Fenofibrate Reduces Age-related Hypercholesterolemia in Normal Rats on a Standard Diet

  • Plasma cholesterol is increased in normal aging in both rodents and humans. This is associated with reduced elimination of cholesterol and decreased receptor-mediated clearance of plasma low-density lipoprotein (LDL) cholesterol. The aims of this study were: (1) to determine age-related changes in plasma lipid profiles, and (2) to determine the effect of fenofibrate, an activator of peroxisome proliferator activated receptor alpha (PPAR $\alpha$), on plasma lipid profiles in normal rats on a standard diet. Male Sprague-Dawley (SD) rats (n=15) were fed standard chow and water from 10 to 25 weeks of age. During that period, we measured daily food intake, body weight, fasting and random blood glucose levels, plasma total cholesterol (TC), triglycerides (TG), and free fatty acid (FFA) levels. At 20 weeks of age, all rats were randomly divided into two groups: a fenofibrate group (in which rats were gavaged with 300 mg/kg/day of fenofibrate) and a control group (gavaged with water). Fenofibrate treatment lasted 5 weeks. There were no significant changes in daily food intake, blood glucose, and plasma TG level with age. Body weight, plasma TC, and FFA levels were significantly increased with age. Fenofibrate significantly decreased plasma concentrations of TC and FFA, which had been increased with age. However, fenofibrate did not influence the plasma concentration of TG, which had not increased with age. These results suggest that fenofibrate might have a novel role in preventing age-related hypercholesterolemia in SD rats on a normal diet.

단백질 종류와 급여수준이 흰쥐의 혈중 지질함량에 미치는 영향 (Effect of Protein in Diet-induced Hypercholesterolemia Rats)

  • 단백질의 종류 및 급여수준이 고콜레스테롤혈증 흰쥐의 체내 지질대사에 미치는 영향을 구명학자 1% 콜레스테롤과 0.25%의 콜산나트륨을 첨가하여 고콜레스테롤 혈증을 유도토록 조제한 실험식이에 대두단백질과 카제인의 공급수준을 달리하여 급여하고 혈청내 지질대사를 관찰하였다. 혈중 총지질 함량은 콜레스테롤 투여시 정상군에 비해 유의적으로 증가되었으며 단백질 급여로 증가가 억제되는 것으로 나타났으며 대두단백질이 더 유의적으로 억제되었다. 트리글리세라이드는 콜레스테롤 투여군이 정상군에 비해 유의적으로 증가되었으며 단백질 급여로 증가가 억제되었으며, 단백질의 종류에 따른 차이는 관찰되지 않았고 인지질 함량은 콜레스테롤 투여군이 정상군에 비해 유의적으로 감소되었다. 총 콜레스테롤 함량과 유리 콜레스테롤 함량은 정상군에 비해 골레스테롤 투여군 모두에서 유의적으로 증가되었으며, 카제인 급여군에 비해 대두단백 급여시 유의적으로 증가가 억제되는 것으로 나타났다. VLDL- 및 LDL-콜레스테롤 함량은 정상군에 비해 콜레스테롤 투여시 유의적으로 증가하였으며, 단백질 급여 수준이 증가할수록 그 증가가 억제되는 것으로 나타난 반면, HDL-콜레스테롤 함량은 콜레스테롤 투여로 인해 유의적으로 감소되었으며 대두단백질이 카제인에 비해 감소 정도가 유의적으로 억제되었다. 이상의 결과에서 단백질의 종류와 급원이 지질함량에 영향을 미치므로써 고콜레스테롤 혈증을 예방·치료하기 위하여 단백 급여, 특히 대두단백 급여가 중요한 요인으로 작용할 것을 사료된다.

중년기 성인의 당뇨병 유병형태에 대한 생리적 지표 및 생활습관 비교 (Comparison of Biological Markers and Lifestyle Factors on the Presence of Diabetes Mellitus in Middle-aged adults)

  • 본 연구는 중년기 성인의 당뇨병 유병형태에 대한 생리적 지표 및 생활습관 요인을 비교, 조사하기 위해 시행되었다. 대상자는 2010-2012년 국민건강영양조사 자료를 이용한 2차 자료 분석이며, 40-64세의 중년기 성인 5,363명을 표본으로 하였다. 자료는 기술적 통계, 교차분석, ANCOVA, 로지스틱 회귀분석으로 분석하였다. 정상군, 전당뇨병군, 당뇨병군을 비교한 결과는 다음과 같다. 체질량지수는 정상군보다 전당뇨병군과 당뇨병군에서 유의하게 높았다. 고혈압, 복부비만여부, 고콜레스테롤혈증여부, 고중성지방혈증여부, 흡연, 음주, 탄수화물 섭취량은 세 군 간에 유의한 차이가 있었다. 생리적 지표 및 생활습관 요인 중 고혈압(OR:1.64, CI 1.35-2.01), 고콜레스테롤혈증(OR:1.36, CI 1.03-1.79), 고중성지방혈증(OR:1.71, CI 1.34-2.18), 복부비만(남;OR:2.31, CI 1.75-3.05, 여;OR:2.20, CI 1.67-2.91), 체질량지수(OR:1.13, CI 1.10-1.17), 음주(OR:1.44, 1.16-1.79)가 당뇨병전단계에 영향을 주는 요인으로 나타났다. 당뇨병에 영향을 주는 요인은 고혈압(OR:2.27, CI 1.68-3.06), 고콜레스테롤혈증(OR:2.31, CI 1.67-3.20), 고중성지방혈증(OR:1.95, CI 1.39-2.73), 복부비만(남;OR:2.31, CI 1.60-3.32, 여;OR:3.26, CI 2.24-4.73), 체질량지수(OR:1.12, CI 1.07-1.18), 흡연(OR:1.59, CI 1.18-2.13)으로 나타났다. 본 연구 결과를 바탕으로 중년기 성인에서 고혈압, 비만, 이상지질혈증, 음주, 흡연 등의 생활습관을 개선할 수 있는 효과적인 당뇨병 예방을 위한 중재 프로그램의 개발을 제언한다.

케일녹즙이 고콜레스테롤혈증 성인남자의 혈청 지질수준 및 인지질 지방산 조성에 미치는 영향 (Effect of Kale Juice on Serum Lipid Levels & Phospholipid Fatty Acid Composition in Hypercholesterolemic Men)

  • 고콜레스테롤혈증(총 콜레스테롤 농도 200 mg/dl 이상)인 성인 남자를 대상으로 12주간 케일녹즙 중재실험을 실시하여 실험 전과 후의 혈청 지질농도와 혈청 인지질 지방산 조성의 변화를 살펴보았다. 혈청 총 콜레스테롤 및 중성지방 농도는 녹즙 섭취 전과 후에 유의한 차이를 보이지 않았으나 HDL-콜레스테롤 농도는 녹즙 섭취 후 유의하게 증가하였고, LDL-콜레스테롤은 유의하게 감소하였으며 atherogenic index 역시 녹즙 섭취 후 유의하게 감소하였다. 대상자들은 녹즙 섭취 전 후로 BMI, 허리둘레, 혈압 등의 신체계측치와 흡연, 음주 등의 생활습관 및 영양소 섭취패턴에 유의한 변화 없이 12주 중재실험기간 동안 평상시 생활패턴을 유지하였다. 실험에 사용된 녹즙에 섬유소가 거의 함유되어있지 않으므로 혈청지질 개선효과는 녹즙에 풍부한 항산화 영양소에 의한 효과로 사료된다. 녹즙섭취 후에 유의한 변화를 보인 혈청 인지질 지방산 조성 중에서 12:0, 14:0, $18:1{\omega}9$, $18:3{\omega}6$ 조성 및 총 단일불포화지방산 조성은 증가하고 $22:4{\omega}6$ 조성은 감소하였다. 특히 올레산($18:1{\omega}9$) 조성은 55~58% 증가하였는데 다중불포화지방산 못지않게 혈중 콜레스테롤 농도를 감소시켜주며, 다중불포화지방산보다 LDL의 산화를 감소시켜 동맥경화증이나 관상심장질환의 위험을 줄여주는 지방산으로 알려져 있다. 12주간의 녹즙섭취는 혈청 인지질의 지방산 조성은 크게 변화시키지 않지만 궁극적으로 지방조직의 지방산 조성에 변화를 초래하고 이로 인해 고콜레스테롤 환자의 혈중 지질농도를 개선시켜줄 것으로 사료된다.

심혈관계 위험요인수준 추정을 위한 지역사회 역학연구 (Distribution and Prevalence Estimation of Cardiovascular Risk Factors through Community Based Health Examination Survey)

  • Cardiovascular disease is very prevalent in Korea, and many risk factors, if properly identified are possibly corrected. However, the study results on prevalence and distribution of risk factors may not be reliable while the risk factors of disease are always issued on health promotion projects conducted recently in a community. The subjects of this study were 854 adults who participated in the health and nutrition survey in a community. They were aged between 20 and 69 and sampled representatively. This study intended to estimate the prevalence and the distribution of risk factors of cardiovascular disease such as hypertension, diabetes mellitus, hypercholesterolemia, and obesity. Systolic blood pressure and diastolic blood pressure levels were estimated at $123.9{\pm}2.2mgHg(men)$, $117.9{\pm}1.7mgHg$(women), and $80.4{\pm}1.5mg(men)$, $74.9{\pm}1.1mgHg(women)$, respectively. Glucose level was estimated at $99.1{\pm}2.3mg/dl$ in men, and $95.7{\pm}1.7mg/dl$ in women. The estimated level of total cholesterol and HDL-cholesterol were $183.4{\pm}3.8mg/dl(men)$, $181.7{\pm}3.1mg/dl(men)$, and $122.0{\pm}4.5mg/dl(women)$, and body mass index was estimated at $24.0{\pm}0.4kg/m^2$ in men and $23.9{\pm}0.4kg/m^2$ in men and $23.9{\pm}0.3kg/m^2$ in women. The prevalence of hypertension was 20.5% for men, and 14.4% for women. The prevalence of diabetes mellitus was estimated to 6.9% for men, and 6.1% for women. The estimated prevalence of hypercholesterolemia was 3.8%(men), 3.9%(women). The rate of obesity was estimated to 28.5%(men), 28.4% (women), respectively. The levels of blood pressure, glucose, and cholesterol were higher in men than in women in almost all the almost ate groups. The prevalence of hypertension for men is about 20%. It was found that the prevalence of diabetes mellitus for males aged between 40 and 59 was rapidly increased. The risk factor with highest prevalence was obesity, and hypertension and diabetes mellitus were the second and third most prevalent.

Lovastatin과 Simvastatin의 고지혈증 치료 비교 (A Comparison of Lovastatin and Simvastatin in Treatment of Hyperlipidemia)

  • Hypercholesterolemia is one of main causes of coronary heart disease(CHD). Clinical trials demonstrated that lowering serum cholesterol levels would reduce incidence of new cardiovascular events and mortality by primary or secondary preventions. The objective of this retrospective study was to compare efficacy and side effects of lovartatin and simvastatin in treatement of hypercholesterolemia. In Boramae Hospital, patients were included when they have taken lovastatin 20 mg or simvastatin 10 mg for 52 weeks with laboratory monitoring for cholesterol at baseline, 3, 6 and 12 month period. As results, total 128 outpatients were included with their total cholesterol level <240 mg/dl and triglyceride level <400 mg/dl at baseline. Total cholesterol and LDL cholesterol of lovastatin group (n=60) and simvastatin group (n=68) were significantly reduced from baseline (p=0.001). Lovastatin maximally reduced total cholesterol by $23.9\%,\;triglyceride\;by\;12.3\%$, LDL cholesterol by $36.1\;\%$ and increased HDL cholerterol by $7.8\%$ and simvastatin reduced by $24.1\%,\;20.5\%,\;34.3\%\;respectively$ and HDL increased by $11.2\%$. There were no significant differences between lovastatin and simvastatin in mean percent change of lipid levels at 12, 24 and 52 weeks from baseline. Cumulative percentage of patients reaching the target LDL cholesterol concentration by 24 weeks was $61.7\%$ in lovastatin and $64.7\%$ in simvastatin. Average time to reach the target LDL goal was 100.1 days in lovastatin and 99.8 days in simvastatin. Both lovastatin and simvastatin also significantly reduced total cholesterol and LDL cholesterol in all subgroups (diabetes mellitus, hypertension, and coronary heart disease). In this study, treatment efficacy in patients with coronary heart disease was lower than other patients. Considering clinical importance of secondary prevention, more intensive treatment is necessary to decrease LDL cholesterol level of 100 mg/dl or lower in patients with coronary heart disease or other clinical atherosclerotic disease. There were no serious side effects during the study period. Digestive side effects were most frequently reported (lovastatin $8.3\%\;vs\;simvastatin\;8.8\%$). In conclusion, both lovastatin and simvastatin were similar in lipid lowering effects and there was no difference in incidence of side effects.

침습, 비침습 혈관레이저에 의한 고콜레스테롤혈증의 치료 효과 환자 유형별 비교 (A Study of Classified Comparison between Intravascular Laser Irradiation Therapy(ILIT) and Extravascular Laser Irradiation Therapy(ELIT) on Hypercholesterolemia)

  • Objective : The purpose of this study was to compare and analyze the effect of ILIT and ELIT according to sex, age, disease on Hypercholesterolemia. Methods : The 73 patients who were irradiated to the Intravascular Laser and the 57 patients who were irradiated to the Extravascular Laser from 1999 to 2005 in Sam-se oriental medicine hospital were the study group. They were irradiated two or three times a week and checked total cholesterol before and after ten times of irradiation. The checked total cholesterol point, 150-199 were normal, 200-239 were borderline, over 240 were abnormal. They were classified into male and female(43 persons, 87 persons), adult and 01d(62 persons, 68 persons), cardiovascular disease and the other diseases(70 persons, 60 persons). Results : 1. Male, adult, and the other diseases groups were more effective in cholesterol reduction and in convertible change of each group than female, old, and cardiovascular disease groups, but only the convertible change of each group had statistic significance. 2. In male, old, and cardiovascular disease, ELIT was more effective than ILIT in cholesterol reduction and convertible change of each group, but this result didn't have statistic significance. 3. In female, ELIT was more effective than ILIT in cholesterol change but this result didn't have statistic significance. ILIT was more effective than ELIT in convertible change of each group and this result had statistic significance. 4. In adult and the other diseases, ILIT was more effective in cholesterol reduction, but this result didn't have statistic significance. ILIT was more effective than ELIT in convertible change of each group too and this result had statistic significance. Conclusion: It was thought to be undesirable to decide which is more effective treatment, but to be more desirable to choose one of ILIT or ELIT according to sex, age, disease than to decide it indiscriminately.

혈청 총콜레스테롤, 공복시 혈당, 혈압의 분포와 심전도상 허혈성 심질환과의 관련성 (Analysis on the Relationships among the Total Cholesterol, Fasting Blood Sugar, Hypertension and Ischemic Heart Disease on EKG Findings)

  • Since the 1960s, Korea has achieved rapid economic growth, longer life expectancy, accelerated urbanization and a westenization of diet. Cardiovascular disease has become the leading cause of death; however the prevalence of ischemic heart disease(IHD) remains low. A cross-sectional multiphasic screening service for 18,426 persons aged $30\sim64$ years in a Medical Aid Program in Kyonggi-do Province was conducted from 1991 to 1993. Total cholesterol(TC), fasting blood glucose(FBG), blood pressure(BP), and electrocardiographic(EKG) data were collected. The result as follows ; 1. On the EKG findings, the age-adjusted prevalence of myocardial ischemia and myocardial infarction was 1.45% in men and 2.06% in women. 2. The mean blood pressure was 122.9/78.8mmHg. The age-adjusted prevalence of hypertension was 11.05% in men and 9.02% in women. The prevalence of hypertension showed increasing tendency according to age increase. In all age group, the prevalence of hypertension was higher in men than women. 3. The mean total cholesterol level was 184.4mg/dl in men and 189.2mg/dl in women. The age-adjusted prevalence of hypercholesterolemia was 4.88% in men and 5.67% in women. The total cholesterol level showed increasing tendency according to age increase, except $55\sim64$ age group in men. 4. The prevalence of hyperglycemia is 5.8%. The age-adjusted prevalence of hyperglycemia is 6.72% in men and 4.50% in women. The prevalence of hyperglycemia showed increasing tendency according to age increase. 5. On the EKG findings, the prevalence of myocardial ischemia and myocardial infarction was higher in hypertension than normal, in all age group of men and women less than 40 years-old. Only in women more t]fan 40 years-old, the prevalence of myocardial ischemia and myocardial infarction was higher in hypercholesterolemia and hyperglycemia. Nevertheless there is not statistical association between ischemia heart disease and previous risk factors in other age group, the prevalence of myocardial ischemia and myocardial infarction was higher in hypertension, hypercholesterolemia and hyperglycemia than normal. The result of this study suggest that relationships between major risk factor of ischemic heart disease and ischemic heart disease is similar to existing theory.

한국 당뇨병 노인의 혈액생화학적 특성, 식이 섭취, 당화혈색소 관련 위험 요인 및 위험도: 국민건강영양조사 제4기(2007-2009)와 제7기(2016-2018) 비교 (Blood Biochemical Characteristics, Dietary Intake, and Risk Factors Related to Poor HbA1c Control in Elderly Korean Diabetes Patients: Comparison between the 4th(2007-2009) and the 7th(2016-2018) Korea National Health and Nutrition Examination Surveys)

  • Objectives: The purpose of this study was to investigate the blood biochemical characteristics, comorbidities, dietary intake, and other risk factors leading to poor glycated hemoglobin (HbA1c) control in elderly Korean diabetes patients over 65 years of age. Methods: Data from the 4th (2007-2009) and the 7th (2016-2018) Korea National Health and Nutrition Examination Surveys (KNHANES) were used. Socio-demographic characteristics (age, gender, region, household income, education level, marital status, nutrition education, diabetes duration and diabetes treatment), lifestyle characteristics (drinking, smoking, regular walking, and subjective health perception), anthropometric characteristics (height, weight, and waist circumference), blood biochemical characteristics (HbA1c, high- density lipoprotein cholesterol, triglycerides, total cholesterol, blood urea nitrogen, and blood creatinine), co-morbidities (obesity, hypertension, hypercholesterolemia, hypertriglyceridemia, and anemia), energy and nutrients intake, food group intake, and HbA1c control-related risk factors were compared. Results: Compared to the 4th survey, the 7th survey showed an increase in diabetes prevalence among men, an increase in the prevalence of diabetes in the older patients, and an increase in the duration of diabetes. The energy ratio from carbohydrate consumption in the 7th survey was lower than in the 4th. Compared to the 4th survey, thiamine and riboflavin intake had improved, and the intakes of vitamin A, vitamin C, and niacin had worsened in the 7th. A comparison of food group intakes showed that there was a decrease in the consumption of whole grains, potatoes, and milk and an increase in the intake of beverages and alcoholic beverages. The risk factors for poor control of HbA1c were the duration of diabetes and co-morbid hypertriglyceridemia in the 4th survey, whereas subjective health perception, obesity, and hypercholesterolemia as co-morbidities were found to be risk factors in the 7th in addition to the risk factors highlighted in the 4th survey. Conclusions: For the future management of elderly Korean diabetes patients, greater care is indicated for men over 75 years, and those with low levels of education. It is necessary to increase the intake of milk and vegetables, and reduce the intake of beverages and alcoholic beverages. In addition, it is necessary to reduce the incidence of obesity, hypercholesterolemia, and hypertriglyceridemia for proper control of blood sugar.

허혈-재관류 : 미세혈관 기능 장애의 기전 (Ischemia-Reperfusion : Mechanism of Microvascular Dysfunction)

  • 미세혈관의 모든 분절(세동맥, 모세혈관, 세정맥)에 있는 내피세포의 기능은 허혈-재관류(ischemia-reperfusion)에 노출시 장애가 발생하게 되는데, 세동맥에서는 혈관확장 능력이 떨어지고, 모세혈관에서는 체액 여과 및 백혈구에 의한 혈관의 막힘(leukocyte plugging)이 증가되며, 세정맥에서는 백혈구-내피세포 접착(leukocyte-endothelial cell adhesion)과 단백의 혈관 외 누출이 증가한다. 활성 산소(reactive oxygen species)와 nitric oxide의 생산의 불균형은 이들 반응을 촉진시키며, 심혈관 질환의 위험 인자인 hypercholesterolemia, 당뇨병, 고혈압 등은 I/R에 대한 미세혈관 반응을 더욱 악화시킨다.
