두 종류의 CAD/CAM 지르코니아 전부도재관의 도재 축성 전후의 변연적합도 비교 (Comparison of marginal fit before and after porcelain build-up of two kinds of CAD/CAM zirconia all-ceramic restorations)

  • 연구목적: 변연적합도는 보철수복물의 성공을 좌우하는 중요한 요소 중의 하나이다. McLean과 von Fraunhofer등에 의하면 120 ${\mu}m$ 이하를 임상적으로 허용할 수 있다고 했는데, 이를 넘는 정확하지 않은 변연은 지각과민을 일으키거나 이차 우식을 일으키고, 치태 축적을 용이하게 하여 보철물의 수명을 단축시키게 된다. 이에 최근에 우리나라에 소개되어 임상적으로 사용되고 있는 CAD-CAM 전부도재 수복물 중 $LAVA^{(R)}$ 시스템 (3M ESPE, St. Paul, MN)과 $EVEREST^{(R)}$ 시스템 (KaVo Dental GmbH, Biberach, Germany)으로 제작한 지르코니아 전부 도재관들이 도재전장 금속주조 수복물과 비교해 보아 코핑 상태일 때와 도재축성 후 각각 변연적합도의 차이가 있는지를 살펴보고자 하였다. 재료와 방법: 상악 제1소구치 덴티폼 치아를 high speed diamond bur 로 교합면 2.0 mm, 순설면 1.0 mm, 그리고 변연은 chamfer margin을 6도 측면 경사 절삭 기계로 균일하게 제작해서 이를 인상 채득하여 금속 지대치를 제작하였다. 이를 덴티폼 모형상에 장착한 후 전악인상을 채득하여 $LAVA^{(R)}$ 수복물 코핑, $EVEREST^{(R)}$ 수복물 코핑, 그리고 도재전장 금속주조 수복물 코핑을 각각 8개씩 제작하였다. 이들 코핑을 염색제를 섞은 Fit-checker $II^{(R)}$ (GC Cor., Tokyo, Japan)를 이용하여 금속 지대치에 접합시킨 뒤, $Microhiscope^{(R)}$ system (HIROX KH-1000 ING Plus, Seoul, Korea)을 이용하여 300배율로 관찰하면서 변연 간격을 ${\mu}m$단위로 측정하였다. 도재축성 후에도 마찬가지 방법으로 모든 수복물의 변연간격을 측정하여 통계 처리를 하였다. 결과: 코핑 상태에서의 평균 변연간격 값은 $EVEREST^{(R)}$ 에서 $52.00{\pm}11.94\;{\mu}m$, $LAVA^{(R)}$$ 56.97{\pm}10.00\;{\mu}m$, 그리고 도재 전장 금속 주조 수복물은 $97.38{\pm}18.54\;{\mu}m$를 보였고, 도재축성 후에 평균 변연 간격값은 $EVEREST^{(R)}$$61.69{\pm}19.33\;{\mu}m$, $LAVA^{(R)}$$70.81{\pm}12.99\;{\mu}m$, 그리고 도재 전장 금속 주조 수복물은 $115.25{\pm}23.86\;{\mu}m$를 보였다. 결론: 1. $LAVA^{(R)}$$EVEREST^{(R)}$ 수복물은 도재 전장 금속 주조 수복물과 비교하여 각각 코핑상태일 때와 도재 축성 후 모두 유의성 있는 (P < .05) 차이를 보이는 우수한 변연 적합도를 나타내었다. 2. $LAVA^{(R)}$$EVEREST^{(R)}$ 수복물 간의 평균 변연 간격 비교에서 코핑 상태일 때와 도재 축성 후에 모두 유의성 있는 차이는 없었다 (P > .05). 3. $LAVA^{(R)}$$EVEREST^{(R)}$ 그리고 PFM 은 각각 코어 상태일 때와 비교하여 도재 축성 후 변연 간격에서 약간 증가되었으나 유의성 있는 차이를 보이지는 않았다 (P > .05).

요추(腰椎) 추간판(椎間板)의 탈출형태(脫出形態)와 한방치료(韓方治療)의 상관성(相關性)에 관한 임상보고(臨床報告) (Clinical Reports on Correlation between the Different Herniated Type and Oriental Medical Treatment)

  • Herniated lumbar intervertebral disc(H.I.V.D) is the most common reason causing low back pain and leg radiating pain. Objective : The purpose of this report is to observe the effects of oriental medical conservative treatment in the different herniated type. Methods : We investigated 30 patients suffering from low back pain with sciatica which were admitted to Taejon Cheonan O. M. hospital from Nov 1, 2000 to Apr 30, 2001. 30 patients had a diagnosis of herniated lumbar interver tebral disc by Lumbar-C.T and Lumbar-M.R.I. we treated 30 patients by oriental medical conservative treatment (Ex: acupuncture, herb-med, physical theraphy, bed-rest, etc.) Results : 1. The mean hospitalization of patients was 21.1 days. that in the bulging type was l4days, protruded type 21days, mixed type 27days, extruded type 30days. that shows the less herniation of lumbar intervertebral disc, the mean hospitalization was more short. 2. In the distribution of the clinical symptoms admitted at that time, low back pain, leg radiating pain were showed in the all types(29patients, 96.6%). and sensory disorder, muscle powerlessness were showed least in the bulg ing type(0%, 37.5%), but that were showed most in the extruded type(50%,75%). 3. In the result of treatment due to clinical syptoms, bulging type was more execellent than any other other types. 4. The less herniation of lumbar intervertebral disc, angle of straight leg raising test was higher. In the result of treatment due to angle of straight leg raising test, bulging type was more execellent than any other other types. 5. In the distribution of physical. test, positive case in the Peyton sign, Ankle Dorsiflexion were showed least in the bulging type(25%, 12.5%), but that were showed most in the extruded type(100%, 75%). 6. In the result of treatment due to physical.test, bulging type was more exec ellent than any other other types. 7. The efficacy of total treatment was 90%(when we set a standard things more than fair), that in the bulging type was 100%, protruded 92%, mixedtype 80%, extruded type 75%. Bulging type had a more remakable effectsthan any other types. Conclusions : The less herniation of lumbar intervertebral disc, clinical syptoms were slighter, physical. test were better, and efficacy of oriental medical conservative treatme nt was remakable high.

Longitudinal Gradients and Seasonal Dynamics of Nutrients, Organic Matter and Conductivity Along the Main Axis of Han-River

  • The purpose of the study was to evaluate spatial and temporal dynamics of nutrients (TN, TP), organic pollution (BOD, COD), and ionic dynamics (electrical conductivity, EC) in the North Han-River, South Han-River, and merged downriver using the dataset of $1998{\sim}2007$, obtained from the MEK (Ministry of Environment, Korea). Accord. ing to interannual nutrient analysis, TN varied slightly in the North Han-River and South Han-River, but decreased in the merged downriver along with BOD. Longitudinal analysis in the water quality showed that BOD, COD, and nutrients had linear decreasing trend along the main axis of headwater-to-downriver. Concentrations of TP and TN in the North Han-River averaged $26.97{\mu}g\;L^{-1}$, $1.696mg\;L^{-1}$, respectively, which were minimum in the three watersheds, followed by South Han-River and then the merged downriver in order. Ratios of TN:TP in the watersheds were >40 in all the sites, indicating that nitrogen may be enough for periphyton or phytoplankton growth and phosphorus may be limited partially. After the North Han-River water is merged with South Han-River, the concentrations of BOD, COD, TN, and TP were similar to the values of $S6{\sim}S7$, respectively or a little bit higher, but increased abruptly in Site M4 (Fig. 3). Thus, mean values of all the water quality parameters in the reach of $M4{\sim}M7$ sites were greater than any other sites. Seasonal data analysis indicated that BOD and EC in the downstream ($S3{\sim}S7$) was greater in the premonsoon than two seasons of the monsoon and postmonsoon, and no significant differences in BOD between the three seasons were found in the upstream ($S1{\sim}S2$). Empirical models of COD in the merged downriver was predicted ($R^2=0.87$, p>0.01, slope = 0.84, intercept = -1.28) well by EC. These results suggest that EC to be measured easily in the field may be used for estimations of nutrients and organic matter pollutions in the merged downriver and these linear models are cost-effective for the monitoring of the parameters.

산화 스트레스 대한 Saccharomyces cerevisiae KNU5377의 항산화 활성의 증가 (Increased Antioxidative Activities against Oxidative Stress in Saccharomyces cerevisiae KNU5377)

  • 산화적 스트레스는 정상적인 대사 과정뿐만 아니라 외부적인 환경에 노출 되었을 때 일어나는 것으로 잘 알려져 있다. 이러한 스트레스를 극복하기 위해 생물체들은 각자의 시스템에 맞게 다양한 항산화 시스템을 진화 발전시켜 왔다. Saccharomyces cerevisiae KNU5377 균주는 고온뿐만 아니라 다양한 스트레스에 대해 내성을 가짐을 확인하였다. 대부분의 스트레스는 궁극적으로는 산화적 스트레스로 귀결된다. 이러한 측면에서 본 연구는 KNU5377 균주가 어떠한 시스템에 의해서 다른 균주보다 스트레스 내성을 가지는지를 밝히기 위해 접근하였다. 수행된 연구결과에서 KNU5377 균주는 항산화 시스템과 밀접하게 관련된 단백질(superoxide dismutase, thioredoxin system, heat shock proteins)과 항산화 관련 물질(trehalose)을 과발현함을 확인하였다. 그러나 이러한 단백질들이 어떠한 조절 시스템에 의해서 균주 특이적인 발현 양상을 보이는지는 현재까지 확인되지 않고 있다. 본 연구는 KNU5377 균주 그 자체의 중요성과 함께 균주 내의 스트레스 내성과 관련된 유용한 유전자를 탐색하여 더욱 우수한 유전자원을 발굴하는데 기여 할 것으로 보인다.

화력발전소용 V2O5/TiO2계 촉매상에서 NH3-SCR 탈질반응으로부터의 N2O 생성 (Formation of N2O in NH3-SCR DeNOxing Reaction with V2O5/TiO2-Based Catalysts for Fossil Fuels-Fired Power Stations)

  • $V_2O_5/TiO_2$계 촉매상에서 $NH_3$에 의한 $NO_x$의 선택적환원은 310의 지구온난화지수를 갖는 $N_2O$의 또 다른 인위적인 배출원이 될 수 있는 것으로 보고되고 있으므로, 본 총설은 화석연료를 연소시키는 화력발전소용 상기 촉매상에서 SCR 탈질반응 동안에 $N_2O$ 생성과 관계되는 주요 변수들의 유의성을 다루고자 한다. $NH_3$-SCR 탈질반응에서 $N_2O$ 배출은 $NH_3$ 산화반응에 더하여 반응 중에 존재하는 $NO_x$$NH_3$ 간의 부반응을 통해 일어나 이 부반응들의 정도는 SCR 촉매의 활성성분인 $V_2O_5$의 함량과 조촉매의 종류($WO_3$$MoO_3$), 반응온도, $NO_2/NO_x$ 비율, 산소농도, 공간속도, 수분함량, 열처리 등과 같은 유입가스 조건과 운전변수 및 화력발전소 현장에 설치된 상용 SCR 탈질공정에서 격은 촉매의 이력에 크게 의존한다. 상기의 모든 변수들이 탈질반응에서 $N_2O$ 생성과 관계된다고 할지라도, 몇몇 핵심변수들이 $N_2O$ 생성에 미치는 영향과 상용 SCR 공정에서 $N_2O$ 생성을 억제할 수 있는 방안이 고찰되었다.

병원 전산시스템 활용에 영향을 주는 컴퓨터불안과 제변수간의 관계 (A Correlation of the Computer Anxiety and the Variables Affecting the Application of a Hospital Computer System)

  • Nowadays, most big hospitals have a computer system to manage their administration. For maxi mum effectiveness in managing the computer system, an analysis of the variables affecting its implementation is necessary from the beginning. This study was done to analyze the variables influencing the operation of a hospital information system (HIS). The theoretical base for this study considered the combined effects of user expectations of computerization, and computer-anxiety. The relationship between variables in the theoretical base were analyzed and the individual characteristics influencing each variable were also analyzed. This study was done in two steps. First, 344 nurses were given an initial questionnaire developed to evaluate the reliability of the items. Based on the results, a second revised questionnaire was administered to 88 nurses who had been working in the areas where HIS was applied. The results of the first and second steps of the study are as follows 1. The initial study was done with nurses who were trained on the computer system briefly before HIS was implemented. The individual characteristics influencing computer anxiety and expectation regarding computer system usage in that initial study included, length of career, type of degree or certification, previous experiences with a computer, training on a computer, desire for computer training, and level of acceptance of a computerized work environment. But in the second study with nurses working in areas of the hospital where HIS was introduced, the work site was the only influencing characteristics. There-fore, in applying a computer system, overcoming work-environment barriers will be more import-ant than any individual characteristics. 2. The computer anxiety of the nurses in both groups, before and after the computer system ap-plication, was below the average level but the expectation of the effects of computerization was above average. The nurses using the computer program showed an above average level of satis-faction with the computer system itself, and with its effect on their efficiency. Therefore, the ability of nurses operating HIS will be positively. predictive. 3. For the variables included in the theoretical framework of the study, all of the correlational coefficients were statistically significant in the analysis of variation correlation. Therefore, the theoretical base of the study, "expectation in con junction with computer anxiety" can be considered an model which can be evaluated. Accord-ing to our analysis, the higher the level of nurses' motivation to use the computer system and the lower the anxiety about computer usage, the higher the possibility of computer system acceptance by nurses. The results of this study showed that in applying a computer system in the hospital, the main characteristic influencing acceptance was where the individual worked rather than personal characteristics such as length of career, type of degree or certification, and previous experiences with a computer. Therefore, it is suggested that the first step in uncovering and eliminating hindrance factors in ap-plication of a computer system should be an analysis of working conditions in relation to the functional content of the computer system. The suitability of the theoretical model based on the hypothesis ap-plied in this study should be further tested.

Rogers 이론에 근거한 가족양상 사정지침개발 (Development of a Family Pattern Appraisal to Guide a Rogerian Nursing Practice)

  • We, clinical nurse specialists practising and guiding student practice in a Community health nursing clinic, wanted to develop a family pattern appraisal consistent with Rogers' conceptual system, the nursing model guiding our practice. We use Rogers' model because it is harmonious with the traditional Korean view of the one human, natural and cosmic world. The purpose of our research was to contribute to science - based nursing practice, not only, one helpful model, but also a model of how to use, in guiding practice, a conceptual system which reflects nurse practitioners' philosophy of nursing, is intellectually satisfying, and enriches meaning in daily nursing life. The research objectives were to review the literature on Rogers' model and analyse it according to Kim's five - level analytical framework, to explore Rogers' definition of family, to review appraisals based on Rogers' model, and to develop a family appraisal which is culturally appropriate for use in our community. This work including the use of the appraisal and its refinement with families in our practice which was done during 1994 and 1995, in Seoul, in the Capital of the Republic of Korea. At the highest level of analysis, Rogers conceptual system emphasizes acausality and multidimentional meaning ; the world view is characterized by process, movement and wholeness. The epistemology Is one of holism and the knowledge base includes all forms of experience, from sensory to mystical, objective, and subjective. At the metaparadigm level, nursing focuses on the unitary human being and the environment. At the level of nursing philosophy, the model identifies human being, nursing, nurse, and illness and health. At the paradigm level the model assumes the irriducibility of the human to parts, noncausality and continual change. Rogers' practice methodology consists of pattern manifestation appraisal and deliberative mutual patterning. Under-standing patterns and patterning of people is the key to helping them achieve their potential. At the theory level, the basic assumptions, key concepts, and homeodynamic principles were identified. Rogers states the family energy field is an undividable, four-dimensional negentropic energy field which is in a larger envircinmental field show-ing such characteristics as cannot be predicted by knowledge of individual family members. Based on the word of Rogers scholars, we chose Rogers' correlates of patterning to understand the family unit as a whole-frequency, rhythms, motion, time perception, sleeping-waking beyond waking, pragmatic -imaginative-visionary to develop the appraisal. We, also used some of Barrel's (1988) criteria including interpersonal network and professional health care access and use, and Cordon's (1982) criteria including self perception - self concept modified to fit the family. Our family Pattern appraisal included 1. Influencirg data, 2. Professional health care access and use, 3. Family self perception-self concept, 4. Family interpersonal network, 5. Sleep-wake-be-yond waking, 6. Pragmatic-imaginary-visionary, 7. Family frequency and rhythm, 8. Family motion, 9. Family time perception. The appraisal was used with four families and modified to eliminate overlap and to make it possible for the family member to express themselves more easily. We plan to gain more experience with the appraisal toward further development of the tool.

가정노인과 양로원노인의 체력, 자기효능, 일상생활활동능력 및 삶의 질에 관한 연구 (Comparisons of physical Fitness, Self Efficacy, Instrumental Activities of Daily Living, and Quality of Life between Institutionalized and Noninstitutionalized Elderly)

  • This study was done to investigate levels of physical fitness, self efficacy(SE), instrumental activities of daily living(IADL), and quality of life (QL), and their relationships among the elderly Data were collected from 47 noninstitutionalized elderly dwell-ing in their own homes and 43 institutionalized elderly living in homes for older people. The ages of the subjects were 65 years and over. The data were col lected from January 20th to February 20th, 1995. Physical fitness was measured with T.K. K dyna mometer, grip dynamometer, stop watch, and Purdue pegboard. Structured questionnaires developed by Kim by Lawton & Brody, and by Ro were adopted to measure SE, IADL, and QL, respectively. The data were analyzed using t-test, ANOVA, and Pearson correlation coefficients. The results were as follows : 1. The noninstitutionalized elderly had higher scores in such measures of physical fitness as body weight, skinfold thickness, flexibility and coordination as compared to the institutionalized elderly. 2. On the following measures of physical fitness, men showed higher levels than women ; in height when standing, height when sitting, body weight, and muscle strength. Women had more trunk flexibility than men. 3. Muscle strength was positively correlated with height when standing, height when sitting, and body weight. The following measures of physical fitness, muscle strength, flexibility and coordi nation, were negatively correlated with "up and go”. 4. The noninstitutionalized elderly had higher scores in SE(t=2.28, p<0.05), IADL(t=2.24, p<0.05), and QL(t=2.41, p<0.05) as compared to the institutionalized elderly. 5. SE was positively correlated with both IADL(r=0.41, p<0.001) and QL(r=0.54, p<0.001), and the latter two variables were a positively correlated (r=0.30, p<0.001). 6. SE was positively correlated with the physical fitness measures of height when standing, body weight, strength of leg muscles, strength of back muscles, grip strength, and level of touching the floor, but negatively correlated with "up and go”. 7. Positive correlations were revealed between IADL and the physical fitness measures of height, strength of leg muscles, level of trunk muscle extension, level of touching the floor, and coordination. The IADL was negatively correlated with “up and go”. 8. QL was positively correlated with body weight (r=0.28, p<0.01) and skinfold thickness (r=0.26, p<0.05). 9. Age was negatively correlated with the physical fitness measures of height when standing, height when sitting, “up and go”, strength of leg muscles, strength of back muscles, grip strength, level of trunk muscle extension, and coordination. 10. Age was also negatively correlated with SE (r=-0.24, p<0.05) and IADL(r=-0.22, p<0.05). The above results suggest that caring elderly in their own homes were more effective and that nursing interventions to enhance physical fitness, SE, IADL, and QL especially for the institutionalized elderly are stressed.

웰니스워드넷: 비정형데이터와 상황적 긍부정성에 기반하여 주관적 웰빙 상태를 무구속적으로 모니터링하기 위한 워드넷 개발 (WellnessWordNet: A Word Net for Unconstrained Subjective Well-Being Monitor ing Based on Unstructured Data and Contextual Polarity)

  • 주관적 웰빙 서비스(subjective well-being service)는 Wellness IT의 주요 서비스이며 개인의 주관적 웰빙 상태를 무구속적이고 비용 효율적으로 측정하는 방법이 중요하다. 이를 위해 감성어휘사전을 활용할 수 있으나 감성어만으로 주관적 웰빙 상태를 측정할 수는 없으며 웰니스 어휘 사전이 별도로 구축될 필요가 있다. 더욱이 기존의 감성어휘사전은 동일한 감정어에 대해 한가지만의 감성값을 제공함으로써 그 용어를 사용한 사람의 특징에 따라 감성값이 변경될 수 있다는 점을 간과하고 있다. 따라서 본 연구의 목적은 현존하는 감성어휘사전 중에서 표현력이 가장 뛰어난 SenticNet을 기반으로 하여 SenticNet에서 제공하는 정보를 통해 스트레스, 우울, 분노, 행복감 등 웰니스 상태를 추정한 결과를 추가한 WellnessWordNet 을 개발하는 것이다. 또한 실제 사람들을 대상으로 WellnessWordNet 에 근거한 웰니스 상태 추정 정확도를 검증해 보았다. 본 논문의 독창성은 WellnessWordNet 웰니스 상태 언어에 대한 값을 제공할 뿐더러, 성별이나 연령과 같은 사람의 특성에 따라 다른 감성값을 제공하는 최초의 감성어휘사전이라는 것이다.

중합광원과 레진 색상이 복합레진의 중합깊이에 미치는 영향 (EFFECT OF LIGHT SOURCE AND SHADE ON DEPTH OF CURE OF COMPOSITES)

  • Purpose of this research is estimating polymerization depth of different source of light. XL 3000 for halo-gen light, Apollo 95E for plasma arc light and Easy cure for LED light source were used in this study. Different shade (B1 & A3) resin composites (Esthet-X, Dentsply, U.S.A.) were used to measure depth of cure. 1, 2, and 3 mm thick samples were light cured for three seconds, six seconds or 10 seconds with Apollo 95E and they were light cured with XL-3000 and Easy cure for 10 seconds, 20 seconds, or 40 seconds. Vicker's hardness test carried out after store samples for 24 hours in distilled water. Results were as following. 1. Curing time increases from al1 source of lights, oui$.$ing depth increased(p<0.05). 2. Depth (that except 1mm group and 2mm group which lighten to halogen source of light) deepens in all groups, Vickers hardness decreased(p<0.05). 3. Vicker's hardness of A3 shade composite was lower in all depths more than B1 shade composites in group that do polymerization for 10 seconds and 20 seconds using halogen source of light(p<0.05), but group that do polymerization lot 40 seconds did not show difference(p>0.05). 4. Groups that do polymerization using Plasma arc and LED source of light did not show Vicker's hardness difference according to color at surface and 1mm depth(p>0.05), but showed difference according to color at 2mm and 3mm depth(p<0.05). The results showed that Apollo 95E need more polymerization times than manufacturer's recommendation (3 seconds), and Easy cure need polymerization time of XL-3000 at least.
