  • In order to investigate experimentally the clinical effects of Soeuminsohapwon that was prescribed to cure cerebral hemorrhage, palpitation etc, the author tested various activities of extract from the Soeuminsohapwhangwon by the method prescribed in the experimental part. The results of the studies were summarized as follows: Suppressive action was not shown on the convulsion induced by strychnine, but significant effect was noted on the convulsion induced by picrotoxin and caffeine. In acetic acid method, analgesic effect was noted. A prolongation of anesthetic time by pentobarbital sodium and antipyretic effect was observed. Relaxing action was noted on the ileum of mice, also same effect was recognized on contraction of the ileum due to acetylcholine, barium chloride and histamine. The expansion of blood vessels by relaxation of smooth muscle and hypotensive action were noted. According to the above results, effects based on oriental medical references were approximate to the actual experimental results.

  • Two hypertensive men with DeBakey type III dissection were admitted due to acute onset of leg ischemia.One patient had ischemia of both legs,The other patient had ischemia of the right leg.Angiograms showed occlusion of aortic bifurcation in one patient and occlusion of right common iliac artery and right renal artery in the other patient.The first patient who had ischemia of both legs was relieved by axillo-bifemoral bypass operation and the second patient with right leg ischemia by femoro-femoral bypass.The dissection of the aorta was successfully managed by conservative measures including hypotensive medication.The bypass grafts was functioning well one year later.The aortic dissection should not be overlooked as an etiology of acute onset of ischemia of the lower extremities.

  • These studies were conducted to investigate the effects of Tongkwan-San water extract on analgesic, sedatative, anti-inflammatory, blood pressure and vasodilating actions, the relaxing action of isolated ileums and actions on the contact dermatitis induced by picryl chloride and on the leakage of the dye into the peritoneal cavity. The results of these studies were summarized as follows: The analgesic effect of Tongkwan-San was noted. The prolongation of anesthetic time of Tongkwan-San was recognized. Spontaneous motilities of isolated ileum of mice were strongly suppressed by Tongkwan-San. It inhibited the contractions of isolated ileum of mice induced by acetylcholine and barium chloride and the contraction of isolated ileum of guinea-pig induced by histamine. Inhibition of the contact dermatitis induced by picryl chloride was recognized. Anti-inflammatory effects in the paw edema induced by histamine and dextran were significantly shown. The leakage of dye into the peritoneal cavity in mice was significantly inhibited. Hypotensive and vasodilating action due to vascular smooth muscle relaxation were noted in rats and rabbits.

Influence of Sopung-Tang on the Blood Pressure Response of the Rat (소풍탕이 흰쥐의 혈압에 미치는 영향)

  • This study was attempted to examine the effect of Sopung-Tang(SPT) on the arterial blood pressure in rats and to elucidate its mechanism of action. SPT given into a femoral vein produced a dose-related vasopressor responses followed by vasodepressor responses. SPT-induced hypotension was significantly inhibited by pretreatment with atropine or propranolol while was not affected by chlorisondamine, Prazosin and cyproheptadine. SPT-evoked hypertensive activity was markedly blocked by pretreatment with prazosin but was not influenced by atropine, chlorisondamine, propranolol and cyproheptadine. Infusion of SPT(15.0 mg/kg/30min) did not affect norepinephrine-induced pressor responses. These experimental results suggest that SPT causes biphasically initial hypertensive activity followed by hypotensive activity, and that this hypertension may be due to the stimulation of peripheral adrenergic alpha-receptors and hypotension may be elicited through stimulation of peripheral cholinergic muscarinic receptors and adrenergic beta-receptors.

Influence of Nicorandil on Aortic Strip's Contractility and Blood Pressure of the Rat

  • The present study was conducted to investigate the effects of nicorandil on arterial blood pressure and vascular contractile responses in the normotensive anesthetized rats and to establish the mechanism of action. Nicorandil (30~300 ${\mu}g/kg$) given into a femoral vein of the normotensive anesthetized rat produced a dose-dependent depressor response. These nicorandil-induced hypotensive responses were not affected by pretreatment with atropine (3.0 mg/kg, i.v.) or propranolol (2.0 mg/kg, i.v.), while markedly inhibited in the presence of chlorisondamine (1.0 mg/kg, i.v.) or phentolamine (2.0 mg/kg, i.v.). Futhermore, after the pretreatment with 4-aminopyridine (1.0 mg/kg/30 min, i.v.) or glibenclamide (50.0 ${\mu}g/kg$/30min) into a femoral vein made a significant reproduction in pressor responses induced by intravenous norepinephrine. In he isolated rat aortic strips, both phenylephrine (10$^{-5}$ M)- and high potassium (5.6 ${\times}\;10^{-2}$ M)-inducedcontractile responses were dose-dependently depressed in the presence of nicorandil (25~100 ${\mu}M$). Collectively, these experimental results demonstrate that intravenous nicorandil causes a dose-dependent depressor action in the anesthetized rat at least partly through the blockade of vascular adrenergic ${\alpha}_1$-receptors, in addition to the well-known mechanism of potassium channel opening-induced vasorelaxation.

  • Analgesic, anticonvulsive, sedative and antipyretic actions of Yangkyuksanwhatang which is composed of nine crude drugs including Rehmanniae Radix, and its effects on isolated ileum and blood vessel were investigated. The results of the studies were summerized as follows; 1. Analgesic and antipyretic effects were observed. 2. Suppressive actions were not shown on the convulsion induced by strychnine, but significant effects were noted on the convulsion induced by picrotoxin. 3. Retroperitoneal Hematoma as a Serious Complication of Endovascular Aneurysmal Coiling

  • Retroperitoneal hematoma (RH) due to radiologic intervention for an intracranial lesion is relatively rare, difficult to diagnose, and can be lifethreatening. We report a case of RH that developed in a patient on anticoagulant therapy following endovascular coiling of a ruptured anterior communicating artery (AcoA) aneurysm. An 82-year-old man presented with a 12-day history of headache. Computed tomography (CT) on admission demonstrated slight subarachnoid hemorrhage, and left carotid angiography revealed an AcoA aneurysm. The next day, the aneurysm was occluded with coils via the femoral approach under general anesthesia. The patient received a bolus of 5,000 units of heparin immediately following the procedure, and an infusion rate of 10,000 units/day was initiated. The patient gradually became hypotensive 25 hours after coiling. Abdominal CT showed a huge, high-density soft-tissue mass filling the right side of the retroperitoneum space. The patient eventually died of multiple organ failure five days after coiling. RH after interventional radiology for neurological disease is relatively rare and can be difficult to diagnose if consciousness is disturbed. This case demonstrates the importance of performing routine physical examinations, sequentially measuring the hematocrit and closely monitoring systemic blood pressures following interventional radiologic procedures in patients with abnormal mental status.

Ginsenosides Inhibit Endothelium - dependent Contraction in the Spontaneously Hypertensive Rat Aorta isn vitro (선천성 고혈압 랫드에서 ginsenosides에 의한 내피의존성수축의 억제작용)

  • Our previous study showed that in vivo treatment of spontaneously hypertensive rats (SHR) with protopanaxatriol ginsenosides (PPT) reduces the blood pressure and inhibits the con- tractions induced by endothelium-derived contracting factor (prostaglandin endoperoxide ($PGH_2$) and superoxide anion) in aorta isolated from SHR. The aim of the present study is to examine whether PPT improves endothelial functions in the isolated thoracic aorta of SHR in vitro. Treatments of aortic rings with PPT, purified ginsenoside $Rg_1$ ($Rg_1$) or indomethacin normalized endotheliuln-dependent relaxation to acetylcholine, but not with protopanaxadiol ginsenosides (PPD) and purified ginsenoside Rb1 (Rb1). The effects of PPT were dose-dependent. PGH,- and oxygen free radical-inducted contractions in rat aorta without endothelium were inhibited by PPT or $Rg_1$, but not by PPD or $Rb_1$. Contractions induced by PGF2$\alpha$, U-46619, a stable thromboxane A2 agonist or KCI (60 mM) were not inhibited by PPT, $Rg_1$ or $Rb_1$. These findings demonstrate that PPT but not PPD scavenges the oxygen-derived free radicals and/or antagonize the effects of $PGH_2$ in the vascular smooth muscle and may explain the hypotensive effect of ginseng in the SHR.

  • The tissue extract (TTE) of a marine snail Telescopium telescopium, collected from the coastal regions of West Bengal, India, was extensively screened for pharmacological and biochemical properties. Telescopium telescopium (TTE) produced significant lysis of washed rat erythrocytes (both direct and indirect), produced haemorrhagic lesions in the skin and also released haemoglobin (in vitro tissue damage) from different tissue samples. TTE was found to produce pro-inflammatory effects when injected into the rat hind paw and also increased peritoneal vascular permeability. Furthermore, intravenous administration of TTE produced a decrease in blood pressure (hypotensive effect) in anaesthetized rats. The extract produced potent esterase activity, as was evident from the breakdown of FDA with subsequent release of fluorescein (in vitro). TTE also demonstrated prominent cholinesterase, phospholipase, phosphatase and protease activities.

  • The general pharmacological actions of recombinant human growth hormone (rHGH) were investigated. It had hypothermic action but neither sedative nor analgesic action. No pharmacological effects were observed in isolated guinea pig ileum and tracheal muscle and rat fundus and uterus. Slight hypotensive action with no effect on respiration was revealed at a dose of 20 IU/kg i.v. of rHGH in rabbits. The rHGH exhibited a weak inhibitory action of glucose tolerance in normal rats, significantly lowered the blood glucose contents in adrenalectomized rats 20 min after i.v. administration (80IU/kg), and produced a significant inhibitory effect on in vitro glycerol release in epinephrine-stimulated epididymal fat pad segments of rats.
