응급구조학전공 학생의 병원 임상실습 만족도에 영향을 미치는 요인 (Influencing Factors on the Satisfaction of the Paramedic Students in Clinical Training)

  • Purpose: This study aims to assess the influencing factors on the level of satisfaction with clinical training and to provide basic data for more efficient clinical training. Method: The study was conducted on 402 paramedic students who have experienced clinical training from September 6 until October 12 in 2011. The questionnaires consisted of 40 questions. We used SPSS 18.0 frequency analysis, technical statistics, t-test, ANOVA, Pearson's correlation coefficients and multiple regression analysis. Result: 1. The level of satisfaction with clinical training showed significant difference between the frequency (F=8.837, p=.000) and clinical training managers (F=5.418, p=.001). 2. The level of satisfaction with clinical training showed the strongest positive correlation with the satisfaction of clinical training hospitals (r=.694, p=.000). 3. Multiple regression analysis revealed the most powerful predictor for satisfaction with clinical training was the satisfaction level of clinical training hospitals(48.2%) and the frequency of clinical training experiences(.8%), the preparation before the clinical training(5.4%), the total duration of clinical training(.7%), and the satisfaction of emergency department education(1.0%). These five variables accounted for 56.1% of the satisfaction of clinical training among paramedic students. Conclusion: As student's satisfaction of the clinical training increases, the quality of paramedics is expected to improve in the future. As a result, the future paramedics can be nurtured to be highly skilled in on-the-scene emergency situations after graduation.

한국어판 리더 레포 관리 측정도구의 신뢰도와 타당도 (Validity and Reliability of a Korean version of Leader Rapport Management)

  • 본 연구는 White 등이 개발한 LRM (leader rapport management) 도구를 한국어로 번역 및 수정하여 도구의 타당도와 신뢰도를 검증하는 방법론적 연구이다. 서울 소재의 2개의 상급병원에서 근무하는 간호사들을 200명을 대상으로 하여 내용 타당도, 구성 타당도, 동시 타당도, 수렴 타당도 및 신뢰도 검증을 실시하였다. 본 연구에서 한국어판 LRM도구는 총 3개 요인으로 구성되었으며 자존감 4문항, 자율성 4문항, 유대관계 4문항으로 총 12문항으로 분석되었다. 또한 신뢰도검증 결과 자존감은 Cronbach's ${\alpha}$ .86, 자율성 Cronbach's ${\alpha}$ .83, 유대관계 Cronbach's ${\alpha}$ .84으로 확인되어 본 연구의 개발도구는 신뢰도와 타당도가 높은 도구임이 검증되었다. 본 연구는 단일 차원의 상사와의 레포를 측정하던 도구들과는 달리 레포의 다차원적인 측면을 고려한 측정도구라는 점과 임상 현장에서 근무하는 간호사들의 효율적인 인적관리를 위해 상사와의 레포 관리 정도를 측정하여 이에 따른 다양한 측면의 관리 방법을 접근할 수 있게 하는 기초자료를 마련했다는데 의의가 있다.

Visceral Leishmaniasis in Rural Areas of Alborz Province of Iran and Implication to Health Policy

  • Visceral leishmaniasis (VL) or kala-azar mainly affects children in endemic areas. This study was conducted to determine the seroprevalence of VL using direct agglutination test (DAT) in children living in rural districts of Alborz Province located 30 km from Tehran capital city of Iran. Multi-stage cluster random sampling was applied. Blood samples were randomly collected from 1,007 children under 10 years of age in the clusters. A total of 37 (3.7%) of the studied population showed anti-Leishmania infantum antibodies with titers of ${\geq}1:800$. There was a significant association between positive sera and various parts of the rural areas of Alborz Province (P<0.002). Two children with anti-Leishmania infantum antibodies titers of ${\geq}1:3,200$ indicated kala-azar clinical features and treated with anti-leishmaniasis drugs in pediatric hospital. The findings of this study indicated that Leishmania infection is prevalent in rural areas of Alborz Province. Therefore, it is necessary to increase the awareness and alertness among physicians and public health managers, particularly in high-risk rural areas of the province in Iran.

병원의 위원회제도에 관한 연구 -서울시내 대학병원을 중심으로- (A Study on the Committees of University Hospitals in Seoul)

  • This study was conducted to identify the problems related to operation of the committee system in university hospitals and to propose the direction for reforming it. Data were collected by surveying 138 working-level administration managers of committees from 12 university hospitals in Seoul in October, 1995. The results were as follows: 1. The purposes of the committee were to promote the reasonable dicision making in the hospitals, to satisfy the requirement of Hospital Accreditation Program and, to execute the administrational instructions in due order. The sort of job which was charged of chairmen and the majority of committee member was doctor. 2. The committees which were to be held the meeting frequently in a year were general management committees(8.25 times/year). But in case of operation rate, medical care-related committees showed the most frequent meeting(90.15%). Most of committees made the regulations and reported the results of their meeting to the CEO of their hospitals by written documents. 3. Most doctors thought the medical care-related committees useful. Medical technicians regarded education and research-related committees as useful, while administrators favored the general management committees. 4. The factors related to the perceived performance of the committee were the kind of job, the class of job and the kind of committee. Also, the perceived performance of the committee was positively related to the usefulness in efficient management of work, and negatively to uncertainties in responsibility and a nominal role of the committee. 5. Most of the respondnts thought that the concern of the top manager about the committee was the most important factor for the improvement of committee system at hospitals. They also regarded that formalization of the objectives and regulation rule and composition of committees with members which work in various fields as essential. Further studies on the organizational and operational cjharacteristics which include general hospitals in various areas are required.

병원 간호단위관리자의 다면평가($360^{\circ}$ feedback)에 관한 연구 (A Study on $360^{\circ}$ Feedback of Nursing Unit Manager in a Hospital)

  • Purpose: The purpose of this study was to develop 360 feedback for nursing unit manager based on the current personnel evaluation system and to evaluate the new tool according to evaluation subjects. Total of 277 subjects of nurse unit managers and staff nurses were participated in this study. Method: The study was conducted in three phases each for development, application, and analysis of 360 feedback. SAS program was utilized for data analysis with descriptive statistics, t-test, and analysis of variance. Result: The evaluation criteria of the developed 360 feedback tool consisted of 13 subscales such as professional knowledge, apprehension & judgement, job performance, applicability, creativity, leadership, responsibility, promptness & accuracy, administrative ability & sense of mission, activeness, cooperation, communicability, and general attitude. The internal consistency of the tool was Cronbach's alpha .939. The evaluation score by! peers(M=4.30) was the highest one, followed by self-evaluation(M=4.23), evaluation by supervisor(M=4.17), and evaluation by subordinate(M=4.10). The differences in the total evaluation scores among the subjects supervisor, self, peer, and subordinate were not statistically significant, but significant differences were found in some subscales scores. Conclusion: Further research is required to test the reliability and validity of the $360^{\circ}$ feedback tool, and to test the outcome and the process of $360^{\circ}$ feedback system.

간호사의 감성지능이 자기효능감과 직무스트레스에 미치는 영향 - 자기효능감의 매개효과 검증 - (The Effect of Emotional Intelligence on Self-Efficacy and Job Stress of Nurses - Mediating Role of Self-Efficacy -)

  • Purpose: This study was to investigate the relationship among emotional intelligence, self-efficacy and job stress of nurses. Method: Data was collected from convenient sample of 265 nurses who work for a university hospital in a city. The Questionnaire measured the level of emotional intelligence, self-efficacy and job stress of nurses. The data were analyzed using descriptive statistics, t-test, ANOVA, scheffe' test, pearson's correlation coefficient and hierarchical multiple regression. Result: The mean score of emotional intelligence was 3.41(${\pm}.37$), self-efficacy was 3.27(${\pm}.45$) and job stress was 3.35(${\pm}.65$). There were significant differences on self-efficacy according to age, marital status and education level. And there were significant differences on job stress according to age, working unit. It was a significant positive correlation between emotional intelligence and self-efficacy. The emotional intelligence and self-efficacy were significantly negative correlation with job stress. The emotional intelligence explained 24.8% of self-efficacy and 1.9% of job stress and self-efficacy explained 2.7% of job stress. And the self-efficacy was showed a mediate variable between emotional intelligence and job stress. Conclusion: To decrease job stress, nursing managers ought to develop the emotional intelligence and self-efficacy of nurses, reinforce as a mediating role between emotional intelligence and job stress

장시간 근로와 산업재해와의 관계 (The Relationship between Long Working Hours and Industrial Accident)

  • Purpose: This study analyzed the relationship between working hours and the occurrence of industrial disasters based on manufacturing businesses that are members of Federation of Korean Trade Unions.Methods: Repliers were managers of the safety & health department in the union, and 215 surveys out of 300 businesses that agreed to participate in the research were analyzed. Results: 94 out of 300 companies, which counted for 43.7%, replied to have experienced industrial disasters. 56 of those (59.6%) had less than 52 working hours per week, and 38 companies (40.4%) had more than 52 working hours per week. 109 companies, which counted for 50.7%, did not have an incident of industrial disaster. Companies with an average of more than 52 hours of working hours per week were shown to have a 2.29 times (95% CI 1.08~4.87) higher possibility of having industrial disasters than those with less than 52 hours of working hours. Conclusion: As the study showed that industrial disasters were more likely to occur in businesses with working hours longer than 52 hours, ways to reduce working hours need to be developed and various safety measures need to be taken to prevent industrial disasters in case workers are undergoing long working hours.

보건교육 정보시스템 개발에 관한 연구 (A Study on Structural Modeling of Activation of the Information System Utilization in the Health Education)

  • The general objectives of this study were to develop a health education management information system to effectively deal with community health problems. This study aimed at 1) to development an health education management information system, and 2) to offer computer-based communication channels among the District Health System components such as health center, health subcenters, and community hospital, 3) lastly, to identify the key issues and effectiveness of health education. Major findings of the study were as follows: The major benefits and significances of this information system included: improvement of quality of health education statistics by reducing manual data processing, improvement of productivity of health educators by reducing paper works, improvement of decision-making capability of managers by providing more information for planning, organizing, and evaluating health education programes, and improvement of communication flow among health institutions. Based on the findings of the study, the following are recommended: (1) The health education information system will connect with computerized information systems of various health-related institutions in a district and computer-based communication channels among them, and of the superior agencies in the future. (2) The major functions of the computerized health education program are: to keep client medical records, to inquire about information on the client and his family's history. (3) The program will provide outputs in various forms, such as files for patient records, data on some chronic diseases, information on the patient and his family members, and various kinds of statistics.

일부 치과위생사의 근무환경이 직무만족과 조직몰입에 미치는 영향 (The effect of dental hygienist's work environment on job satisfaction and organizational commitment)

  • Objectives: The purpose of this study is to analyze the descriptive correlation of the commission intended to provide effective working practices to enhance job satisfaction and organizational engagement by examining the work environment of the dental hygienist. Methods: After the IRB approval, the subjects were dental hygienists working for dental hospitals and clinics in Busan from May 1st to May 31th of 2017. And the final 153 questionnaires were analyzed to SPSS 23.0 for windows, SPSS Inc. Chicago, IL. USA. The data was analyzed using average standard deviation, t-test, one-way ANOVA and multiple regression analysis. Results: The average working environment of the subjects was found to be $3.37{\pm}0.53$ points with the average job satisfaction of the subjects of $3.29{\pm}0.50$ points and the average organizational commitment of the subjects of $3.25{\pm}0.59$ points. As the working conditions of the working environment are higher, it revealed that the higher the working environment, the better the job satisfaction. Conclusions: In conclusion, this study showed that the ability to support managers' abilities, leadership, and dental hygienists is the most influential factor in job satisfaction and organizational involvement. Also, hospital administrators are encouraged to provide excellent work and leadership skills to enhance both the quality of work and that of work ethic.

간호사의 공감만족에 대한 국내 연구 분석 (Analysis of Research on Compassion Satisfaction among Nurses)

  • 본 연구는 간호사의 공감만족에 대한 국내 연구의 동향을 분석하여 향후 연구에 대한 방향을 제시하고자 시도되었다. 연구조사 데이터베이스는 학술정보서비스, 국회도서관, KISS, NDSL을 이용하였다. 주요어는 간호사, 공감만족, 전문직 삶의 질 로 검색하여 최종 18편의 논문을 분석 하였다. 연구 설계는 서술적 조사연구 위주였고, 연구 대상은 병원에 근무하는 간호사로 나타났다. 공감만족과 관련이 있는 일반적 특성으로는 연령, 직위, 경력, 교육상태 등이었으며, 관련변수로는 소진, 공감피로, 직무만족, 극복력 등으로 나타났다. 공감만족과 관련이 있는 변수를 살펴본 결과 나이가 어리고 경력이 짧은 미혼인 간호사의 공감만족이 상대적으로 낮은 것으로 나타나 조직적 차원에서 지지 프로그램이 요구된다. 간호사의 공감만족은 임상실무에서 간호의 질 향상과 간호인력 관리에 기여 할 것이다. 따라서, 간호사가 간호를 하면서 높은 공감만족을 가질 수 있도록 간호 관리자의 관심과 노력이 매우 필요하다. 본 연구의 결과를 통해 국내 간호사에게 적합한 도구개발 연구, 간호사의 공감만족에 대한 본질적인 실체를 밝히는 질적 연구와 프로그램을 개발하여 효과를 검증하는 중재 연구를 제언한다.
