Development of Case Management System and Analysis of Economic Feasibility under the Fee-For-Service Reimbursement (행위별 수가 지불제도 하에서의 사례관리시스템 개발 및 경제성 분석)

  • Background : Recently, we have experienced various changes in the healthcare environment. Healthcare organizations are facing a financial crisis due to more competitive relationships among themselves as well with low health insurance fees. The purposes of the current study were: (1) to develop a data warehouse-based system for evaluating and monitoring the case management activities, and (2) to measure and analyze its effects. Methods : In order to collect the data for the study, the database on discharged patients was utilized at a university hospital located in Seoul from June 1, 2002 through December 31, 2002. Initially, a data warehouse was built for the case management system. The case management activities were analyzed using structured methodology to establish the case management system. Results : The findings of this study were as follows: (1) A case management system was developed to make it possible to monitor of healthcare quality and resource utilization. The Case management System included monitoring functions regarding utilization reviews, critical pathways, and clinical indicators. (2) Utilizing the case management system, unplanned readmissions were documented among total discharged patients during two months from November 1, 2002 through December 31, 2002. The unplanned readmission rate was 2.3%(276 patients) in total of 11,960 discharged patients. Among them 81 patients(0.7% of total discharges, 29.3% of unplanned readmission) were readmitted to the same physician in charge under the same diagnosis. No significant differences were found in the demographic variables such as gender and age among the patients. (3) After implementing the case management system, 2.9% of average length of stay reduced. Applying cost-benefit analysis, the 2.9% reduction of length of stay represents net profit of 279,592,000 in the year of 2004. In addition, applying value acceleration analysis, cumulative net benefit of 1,481,000,000 was expected by the year of 2007. Also we were able to expect 247,800,000 of cumulative benefit for the prospective 5 years in value linkage analysis. It represents average 787,700,000 of pure net benefit a year. Conclusion : The value of present study would be not only implementing the knowledge management system into the existing case management activities, but also evaluating its effects and estimating its financial benefits. This study suggested that the case management system would be a supportive tool for monitoring and improving the quality of healthcare, and a cost-effective tool for increment of healthcare organization's financial benefit.

French 'Minima Sociaux's Scheme, Benefit Determination Rule and its Appreciation : A Study on Social Assistance in Europe (프랑스 사회적 미니멈(Minima sociaux)의 구조 및 급여 체계 : 유럽 공공 부조 제도의 한 연구)

  • The purpose of this article is to explore French 'minima sociaux', focusing its scheme, benefit determination rule and its level appreciation. First, on the its structure domain, French 'minima sociaux' presents the plural system in which there is eight categorical benefits and one general benefit. Il is the representation of the intention to guarantee minimum income for the dead zone people out of the social insurance application and also a historical product in different period, by different logic of benefits implementation. Second, comparing nine benefits based on the benefit determination rule, level of benefits for the poor without work ability is higher than one for the poor with work ability. Il represents one polarized perception toward for the poor according to have or not its work ability. Third, comparing level of 'minima sociaux' with relative poverty line, the level of the most 'minima sociaux' is placed under the poverty line. Nevertheless, it must not forget that 'minima sociaux' plays its role as the fundamental alternative for poverty alleviation, but not the only alternative. Fourth and finally, comparing with minimum income guarantee(SMIC in french), level of RMI benefit is estimated merely on the 50% of SMIC. We can consider that it is the result of the interaction of the complex factors, as the limited role of the state toward the minimum income guarantee for the RMI beneficiary and the intervention the logic of status instead of the logic of need, etc..

The Relationship among Trust of Instructors, Leisure Benefit and Resilience for Golf Participants (골프참여자의 지도자 신뢰와 여가 이득 및 회복탄력성의 관계)

  • The purpose of this study was to investigate the relationship among trust of instructors, leisure benefit and resilience for golf participants. To achieve the goal of this study, a total of 250 questionnaires were distributed and 250 copies were collected back. Out of those returned questionnaires, insincerely replied or double-replied questionnaires were excluded and finally 234 questionnaires were analyzed for this study. For analysis of the data, frequency analysis, exploratory factor analysis, reliability analysis, confirmatory factor analysis, correlation analysis, and structural equating modeling were conducted using SPSS 18.0 and AMOS 18.0. Main findings were as follows: First trust of instructors had a positive effect on leisure benefit. Second, leisure benefit had a positive effect on resilience. Third, trust of instructors had a positive effect on resilience. Fourth, leisure benefit mediated the relationship between golf participant trust of instructors and resilience. Therefore, it is necessary to make efforts to make golf participation lead to satisfaction, and in addition, it is necessary to seek various and convenient services so that various benefits can be generated through golf, and even psychological management through careful observation of golf participants. Is judged.

Cost-Benefit Analysis of Telepresence System for Public Sector Relocation (공공기관 지방이전에 따른 영상회의 시스템 도입의 경제성 분석)

  • The purpose of this study is to analyze the economic effects of telepresence system for public sector relocation. As a Green IT system, the importance of telepresence is being recognized increasingly as a means to replacing a majority of business trips and thereby reducing greenhouse gases emitted in the process. However, there are only a few related researches that analyze the economic effects of telepresence at the moment. This study conducts the cost-benefit analysis of 157 public sectors which are to relocate from 2012 based on the sample case of National Information Society Agency. The results obtained reveals that telepresence would save about 180 billion won and reduce about 580t CO2 emission. This study are expected to provide the academic framework for organizations contemplating the adoption of Green IT system moving forward.

The Effect of Organizational Relations and Technology Factors on Logistics Performance of Logistics Firms (물류기업의 조직간 관계요인 및 기술요인이 물류성과에 미치는 영향)

  • This study The purpose of this study is to analyze the effect of relationship factors and technical factors on the logistics performance of logistics companies. As a result of the analysis, information sharing, mutual trust, and mutual benefit of the inter-organizational relationship factors as factors affecting the logistics performance were analyzed as the factors that positively influence the logistics performance. In the technology factor, IT infrastructure and technology reliability are analyzed as factors that positively influence logistics performance, but system operation capacity is analyzed as a factor that does not positively affect logistics performance. Based on the results of this analysis, it can be confirmed that logistics companies can maximize the logistics performance by mutual sharing of information, mutual benefit and trust, and it is also confirmed that technical reliability is also an important factor.

  • The study derives various anticipated benefits resulting from locating small and medium sized IT service firms in a specific industrial cluster area. The study also derives IT service agility factors to obtain competitive advantages vital to real-time global enterprises. The anticipated benefits were then used to analyze how they improve the firms' agility. Using questionnaire data, factor analyses were used to figure out eight anticipated benefit factors including coordination, information sharing, business costs, speciality, communication, trust improvement, mutual business understanding, and reciprocity. The eight empirically derived benefit factors were then used to examine how they affect the IT service agility including flexibility, speed, responsiveness, and lean efficiency. The results of regression analyses showed that each one of the anticipated benefit factors except communication influence one or more IT service agility factors.

SPB(Seamlessness-Privacy-Benefit) Paradigm for Construction of Highly-Connected Information Society (고도 정보 연계 사회의 구축을 위한 SPB(Seamlessness-Privacy-Benefit) 패러다임)

  • The purpose of this research is two-fold : (1) we propose the paradigm to construct a connected information society according to a judgment where a coming information society will evolve into highly-connected information society having "seamlessness" as a keyword, and (2) we draw guidelines to construct infrastructures and establish systems and business models of highly-connected information society based on suggested paradigm. To achieve this, this research conducts the followings. First, we suggest reasons why a coming information society can be appropriate to be considered as highly-connected information society by examining two perspectives on the coming information society. Second, we explain the definition and characteristics of highly-connected information society and propose the SPB paradigm. Third, we analyze the utility of SPB paradigm by evaluating practices. Finally, we suggest four strategies to construct highly-connected information society based on advanced research findings and future research directions.

대형 가격할인점 이용이 소비자편익(消費者便益)에 미치는 효과에 관한 일고(一考)

  • The aim of this study is to analyze the consumers perception of price and the economic benefit of shopping at the various discount-stores. We found that there is a significant difference between the real price level of discount-stores and the firms purporting discount level of their commodities. We also, found that there is a bias between the consumers perception of their monetary profit and the actual economic benefit. It's recommended that consumers should give objective judgement on price level of discount-stores and appreciate rationally the exact profit for them to take the advantage in their everyday life. To increase consumer welfare, it's desirable to improve labeling system on retail prices and regulations on distribution industry as well as the consumers perception of price and their profit level as mentioned above. Additionally, it should be followed that the business side build the consumer-oriented management system.

  • If private discipline replace the strict administrative regulation, fundamental rights of protective essence can be relativized while if may be beneficial for the individual freedom. Assigning the state power to the private organization is equivalent to giving up the practice of individual liberty to the organized authority. It is important to make use of public law mechanism, particularly under the imperfect juristic system. Regarding the prohibition of prior censorship, it is necessary to consider whether it is desirable to prioritize the freedom of speech over the rest of the legal benefit. Apart from this necessity, new advertisement screening mechanism was irreversibly - and controversially - introduced for the health functional food since the unconstitutional verdict of the prior screening of advertisements. With this, the state must secure the broad order through legal regulations and norms in order to freedom of speech and other legal benefit can be harmoniously and practically guaranteed.

A Study on Cost-Benefit analysis for Geographic Information Systems in Local Governments (지자체 GIS사업을 위한 비용효과분석 연구)

  • Because efficiency of the 1st-phase NGIS investment(1995-2000) has not been clearly measured, the action taken in the 2nd-phase NGIS project requires NGIS budgets to be evaluated in terms of effectiveness. Especially, the effective investments in local governments are critical for the NGIS projects, because they execute the much larger amount of budgets in total than other GIS projects. As indicated, for the successful NGIS implementation, it is important to obtain continuous political and financial supports from decision makers. As a persuasion measure for the budget appropriation, CBA(Cost-Benefit Analysis) and CEA(Cost-Efficiency Analysis) can play an important role for the decision makers. The major concern of this paper is how to measure the costs and benefits of the GIS implementation by considering important characteristics of the GIS projects in local governments, and existing theories are reviewed for this concern. The GISs in local governments can have different stages in terms of its evolution and the effectiveness of the applications can be represented variously. To identify categories for measuring costs and benefits of the various GISs, case studies and success stories are reviewed from both the foreign and domestic research. The categories of costs and benefits are determined from the tangible and intangible aspects. The categories for the quantitative and qualitative measure are proposed to evaluate the GISs in local governments. After measuring costs and benefits, three key evaluation methods in cost-benefit analysis are suggested as follows: 1) the benefit: cost ratio (B/C), 2) Internal Rate of Return (IRR), and 3) the net present values (NPV) of the costs and benefits. The sensitivity and uncertainty analysis are also helpful to make a decision for the GIS budget appropriation in local governments. In conclusion, although cost-benefit analysis is not an easy undertaking, it is certain that it can play an important role in the future for the GIS funding decisions in local governments.
