소평탕(消平湯)이 RIN-m5F 세포에서 인슐린 분비 및 유전자 발현에 미치는 영향 (Effect of Sopyung-tang Extract on Insulin Secretion and Gene Expression in RIN-m5F Cells)

  • Background : At high glucose levels in $\beta$-cells, cell viability and insulin secretion are decreased by glucotoxicity. Sopyung-tang(SPT) had an effect on blood glucose level decrease and antioxidant enzyme activities in streptozotocin-induced diabetic rats. Objectives : This study performed a series of experiment to verify the effects of SPT extract on the cell viability, antioxidant enzyme activities, insulin secretion and insulin mRNA expression at hyperglycemic states of RIN-m5F. Methods : After treatment at various concentrations of SPT added to the RIN-m5F cells, cell viability by MTT assay, free radical-scavenging activity, SOD activity and insulin secretion were measured. Additionally, insulin-related gene expression was measured using real-time RT-PCR. Results : Compared to the control group, SPT extract showed considerable effects on RIN-m5F cell viability, DPPH radical-scavenging activity, superoxide dismutase (SOD) activity, insulin secretion and insulin-related gene expression. Conclusions : This study showed that SPT extract has an effect on $\beta$-cell cell viability, insulin secretion and insulin-related gene expression. Thus, SPT extract may be used for treatment of diabetes and its complications. Further mechanism studies of SPT seem to be necessary on the glucotoxicity and oxidative stress.

Oxidative and Anti-oxidative Status in Blood of Streptozotocin-induced Diabetic Piglets

  • Eight LW${\times}$D crossbred, castrated weanling piglets were used to examine the effect of hyperglycemia by streptozotocin (STZ)-injection on oxidative and anti-oxidative status in circulating fluid. Every two of the eight piglets were intravenously administrated STZ at a dose of 0 (control), 100, 125 or 150 mg/kg BW, respectively, and on 15th day after the STZ-injection, some markers of the oxidative stress in circulating fluid were measured to evaluate oxidative and anti-oxidative status in the piglets. First, piglets with hyperglycemia were selected from the STZ-injected piglets as measured by the levels of fasting plasma glucose (FPG) during 2 weeks after the STZ-injection. Additionally, data obtained from the intravenous glucose tolerance test (IVGTT) on 14th day were analyzed. Secondly, the data obtained in this experiment were divided into the control group and the hyperglycemic (STZ) group, and compared. The FPG level or area under curve (AUC) for plasma glucose during the IVGTT in the STZ-induced diabetic piglets was slightly significantly (FPG, p = 0.070; AUC, p = 0.072) higher compared with the control. On the other hand, the plasma level of lipid peroxidation in the STZ-induced diabetic piglets was significantly (p<0.05) higher compared with the control. These results raise the possibility that STZ-induced diabetic piglets produced in this study can be used as a diabetic animal model to research the pathogenic mechanisms or therapy of complications in diabetic mellitus.

Ob/ob mouse에서 오정환(五精丸)이 혈당, 고지혈증, Polyol Pathway 및 항산화작용에 미치는 영향 (Effects of Ojung-hwan on Blood Glucose, Hyperlipidemia, Polyol Pathway and Antioxidative Mechanism in Ob/ob Mouse)

  • Objectives : Diabetes is a disease in which the body does not produce or properly use insulin. Etiological studies of diabetes and its complications have shown that oxidative stress might play a major role. Therefore, many methods have been tried to regulate free oxygen radicals for treating diabetes and its complications. Ojung-hwan, composed of five crude herbs, has been considered effective for treating symptoms of aging. In male ob/ob mouse of severe obesity, hyperinsulinemia and hyperlipidemia, which are features of NIDDM, the hyperglycemic activities and mechanisms of Ojung-hwan were examined. Methods : Mice were grouped and treated for 5 weeks as follows. Both the lean (C57/BL6J black mice) and diabetic (ob/ob mice) control groups received standard chow. The experimental groups were fed a diet of chow supplemented with 30 and 90 mg Ojung-hwan per 1 kg of body weight for 14 days. The effects of Ojung-hwan extract on the ob/ob mice were observed by measuring the serum levels of glucose, insulin, lipid components, and the kidney levels of superoxide anion radical (${\cdot}\;O{_2}{^-}$), MDA+HAE, GSH/GSSG ratio, and also the enzyme activities involved in polyol pathway. Results : Ojung-hwan lowered the levels of serum glucose and insulin in a dose-dependent manner. Total cholesterol, triglyceride and free fatty acid levels decreased, while the HDL-cholesterol level increased, in Ojung-hwan treated groups. Renal aldose reductase and sorbitol dehydrogenase activities increased in the ob/ob mice, whereas they were inhibited in the Ojung-hwan treated groups. Ojung-hwan inhibited the generation of ${\cdot}\;O{_2}{^-}$ in the kidney. Finally, MDA+HAE levels increased and GSH/GSSG ratio decreased in the ob/ob mice, whereas they improved in the Ojung-hwan treated groups. Conclusions : Ojung-hwan showed antidiabetic and antihyperlipidemic activities by regulating theactivities of polyol pathway enzymes, scavenging reactive oxygen species and reducing the MDA+HAE levels in the ob/ob mice.

In vivo Investigation of Anti-diabetic Properties of Ripe Onion Juice in Normal and Streptozotocin-induced Diabetic Rats

  • The acute and subacute hypoglycemic and antihyperglycemic effects of drinkable ripe onion juice (Commercial product name is "Black Onion Extract") were investigated in normal and streptozotocin-induced diabetic rats. For tests of acute and subacute hypoglycemic effects, ripe onion juice (5 and 15 mL/kg b.w.) was administered by oral gavage to normal Sprague Dawley rats and measurements of fasting glucose levels and oral glucose tolerance tests were performed. Tolbutamide was used as a reference drug at a single oral dose of 250 mg/kg b.w. To test anti-hyperglycemic activity, the ripe onion juice was administered to streptozotocin-induced diabetic rats by oral gavage at single dose of 15 mL/kg b.w. per day for 7 consecutive days. Oral administration of the ripe onion juice at either dosed level of 5 or 15 mL/kg b.w. showed no remarkable acute hypoglycemic effect in normal rats. The two dosed levels caused a relatively small reduction, only 18% and 12% (5 and 15 mL/kg b.w., respectively) decrease in glucose levels at 2 h after glucose loading in normal rats. However, at 3 h after glucose loading, blood glucose levels in the ripe onion juice-dosed rats were decreased to the corresponding blood glucose level in tolbutamide-dosed rats. Although showing weak hypoglycemic potential compared to that of tolbutamide, oral administration of ripe onion juice (15 mL/kg b.w.) for a short period (8 days) resulted in a slight reduction in the blood glucose levels that had elevated in Streptozotocin-induced diabetic rats. In conclusion, these results suggest that the commercial product "Black Onion Extract" may possess antihyperglycemic potential in diabetes.

Altered Gene Expression Profiles in the Lungs of Streptozotocin-induced Diabetic Mice

  • Diabetes mellitus is a common heterogeneous metabolic disorder, characterized by deposition of extracellular matrix, oxidative stress, and vascular dysfunction, thereby leading to gradual loss of function in multiple organs. However, little attention has been paid to gene expression changes in the lung under hyperglycemic conditions. In this study, we found that diabetes inuced histological changes in the lung of streptozotocin-induced diabetic mice. Global gene expression profiling revealed a set of genes that are up- and down-regulated in the lung of diabetic mice. Among these, expression of Amigo2, Adrb2, and Zbtb16 were confirmed at the transcript level to correlate significantly with hyperglycemia in the lung. We further evaluated the effect of human umbilical cord-derived perivascular stem cells (PVCs) on these gene expression in the lung of diabetic mice. Our results show that administration of PVC-conditioned medium significantly suppressed Amig2, Adrb2, and Zbtb16 upregulation in these mice, suggesting that these genes may be useful indicators of lung injury during hyperglycemia. Furthermore, PVCs offer a promising alternative cell therapy for treating diabetic complications via regulation of gene expression.

Transient Neonatal Diabetes Mellitus Managed with Continuous Subcutaneous Insulin Infusion (CSII) and Continuous Glucose Monitoring

  • Neonatal diabetes mellitus can be categorized as transient, permanent, or syndromic, and approximately half of the cases are transient. We present a case involving a term newborn who showed overt progression of transient neonatal diabetes mellitus, with complete remission within 6 months. On the second day of life, the patient presented with tachypnea, hyperglycemia, and decreased serum levels of C-peptide and insulin. Continuous subcutaneous infusion of insulin and continuous glucose monitoring were well tolerated. The patient showed a normal growth pattern, with no hyperglycemic or hypoglycemic episodes at 6 months of age. As it is rare and often asymptomatic, hyperglycemia may be attributed to various factors, including intrauterine environment, perinatal stress, and diverse genetic background. Therefore, consistent blood glucose monitoring and prompt early insulin therapy are crucial for any term newborns with persistent hyperglycemia, to prevent further diabetic complications. Moreover, continuous subcutaneous insulin infusion and the utilization of continuous glucose monitoring devices are the most effective and practical management strategies.

Mesangial 세포에서 고포도당에 의해 유도되는 insulin-like growth factor 분비 촉진작용에 대한 ginsenosides의 차단 효과 (Ginsenosides Protect the High Glucose-induced Stimulation of IGFs in Mesangial Cells)

  • 인삼은 고전적으로 항당뇨 효과가 있는 것으로 보고되고 있다. Insulin-like growth factor (IGF) 역시 당뇨병성 신증의 발병 초기에 중요한 역할을 하는 것으로 알려져 있다. 이에 본 연구에서는 mesangial 세포에서 고포도당에 의한 IGF 분비에 대한 ginsenoside의 차단 효과 및 이와 관련된 신호전달계를 알아보았다. 결과는 다음과 같다. 고포도당에 의해 증가 되었던 IGF-I 및 IGF-II 분비 촉진 작용은 GTS, PD 및 PT 처리 시 차단되었으며, 세포 성장 촉진작용에서도 같은 효과를 볼 수 있었다. 아울러 고포도당에 의한 산화성 스트레스 종가, GSH 감소, AA 방출 증가 작용 및 $PGE_2$ 합성 증가 작용은 GTS 처리시 현저하게 차단되었으며 PD 및 PT 처리 시 역시 억제 되는 것으로 나타났다. 이상의 결과를 볼 때 mesangial 세포에서 ginsenoside는 산화성 스트레스 및 arachidonic acid 활성 경로를 억제하여 고포도당에 의한 IGFs 분비 작용을 차단하는 것으로 나타났다.

울산 지역 당뇨 질환 노인의 항산화 영양소 섭취실태 및 혈액 항산화 영양상태 (Dietary Antioxidant Vitamins Intakes and Plasma Antioxidant Levels in Korean Elderly with Diabetes Living in Ulsan)

  • Diabetic mellitus in an older population is associated with increased basal oxidative stress and free radical accentuated by hyperglycemic challenge. Enhanced free radical in diabetic elderly can cause the oxidative damage and such damage can be protected by antioxidant defense system. It is believed that vitamin C, A and E are the most abundant and effective antioxidants in human plasma. The purpose of this study was to determine the antioxidant status in Korean diabetic elderly using the case-control study. The antioxidant status was examined by determining plasma levels of antioxidant vitamins (vitamin C, A, E, ${\beta}$-carotene), total antioxidant status (TAS) and thiobarbituric acid reactive substance (TBARS) and intakes of vitamin C, A, ${\beta}$-carotene and retiol. Fasting glucose and HbA1c levels and serum lipid profiles (triglyceride (TG), total cholesterol, HDL-cholesterol and LDL-cholesterol) were also determined. Diabetic subjects were 122 elderly persons over 60 years old, visiting public health center, and control subjects were 96 healthy elderly persons living in Ulsan, Korea and they were matched by age, gender, smoking and drinking status. The diabetic and control subjects were divided into sub-groups according to the status of using diet therapy and vitamin supplement. The subjects were interviewed to collect data on their general characteristics, disease history, vitamin supplement, diet therapy and health-related habits by questionnaires. Their dietary intakes were obtained by means of semi-quantitative food frequency questionnaires (SQFFQ). Fasting plasma glucose and HbA1c levels were significantly higher in diabetes than in control subjects, and plasma total cholesterol level of diabetes was not significantly different from that of control subjects. However serum HDL cholesterol level of diabetes was significantly lower and serum TG level of diabetes was significantly higher than those of control group. The average vitamin A and ${\beta}$-carotene intakes of diabetes were significantly higher than those of control subjects. There was no significant difference in plasma vitamin C, ${\beta}$-carotene, and TBARS levels between two groups, but plasma vitamin A, E and TAS levels were significantly higher in diabetes than those in control group. Plasma vitamin A and TAS levels of diabetic subjects using diet therapy were higher than those of control using diet therapy, and plasma vitamin E, ${\beta}$-carotene and TAS levels of diabetic subjects using vitamin supplements were significantly higher than those of controls using vitamin supplements. These results suggested that diabetic mellitus could enhance antioxidant defences against reactive oxygen species and interest in healthy eating such as consumption of more antioxidant nutrients.

PC12 신경세포에서 고당 및 과산화수소로 유도된 산화적 스트레스에 대한 곰취 추출물의 효과 (Effect of gomchwi (Ligularia fischeri) extract against high glucose- and H2O2-induced oxidative stress in PC12 cells)

  • 본 연구에서는 곰취(Ligularia fischeri)의 in vitro 혈당 억제 가능성과 고당으로 인한 산화적 스트레스에 대한 신경세포 보호효과 및 대표적 생리활성물질을 분석하였다. 곰취 아세트산에틸 분획물은 다른 분획물들보다 뛰어난 총 페놀 함량(223.33 mg GAE/g)을 나타내었으며, 또한 인체에서 당을 흡수하기 위해 필요한 효소인 알파글루코시데이스 억제효과에 의해 당뇨에 의한 고혈당을 줄여줄 수 있을 것으로 기대된다. 유의적 ABTS 라디칼 소거활성 및 말론다이알데하이드(MDA) 생성 억제효과를 갖는 곰취 아세트산에틸 분획물은 과산화수소 및 고당으로 유발시킨 산화적 스트레스 감소 및 이로 인한 신경세포 보호효과를 나타내었다. 곰취 아세트산에틸 분획물의 주요 생리활성 물질을 확인하기 위해 HPLC분석을 실시한 결과, 대표적인 페놀성 화합물은 클로로겐산의 이성질체인 3,5-DCQA가 존재하는 것으로 확인되었다. 본 연구 결과를 바탕으로 고려할 때, 곰취는 고당으로 유도되는 산화적 스트레스로부터 신경퇴행성 질환 예방 소재로서의 활용가능성이 있을 것으로 추정된다.
