Adherence to Clinical Guideline for Endotracheal Suction in ICU Nurses (중환자실 간호사의 기관내 흡인 임상지침 수행 현황)

  • Purpose: This study aimed to evaluate the adherence to the clinical guideline for endotracheal suction in nurses working at intensive care units (ICU) and to identify the characteristics of nurses with good adherence. Methods: This study was a descriptive study to evaluate the pattern of adherence and its related factors to endotracheal suction. One hundred fifty five nurses working at ICU participated in this study. We used a questionnaire developed based on American Association for Respiratory Care (AARC) guidelines and other associated factors from previous studies. Results: Around half of the participants reported that their adherence to the clinical guideline was poor. Items deviated from the recommended guideline were reasons for initiating a suctioning, applied suction pressure ranged from 20 to 200mmHg, and applied catheter size from 6 to 17 french. Other factors deviated were the depth of inserted catheter, and inappropriate use of normal saline instillation. The most significant factor was related to hospital; the misused or misled clinical protocol. Conclusions: The adherence to the clinical guidelines of the endotracheal suction in ICU nurses was not appropriate, which might contribute to the patient health outcomes. More enhanced continuing education as well as hospital regulation is warranted.

Prevalence and Related Risk Factors of Delirium in Intensive Care Units as Detected by the CAM-ICU (CAM-ICU로 평가한 중환자실의 섬망 발생률과 섬망 발생 위험요인)

  • Purpose: Screening of delirium using delirium assessment tools could promote delirium detection, however, there is lack of report about regular delirium assessment in Korea. This study was intended to describe the prevalence and related risk factors of delirium in intensive care unit (ICU). Methods: The Confusion Assessment Method for the ICU (CAM-ICU) data which were evaluated by nurses in ICUs was obtained through retrospective chart review. Data were analyzed using descriptive statistics, Chi-square test, t-test, Mann-Whitney U test, and stepwise logistic regression. Results: Delirium was evaluated in 125 patients. The incidence rate of delirium was 27.2% with a high prevalence of hypoactive delirium compared to hyperactive delirium (61.8 vs. 38.2%). Those with delirium were older, had hypertension, stayed longer in hospital, receiving ventilator support, had more number of catheters, had low serum protein and albumin level. Delirium incidence also varied according to diagnosis. Age, diagnosis of gastrointestinal disease, and application of ventilator were the significant risk factors for the incidence of delirium. Conclusion: Routine delirium screening is important for early detection of delirium. Identification of high-risk group and running delirium prevention programs could improve early recognition of delirium in ICU.

Impact of Unit-level Nurse Practice Environment on Nurse Turnover Intention in the Small and Medium Sized Hospitals (중소병원 간호단위의 간호근무환경이 간호사의 이직의도에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the impact of the unit-level nurse practice environment on nurse turnover intention in the small and medium sized hospitals. Methods: A cross-sectional study was conducted using a questionnaire survey with 308 nurses in 38 nursing units of 6 small and medium sized hospitals, having over 200 beds and under 300 beds and located in B metropolitan city. Data were collected from July 20 to August 10, 2011. Data were analyzed using hierarchical multiple regression. Results: The mean turnover intention in nurses of small and medium sized hospitals was $3.52{\pm}0.53$. Factors affecting turnover intention in the nurses included age, work unit, monthly income, number of night-duties, work hours per day and unit-level nurse practice environment. The unit-level nurse practice environment accounted for 15% of turnover intention when other variables were controlled. Conclusion: The results of the study indicate that nurse turnover intention is associated with the nurse practice environment at the unit level. Small and medium sized hospitals can improve nurse retention and lower turnover intention by changing the nurse practice environment of unit, such as creating better support services and nurse participation in hospital affairs.

The Experiences of Patients in Intensive Care Units(ICU) (중환자실 환자의 경험)

  • Purpose: The purpose of this study was to describe the essence of the experiences of patients in an ICU, and to understand them from the patients' point of view. Methods: Participants in this study were six patients in P hospital. Data collection consisted of in-depth interviews and an observation method done from January to April in 2005. The method was analysis using the phenomenological method proposed by Colaizzi(1978). Results: The themes were classified into eight theme clusters. The eight theme clusters were finally grouped into four categories, 'shock', 'pain', 'gratefulness' and 'pleasure of revival'. Conclusion: The ICU patients had negative experiences in physical mental critical situations, but also positive experiences in consolation and nurses and families' encouragement. Therefore, ICU nurses must support patients and their families to minimize the negative experiences and maximize the positive experiences.

Risk Factors for Deliberate Self-extubation (기관 내 삽관환자의 의도적 자가발관 위험요인)

  • Purpose: This study was conducted to analyze intubation survival rates according to characteristics and to identify the risk factors affecting deliberate self-extubation. Methods: Data were collected from patients' electronic medical reports from one hospital in B city. Participants were 450 patients with endotracheal intubation being treated in intensive care units. The collected data were analyzed using Kaplan-Meier estimation, Log rank test, and Cox's proportional hazards model. Results: Over 15 months thirty-two (7.1%) of the 450 intubation patients intentionally extubated themselves. The patients who had experienced high level of consciousness, agitation. use of sedative, application of restraints, and day and night shift had significantly lower intubation survival rates. Risk factors for deliberate self-extubation were age (60 years and over), unit (neurological intensive care), level of consciousness (higher), agitation, application of restraints, shift (night), and nurse-to-patient ratio (one nurse caring for two or more patients). Conclusion: Appropriate use of sedative drugs, effective treatment to reduce agitation, sufficient nurse-to-patient ratio, and no restraints for patients should be the focus to diminish the number of deliberate self-extubations.

A Study of Handwashing by Intensive Care Unit Nurses according to the Content of Nursing Faculty Practice (중환자실 간호사의 간호업무내용에 따른 손씻기에 관한 연구)

  • Purpose: This study was done to determine the rate for handwashing by intensive care unit nurses according to the content of nursing tasks, to investigate the relationship between hand washing practice evaluated by nurses themselves and their actual practice observed, and finality to provide basic materials for strategy for hand washing education. Method: Data were collected by observing 27 nurses working in intensive care units of a hospital in Uijeongbu, Gyeonggi-do and by using observation and a structured self-assessment tool. Collected data were analyzed with SPSS and SAS. Results: The handwashing rate for the nurses was 4.3%. The handwashing rate was high in proportion to the risk of cross infection. In addition, the handwashing rate was highest in nurses working in the neurosurgery intensive care unit. The average score for self-assessment of handwashing was $49.42{\pm}3.78$ points and it was higher than their actual practice of handwashing. Conclusion: In order to improve handwashing by nurses, it is necessary to educate them on the importance of handwashing. In addition, there should be strategies for standardizing knowledge and attitudes to handwashing and inducing nurse:3 to practice hand washing in compliance with the policies and working conditions of the institution.

A Qualitative Content Analysis of the Person-Centered Care Experienced by Critical Care Patients (중환자실 환자가 경험한 인간중심 간호에 관한 질적 내용분석)

  • Purpose : To explore the person-centered care (PCC) experienced by critical patients. Method : This qualitative study deductively examined the attributes of the PCC model proposed by Jakimowicz and Perry. The participants were 16 patients who were recently discharged from intensive care units at a university hospital. Data were collected through in-depth interviews and were analyzed using the deductive method of content analysis. Results : A total of 4 categories, 16 subcategories, and 33 codes were generated from 171 meaningful statements. The final 4 categories were "compassionate presence," "professional interaction," "outstanding competency," and "patient identity." These were consistent with the main attributes of Jakimowicz and Perry's model. However, most of the codes belonged to the "compassionate presence" and "patient identity" categories. Among the attributes of the model, "continuity of nursing," "therapeutic relationship," "expert knowledge," "clinical knowledge," "evidence-based intervention," and "patient's rights" were not derived as codes. Conclusion : These findings deepen the understanding of the PCC model from the patient's point of view. The main attributes of PCC identified in the current study can be applied to the development of practical guidelines for intensive care nursing. In addition, we recommend the development of a PCC measurement tool for critical care patients.

Emotional Response of ICU Patients' Family toward Physical Restraints (중환자실 환자의 억제대 적용에 대한 가족의 정서적 반응)

  • Purpose: The purpose of this study was to investigate the emotional response of family members of physically restrained patients in the intensive care units (ICUs). Methods: The study subjects were 200 family members of ICU patients who had been on physical restraints in two university hospitals. Data were collected using the "Instrument of family's emotional response toward physically restrained patients". Results: The mean score of familial emotional response was 2.69 out of a possible 5. The subcategory of acceptance was the highest with 3.56 points followed by depression (3.02), helplessness (2.94), anxiety (2.87), shock (2.74), avoidance (2.64), and grudge (2.08). Multiple stepwise regression analysis indicated that the age of family members, side effects of restraints, and information provision were the variables influencing on negative emotional response of family. Conclusion: Family members showed slightly negative emotional response toward the physical restraints. This finding could be influenced by their limited knowledge of the need for the restraints. Educational programs or fact sheets to be given to family members may be helpful.

Effect of Sleep Disturbance on Fatigue, Sleepiness, and Near-Miss among Nurses in Intensive Care Units (중환자실 간호사의 수면장애가 피로, 졸음과 근접오류에 미치는 영향)

  • Purpose : This study aims to investigate the differences in fatigue, sleepiness, and near-miss according to sleep disturbance among shift nurses in intensive care units (ICUs). Methods : A cross-sectional study in a tertiary hospital was performed. A total of 122 shift nurses working in the six ICUs were recruited. They completed self-reported questionnaires about sleep disturbance, fatigue, sleepiness, and near-miss in the past two weeks. Results : The prevalence of reported sleep disturbance was 30.3% (37 out of 122 subjects). Compared to the non-sleep disturbance group, the sleep disturbance group reported significantly more sleepiness (11.46 vs. 8.86) and higher fatigue (82.62 vs. 69.39). The sleep disturbance group showed higher rates of near-miss (78.4 vs. 57.6%) and a higher frequency of them (4.49 vs. 2.11/2weeks) compared to the non-sleep disturbance group. Medication error was the most common type of near-miss. Conclusions : This study suggests that sleep disturbances could increase fatigue, sleepiness, and near-miss among ICU shift nurses. Personal and organizational programs should be developed to support the sleep of ICU nurses.

Quality Control of Diagnostic X-ray Units for Animal Hospital (동물병원의 방사선발생장치 정도관리에 대한 연구)

  • The purpose of this study was to investigate the actual conditions of radiation safety supervision in animal clinics using quality assurance (QA) and quality control (QC) of diagnostic X-ray units. The surveys for QA/QC, equipment condition, and safety supervision were carried out in 18 animal clinics randomly. The QA/QC included reproducibility of dose exposure, kVp, mAs, collimator accuracy test, collimator luminance test, X-ray view box luminance test, grounding system equipment test and external leakage current test. As a result, 44.44% of reproducibility of dose exposure was proper, 81. 25% of kVp test was good, and 100% of mAs test was appropriate. Also, 66.66% of collimator accuracy test was proper, 61.11% of collimator luminance test was good, 53.13% of X-ray view box luminance test was suitable. In addition, only 5.55% of grounding system equipment and ground resistance was proper, 63.64% of external leakage current test was appropriate in grounding system equipment test.
