The Effects of Group Exercise, Manual Therapy and Home Exercise on Pain, Range of Motion and Function in Patient with Adhesive Capsulitis

  • Purpose: The purpose of this study was to determine the effects of group exercise, manual therapy, and a home exercise program on VAS (visual analogue scale), ROM (range of motion) of shoulder, and functional outcome in adhesive capsulitis. Methods: The study was conducted with 45 patients. All subjects were randomly assigned to 3 groups: group exercise (n=15), manual therapy (n=15), home exercise (n=15). The subjects performed an intervention program 3 times a week for 4 weeks a total of 12 times. Outcome measurements were VAS, ROM, and Constant-Murley score outcome measures. Measurements were taken four times, at baseline, 4 weeks later, 8 weeks later, and 12 weeks later. Results: VAS, ROM of the shoulder, and functional outcomes showed improvement in all groups at the final follow-up (p<0.05). VAS and ROM showed greater improvement with group exercise and manual therapy than home exercise, however, there were no significant differences between the two groups (p>0.05). Functional outcomes showed the greatest improvement in group exercise at the final follow-up. This improvement was greater than with manual therapy or home exercise (p<0.05). Conclusion: Group exercise and manual therapy were more effective for improving pain, ROM, and function than home exercise in adhesive capsulitis. In particular, group exercise is more effective in functional recovery than other physiotherapy interventions.

The Inter-Rater Reliability of Measurements of Active Craniocervical Range of Motion With Smartphone in Patients With Stroke (뇌졸중 환자에서 스마트폰 어플리케이션을 이용한 능동적 머리목관절 가동범위 측정에 관한 측정자간 신뢰도 연구)

  • Background: Numerous studies have used smartphone applications to measure the range of motion in different joints. In addition, studies measuring the active range of motion (AROM) of the craniocervical joint have revealed high reliability. However, the subjects in these studies were all healthy subjects. No study has yet been conducted to measure the inter-rater reliability for the AROM of the craniocervical joint in stroke patients. Objects: The purpose of this study was to investigate the inter-rater reliability of the AROM of the craniocervical joint using a smartphone. Methods: The participants included 21 subjects who had strokes (17 males and 4 females). Two raters evaluated six types of craniocervical AROM, including flexion, extension, lateral flexion to the hemiplegic side, lateral flexion to the non-hemiplegic side, rotation to the hemiplegic side, and rotation to the non-hemiplegic side, using a goniometer and a smartphone to investigate inter-rater reliability. The inter-rater reliability was analyzed by intraclass correlation coefficients (ICC). Results: The inter-rater reliability of the smartphone was good for extension, lateral flexion to the hemiplegic side, lateral flexion to the non-hemiplegic side, and rotation to the hemiplegic side [ICC(2,k)=.86~.88] and excellent for flexion [ICC(2,k)=.95]. The inter-rater reliability for rotation to the non-hemiplegic side was moderate [ICC(2,k)=.72]. Conclusion: These results suggest that the smartphone offers high inter-rater reliability for measurements of the craniocervical AROM in patients with stroke.

  • This study is approached the early intervention of children with disability in the aspect of family. The family of didabled children are identified as special individual with responsibilities and roles beyond those of other family. Physical Therapy need of the disabled child should be sought in the context of the family and socio-cultural environment. The process of early developmental facilitation is defined as a approach of the family. As family is the most influential factor in the child developmental facilitation, treatment and education. The family tend to seek complete information about disabilities, desire accurate, straight foreward about their child.

  • This study investigated effect to injure cerebrovascular accident patients who divided cerebral hemorrhage and cerebral infarction of sex the distinct on as a result of body composition. cerebral infarction patients of cerebrovascular accidents were observed the higher ratio than cerebrovascular hemorrhage patients. Experimental group were sixteen subjects who had received. we used assessment instruments such as in body to assess body composition. The result were as follows; 1. Change in body composition according to the come of onset, there were not differenced significantly between two groups. 2. Change in body composition according to the sex, the soft lean mass were differenced significantly(p<.05). but the other variations were not differenced significantly between two groups. 3. Change in body composition according to the region of paralysis, the soft lean mass were differenced significantly(p<.05). but the other variations were not differenced significantly between two groups.

Comparing the Muscle Strength of the Iliopsoas with the Muscle Activity of the Rectus Femoris according to Knee Flexion Angles in Supine and Sitting Positions (바로 누운 자세와 앉은 자세에서 무릎관절 굽힘 각도에 따른 엉덩허리근의 근력과 넙다리곧은근의 근활성도 비교)

  • Purpose : The muscle strength of iliopsoas (IL) was measured commonly in sitting position with hip and knee flexed 90°. However, there is no study to determine the muscle strength of IL in various test positions. Therefore, the purpose of this study was to compare the muscle strength of IL and muscle activity of rectus femoris (RF) according to test position and knee flexion angle. Methods : Twenty healthy subjects were participated for this study. The muscle strength of IL and muscle activity of RF were measured by hand-held dynamometer and surface electromyography during maximum voluntary isometric contraction (MVIC) of IL, respectively. The muscle strength of IL and muscle activity of RF was measured in 4 conditions as follows; 1) knee flexion angles 90 ° in supine, 2) 130 ° in supine position, 3) 90 ° in sitting, 4) 130 ° in sitting. Each condition were performed randomly by three repetitions. Results : The muscle strength of the IL was the main effect on the test position and knee flexion angle (p<.05), and the muscle activity of RF was the main effect only on the knee flexion angle (p<.05). There was also no interaction between the factors (p>.05). In supine position, the muscle strength of IL in knee flexion 130 ° was significantly less than that in knee flexion 90 ° (p<.0125). In knee flexion 90 °, the muscle strength of IL in supine position was significantly greater than that in sitting position (p<.0125). The muscle activity of RF in knee flexion 130 ° was significantly less than that in knee flexion 90 ° in supine and sitting positions (p<.0125). Conclusion : When the muscle strength of IL was measured in clinic and sports fields, the supine position with knee flexion 130 ° was recommended to prevent the muscle activation of RF and to maintain the trunk stability.

  • Interleukin-2 (IL-2), a glycoprotein mainly secreted by CD4+ T helper Iymphocytes, has been developed to use recombinant cytokine to augment the immune response against cancer since IL-2 not only stimulates T Iymphocytes but also enhances natural killer (NK) cell activity. In order to evaluate the immunological safety of recombinant mouse IL-2 (rmIL-2) in cancer therapy, renal cell carcinoma was established in the flank by s.c. injection of renca cell line. Tumor-bearing BALB/c mice were treated with I.p. injections with $2{\times}10^5$ Lu rmIL-2. Even though the tumor size was diminished, there were not significant recovery of body and relative lymphoid organ weights including thymic atrophy in rmIL-2 immunotherapy. Distribution ratios of T cell subsets in thymus were analysed using flow cytometry. Without regard to dosage of rmIL-2, the ratio of CD3+CD4-CD8- T cells was increased in accordance with survival of solid tumor but that of CD4+CD8+ T cells was decreased dramatically. Emergence of autoantibodies (ANA, anti-dsDNA, and anti-histone) in blood was measured after rmIL-2 treatment. The results showed that the levels of ANA and anti-dsDNA did not significantly changed, but the level of anti-histone was increased significantly owing to rmIL-2 therapy. These results indicate rmIL-2 immunotherapy is to induce the autoimmune potential, and the anti-histone measurement as a biomarker of autoimmunity is useful in cancer immunotherapy.

The Effects of Olfactory Stimulation on the Balance Ability of the Elderly (후각자극이 노인의 균형에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effect of olfactory stimulation on the balance ability of the elderly. Design: Randomized controlled trial. Methods: A total of 20 subjects were randomly assigned to the olfactory stimulation group (2 males and 8 females) and deception olfactory stimulation group (3 males and 7 females). Subjects were subjected to 30 second 3 times olfactory stimulation and deception olfactory stimulation, and the assessment measured pre and post postural distance and postural speed. Results: In the elderly, the olfactory stimulation group was significantly improved the postural distance and postural speed in the eyes closed state than the deception stimulation group (p<.05). Conclusion: As the information is provided through the olfactory stimulation, the postural distance and postural speed are significantly improved. therefore, it is expected that the olfactory stimulation in the elderly will be an effective intervention method to prevent fall.

The Effect of Treadmill Exercise on the Expression of IL-1β in the Spinal Cord in Osteoarthritis Rats (트레드밀 운동이 뼈관절염 흰쥐의 척수 내 IL-1β 발현에 미치는 영향)

  • PURPOSE: This study examined the effects of treadmill exercise of diverse intensities on the expression of IL-$1{\beta}$ (interleukine-$1{\beta}$) in the spinal cord in osteoarthritis rats. METHODS: The authors applied treadmill exercise of diverse intensity for 4 weeks to Sprague-Dawley rats to which intra-articular injection of monosodium iodoacetate(MIA, $3mg/50{\mu}l$, diluted in saline) was applied in the right knee joint to induce osteoarthritis. The four-week exercise was not applied to the control group(CG, n=15), while exercise of applicable intensity was applied to the low-intensity exercise group(LEG, n=15), moderate-intensity exercise group (MEG, n=15), and high-intensity exercise group(HEG, n=15) for four weeks. Observations were made of expression of IL-$1{\beta}$ in the spinal cord in osteoarthritis rats using western blot analysis. RESULTS: There were significant differences(p<.05) in the comparison of expression of IL-$1{\beta}$ in the spinal cord between the four groups involved. And the LEG and MEG had reduced expression of IL-$1{\beta}$ significantly than the CG(p<.05); in particular, the MEG showed the lowest expression. On the other hand, the HEG had more elevated expression of IL-$1{\beta}$ significantly than the CG(p<.05). CONCLUSION: As a result, factors that induce neuropathic pain such as IL-$1{\beta}$ are reduced; thus, the recovery of damaged neurons is improved and neuropathic pain is reduced. Further, when prescribing exercise to treat osteoarthritis patients, exercise of moderate intensity suitable for patients' physical conditions, rather than high intensity, maximizes the effects of this therapy.

The Effect of Methylprednisolone Pulse Therapy against Steroid Resistant Nephrotic Syndrome in Children (스테로이드 저항성 신증후군 환아에서의 Methylprednisolone 충격 요법의 치료 효과)

  • Purpose : The Childhood Steroid Resistant Nephrotic Syndrome (SRNS) has a poor prognosis and there has been no effective therapy against SRNS of children. In 1990, Mendoza have reported that methylprednisolone pulse therapy was effective against SRNS of children. But in 1992, Waldo have reported that methylprednisolone pulse therapy was not as effective as in the report of Mendoza. So, retrospectively, we have studied 20 korean children with SRNS to evaluate the effect of methylprednisolone pulse therapy, Methods : Mothylprednisolone pulse therapy were given to 20 korean children with SRNS who admitted to Seoul National University Hospital from 1990 to 1995 and follow up was done Results : 1) During methylprednisolone pulse therapy, remission of nephrotic syndrome was induced in 45% of patients. 2) during follow up after the end of methylprednisolone pulse therapy, remission of nephrotic syndrome was maintained in 45% of patients. 3) 25% of patients has progressed to chronic renal failure. Conclusion : We think that the methylprednisolone pulse therapy is a effective therapy against SRNS of children with the 45%, remission rate of of SRNS in Korean Children
