  • A key component of aviation safety is to eliminate the errors in commercial aircraft air data systems to ensure stable aviation operation. Although the technical aspects such as the maintenance and inspection play a pertinent role, human errors are expected to have a similar or even larger influence on the aviation safety. Aviation maintenance and inspection tasks are often performed by a complex organization, in which individuals perform a variety of tasks in an environment involving time pressure, sparse feedback, and complex conditions. These situational characteristics, combined with the general tendency of human error, may lead to various types of errors, which may have critical consequences such as accidents and loss of life. For instance, if an amber message "IAS DISAGREE" is displayed on the primary flight display while the aircraft is rolling on the runway to takeoff, the crew immediately performs a rejected takeoff operation and troubleshoots the air data system. This paper proposes alternative approaches to address the occurrence of defects due to the human factors involved in the practical processes of the air data system of commercial aircraft.

GEMS모델을 이용한 선박충돌사고의 인적과실 유형 분석 (Classification of Human Errors in Ship′s Collision using GEMS Model)

  • 지금까지(1988∼2000년) 국내 해양안전심판원에서 재결한 선박충돌사고의 원인분류 통계데이터를 살펴보면 전체 2,290건의 인적과실(Human Error) 중에서 상대선박에 대한 경계의무소홀이 929건으로 약 40.6%나 되는 가장 많은 비중을 차지하고 있는 것으로 파악되고 있다. 선박충돌사고는 좌초사고와 더불어 인적과실로 기인한 수많은 인명피해와 재산피해 및 해양환경오염을 유발하는 심각한 해양사고로서 이에 대한 사고원인을 철저히 분석하고 그 예방대책 마련이 무엇보다 중요한 과제가 되고 있다. 따라서, 본 연구에서는 선박충돌사고의 인적과실을 분석하기 위한 목적으로 목포해양안전심판원에서 재결한 선박충돌사고(1990∼2002, 65건)에 대하여 상대선 경계의무소홀이나 동정감시 불충분으로 기인하여 발생한 충돌사고를 조사항목별로 분석하고, GEMS 동적모델을 이용하여 선박충돌사고의 인적과실 유형을 체계적으로 분류하였다.

  • 건설종사자가 일으킨 휴먼에러가 건설 사고재해의 중요한 원인 중 하나이므로 사고재해를 줄이기 위해서 건설근로자의 휴먼에러를 줄이는 것이 중요하다. 그러나, 건설업의 특성상 건설근로자는 현장이라는 공통의 틀 안에서 작업을 하고 있으므로, 본 연구는 조직적 관점에서 건설 현장 조직의 안전의식 수준이 건설근로자 휴먼에러에 미치는 영향을 파악하였다. 분석 결과 휴먼에러가 사고재해에 영향을 미치며, 건설 현장의 안전의식 수준이 건설근로자의 휴먼에러에 영향을 미친다는 것을 알 수 있었다. 특히, 잘못은 안전의식의 거의 모든 영역에서 현장의 안전의식 수준과 상관관계가 있는 것으로 나타났다. 본 연구는 건설 현장의 안전의식 수준을 높이면 건설근로자가 잘못된 행동(행위착오, Slip)을 할 가능성이 작아져 궁극적으로 건설 현장의 사고재해를 줄일 수 있다는 것을 파악한 점에서 의의가 있다고 할 수 있다.

LPG 이송작업시 인적과오에 대한 사상수목분석

  • LPG refueling include a lot of risk done by human beings, dealing with highly combustible gas, so, during the refueling, the leakage initiated by human errors can result in a catastrophic accident. Therefore, this research tried to show what kind of tasks would include the high probability of the human errors and what should be considered for effective safety management in the LPG refueling. At first, 4 typical cases were taken through surveying various accident cases, and then a prototype of the refueling task was presented. And each task was analysed by FTA and ETA. The results showed that overpressure occupies 64.64% of the major reasons for gas leakage, and its probability was approximately 6.62E-06. Among the tasks, gas leakage resulted from mal-assembly of lorry hoses had the highest rate, and human errors related to opening and closing valves of pipe lines were most frequent. Also, the effects of confirming tasks were analyzed quantitatively.

A study for the reduction of the SW reliability test time and human errors using the SW reliability test automation

  • In this paper, we propose a way to reduce the SW reliability test time and human errors by utilizing the Lean Six Sigma methodology as a troubleshooting tool. Recently, SW technology is rapidly changing and the importance of SW management has been recognized. Accordingly, the SW reliable test for the SW development outcomes in one of the methods for SW quality improvement is required. However, it consumes a lot of development costs, has the time constraints and human errors in SW reliable test, so, it is difficult to meet the requirements for SW test.

Human Error Identification based on EEG Analysis for the Introduction of Digital Devices in Nuclear Power Plants

  • Objective: This paper describes an analysis of electroencephalography(EEG) signals to identify human errors during using digital devices in nuclear power plants(NPPs). Background: The application of an advanced main control room(MCR) has accompanied with lots of changes in different forms and features by virtue of new digital technologies. The characteristics of these digital technologies and devices provide several opportunities for the use of interface management. It can integrate into a compact single workstation in an advanced MCR, allowing workers to operate the plant with minimum physical burden under any operating condition. However these devices may introduce new types of human errors, and thus we need a means to assess and prevent such errors especially those related to digital devices. Method/Conclusion: The EEG data are relatively objective, and thus we introduce several measures to EEG analysis for obtaining the feasibility of human error identification. Application: This study may support to ensure the safety when applying digital devices in NPPs.

철도기관사들의 개인적 특성과 인적오류사고 발생에 대한 비교 분석 (A Study on Analysis between Accidents Caused by Human Errors and Personal Characteristics of Railway Drivers)

  • To verify the effect of driver's personal characteristics of driver on the accident frequency through railway accidents caused by human errors and the relationship with aptitude test. To prove the relevance between the driver's personal characteristics and human error accidents. Accident data from 2010 to 2011 was analyzed which collected from a train crew department in K national corporation, and 31 drivers gave an personal interview from Sep. 2011 to Nov. 2011 who had controlled a train alone and caused an accident. Compared between driver's personal characteristics and accident rate, and accident induction possibility surveyed from normal person and disqualified in aptitude tests. Accidents was occurred with the age 40s (27%) and 50s (25%), and with the experience between 15 years and 20 years (38%) and over 20 years (30%). Because more aged, more experienced, it can be seen in the correlation between driver's age and accidents induction caused by human errors like illusion. First of all it must be checked whether working conditions and environmental factors are human error-prone. Most accidents occur when received civil complaints or manager at the riding. Therefore accidents can be prevented when investigated through subsequent surveys how often human error happens, even though no accident, and safety device installed based on the error frequency.

국내 화학사고의 휴먼에러 기반 분석에 관한 연구 (A Study on the Analysis of Human-errors in Major Chemical Accidents in Korea)

  • This study analyses the types, related operations, facilities, and causes of chemical accidents in Korea based on the RISCAD classification taxonomy. In addition, human error analysis was carried out employing different human error classification criteria. Explosion and fire were major accident types, and nearly half of the accidents occurred during maintenance operation. In terms of related facility, storage devices and separators were the two most frequently involved ones. Results of the human error-based analysis showed that latent human errors in management level are involved in many accidents as well as active errors in the field level. Action errors related to unsafe behavior leads to accidents more often compared with the checking behavior. In particular, actions missed and inappropriate actions were major problems among the unsafe behaviors, which implicates that the compliance with the work procedure should be emphasized through education/training for the workers and the establishment of safety culture. According to the analysis of the causes of the human error, the frequency of skill-based mistakes leading to accidents were significantly lower than that of rule-based and knowledge based mistakes. However, there was limitation in the analysis of the root causes due to limited information in the accident investigation report. To solve this, it is suggested to adopt advanced accident investigation system including the establishment of independent organization and improvement in regulation.

국내 원자력발전소 인적오류 저감을 위한 Crew Resource Management 교육훈련체계 개발 (Development of a Crew Resource Management Training Program for Reduction of Human Errors in APR-1400 Nuclear Power Plant)

  • The nuclear power industry in the world has recognized the importance of integrating non-technical and team skills training with the technical training given to its control room operators to reduce human errors since the Three Mile Island and Chernobyl accidents. The Nuclear power plant (NPP) industry in Korea has been also making efforts to reduce the human errors which largely have contributed to 120 nuclear reactor trips from the year 2001 to 2006. The Crew Resource Management (CRM) training was one of the efforts to reduce the human errors in the nuclear power industry. The CRM was developed as a response to new insights into the causes of aircraft accidents which followed from the introduction of flight recorders and cockpit voice recorders into modern jet aircraft. The CRM first became widely used in the commercial airline industry, but military aviation, shipboard crews, medical and surgical teams, offshore oil crews, and other high-consequence, high-risk, time-critical industry teams soon followed. This study aims to develop a CRM training program that helps to improve plant performance by reducing the number of reactor trips caused by the operators' errors in Korean NPP. The program is; firstly, based on the work we conducted to develop a human factors training from the applications to the Nuclear Power Plant; secondly, based on a number of guidelines from the current practicable literature; thirdly, focused on team skills, such as leadership, situational awareness, teamwork, and communication, which have been widely known to be critical for improving the operational performance and reducing human errors in Korean NPPs; lastly, similar to the event-based training approach that many researchers have applied in other domains: aircraft, medical operations, railroads, and offshore oilrigs. We conducted an experiment to test effectiveness of the CRM training program in a condition of simulated control room also. We found that the program made the operators' attitudes and behaviors be improved positively from the experimental results. The more implications of the finding were discussed further in detail.

항공기 유사호출부호와 항공교통관제 인적오류 상관관계 연구 (Correlation of similar call-signs aircraft and human error in ATC)

  • As the ultimate goal of the Aeronautical communication is delivery of information, it is essential to communicate via accurate use of language; there are possibilities of errors occurring as the exchange of information between pilot and air traffic controller is conducted through radio communication. Especially, there are frequently reported errors in radio communication in the event that two or more aircrafts using similar call-signs are operated in one sector. In this paper, a list of errors, which can possibly caused by air traffic controllers when controlling airplanes using similar call-signs, was derived through preceding researches and theories related to similar call-signs and human errors. Based on this list, an experiment was conducted on 52 air traffic controllers working at "J" international airport. For this experiment, ATC2K which is a simulated approach control radar developed in Korea, was used; and a regression formula was derived by analyzing the quantity of frequency change in error occurrence when controlling two or more airplanes using similar call-signs base on a premeditated scenario.The data presented in this research has useful findings and concepts for understanding the errors that can possibly occur while controlling airplanes using similar call-signs.
