Radionuclide cardiac studies lend themselves exceptionally well to functional imaging. This is especially true for gated blood pool scan (GBP). Making functional images is also possible in radionuclide angiocardiography (RNAC). In this study we tried to validate the functional images obtained from RNAC by comparing it with GBP. Twenty three patients (16 patients with coronary artery diseases, 5 with hypertensive heart diseases, and 2 with nonspecific chest pains) underwent simultaneous RNAC and GBP at the same position (LAO $45^{\circ}$). From both studies, global ejection fraction, regional ejection fraction, phase image, amplitude image, stroke image, paradox image, maximum ejection and maximum filling rates were obtained. Global ejection fraction are almost same in both studies. Regional ejection fractions of apex and inferior portion of left ventricle calculated from RNAC are well correlated with those of GBP. Phase and paradox image, maximum ejection and maximum filling rates were obtained. Global ejection fraction are almost same in both studies. Regional ejection fractions of apex and inferior portion of left ventricle calculated from RNAC are well correlated with those of GBP. Phase and paradox images of RNAC are very similar to those of GBP. However, amplitude and stroke images are different. Regional ejection fractions of the left ventricular base, maximum ejection and maximum filling rates obtained from RNAC are significantly different from those of GBP. In conclusion, albeit all of functional images of RNAC is not same as GBP, regional walt motions and global left ventricular function are expected to be successfully analyzed by phase and paradox image and ejection fraction.
Hypertension excessively affects national health is the basic disease that causes death and disablement after the middle-aged. Therefore, The detection and control of hypertension is essential to health care. In general, It is known that the systolic blood pressure should be below 140mmHg, the diastolic blood pressure should be below 90mmHg. Presently, the treatment of hypertension is mostly taking anti-hypertension drug, diet and stability. but Taking anti-hypertension drug has such side effects as depression. and that, We were making a study of various treatments of hypertension. In a course of hypertension study, We applied aromatherapy to the control of hypertension. Aromatherapy is the practice of using of volate plant oils including essential oils, for psychological and physical well-being. Essential oils are drawn into the body by the sense smell not only does the aroma of the true plant stimulate the brain to the trigger a positive effect, but supply physical benefits also. In Europe and North America, The study and development of aromatherapy is lively in progress. In the control of hypertension, We applied Lavender, Majoram, Ylang-Ylang to forty-two hypertensive patients. After aromatherapy for controlling hypertension. We measured the changes of the systolic blood pressure & the diastolic blood pressure of patients by time. As the result of above study, We identified the significance of blood pressure down by aromatherapy.
lridology, a form of complementary and alternative medicine (CAM), is the diagnosis of medical conditions through noting irregularities of the pigmentation in the iris. lridological constitution has a strong familial aggregation and is implicated in heredity. Apolipoprotein E (apoE) gene polymorphism is one of the most well studied genetic markers of vascular disease. I investigated the relationship between iridological constitution and apoE polymorphism. I classified 87 hypertensive patients with family history of cerebral infarction and 79 controls according to iris constitution, and determined apoE genotype. Neurogenic type in hypertensives was 32.2% compared with 16.5% in controls (P<0.001). No differences in the apoE genotypes frequencies were observed in patients compared with those in controls ($x^2=0.726$, df-=2, P=0.696). However, in a population with ${\varepsilon}3/{\varepsilon}4$ genotype, the frequency of neurogenic constitution was significantly higher in hypertensives than in controls (60% vs. 0%) ($x^2=5.265$, df=l, P=0.022). These results could imply that apoE ${\varepsilon}3/{\varepsilon}4$ genotype and neurogenic iris constitution are risk factors for hypertension.
Objectives : The study was accomplished to find out how korean medical doctor take oriental medical service for hypertension patients in clinical practice Methods : Survey questions were developed based on consensus of 2 clinician, 2 oriental internal medicine specialist and 1 acupuncture specialist. The list of the Korean oriental medical doctors is provided by the Association of the Korean Oriental Medicine. The questionnaire were distributed via E-mail to 9,465 members of Korean oriental medical doctors from 19th August 2008 to 11th September 2008. The most common medical diagnosis method was pattern diagnosis(64.2%). Saam and five element acupuncture were as frequent as 32.3% of acupuncture prescription principle. The most common acupuncture points were region of shoulder and back acupoints inclued $GB_{21}$, $GV_{14}$(25%). The most common herbs prescription principle were 'constitution prescription'(35.4%), 'pattern prescription'(12.6%). The most common opinion for the revitalization of oriental medical service were 'medical insurance system of herbal medicine'(28.8%) and 'research of oriental medical treatments in hypertensive patients'(26.8%). Conclusions : This survey provides unique insight into the perception, treatment method of the Korea medical doctor at oriental medical service for hypertension patient. This study provides a wealth of information and a fresh raft of questions which will inform future research and policy-making.
Purpose: To investigate the effectiveness of aromatherapy on blood pressure, heart rate variability, aortic pulse wave velocity and the aortic augmentation index of essential hypertensive patients. Methods: Using a coin toss, 22 participants were assigned to the experimental group and 20 to the control. The experimental group was given a blend of oils of lemon (Citrus limonum), lavender (Lavandula angustifolia), and ylang ylang (Cananga odorata) which were prepared in the ratio of 2:2:1, respectively. The control group was given an artificial lemon fragrance of Limonene (35 cc) and Citral (15 cc) mixture. The experiment, inhalation, was conducted for 3 weeks (2 min per inhalation, 2 times per day) to both groups. Results: There was a noticeable difference in systolic blood pressure between the groups (p=.001), however the difference in diastolic blood pressure between the two groups was not significant. There was a notable difference in sympathetic nerve system activity of heart rate variability (p=.047). However, the differences in aortic pulse wave velocity or the aortic augmentation index were not significant. Conclusion: Aromatherapy is effective in lowering systolic blood pressure and sympathetic nerve system activity.
Radioimmunoassay for the measurement of plasma renin activity (PRA) was performed in 43 normal Koreans and 45 patients with essential hypertension. Plasma samples were drawn in supine position in the morning and after upright posture for 4 hours. Urinary sodium excretion rates were measured in the concurrent 24 hour urine samples, as an index of their sodium balance. The results were as follows: 1. There was an inverse correlation between 24hr sodium excretion and PRA. The normal values of PRA in supine position ranged from 1.0 to 7.0 ng/ml/hr. when 24 hour sodium excretion were between 50 to 150 mEq. PRA in elderly tended to be low. 2. When stimulated by 4 hour upright posture, PRA increased by 2.6 times from the baseline value. 3. Of the 45 patients with essential hypertension, PRA was low in 10 cases (22.2%), normal in 28 cases (62.2%), and high in 7 cases (15.6%). 4. In the normal and high renin groups, who tended to be younger in ages, mean diastolic blood pressure and BUN were higher than in low renin group. Though hypertensive retinopathy and left ventricular hypertrophy in ECG were more prevalent in the former, no significant differences were noted as in the case of serum cholesterol. 5. There were 8 cases of cardiovascular complications (7 with cerebral vascular accident, 1 with myocardial infarction); 3 in low renin group (30%), 2 in normal renin (7.1%) and 3 in high renin group (42.9%). This figure indicated higher rate of cardiovascular complications in high renin groups, and lower rate in normal renin group. But the incidence of the complication was not significantly low in low renin group.
This study was to examine the effect of the Self-efficacy Promotion Exercise Program to the elderly hypertensive with Stage 1. This research was the quasi-experimental study. The Self-efficacy Promotion Exercise Program combined to Aerobic exercise which is incrementally at a level of 40~60% HRmax and the source of the Self-efficacy Promotion Exercise. This research was carried out to experimental(33 patients) and control(32 patients), 60 minutes per day, 3 times a week for 12 weeks. The Self-efficacy Promotion Program was effect on enhancing the general(p<.001) and exercise self-efficacy(p<.001), the self-care behaviors(p<.001) and decreasing the systolic blood pressure(p<.001) to the elderly hypertensives. The 12 weeks Self-efficacy Promotion Exercise Program will be considering to useful for various care settings to the elderly hypertensives.
Purpose: The purpose of this study was to evaluate the effect of aerobic exercise on serum lipids, body weight, and body mass index in adults with hypertension using a systematic review and meta-analysis. Methods: Six electronic databases (PubMed, EMBASE, Co-chrane library, CINAHL, PsycINFO, SPORTDiscus) and five domestic databases were searched for randomized controlled trials studies. We used random effect models to derive weighted mean differences (WMD) and their 95% confidence intervals (CI) of aerobic exercise on serum lipids, body weight, and body mass index. Results: Nineteen studies met our inclusion criteria. In comparison with the control group, triglyceride level (TG) (-13.49 mg/dL, 95% CI: -26.74 to -0.24) and body weight (-0.62 kg, 95% CI: -1.08 to -0.16) difference between follow-up and baseline was significantly reduced in the aerobic exercise group. However, no difference was found in total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C) and body mass index (BMI). Conclusion: Aerobic exercise is effective in reducing the TG level and body weight in adults with hypertension. In order to improve levels of serum TC, LDL-C and HDL-C of hypertensive patients, it's needed to combine other intervention methods such as diet therapy.
Objectives: The purpose of this study is to identify the characteristics between therapy continuity group and therapy discontinuity group and to develop management program for Korean patients with hypertension and diabetes. Methods: The subject of the study were 109 therapy continuity and 66 therapy discontinuity of Korea hypertension diabetes Daegu initiative. The data collection was performed from December 5 to December 30, 2008. Analysis of data was done by using descriptive statistics, chi-square test, t-test and ANCOVA with SPSS program. Results: 1) The groups were significantly correlated with such variables systolic BP(F=4.518, p=0.035) and diastolic BP(F=17.793, p=0.000). 2) The groups with hypertensive were significantly correlated with such variables perceived susceptibility of disease($\chi^2$=25.053, p=0.000), perceived barrier of health behavior($\chi^2$=12.584, p=0.006), drinking($\chi^2$=27.545, p=0.000), diet($\chi^2$=8.645, p=0.013), regular taking medicine($\chi^2$=92.415, p=0.000) and regular measurement of BP($\chi^2$=6.045, p=0.049). 3) The groups with diabetic were significantly correlated with such variables perceived seriousness of disease($\chi^2$=6.128, p=0.047), perceived susceptibility of disease($\chi^2$=8.079, p=0.018), health knowledge and attitude(F=8.418, p=0.006), drinking($\chi^2$=6.276, p=0.043), diet($\chi^2$=7.275, p=0.026), regular taking medicine($\chi^2$=33.083, p=0.000) and regular measurement of glucose($\chi^2$=7.233, p=0.027). Conclusion: The above findings indicate that it is necessary to develop and apply special management programs according to the therapy discontinuity group.
In diabetes mellitus, renal disease is a common complication, characterized by increased urinary albumin excretion and reduced eGFR. According to KDIGO CKD stage classification, Korean characteristics were analyzed according to urinary albumin and eGFR using the National Health and Nutrition Examination Survey VI raw data. According to KDIGO classification, diabetic patients were classified as Low risk 72.0%, Moderate risk 19.3%, High risk 5.6% and Very high risk 3.0%. Low risk decreased from 74.7% to 52.2%, and moderate to very high risk increased from 25.4% to 47.8% as the duration of diabetes mellitus was prolonged. The risk factors were CKD stage 1 (HR 2.064) to stage 4 (HR 11.049), the highest risk of hypertension. The incidence of renal disease was elevated according to duration of hypertension and HR 0.42 of kidney disease was decreased in the group maintaining proper blood pressure. In the hypertensive patients, the group administered with target blood pressure had a reduction of the kidney disease by 42% than the group with the hypertension. Therefore, controlling and managing hypertension to target blood pressure is important for the prevention of kidney disease.
