데이컴 기법을 이용한 일개 권역응급의료센터 간호사의 직무 분석 (Nurses' Job Analysis in a Regional Emergency Center Using the DACUM)

  • Purpose: This study aimed to establish the role of and to analyze the job of nurses in a regional emergency center using the Developing a Curriculum(DACUM) process, a job analysis technique. Methods: The DACUM committee was organized, and three workshops were held from June 1st to July 4th, 2016. One hundred two nurses in a regional emergency center participated in a survey from October 3rd to 10th, 2016. The frequency, importance and difficulty of each tasks and task elements in the DACUM were investigated on a Likert scale. Results: The roles of nurses in a regional emergency center were identified and the DACUM chart was developed with 6 duties, 29 tasks and 153 task elements. According to the survey, some tasks such as 'CPR,' 'acute respiratory distress care,' and 'drug administration' showed high frequency and importance or difficulty in all nursing units in common, and different results reflected the characteristics of each nursing unit were also found. Conclusion: The results showed the feature of the regional emergency center. Based on these results, it is necessary to develop educational programs for practice.

간호학생의 감정노동이 소진에 미치는 영향: 감성지능의 조절효과 (Impact of Nursing Students' Emotional Labor on Burnout during Nursing Practice in a Hospital: Moderating Effect of Emotional Intelligence)

  • Purpose: This study was done to: (a) identify levels of emotional labor that nursing students perceived when caring for patients in the hospital during their nursing practicum, (b) examine the impact of emotional labor on burnout, and (c) identify the possible moderating effect of emotional intelligence on the relationship between emotional labor and burnout for nursing students. Methods: A descriptive study was conducted with a sample of 171 students enrolled in a 4-year undergraduate nursing program in one university located in Korea, and had a practicum experience at least one semester before the study began. Data were collected with self-report questionnaire, and analyzed with SPSS 21.0 program, using hierarchical multiple regression. Results: Mean score for emotional labor was 3.17 with a range of 1 to 5. For nursing students, emotional labor significantly affected burnout in the nursing practicum (F=15.763, p<.001), and explained 30.1% of the variance in burnout. The degree of emotional intelligence acted as a moderator on the relationship between emotional labor and burnout at significant level (F=15.345, p<.001). Conclusion: This research shows that nurse managers as well as nurse educators need to pay attention to nursing students' emotional labor during nursing practicum in the care units of hospitals.

기관 내 삽관환자의 의도적 자가발관 위험요인 (Risk Factors for Deliberate Self-extubation)

  • Purpose: This study was conducted to analyze intubation survival rates according to characteristics and to identify the risk factors affecting deliberate self-extubation. Methods: Data were collected from patients' electronic medical reports from one hospital in B city. Participants were 450 patients with endotracheal intubation being treated in intensive care units. The collected data were analyzed using Kaplan-Meier estimation, Log rank test, and Cox's proportional hazards model. Results: Over 15 months thirty-two (7.1%) of the 450 intubation patients intentionally extubated themselves. The patients who had experienced high level of consciousness, agitation. use of sedative, application of restraints, and day and night shift had significantly lower intubation survival rates. Risk factors for deliberate self-extubation were age (60 years and over), unit (neurological intensive care), level of consciousness (higher), agitation, application of restraints, shift (night), and nurse-to-patient ratio (one nurse caring for two or more patients). Conclusion: Appropriate use of sedative drugs, effective treatment to reduce agitation, sufficient nurse-to-patient ratio, and no restraints for patients should be the focus to diminish the number of deliberate self-extubations.

중환자실 간호사의 수면장애가 피로, 졸음과 근접오류에 미치는 영향 (Effect of Sleep Disturbance on Fatigue, Sleepiness, and Near-Miss among Nurses in Intensive Care Units)

  • Purpose : This study aims to investigate the differences in fatigue, sleepiness, and near-miss according to sleep disturbance among shift nurses in intensive care units (ICUs). Methods : A cross-sectional study in a tertiary hospital was performed. A total of 122 shift nurses working in the six ICUs were recruited. They completed self-reported questionnaires about sleep disturbance, fatigue, sleepiness, and near-miss in the past two weeks. Results : The prevalence of reported sleep disturbance was 30.3% (37 out of 122 subjects). Compared to the non-sleep disturbance group, the sleep disturbance group reported significantly more sleepiness (11.46 vs. 8.86) and higher fatigue (82.62 vs. 69.39). The sleep disturbance group showed higher rates of near-miss (78.4 vs. 57.6%) and a higher frequency of them (4.49 vs. 2.11/2weeks) compared to the non-sleep disturbance group. Medication error was the most common type of near-miss. Conclusions : This study suggests that sleep disturbances could increase fatigue, sleepiness, and near-miss among ICU shift nurses. Personal and organizational programs should be developed to support the sleep of ICU nurses.

외국인 환자 간호조직 실태 조사 (A Survey on Nursing Organizational Service for Foreign Patients in Tertiary Hospitals)

  • Purpose: The purpose of this study was to investigate the status of organizational nursing service for foreign patients in tertiary hospitals in order to build up a database for nursing care policy development for foreign patients. Methods: The subjects of this study were nursing managers or department managers of international care center in 36 tertiary hospitals nationwide. Questionnaires were consisted of the items measuring the details of and evaluations for nursing care for foreign patients within the given organizations and any organizational support for culturally competent care. The data from 35 hospitals out of 36 were analyzed finally. Results: The organizational nursing service for foreign patients was rated 6.49 points out of 10. Organizational support for culturally competent care was 2.61 points out of 4. Challenges to improving nursing care for foreign patients were listed such as developing interpreter services and international care units without accompanying by a family member, improving nursing staffs' attitudes and behaviors for providing culturally competent care, and preparing various medical documents using multiple languages. Conclusion: In order to improve nursing care for foreign patients in tertiary hospitals, we need to be more aware of cultural knowledge and sensitivity toward the care needs of foreign patients.

중환자실 간호사의 요통 영향요인 (Factors Affecting Low Back Pain in Nurses in Intensive Care Unit)

  • Purpose: This study was conducted to identify the factors that influence low back pain in nurses in Intensive Care Units (ICUs). Methods: A descriptive-correlational design was used, with a convenience sample of 116 nurses from one university hospital in Seoul. The data collected were processed using SPSS Window 17.0 Program for actual numbers and percentages, differences in the dependent variable according to general characteristics, and mean, standard deviations, correlation coefficients and multiple regression analysis. Results: The factor influencing low back pain in nurses in ICUs was functional ability (${\beta}$=.652). This one factor explained 41.9% of low back pain in nurses in ICUs. Conclusion: These results show that functional ability was related to nurses' low back pain in nurses in ICUs. These results further suggest that more research is needed to develop program involving functional ability to prevent and management of low back pain in nurses in ICUs.

환자특성에 따른 의료이용에 대한 환자만족도 비교 (The Relationship between Patient Characteristics and Satisfaction with Hospital Care)

  • Purpose: The purpose of this study was to identify the relationship between patient characteristics and patient satisfaction. Methods: A cross-sectional questionnaire survey was conducted in an acute care hospital. The subjects were 317 patients discharged from general medical and surgical nursing care units during September, 2005. Patient satisfaction was measured using the short-form satisfaction scale of Hwang and Park(2001). Additional information about patient characteristics, including general demographics and health care utilization variables, was collected from the hospital information systems. Multiple regression analysis was performed to determine patient characteristics influencing patient satisfaction. Results: Patients were satisfied with hospital care with an average of 4.10 on a five-point Likert scale. Patient characteristics explained 13.5% of the variance of patient satisfaction. The significant factors influencing patient satisfaction were patients' age and perceived health status. There was no significant relationship between structural variables and patient satisfaction. Conclusion: This study showed that patients' characteristics were significant factors explaining patient satisfaction. Therefore, these characteristics should be adjusted in reporting patient satisfaction as an indicator for hospital-level or department-level rating.

중환자실 간호사의 업무부담 관련 요인: 체계적 문헌고찰 및 메타분석 (Factors Related to Workload of Intensive Care Unit Nurses: Systematic Review and Meta-Analysis)

  • Purpose: This study aimed to identify factors related to the workload of intensive care unit nurses through a systematic literature review and meta-analysis to provide basic data to explore the direction of development of nursing staffing standards. Methods: This study involved quantitative studies about nurses working in intensive care units related to nursing workload published in English or Korean since 2000. Search terms included 'intensive care unit', 'nursing workload', and their variations. Databases such as RISS, DBpia, MEDLINE(PubMed), CINAHL, PsycINFO, and Web of Science were utilized. Quality assessment was conducted using the Joanna Briggs Institute's Critical Appraisal Checklist for Analytical Cross-Sectional Studies. JAMOVI software facilitated the analysis of effect sizes, employing a meta-analysis approach for 7 studies with correlational or regression data. Results: From 16 studies on the workload of intensive care unit nurses, a total of 20 patient and nurse-related factors were identified. Patient-related factors included severity of illness, length of stay, and age. Meta-analysis was conducted for three patient-related factors: age, severity of illness measured by SAPS 3, and length of stay. Only severity of illness measured by SAPS 3 was significantly associated with nurse workload (Zr=0.16, p<.001, 95% CI=0.09-0.24). Conclusion: In previous studies, the characteristics of intensive care units and patients varied across studies, and a variety of scales for measuring workload and severity of illness were also used. Sustained research reflecting domestic intensive care unit work environments and assessing the workload of intensive care unit nurses should be imperative.

병원간호사의 욕창간호 지식수준 (Knowledge Level of Pressure Ulcer Among Hospital Nurses)

  • The purpose of this study was to evaluate the knowledge level of pressure ulcer among hospital nurses through a cross-sectional survey by using the pressure ulcer knowledge assessment tool of Beitz et al.(1998). The total of subjects was 160 voluntary participants (60 were from Hospital A and 100 were from Hospital B) working at adult patients' units in two university hospitals located in the same city. None of the hospitals had expert nurses of pressure ulcer nor provided a regular pressure ulcer education program during the past one year. The survey tool consisted of 32 true-false items which were grouped into the risk factors knowledge category(13 items), the wound assessment knowledge category(4 items), and the treatment methods knowledge category(15 items). An internal consistency reliability test of the tool yielded an overall coefficient of 0.72; the coefficient for the risk factors knowledge category was 0.40, that for the wound assessment knowledge category was 0.33; that for the treatment knowledge category was 0.54. The main findings of the study are as follows. 1) Demographic characteristics of the two hospital nurses were similar except for the educational level(p=0.029) and the work units(p=0.001). These observations were maintained even if Hospital A and B were separately compared. 2) The knowledge level of the subjects about pressure ulcer in general was low as indicated by 53.3 points(of 100 possible points) on average. The knowledge level about treatment methods of pressure ulcer was the lowest as indicated by 47.0 points on average. 3) No correlation between the knowledge level of the pressure ulcer and the demographic characteristics of nurses was found except that the knowledge level of I.C.U. nurses was significantly higher(p=0.0003) than that of nurses in other units(p=0.2926) in the case of Hospital A. 4) The knowledge level of nurses in Hospital B was higher than for nurses in Hospital A. The reason was not identified, but it seems that it is related to the role of the I.C.U. or some other factors. The study results confirmed the existing literature that knowledge level of nurses about pressure ulcer is low regardless of age, educational level, or work experience. However, the working place(unit) affected the knowledge level. Further research on the exact reason for the differences in the knowledge level is needed in the future.

간호과정 적용 평가도구의 타당도 및 신뢰도 조사 - 욕창, 낙상예방 및 통증 간호를 중심으로 - (Validity and Reliability Assessment of the Performance Measures Based on the Nursing Process for Prevention and Management of Pressure Ulcers, Falls and Pain)

  • Purpose: This study was conducted to determine the validity and reliability of performance measurement tools based on the nursing process for prevention and management of pressure ulcers, falls and pain. Methods: The performance measurement tools were reviewed by a panel of experts and refined on the basis of the panel's suggestions. The validity of the performance measurement tools was measured by surveying hospital nurses. The reliability of these tools was tested by having nursing experts use the tools in five nursing units to assess nursing performance in prevention and management of pressure ulcers, falls and pain. Results: The performance measurement tools in this study were found to be acceptable as tools to evaluate quality of nursing care in pressure ulcers, falls and pain. The reliability of the performance measurement tools was acceptable. Conclusion: These results indicate that the performance measurement tools developed in this study are valid and reliable instruments to monitor and improve quality of nursing care in prevention and management of pressure ulcers, falls and pain.
