Combination Treatment of Korean Medicine with Guibiondamtang-gamibang for Hwa-Byung Patients: 2 Cases Report (귀비온담탕 가미방을 포함한 한방치료로 호전된 화병 환자 2례 : 증례 보고)

  • Objectives: This case report aims to report the clinical effectiveness of the combination treatment of Korean medicine with Guibiondamtang-gamibang on Hwa-byung patients. Methods: Case 1 experienced stress at work and complained of chest discomfort, nausea, dizziness, insomnia. Case 2 experienced huge stress about money, and complained of palpitation, chest discomfort, dizziness and insomnia. They received combination treatment of Korean medicine during hospitalization. The treatment included Guibiondamtang-gamibang, acupuncture, moxibustion, pharmacoacupuncture, and fumigation. The effects were evaluated through Hwa-byung scale, instrument of oriental medical evaluation for Hwa-byung, insomnia severity index (ISI) and heart rate variability (HRV) test. Results: After the treatment, the clinical symptoms such as chest discomfort, palpitation, dizziness, nausea and insomnia were improved. Conclusions: This case report shows that the combination treatment of Korean medicine with Guibiondamtang-gamibang may be effective for treating Hwa-byung patients.

A Performance Comparison of mSE-MMA and mDSE-MMA Adaptive Equalization Algorithm in 16-QAM Signal Transmission (16-QAM 신호 전송에서 mSE-MMA와 mDSE-MMA 적응 등화 알고리즘의 성능 비교)

  • This paper related with the performance comparison of mSE-MMA and mDSE-MMA adaptive equalization algorithm which is possible to reduce the intersymbol interference that occurs in the nonlinear communication channel transmitting 16-QAM signal. This two algorithm is possible to reduce the computational load compared to the current MMA algorithm, it has the degraded equalization performance due to simplified arithmetic in order to applying the mobile communication terminal. In order to improve the performance degradation, they controls the step size according to the existence of arbitrary radius circle of equalizer output compared to transmitted symbol point. The variation of step size according to this principle is applied to the SE-MMA and DSE-MMA, namely mSE-MMA and mDSE-MMA algorithm, the algorithm's performance were compared in the same channel and noise environment by computer simulation. As a result of simulation, the mSE-MMA has more superior to the mDSE-MMA in residual value of every performance index and SER performance, and the vice versa result in convergence speed.

  • This paper evaluates the performance of CCA (Compact Constellation Algorithm) adaptive equalization algorithm by varying the step size for minimization of the distortion effect in the communication channel. The CCA combines the conventional DDA and RCA algorithm, it uses the constant modulus of the transmission signal and the considering the output of decision device by the power of compact slice weighting value in order to improving the initial convergence characteristics and the equalization noise by misadjustment in the steady state. In this process, the compact slice weight values were fixed, and the performance of CCA adaptive equalization algorithm was evaluated by the varing the three values of step size for adaptation. As a result of computer simulation, it shows that the smaller step size gives slow convergence speed, but gives excellent performance after at steady state. Especially in SER performance, the small step size gives more robustness that large values.

  • This paper proposes a dolphin whistle mimicking underwater communication method using FSK (Frequency Shift Keying) and method to improve BER (Bit Error Rate) performance by using multipath gain combining. The proposed method divides whistle sound into short time intervals and transmits FSK modulated signal that ensures orthogonality of the symbol. Multipath gain can be obtained by using characteristic of mimicked signal frequency that varies with time. To demonstrate the performance of the proposed method, computer simulations and lake experiments were conducted. Computer simulation results show that an additional multipath gain is obtained by multipath. From lake experiments, when symbol length is 20 msec and modulation band is 900 Hz, the proposed FSK method with multipath combining gain obtains BER of 0.002, which is better than CSS (Chirp Spread Spectrum) with BER of 0.185. he proposed based on FSK method has higher imitation degree than the CSS method by analyzing mean cross-correlation value in the time - frequency domain of the imitated signal and actual whistle signal.

  • As dental implant surgery and bone grafts were widely operated in Korean dentist, many bone substitutes are commercially available, currently. For commercially used in Korea, all bone substitutes are firstly evaluated by the Ministry of Health and Welfare (MOHW) for safety and efficacy of the product. After being priced, classified, and registration by the Health Insurance Review and Assessment Service (HIRA), the post-application management is obligatory for the manufacturer (or representative importer) to receive a certificate of Good Manufacturing Practice by Ministry of Food and Drug Safety. Currently, bone substitutes are broadly classified into C group (bone union and fracture fixation), T group (human tissue), L group (general and dental material) and non-insurance material group in MOHW notification No. 2018-248. Among them, bone substitutes classified as dental materials (L7) are divided as xenograft and alloplastic bone graft. The purpose of this paper is to analyze alloplastic bone substitutes of 37 products in MOHW notification No. 2018-248 and to evaluate the reference level based on the ISI Web of Knowledge, PubMed, EMBASE (1980-2019), Cochrane Database, and Google Scholar using the criteria of registered or trademarked product name.

A Performance Evaluation of QE-MMA Adaptive Equalization Algorithm based on Quantizer-bit Number and Stepsize (QE-MMA 적응 등화 알고리즘에서 양자화기 비트수와 Stepsize에 의한 성능 평가)

  • This paper relates with the performance evaluation of QE-MMA (Quantized Error-MMA) adaptive equalization algorithm based on the stepsize and quantizer bit number in order to reduce the intersymbol interference due to nonlinear distortion occurred in the time dispersive channel. The QE-MMA was proposed using the power-of-two arithmetic for the H/W implementation easiness substitutes the multiplication and addition into the shift and addition in the tap coefficient updates process that modifies the SE-MMA which use the high-order statistics of transmitted signal and sign of error signal. But it has different adaptive equalization performance by the step size and quantizer bit number for obtain the sign of error in the generation of error signal in QE-MMA, and it was confirmed by computer simulation. As a simulation, it was confirmed that the convergence speed for reaching steady state depend on stepsize and the residual quantities after steady state depend on the quantizer bit number in the QE-MMA adaptive equalization algorithm performance.

Comparison of Sleep Duration, Social Jetlag, and Subjective Sleep Disturbance in Rotating Shift Nurses According to the Chronotype (순환교대근무 간호사의 일주기유형에 따른 근무조별 수면시간, 사회적 시차, 주관적 수면장애 비교)

  • Purpose: The purpose of this study was to investigate the sleep duration, social jetlag (SJL), and subjective sleep disturbance according to the individual chronotype in rotating shift nurses. Methods: A total of 344 rotating 3-shift nurses (mean age 28.67 years) were recruited at one university affiliated hospital. They completed the following questionnaires, which were used to assess their chronotype and sleep: the morningness-eveningness questionnaire (MEQ), self-reported sleep duration of work days (SDWshift) and free days (SDFshift) in each shift (day [D], evening [E], night [N]), and sleep disturbance (Insomnia severity index, ISI). SJLshift was calculated as the difference in midsleep (MS=sleep onset+1/2 sleep duration) between work days (MSW) and free days (MSF). Results: Subjects were divided into 3 chronotype groups according to the MEQ; morning (MG, 4.4%) intermediate (IG, 57.8%), and evening groups (EG, 37.8%). SDWD was shortest (4.68 hr) and SDFE was longest (8.93 hr) in the EG. SJLD was longest in the EG (3.77 hr), and SJLN was longest in the MG (7.37 hr). The prevalence of sleep disturbance was 33.3% (MG), 29.6% (IG), and 40.0% (EG), respectively, without any statistical significance. Conclusion: In order to improve the sleep of shift workers, it is recommended that the evening chronotypes should reduce the day shifts and the morning chronotypes should reduce the night shifts. We also propose a study to determine whether these shift assignments can improve the sleep in shift nurses.

The Mediating Effect of Depression on the relationship between Emotional Labor and Somatic Symptoms and Insomnia in Non-Medical Hospital Workers (비의료직 병원 종사자들의 감정노동과 신체증상 및 불면증 간의 관계에서 우울의 매개효과)

  • The purpose of this study is to prevent risk of emotional labor and health through verifying the mediating effects of depression on the relationship between emotional labor and somatic symptoms and insomnia in non-medical hospital workers. For this study, a sample of 533 tertiary hospital's non-medical workers completed the questionnaires of the Korean Emotional Labor Scale, PHQ-15, ISI, PHQ-9. The data was analyzed using the SPSS 25.0 program. The results can be summarized as follows. 1. Among the emotional labor sub-factors, emotional dissonance had a significant effect on depression, somatic symptoms and insomnia. Other sub-factors had a relatively small effect or had no effect. 2. The results showed the partial mediating effects of depression on the relationship between emotional labor and somatic symptoms. 3. The results showed the full mediating effects of depression on the relationship between emotional labor and insomnia. On the basis of the results, we suggest the necessity of dealing with emotional dissonance and depression in non-medical hospital workers' emotional labor and health issues.

Efficacy and Safety of Banhahubak-Tang for Depression Treatment: Study Protocol for a Randomized, Waitlist-Controlled Trial

  • Objectives: Depression is a highly prevalent disease, and the market for antidepressant drugs continues to grow at a steady rate. Although current antidepressants are reported to be effective, because of their low remission rate and side effects, new antidepressants are needed. The use of Banhahubak-tang (BHT) to treat the symptoms of depression is supported by experimental evidence. The proposed study will evaluate the efficacy and safety of BHT in treating depression. Methods: A randomized, waitlist-controlled, parallel clinical trial will be conducted to assess the efficacy of BHT in depression. A total of 84 participants with depression will be randomized into the intervention group or waitlist-control group at a 1:1 ratio. Patients in the intervention group will be administered BHT three times a day for four weeks and followed up for four more weeks after therapy completion. Patients in the waitlist-control group will undergo the same intervention and follow-up after a four-week waiting period. The primary outcome is change in the Korean version of the Hamilton Depression Rating Scale (K-HDRS) scores for major depressive disorders after four weeks. The secondary outcomes include scores on the K-HDRS, Korean Symptom Check List 95 (KSCL-95), Insomnia Severity Index (ISI), State-Trait Anxiety Inventory-Korean version (STAI-K), State-Trait Anger Expression Inventory-Korean version (STAXI-K), and the Instrument on Pattern Identifications for Depression and EuroQol-5 Dimension (EQ-5D). Conclusions: This trial will provide high-quality clinical evidence of the efficacy and safety of BHT in the management of depression.

Utilization of Psychological Tools for Critical Pathway Based Mental Evaluation and Diagnosis (CP 기반 정신 평가 및 진단을 위한 심리검사의 활용)

  • Objectives: For diagnosis and evaluation, evaluation tools are needed. Various tools can be used to diagnose and evaluate mental disorders. Among them, psychological tests are widely used. For Korean Standard Classification of Diseases (KCD) diagnosis, psychological tests are also required. Currently developed critical pathway (CP) presented tools for diagnosis and evaluation of mental disorders. The CP suggests the use of tools based on the Clinical Practice Guideline (CPG). Therefore, CPG-based tools should be able to be used in the clinical scene of Korean Medicine for diagnosing and evaluating mental disorders according to CP. Methods: Tools suggested by CPs are summarized. The degree of utilization of tools in CPGs is also summarized. A review was conducted by Korean Medicine neuropsychiatrist experts on tools and user's usage plan. Results: As a result, developed CPs suggested using 19 tools for anxiety disorder, 13 tools for insomnia), 12 tools for Hwabyeong, and 9 tools for dementia. In CPG, 48, 34, 44, and 44 tools were used for anxiety disorder, insomnia, Hwabyeong, and dementia, respectively. Among tools presented in CP, HAM-A, HAM-D, CGI, SAS, and TESS for anxiety disorder, CPG, ISI, and PSQI for insomnia disorder, CPG, STAI, and STAXI for Hwabyeong were frequently used in CPG. For dementia CPG, MoCA, MMSE, HDS, ADL, and ADAS-cog were frequently used. Among them, MoCA, ADL, and ADAS-cog were suggested tools in CP. Conclusions: As a result of analyzing tools suggested and used in the developed CPs and CPGs, it was verified that various tools were used in each study. Most of them were symptom and behavioral evaluation scales. Therefore, symptoms and behavior evaluation scales used more frequently should be able to be used in the clinical scene of Korean Medicine.
