified according to the published years, study designs, techniques of treatment and evaluation method. Results The total number of studies selected was 12, which included 8 case reports, 2 randomized controlled trials (RCT), and 2 non-RCT. In these studies, acupuncture, pharmacopuncture, Chuna, and herbal medicine were used for treatment. Most studies used visual analogue scale as primary outcome. Conclusions Based on the results of the collected studies, we analyzed the clinical trends of Korean medicine treatments for iliopsoas muscle. The results showed that a variety of Korean medicine treatments can be applied to the iliopsoas muscle and positive effects can be confirmed.

  • Objectives : This study is designed to find out the influence of acupuncture treatments on Iliopsoas muscles of disc herniated patients. Method : Control group took usual acupuncture therapies and sample group took acupuncture therapies on their Iliopsoas muscles and usual acupuncture therapies. To Conclude the results, we studied Numerical Rating Scale(NRS) score and changes of SLR-test angle statistically. Results : The improvement rate of the group who took Iliopsoas muscles and usual acupuncture therapies was higher than the group who only took usual acupuncture therapies. Conclusion : In this study, we found out that the treatment on Iliopsoas muscles is effective to low back pain patients.
