Development of Pressure Ulcer Management Guideline by Adaptation Process (수용개작방법을 활용한 욕창간호 실무지침 개발)

  • Purpose: This study was done to develop an evidence-based nursing clinical practice guideline (PU CPG) for pressure ulcer prevention and management in Korea. Methods: The guideline adaptation process was used and conducted according to the guideline adaptation manual developed by Gu et al. (2012) which consists of three main phases and 9 modules including a total of 24 steps. Results: The newly developed PU CPG included the introduction, pressure ulcers, summary of recommendations, recommendations, references, appendices, and glossary. The total number of recommendations was 148 in 4 sections (organizational policy, assessment, pressure ulcer prevention and management, and education) and 16 domains. Of the recommendations 4.7% were graded A, 16.9%, B, and 78.4%, C. Conclusion: Results indicate that this new PU CPG is an evidence-based practice guidance for pressure ulcer prevention and management and can be recommended for dissemination and utilization by nurses nationwide to improve the quality of pressure ulcer prevention and management. Regular revision is recommended.

Useful evaluation of 3D target location correction by using Xsight spine tracking system in CyberKnife (사이버나이프에서 Xsight spine tracking system을 이용한 3D 표적위치보정의 유용성 평가)

  • The purpose of this study is to evaluate utility of rotating adjustment using Xsight spine tracking system in 3D DOF location adjusting method, to minimize error between 6D DOF and 3D DOF in fiducial tracking system. In this study, the result of 6D DOF target location error is $0.124{\pm}0.058mm$, using fiducial inside tumor 3D DOF $0.673{\pm}0.142mm$, outside tumor $1.126{\pm}0.253mm$, apply with Xsight spine tracking system 3D DOF $0.542{\pm}0.103mm$. As the experiment shows, it was demonstrated that rotating adjustment through Xsight spine tracking system is valuable in case of treatment in 3D DOF location error that makes increasing accuracy and dose distribution each approximately 48% and 3%. In accordance with result of this study is useful rotation.

A Study on the Academic Achievement and the Needs of Prior Field Learning in a College (일개 대학의 선행 현장수업의 필요성과 학업성취도에 관한 연구)

  • Purpose: The purpose of this study is to examine the needs of prior field learning and the academic achievement of field experience learning in a college. Methods: This study was performed from May 1 to October 30, and students were given questionnaire. The research questionnaire as follows: (1) to investigate the academic achievement after field experience learning, (2) to verify the needs of field experience learning. A statistical analysis was performed using SPSS 17.0 for window version. Results: The results was as follows : First, satisfaction of field learning had scored good(47.2%) in lesson goal, good(51.8%) in acquisition of knowledge and techniques, good(51.0%) in preparation of study and good(45.9%) in association. Second, curriculum of field learning had scored normal(35.5%) in prior education, good(47.4%) in composition, good(50.8%) in guidance and good(47.2%) in contents. Third, curriculum of field learning had scored good(44.6%) in duration, good(46.1%) in numbers, good(51.3%) in convenience and normal(38.1%) in means of transportation. Forth, needs of field learning had scored good(46.6%) in field learning of practicum, good(48.2%) in field learning of theory subject, 3-4 times(42.0%) in frequency of field learning and 2hours(57.3%) in a field learning hour. Conclusion: These findings suggest that college student's thinking of field experience learning is positive. Field experience learning provided that college students have directly an opportunity of gaining valuable experience to feel the field.

Usefulness of a new polyvinyl alcohol hydrogel (PVA-H)-based simulator for endoscopic submucosal dissection training: a pilot study

  • Background/Aims: We developed a new endoscopic submucosal dissection (ESD) simulator and evaluated its efficacy and realism for use training endoscopists. Methods: An ESD simulator was constructed using polyvinyl alcohol hydrogel sheets and compared to a previous ESD simulator. Between March 1, 2020, and December 30, 2021, eight expert endoscopists from three different centers analyzed the procedure-related factors of the simulator. Five trainees performed gastric ESD exercises under the guidance of these experts. Results: Although the two ESD simulators provided overall favorable outcomes in terms of ESD-related factors, the new simulator had several benefits, including better marking of the target lesion's limits (p<0.001) and overall handling (p<0.001). Trainees tested the usefulness of the new ESD simulator. The complete resection rate improved after 3 ESD training sessions (9 procedures), and the perforation rate decreased after 4 sessions (12 procedures). Conclusions: We have developed a new ESD simulator that can help beginners achieve a high level of technical experience before performing real-time ESD procedures in patients.

  • The liver imaging reporting and data system (LI-RADS) has been developed with the support of the American College of Radiology to standardize the diagnosis and evaluation of treatment response of hepatocellular carcinoma (HCC). The CT/MRI LI-RADS version 2018 has been incorporated in the American Association for the Study of Liver Diseases guidance. This review examines the effect of CT/MRI LI-RADS on the standardized reporting of liver imaging, and the evidence in diagnosing HCC and evaluating treatment response after locoregional treatment using CT/MRI LI-RADS. The results are compared with other HCC diagnosis guidelines, and future directions are described.

The Effect of Career Barrier, Satisfaction and Career Decision Making Pattern on Career Search Behavior in Male Nursing Students (남자 간호대학생의 진로장벽, 전공만족도 및 진로의사결정유형이 진로탐색행동에 미치는 영향)

  • Purpose: The study was conducted to investigate the effect of career barrier, satisfaction, and career decision making pattern on career search behaviors in male nursing students. Methods: Data were collected through structured questionnaires that included general characteristics, career barrier, satisfaction, career decision making pattern and career search behaviors and were analyzed by descriptive statistics, ANOVA, t-test, Pearson correlation coefficient and multiple regression using SPSS/PC+ program. Results: The results of the research suggest the two aspects of career search behaviors as being self-exploration and environmental exploration. The rational pattern, among the types of satisfaction and career decision making pattern, was found to have a meaningful effect on self-expression. On the other hand, dependent pattern, among the satisfaction and career decision making pattern, was found to have an effect on career search patterns. Conclusion: It is considered that, based on the results of this study, diversified approaches that consider individual career decision making patterns such as rational pattern and dependent pattern as well as the methods to enhance the satisfaction of male nursing college students for career guidance are needed.

Radiofrequency catheter ablation of atrioventricular nodal reentry tachycardia in children and adolescents: a single center experience

  • Purpose: Atrioventricular nodal reentry tachycardia (AVNRT) is less common in pediatric patients than in adult patients. Thus, data for pediatric AVNRT patients are insufficient. Hence, we aimed to analyze the patient characteristics, treatment, and any recurrences in pediatric AVNRT patients. Methods: We reviewed the records of 50 pediatric AVNRT patients who had undergone radiofrequency catheter ablation (RFCA) between January 1998 and December 2016 at a single regional center. The patients were aged ${\leq}18years$. Results: Among 190 pediatric patients who underwent RFCA for tachyarrhythmia, 50 (26.3%; mean age, $13.4{\pm}2.6years$) were diagnosed as having AVNRT by electrophysiological study. Twenty-five patients (25 of 50, 50%) were male. Twenty patients (20 of 50, 40%) used beta-blockers before RFCA. All patients had no structural heart disease except 1 patient with valvular aortic stenosis and coarctation of the aorta. RFCA was performed using the anatomic approach under fluoroscopic guidance. The most common successfully ablated region was the midseptal region (25 of 50, 50%). Slow pathway (SP) ablation and SP modulation were performed in 43 and 6 patients, respectively. Complication occurred in 1 patient with complete atrioventricular block. During follow-up, 6 patients had recurrence of supraventricular tachycardia, as confirmed by electrocardiography. Among them, 5 underwent successful ablation at the first procedure. In 1 patient, induction failed during the first procedure. Conclusion: RFCA is safe and effective in pediatric AVNRT patients. However, further research is needed for establishing the endpoints of ablation in pediatric AVNRT patients and for identifying risk factors by evaluating data on AVNRT recurrence after RFCA.

Fluoroscopic Radiation Exposure during Percutaneous Kyphoplasty

  • Objective: The author measured levels of fluoroscopic radiation exposure to the surgeon's body based on the different beam directions during kyphoplasty. Methods: This is an observational study. A series of 84 patients (96 vertebral bodies) were treated with kyphoplasty over one year. The patients were divided into four groups based on the horizontal and vertical directions of the X-Ray beams. We measured radiation exposure with the seven dosimetry badges which were worn by the surgeon in each group (total of 28 badges). Twenty-four procedures were measured in each group. Cumulative dose and dose rates were compared between groups. Results: Fluoroscopic radiation is received by the operator in real-time for approximately 50% (half) of the operation time. Thyroid protectors and lead aprons can block radiation almost completely. The largest dose was received in the chest irrespective of beam directions. The lowest level of radiation were received when X-ray tube was away from the surgeon and beneath the bed (dose rate of head, neck, chest, abdomen and knee: 0.2986, 0.2828, 0.9711, 0.8977, 0.8168 mSv, respectively). The radiation differences between each group were approximately 2.7-10 folds. Conclusion: When fluoroscopic guided-KP is performed, the X-Ray tube should be positioned on the opposite side of the operator and below the table, otherwise the received radiation to the surgeon's body would be 2.7-10 times higher than such condition.

Rearing Behavior and Rearing Stress of Fathers with Children of Preschool and School Age

  • Purpose. This study was conducted to compare the paternal rearing behavior and rearing stress level between fathers with a preschooler and fathers with school children so that it can be utilized as a basic source for developing parental rearing education programs. Methods. A descriptive comparative methods was conducted to identify the paternal rearing behavior and paternal rearing stress. Respondents were 361 fathers who had either preschoolers (n=189) or children of elementary age (n=l72). Results. Comparing the two group's means, the rearing activity score and rearing stress there were significant differences. In the school children's group's father, 'outdoor activity' and 'guidance on discipline activity' were significantly higher than the other group. In the preschool children's fathers group, 'play interaction activity' was statistically significant higher than the other, and the child-part mean score of paternal rearing stress was significantly higher than the other group. The correlation between paternal rearing behavior and paternal rearing stress, it can be shown that more paternal rearing behavior means less paternal rearing stress. Conclusion. These results of this study will help design rearing program for fathers that have either preschool children or school children by providing the basic data for paternal rearing behaviors and paternal rearing stress.

Literary Review of Therapeutic Activity of Psychiatric Patients (정신과 환자의 치료적 활동에 관한 문헌적 고찰)

  • More and Tore emphasis is put on moral treatment in the care of psychiatric patients. Methods employed for moral treatment rake use of therapeutic activity for the mentally ill patient. In some psychiatric hospitals all of the therapeutic activities are categorized as occupational therapy, while in others these activities are departmentally categorized -recreational, musical, occupational and bibliotherapy. All of these constitute separate departmental therapies. By research, it is known at this time, that the above activities have been of great help in personality integration, reeducation and early recovery of patients, as well as in their social adjustment after discharge. The goal of these therapeutic activities incorporate the goals set by healthier interpersonal relationship and psychotherapy. Put the therapeutic activities require a prolonged time of therapy for effectiveness, economic support, special trained personnel, careful planning, guidance and proper evaluation. Therefore this treatment is a very complicated one. But it may be the most profitable therapy for certain patients. One true fact about this treatment is, that simple recreation, occupation, music, reading, etc., make the patients life in the ward more interesting and have a good effect on the interpersonal relationship between patients. Thus it has been proved that this kind of treatment demands the active and full participation of the personnel and the full support of the hospital administration.
