The Intervention and Outcome Measurement Tools of Children With Attention Deficit Hyperactivity Disorder: A Systematic Review (주의력결핍과잉행동장애 아동의 중재방법과 측정도구에 관한 체계적 고찰 : 국제 기능.장애.건강 분류 모델의 분류기준에 근거하여)

  • Objective : The purpose of this study was to provide for the intervention and outcome measurement tools of children with Attention Deficit Hyperactivity Disorder (ADHD) through ICF model. Methods : The systematic review methods were used. Papers published in the journal between January, 2000 and July, 2014 were searched through MEDLINE/PubMed, Sciencedirect, Ovid. The main terms searched were 'ADHD, Children, intervention, outcome measure', and 8 papers were analyzed. Results : 1. The subjects of ADHD were pure ADHD (75.8%), ADHD with dyslexia (9.1%), ADHD with conduct disorder (5.8%), ADHD with tic disorder (3.8%), ADHD with DCD (3.0%), ADHD with emotional disorder (2.5%). 2. The nonpharmacologic intervention of ADHD were functioning and disability (80%) and contextual factors (20%). Most frequently used intervention were body function and structure (60%). 3. The outcome measurement tools of ADHD were functioning and disability (80.5%) and contextual factors (19.5%). Most frequently used outcome measurement tools were body function and structure (70.8%). Conclusion : This study can provide information on the intervention and outcome measurement tools of ADHD.

The Preliminary Study for Development of Occupational Therapy Model Focused on Improving Living Functions within the Community Care System (커뮤니티 케어 제도 내 생활기능 향상 중심의 작업치료 모델 개발을 위한 기초 연구)

  • Objective : This study conducted a preliminary study to develop a occupational therapy model focused on improving living functions within the community care system. Methods : From June to July, 2018, the literature on community care was researched, focusing on cases of Japan's Management Tool for Daily Life Performance (MTDLP), Sweden, United Kingdom, Germany and domestic S Elderly Care Centers and I Health Centers. Based on this information, a group of experts developed a occupational therapy model within the community care system. Results : Assessment tools such as occupation-based health promotional table, interest checklist, occupational goals for improving living functions, sheet for evaluation of living functions, survey of daily life time (weekday and weekend), and sheet for transition of living functions were developed to conduct evaluation for occupational therapy. The improving living functions program, analysis of activities based on ICF model, lifestyle redesign program, cognitive exercise therapy, the Lee Silverman Voice Treatment (LSVT), hospice, and home modification were also organized interventions already in place by occupational therapists. Conclusion : This study showed specific measures and models for the implementation of occupational therapy within community care systems. Occupational therapy is positioned as a specialized area that is essential to the client, and we look forward to the use of this model.

A Study on Development of the Job Functional Assessment Scale for people with Disabilities (장애인 근로능력평가 척도 개발에 관한 연구 -국민연금장애연금수급자를 중심으로)

  • The purpose of this study was to develop the Job Functional Assessment Scale. This study was involved took third steps. First, 93 preliminary items were developed based on theoretical literature and International Classification of Functioning, Disability and Health (ICF) Model. To verify the content validity, 3 rehabilitation experts rated those items. Second, the scale consisting of 73 items was administered to sample of 880 people with disabilities. Finally, items of the scale were reduced to 58 items. Factor analysis showed evidence of construct validity of the scale. Internal consistency (Cronbach's ${\alpha}$) of total score showed .94 and Cronbach's ${\alpha}$ of 7 subfactor demonstrated .74~.90. Thus, Job Functional Assessment Scale demonstrated satisfactory convergent validity and discriminant validity. In addition, the practical use of the scale was discussed.

Early Detection of Intravenous Infiltration Using Multi-frequency Bioelectrical Impedance Parameters: Pilot Study

  • In this study, bioelectrical impedance analysis, which has been used to assess an alteration in intracellular fluid (ICF) of the body, was applied to detect intravenous infiltration. The experimental results are described as follows. Firstly, when infiltration occurred, the resistance gradually decreased with time and frequency i.e., the resistance decreased with increasing time, proportional to the amount of infiltrated intravenous (IV) solution. At each frequency, the resistance gradually decreased with time, indicating the IV solution (also blood) accumulated in the extracellular fluid (ECF) (including interstitial fluid). Secondly, the resistance ratio started to increase at infiltration, showing the highest value after 1.4 min of infiltration, and gradually decreased thereafter. Thirdly, the impedance ($Z_C$) of cell membrane decreased significantly (especially at 50 kHz) during infiltration and gradually decreased thereafter. Fourthly, Cole-Cole plot indicated that the positions of (R, $X_C$) shifted toward left owing to infiltration, reflecting the IV solution accumulated in the ECF. The resistance ($R_0$) at zero frequency decreased continuously over time, indicating that it is a vital impedance parameter capable of detecting early infiltration during IV infusion. Finally, the mechanism of the current flowing through the ECF, cell membrane, and ICF in the subcutaneous tissues was analyzed as a function of time before and after infiltration, using an equivalent circuit model of the human cell. In conclusion, it was confirmed that the infiltration could be detected early using these impedance parameters during the infusion of IV solution.

  • The purpose of this study is to investigate the nature of the experience of Emotion Focused Psychology Coaching, and what kind of changes happened for the participants through the experience. For this, in-depth interviews were conducted with four participants who had experienced five sessions of Emotion Focused Psychology Coaching. The interviews were analyzed using the phenomenological analysis method of Colaizzi. Emotion Focused Psychology Coaching is a new coaching psychology model that combines Greenberg's Emotion-Focused Therapy (EFT) and the ICF Core Competencies. The findings show that prior to the experience of Emotion Focused Psychology Coaching, the participants had avoided or suppressed their emotions, which in turn prevented them from exploring effective interpersonal relationships or alternatives in their actions. On the other hand, after they experienced Emotion Focused Psychology Coaching, their perception of emotions, emotion regulation, emotional expression, empathy for the other, etc. were developed in adaptive ways and the participants could present effective alternatives.

Multiple Relationships Between Impairment, Activity and Participation-based Clinical Outcome Measures in 200 Low Back Pain

  • Background: The International Classification of Functioning, Disability and Health (ICF) model, created by the World Health Organization, provides a theoretical framework that can be applied in the diagnosis and treatment of various disorders. Objects: Our research purposed to ascertain the relationship between structure/function, activity, and participation domain variables of the ICF and pain, pain-associated disability, activities of daily living (ADL), and quality of life in patients with chronic low back pain (LBP). Methods: Two-hundred patients with chronic LBP (mean age: 35.5 ± 8.8 years, females, n = 40) were recruited from hospital and community settings. We evaluated the body structure/function domain variable using the Numeric Pain Rating Scale (NPRS) and Roland-Morris disability (RMD) questionnaire. To evaluate the activity domain variable, we used the Oswestry Disability Index (ODI) and Quebec Back Pain Disability Scale (QBDS). For clinical outcome measures, we used Short-form 12 (SF-12). Pearson's correlation coefficient was used to ascertain the relationships among the variables (p < 0.05). All the participants with LBP received 30 minutes of conventional physical therapy 3 days/week for 4 weeks. Results: There were significant correlations between the body structure/function domain (NPRS and RMD questionnaire), activity domain (ODI and QBDS), and participation domain variables (SF-12), rending from pre-intervention (r = -0.723 to 0.783) and postintervention (r = -0.742 to 0.757, p < 0.05). Conclusion: The identification of a significant difference between these domain variables point to important relationships between pain, disability, performance of ADL, and quality in participants with LBP.

International and domestic research trends in longitudinal connectivity evaluations of aquatic ecosystems, and the applicability analysis of fish-based models (수생태계 종적 연결성 평가를 위한 국내외 연구 현황 및 어류기반 종적 연속성 평가모델 적용성 분석)

  • Recently, stream longitudinal connectivity has been a topic of investigation due to the frequent disconnections and the impact of aquatic ecosystems caused by the construction of small and medium-sized weirs and various artificial structures (fishways) directly influencing the stream ecosystem health. In this study, the international and domestic research trends of the longitudinal connectivity in aquatic ecosystems were evaluated and the applicability of fish-based longitudinal connectivity models used in developed countries was analyzed. For these purposes, we analyzed the current status of research on longitudinal connectivity and structural problems, fish monitoring methodology, monitoring approaches, longitudinal disconnectivity of fish movement, and biodiversity. In addition, we analyzed the current status and some technical limitations of physical habitat suitability evaluation, ecology-based water flow, eco-hydrological modeling for fish habitat connectivity, and the s/w program development for agent-based model. Numerous references, data, and various reports were examined to identify worldwide longitudinal stream connectivity evaluation models in European and non-European countries. The international approaches to longitudinal connectivity evaluations were categorized into five phases including 1) an approach integrating fish community and artificial structure surveys (two types input variables), 2) field monitoring approaches, 3) a stream geomorphological approach, 4) an artificial structure-based DB analytical approach, and 5) other approaches. the overall evaluation of survey methodologies and applicability for longitudinal stream connectivity suggested that the ICE model (Information sur la Continuite Ecologique) and the ICF model (Index de Connectivitat Fluvial), widely used in European countries, were appropriate for the application of longitudinal connectivity evaluations in Korean streams.

Estimation of Urea and Vitamin B12 Concentrations in the Body using Two Compartment Model (신 부전증 환자의 요소 및 비타민 B12 의 체내농도 추정)

  • Removal of urea and vitamin B12 was simulated using two pool models with closed loop dialysis, open loop dialysis, and hemodiafiltrations with predilution and postdilulion. It was found that urea removal was limited by dialyzer clearance, however B12 removal was limited both by the resistance between ECF and ICF and by dialyzer clearance. Open loop dialysis was better than closed loop dialysis in terms of dialysate requirement and removal efficiency. Residual renal function plays an important role in removing vitamin B12. Dialysis frequency more than twice/wide does not have great effect on removal efficiency, but has the effect of reducing the difference between maximum and minimum cancentrations during dialysis period.

The Effects of Activity and Family Support on the Participation Restriction of Chronic Stroke Patients (만성 뇌졸중 환자의 참여제한에 활동과 가족지지가 미치는 영향)

  • The purpose of this study was to identify the factors determining the participation restriction of chronic stroke patients based on international classification of functioning, disability, and health (ICF) model. Sixty-eight stroke patients participated. The participants were assessed participation restriction using the Korean version of London handicap scale (K-LHS), modified Barthel index (K-MBI) to measure activities of daily living, Berg balance scale (K-BBS) to assess balance, and the center for epidemiologic studies depression (K-CES-D) to gauge depression. Also, 3 minutes walking test (3MWT), gait velocity, asymmetric posture, and family support were assessed. A stepwise multiple regression analysis was used to explore the factors determining participation restriction. There were no significant different in the K-LHS and K-MBI results by gender (p>.05). Correlations between the K-LHS and K-MBI (r=-.656), K-BBS (r=-.543), K-CES-D (r=.266), 3MWT (r=-.363), gait velocity (r=.348), and family support (r=-.389) were significant (p<.05). Also, the K-MBI and family support were the factors that determined participation restriction (p<.05) and that 40.2% of the variation in the K-LHS can be explained. Therefore, it is suggested that evaluation and intervention of patient's activity level and extent of family support is necessary to reduce participation restriction of chronic stroke patients.

A Comparative Study of Oswestry Back Pain Disability Questionnaire Versus Computer Adaptive Testing for Measuring Back Pain

  • The aim of the present study was to compare measurement precisions of the Oswestry Back Pain Disability Questionnaire (ODQ) and a computer adaptive testing (CAT) method. The ODQ has been regarded as one of the most reliable condition-specific measure for back pain for decades. Cross-sectional study was carried out with two independent convenient samples from two out-patient rehabilitation clinics for back pain ($n_1=42$) and non-back pain group ($n_2=42$). Participants were asked to fill out the ODQ and CAT of International Classification of Functioning, Disability and Health-Activity Measure (ICF-AM). A series of Rasch analyses were performed to calculate person ability measures. The CAT measures had greater relative precision in discriminating the groups than did the ODQ measure in comparisons of the relative precision. The CAT measure appears to be more effective than did the ODQ measure in terms of measurement precision. By administering test items calibrated in a way, CAT measures using item response theory may promise a means with measurement precision as well as efficiency.
