Mechanism of Decrease in Lung Injury by Low Dose of Endotoxin During Hyperoxia in the Rats (저용량의 내독소가 쥐에서 고농도의 산소에 의한 급성폐손상을 경감시키는 기전)

  • 배 경 : 쥐를 고농도의 산소에 60시간 이상 노출시켰을 때 급성 폐손상이 유발되지만 내독소를 저용량으로 투여시에는 이러한 폐손상이 경감된다고 알려져 있으나 그 기전에 대하여는 확실히 밝혀지지 않고 있다. 산화질소(nitric oxide, NO)는 내독소나 염증성 사이토카인(cytokine) 등의 자극에 의해서 폐내 여러 염증세포에서 만들어지며 이 산화질소는 경우에 따라 우리 몸에 이롭거나 해로운 양면성을 지니고 있다. 저자들은 쥐에서 고농도의 산소에 의한 폐손상이 저농도의 내독소 투여로 경감되는 기전에, 산화질소가 중요한 역할을 하는지 또는 황산화효소나 다른 항염증성 사이토카인이 중요한 역할을 하는지를 규명하고자 하였다. 방 법 : 총 120마리의 쥐 (Sprague-Dawley rat)를 24마리씩 5군으로 나누어 대조군은 실내 공기를, 고농도 산소군은 100%의 산소를 100%의 산소를 60시간 투여하였고 내독소군은 100% 산소 투여시 2일간 저용량의 내독소를 투여하였다. 다른 두 군은 산화질소 합성 억제물인 aminoguanidine(AG)과 N-nitro-L-arginine methyl ester (L-NAME)를 각각 2일간 고농도 산소와 내독소에 더하여 투여하였다. 각각의 군에서 폐손상의 정도와 사망률을 관찰하고 superoxide dismutase(SOD), catalase, nitric oxide, IL-6, IL-11을 기관지폐포세척액에서 측정하고, 고농도산소 투여군의 폐조직에서 iNOS synthase rnRNA의 발현을 비교하였다. 결 과: 1. 100%의 산소에 60시간 노출시켰을 때 쥐의 사망률은 8.3% 이었고 내독소 투여군은 4.2%, NAME 투여군이 37.5%, AG 투여군이 25%로 산화질소 합성 억제제에 의하여 사망률의 증가가 관찰되었다. 2. 폐의 손상 정도를 나타내는 폐의 wet/dry 중량비와 늑막액도 100%의 산소에 노출된 군에서 증가되었고 내독소 투여에 의하여 감소되었으며 NAME나 AG 투여군에서는 오히려 증가되었다. 3. 이러한 내독소에 의한 폐손상 억제효과가 항산화효소인 SOD나 catalase, 또는 protective cytokine인 IL-6나 IL-11등의 증가와 관련이 있는지를 관찰하였으나 이들 모두에서 유의한 변화를 관찰하지 못하였다. 4. 산화질소는 100% 산소에 노출시킨 군에서도 증가하였으나 내독소 투여군에서 유의하게 더욱 증가하였고 이는 L-NAME 나 aminoguanidine의 투여시 감소하였다. 5. iNOS mRNA의 발현도 내독소 투여군에서 유의하게 증가하였다. 결 론 : 쥐의 고농소 산소 투여에 의한 폐손상은 저용량의 내독소 투여로 경감되며, 이는 주로 내독소 투여에 의한 iNOS mRNA의 발현을 유도하여 생성된 산화질소의 증가에 기인하는 것으로 생각된다.

  • 이 연구는 브랜드 네임 스펙트럼 이론을 이용하여 병원 명칭의 특성을 분석하였다. 연구대상은 수도권 지역에 위치하는 모든 병원을 대상으로 하였고, 명칭은 의미있는 단위로 분류한 후 스펙트럼 이론에 따라 분류하였다. 분석결과 임의적 명칭의 빈도가 가장 높았고, 다음으로는 암시적, 서술적, 그리고 조어적 명칭의 순이었다. 단일 스펙트럼 구성된 명칭이 가장 많았으며, 스펙트럼이 2개와 3개로 구분된 명칭의 순이었다.

A Study on the Relationship between the Name Awareness of Public Medical Institutions and Patients' Intent to Revisit (공공의료기관 인지도와 재방문여부와의 관계 연구)

  • This study has significance in terms of its intent to contribute to the increase in hospital revisits by proposing measures to enhance name awareness through an examination of the relationship between the name awareness of public medical institutions and patients' revisit, and its corresponding influences. The study findings were drawn by employing analysis methods such as frequency analysis, reliability and exploratory factor analysis, t-tests, ANOVA, correlation analysis, and hierarchical multiple regression analysis. The results of the correlation analysis showed statistically significant correlations between all independent variables and the patients' revisit or not. The perception of service quality and the environmental factor of medical treatment increased the explanatory power of independent variables, thereby exhibiting a high correlation with the revisit to medical institutions. The usual image, medical service, and medical treatment environment were revealed to have a positive influence on the patient's intent to revisit. This study could confirm a correlation between the name awareness of medical public institutions and the patients' revisit or not. Consequently, to improve the quality and infrastructure of their medical services, public medical institutions are required to perform studies on measures to increase the revisits of patients by identifying their image, medical services, and treatment environment on a consistent basis.

Involvement of nitric oxide and prostanoid on the endothelium -dependent vasodilatation by acetylcholine in the isolated rabbit renal artery (토끼 적출 신동맥에 있어서 acetylcholine에 의한 내피세포 의존성 이완작용에 대한 nitric oxide와 prostanoid의 연관성)

  • The effect of the nitric oxide synthase(NOS) inhibitor, $N^{G}$-nitro-L-arginine methyl ester (L-NAME), and the prostanoid synthesis inhibitor, indomethacin, on the vasodilatation produced in response to acetylcholine(Ach) on the isolated rabbit renal artery was examined. The vasodilatory reponses to Ach($10^{-8}-3{\times}10^{-5}M$) were completely absent in thevessel which the endothelium had previous been removed. L-NAME($10^{-4}M$) significantly reduced the vasodilatory reponse to the Ach($10^{-8}-3{\times}10^{-5}M$). When L-arginine ($10^{-3}M$) was also present in the organ bath along with L-NAME($10^{-4}M$), this inhibitory effect of L-NAME ($10^{-4}M$) on the vasodilatory response to Ach ($10^{-8}-3{\times}10^{-5}M$) was significantly attenuated, Indomethacin ($10^{-6}M$) did not significantly affect the vasodilatory responses to Ach ($10^{-8}-3{\times}10^{-5}M$). The inhibition by L-NAME ($10^{-4}M$) and indomethacin ($10^{-6}M$) on vasodilatory response to Ach was significantly greater than the inhibition due to L-NAME ($10^{-4}M$) alone. The present study has established that Ach induce relaxation via and endothelium-dependent mechanism, this relaxation to Ach involves both nitric oxide(NO) and prostanoid in the isolated rabbit renal artery.

Blunted Indomethacin-Induced Downregulation of Aquaporins by Nitric Oxide Synthesis Inhibition in Rats

  • The present study was aimed to determine whether nitric oxide (NO) plays a role in the regulation of aquaporin (AQP) channels in the kidney. Male Brattleboro rats ($250{\sim}300\;g$ body weight) were used. The experimental group was treated with $N^G$-nitro-L-arginine methyl ester (L-NAME, 100 mg/L drinking water) for 1 week, and cotreated with indomethacin (5 mg/kg, twice a day, i.p.) for the last two days. Control groups were treated with either L-NAME for 1 week, indomethacin for 2 days, or without any drug treatment. The abundance of AQP1, AQP2 and AQP3 proteins in the kidney was determined by Western blot analysis. Indomethacin downregulated AQP channels, whereas L-NAME by itself showed no significant effects on them. The indomethacin-induced downregulation of AQP2 and AQP3 was significantly blunted in L-NAME-treated rats, while that of AQP1 was not affected. These results suggest that endogenous NO, when stimulated, may downregulate AQP channels that are specifically regulated by AVP/cAMP pathway in the kidney.

Dexmedetomidine inhibits vasoconstriction via activation of endothelial nitric oxide synthase

  • Despite the complex vascular effects of dexmedetomidine (DEX), its actions on human pulmonary resistance arteries remain unknown. The present study tested the hypothesis that DEX inhibits vascular tension in human pulmonary arteries through the endothelial nitric oxide synthase (eNOS) mediated production of nitric oxide (NO). Pulmonary artery segments were obtained from 62 patients who underwent lung resection. The direct effects of DEX on human pulmonary artery tension and changes in vascular tension were determined by isometric force measurements recorded on a myograph. Arterial contractions caused by increasing concentrations of serotonin with DEX in the presence or absence of L-NAME (endothelial nitric oxide synthase inhibitor), yohimbine (${\alpha}_2$-adrenoceptor antagonist) and indomethacin (cyclooxygenase inhibitor) as antagonists were also measured. DEX had no effect on endothelium-intact pulmonary arteries, whereas at concentrations of $10^{-8}{\sim}10^{-6}mol/L$, it elicited contractions in endothelium-denuded pulmonary arteries. DEX (0.3, 1, or $3{\times}10^{-9}mmol/L$) inhibited serotonin-induced contraction in arteries with intact endothelium in a dose-dependent manner. L-NAME and yohimbine abolished DEX-induced inhibition, whereas indomethacin had no effect. No inhibitory effect was observed in endothelium-denuded pulmonary arteries. DEX-induced inhibition of vasoconstriction in human pulmonary arteries is mediated by NO production induced by the activation of endothelial ${\alpha}_2$-adrenoceptor and nitric oxide synthase.

Classification of Aroma Essential Oils by Sasang Constitution (아로마 에센셜 오일의 사상체질의학적 분류제목)

  • 1. Objectives: Aromatherapy is one of the alternative medical treatments by flavors of essential oils extracted from plants. This study was to investigate the classification of aroma essential oils by Sasang Constitution for clinical applications. 2. Methods: 1) We searched scientific name of aroma essential oils and scientific name of herbs, classify aroma essential oils according to Sasang Constitution. 2) If the method 1) is impossible, sort them by regard of aroma essential oils' flavors, specific characters and efficacies. 3. Results: Aroma essential oils suitable for Soeumin are 27 kinds, for Soyangin and Taeeumin are each 16 kinds, and for Taeyangin are 5 kinds.(Duplicates are allowed.) The cases which can not be classified clearly, are 10 kinds, so they are able to consider the Constitution of the more than two-phase. 4. Conclusions: Most of aromatic herbs are warm and spicy by pre-reports. So, aroma essential oils suitable for Soeumin are most often. After this study, studies about Sasang Constitutional herb recipes and more clinical studies are needed.

Clinical Study of Acupuncture Effect by Measuring Amperage from Chong points (정혈(井穴)의 도전량(導電量) 측정에 의한 무자(繆刺) 및 거자법(巨刺法)의 유용성에 관한 임상 고찰)

  • Objective : This study is performed to evaluate the clinical effect of acupuncture to acu-points in the normal side by means of amperage measured from chong points. Methods : Among the outpatients with ankle sprain who visited to Department of Acupuncture & Maxibustion, Jaseng oriental medicine hospital from 21, December 2000 to 10, February 2001, we selected 11 persons who showed normal finding in the X-ray. They were divided into two groups. One group was treated with acupuncture to acu-points in the painful side and the other was treated with acupuncture to acu-points in the normal side. Before and after acupuncture, we measured the amperage from their chong points. We selected 10 persons who we thought they were healthy as a control group. We also measured the amperage from their chong points. Results : Between the control and the ankle sprain group, there were significant differences of differences between the amperages of Rt. chong point and Lt. thong point of the same name. Between the group treated with acupuncture to acu-points in the painful side and the group treated with acupuncture to acu-points in the normal side, there were no significant differences of differences between the amperages of Rt. chong point and Lt. chong point of the same name. Conclusion : This result indicated that the acupuncture to acu-points in the painful side was not less effective than the acupuncture to acu-points in the normal side, and so the more study is needed continuously.

The Comparison of Curriculums of Physical Therapy Education Department in the World (세계 물리치료학과 교과과정 비교 분석)

  • Purpose : The purpose of this study was to investigate curriculums of physical therapy department in order to provide the basic data for developing educational standard. Methods : We identified the curriculums of physical therapy department in the world from January to may 2010 by internet, e-mail, fax and telephone. We compared the credits, education semesters, the names of departments, the numbers of departments for physical therapy education in all countries of the world and suggested some alternative for improvement. Results : The mean credits of 25 senior colleges were 140 and the mean credits of 22 Junior colleges were 136.6 in south Korea. The mean educational semesters and the mean credits were 9 semesters, 175.8credits of 1,363 universities in 73countries in the world. Of the 1,289 universities in the world, South Korea was ranked the fifth in the number of existing physical therapy education institutions with the number of 72. The name of department were comprised of physical therapy(1,289, 94.6%) and compound name(75, 5.4%) Conclusion : We suggest that the KAPT gradually rearranges senior college(4years), 170 creditS and consider time of community services and forms commission on accreditation in physical therapy education for developing educational standard. Also, we suggest to change the name of department into compound name because the student can have a chance to studies various subjects and get a chance to more occupations. We hope that the results of study can be helpful in order to rearrange the curriculum and the KAPT encourages the universities about following the educational standard.

The Effects of Brand Evidence on the Customer Satisfaction and Brand Verdict in Hospital (병원 브랜드 증거가 고객만족과 브랜드 판단에 미치는 영향)

  • The objective of this study is to identify the effect of brand evidence on customer satisfaction, and the effect of customer satisfaction on brand verdict in case of hospital management. To achieve this objective, 250 hospital customers were surveyed in Busan and Gyeongnam province. Of these, 220 respondents were used for the analysis. The research results show that brand name, employee service, core service and feelings have a positive effect on customer satisfaction. However, services cape failed to affect customer satisfaction.
