Methods: The participants were 12 APNs who had experience in home health nursing for over 5 years. Data were collected through two focus group interviews between August and September, 2017. The results were analyzed using a qualitative content analysis. Results: Out of 10 sub-categories and 29 codes, 4 categories emerged: "crushed by the responsibilities as the only healthcare professional", "worn out by family who interfere with work", "anxiety about my safety being threatened" and "using self-learned know-hows". Conclusion: APNs face difficulties due to the environmental characteristics of the homes, the characteristics of chronically critically ill (CCI) patients and their families, and legal or systematical limitations when dealing with problems based on their personal competence. • The nursing home for the elderly has a weak point for evacuation from the fire because most of all the users are suffering from a dementia or paralysis. When it comes to night shift, it is much difficult to get them evacuated due to the lack of staffs. In addition to that, the nursing home for the elderly has such a strict control system for access that does not have a utility for escape opening. That is why they need the differentiated evacuation planning. This study is to consider the point at issues on the present conditions of law, system, and evacuation plans for the nursing home for the elderly that has some difficulties because of impaired functions of residences. This study dealt with the point at facilities and operations of the emergency evacuation system. First of all, for the point at the facilities, I found that it need to be considered on the characteristics of the elderly who has any difficulties of the self evacuation. And the established regulation did not reflect the characteristics of closed area of the nursing home for the elderly. The second, for the point at the operation of the institutions, it is pointed that there seems to be a lack of ways to manage to a emergency situation and educations on the operation staff. And the last, for the point at the operation, the requirement for the managers of fire prevention facilities are similar to that of a normal building. Furthermore there is a lack of corporation with the fire station.

The experimental group of the wives had lower stress index than the control group, the experimental group of the husband showed lower stress index than the control group, (t=-3.14, p=.002). The wife (t=-3.75, p=.001) and husband (t=-4.20, p=.001) of the experimental group showed lower depression scores than the control group. Both the wife (t=3.86, p=.001) and husband (t=5.28, p=.001) showed higher scores for quality of life compared to the control group. It was found that this holistic healthcare home visit program is an effective program to make improvements on stress, depression and quality of life for multi-cultural couples. • To improve the quality of healthcare and build up the health delivery system, electronic health record system is important for healthcare institutions providing health services. However, the systems currently operated in domestic healthcare institutions use independent software interface with distributed environment for data search and process. Therefore, it raises an additional expense to buy or develop each interface module when there is a link to new system and the problem of complexity. To solve these problems, this study implemented the service oriented architecture basis for home nursing service and carried out an assessment. The study defined the process modeling and business requirements based on the scenario of service and drew 17 suitable services based on five verification items for service design. To draw the final service, seven services were selected by service litmus test (SLT). The study found that, by the improvement of business process for healthcare service support department, SOA based information system has an effect of time reduction of patient processing time. In conclusion, to enable the hospital information system to cope with various demands of healthcare consumers, it is necessary to consider the application of SOA technology which has excellent interoperability, reusability and maintenance.

  • Purpose: This study aimed to clarify the needs for hospital-based home care nursing medical services in elder care institutions by analyzing the details and frequency of medical services provided by, and the needs for, hospital based home care nursing in select institutions in Korea. Methods: Seventy-seven staffs at elderly care institutions located throughout the country completed self-report questionnaires between February 1 and May 31, 2009. SPSS ver. 14.0 was used for data analysis regarding frequency and percentage, mean and standard deviation. Results: Forty-eight hospital-based home care nursing medical services in eight domains were identified as being needed in elderly care institutions. The most commonly used medical services were providing instruction in oral drug administration, checking drug beneficial/adverse effects, and administering blood glucose test, while the most needed medical services requiring hospital based home care nursing were complex pressure ulcer care, followed by diabetic foot ulcer management and nutrient injection. Conclusion: The present results should provide fundamental data for better healthcare services with hospital based home care nursing at elderly care institutions as part of a 'win-win' strategy through which medical expenses are reduced, insurance costs are kept stable, and safe and high-quality medical services are provided for residents of elder care institutions. Political decisions intended to promote visits by hospital based home care nurses to elder care institutions would be a prudent course.
