Developing Scenario for Elderly Residents' Behaviors at Home Using Persona-based Scenario Method (퍼소나 기반 시나리오 방법론을 적용한 주택에서의 노인 행위 시나리오 개발)

  • An analysis of elderly behavior at home needs to be meticulously understood in order to build a spatial environment where the elderly can live safe and independent lives. To this end, the following study analyzed elderly behavior that occurs in the home, constructed personas, and developed persona-based scenarios of elderly behavior at home. Persona was composed of behavioral characteristics, spatial characteristics, and personal characteristics (gender, age, disease etc) that have a direct effect on living activities in the home, so it was intended to express a more real persona. The study developed six personas based on the behavioral and mobility characteristics of the elderly, and scenarios for each persona developed bsed on the following eight behaviors: toilet use, bating/Shower, washing and grooming, having meal, sleeping, dressing/undressing, laundering clothes, and getting out of home). Through this, the study determined the characteristics and difficulty level of various sub-behaviors and actions that occur in elderly behaviors at home. It also determined characteristics related to the use of physical elements such as equipment and furniture that are a part of those sub-behaviors. The behavior scenarios developed in this study can be utilized to understand the difficulties involved in elderly behavior, and to present a spatial plan and remodeling direction that alleviates such difficulties. This study is also significant that it investigates the applicability of persona-based scenario method on residential architecture.

A Study on Local Variations of Elderly Welfare Facilities by Care Type (노인복지시설 유형별 지역적 편차에 관한 연구)

  • This study purposed to examine elderly welfare facilities by type, to analyze their local variations, and ultimately to contribute to the expansion of elderly welfare infrastructure. The results are expected to help inspect elderly welfare infrastructure for providing the aged with social services before the execution of 'the insurance for elderly long term care' and establish welfare facilities by area in the future. For these purposes, we used the national data "The Current State of Elderly Welfare Facilities in 2007" produced by the Ministry of Health and Welfare. We digitized elderly welfare facilities in 163 cities and counties by type and analyzed them by area. We also examined the differences in the local distribution of representative elderly welfare facilities such as elderly welfare centers, home based facilities (home helper centers), asylums for the aged and elderly care facilities in 16 cities and provinces. Furthermore, we analyzed differences and problems in their local distribution urban areas, mixed areas of urban and rural communities, and rural areas. In addition, we studied the current state of institutionalized care and home based care, which are two major directions of current elderly welfare policies, based on the local distribution of facilities and analyzed differences in the trends according to area. According to these results, the urban had more home based care facilities than the rural. However, the rural had more institutionalized care facilities than urban. Also, each local self-governing body showed unique characteristics. Therefore, these results suggest that we need to establish elderly welfare policies based on the distribution of facility types by area.

A Narrative Inquiry of Experience of the Elderly's Living with Chronic Disease in Hospital-based Home Care Services (병원중심가정간호를 받는 만성질환 노인의 삶의 경험에 관한 내러티브 탐구)

  • Purpose: The purpose of this study is to document the experiences of elderly persons with chronic diseases who are under hospital-based home care services and to find out how they describe the experiences using a narrative inquiry method. Methods: The participants are 7 elderly patients over the age of 70. Data collection and analysis were conducted through Jeong Gwang Soon's six-stages: observing phenomena, selecting participants, talking, talking again, writing, and confirming. Results: The results of data analysis were classified into 4 themes of overall experience related to chronic diseases: response to home health care, overcoming chronic disease, life reconstruction, and 22 categories as common denominators extracted from the elderly persons' lives. Conclusion: This research makes the experiences of the elderly living with chronic diseases more understandable, and it can be utilized as a effective nursing praxis to improve the quality of elderly persons with chronic diseases in hospital-based home care services.

  • Purpose: This study aims to present policy and practical intervention directions by analyzing the level of disaster information understanding ability and influencing factors of the elderly at home and the elderly at facilities. Method: The survey was conducted on the elderly at home and the elderly at home over the age of 65 across the country, including the ability to understand disaster information scale (E-DLS). Results: This study found that the average monthly income, smart phone use, and social support for the elderly at home had a statistically significant impact on their ability to E-DLS. Conclu- sion: Through this study, various measures were proposed to improve the ability of the elderly to understand disaster information.

Development and Clinical Use of Assessment Indicators for Improving Medication Adherence of Elderly Residents in Nursing Home (노인요양원 입소자의 복약순응도 향상을 위한 평가지표의 개발 및 임상활용)

  • Background: South Korea is rapidly being an aging-society and the demand of long-term care insurance services for elderly patients is rising. In addition, because the elderly taking multiple medicines, the adherence is lowered and the adverse events are easily occurred. Therefore, many are interested in introducing the geriatric pharmacy specialist to manage this situation. Purpose: By applying a similar program such as the geriatric pharmacy specialist working in nursing home, we conducted this study to evaluate the potential contribution to both the health insurance services and financial savings for the elderly patients. Methods: We conducted a trial in an elderly nursing home to collaborate between doctor and pharmacist making a checklist for improving medication adherence and establish a consultation system. Also we applied a smart phone application in the pharmaceutical care processes. Results: Thereby completing the drug therapy related checklist apply to nursing facilities in South Korea. And we got a performance that improves medication adherence when used in the clinical practice settings. Conclusion: By introducing a training program of pharmacy care managers and geriatric professionals such as the United States and Japan, we revealed the elderly nursing home residents and vulnerable elderly people living alone were improved the medication adherence and it will contributed to the health and quality of life of the patients.

  • This study aimed to examine the possibility of remodeled nursing home utilizing a closed school in Kunsan, where the number of closed schools increases and the ratio of elderly population grows. Major contents of research were understanding architectural characteristics of closed school and elderly housing in the sample area, and a suggestion of remodeling a closed school into nursing home. Conclusions can be summarized as follows; understanding the high ratio of elderly population and increases of closed schools in sample area, verification of overall possibility in remodeling a closed school into elderly facility, confirmation of availability in changing regular class room to residential rooms, and some proposals of principal points in floor planning - homelike atmosphere by small scaled residential units(10 persons), spatial hierarchy for residents' mental comfort, sliding door for accessibility, kitchen and dining space in residential unit for the residents' participation in houseworks.

Research and Development of Urban Health Infrastructure for Home Health Care for the Elderly (도시의 노인보건방문서비스를 위한 자원 및 모델 개발에 관한 연구)

  • Home health care is one of the important components of health care services. Today, the need and demand for the home health care is increasing. To assess the effects of home visit health services by public health nurses in health center on the stat of health, use of medical services and quality of life by elderly people living at home, a randomized controlled trial was implemented for 65 years or more old people randomly allocated to intervention(93) and control(118) group. Intervention group received 2 visits a month over 6 months. Control group received no home visits. The data was collected in a one-year follow-up survey conducted at Kyongju before and after the intervention which composed of health risk assessment, risk factor education and health related behavior counseling. The prevalence rate of chronic illness was more decreased in intervention group than control group after intervention. The intervention group visited medical facilities less frequently than control group. And the home visit health services encouraged the elderly to practice regular exercise. After the intervention, the score of ADL(activities of daily living), LSI(life satisfaction index) and SSI(social support index) in intervention group were more increased than control group. And the increase of scores was more prominent in 70 years or more old people, female, non-smoker and non-chronic illed elderly rather than others. In conclusion, the regular home visit health services provided by public health nurses were beneficial for the elderly in terms of health promotion and quality of life.

Home visiting oral health program of Longterm home care service and the change of some pathogenic microorganism counts in denture (재가장기요양기관의 방문구강보건교육 전·후 의치의 일부 병원성 미생물의 집락수 변화)

  • Purpose: This study is a repetitive comparative analysis of a qualitative case study that carried out a home visiting oral health education program. Method: This study conducted an interview survey through medical examination by interview before and after the home visiting oral health education and measured some pathogenic microorganism counts in the denture. Results: There was a positive change in the self oral care ability of the elderly at home after home visiting oral health education, including the behavior of self management of dentures, and some pathogenic microorganism counts in the dentures. Conclusion: The home visiting oral health education of home care service centers can improve oral health care for the elderly at home.

A Study on Social Support and the Quality of Life in the Elderly(The comparative analysis between home residents and institution residents) (노인의 사회적 지지와 삶의 질에 관한 연구 -일반가정노인과 양로원노인을 대상으로-)

  • Recent improvements in the standard of living, national income and medical care, and a decline in the infant death rate which have occurred related to economic growth and modernization, have led to a longer average life-span and a higher ratio of elderly people in the total population, Therefore, not only in the field of nursing science, but also in the field of many other discipline, issues concerning the elderly have been given increasing interest. A great deal of effort has been spent on increasing the quality of life for elderly people. This study was conducted to analyze the correlation between social support and quality of life. The sample consisted of III subjects residing at home and 107 subjects residing in institutions all of whom were over 65 years of age. The data collection period was from October 23, 1990 to January 26, 1991. Social support was measured using the Norbeck social Support Questionnaire developed by Norbeck, translated by Oh, Ka Sil and quality of life was measured using the QOL scale developed by No, You Ja. Data were analyzed using pereentages, t-test, Pearson Correlation Coefficient and ANOVA. The results of this study are as follows : 1. There was a statistically signifivant difference in the level of social support between the two groups (t=-8.83, p<.001), The elderly at home reported a much higher level of social support. 2. There was a statistically significant difference in the level of QOL between the two groups (t=-5.77, p<.001) . The elderly at home reported a much more positive quality of life. 3. There was a positive correlation between social support and QOL for the elderly at home ard it was statistically significant (r=.32, p<.001). 4. There was a postitive collealtion between social support and QOL for the elderly in institutions and it was also statistically significant (r=.19, p<.05). 5. The relationship between the general characteristics of the elderly at home and the variables of social support and of QOL were as follows ; 1) according to sex(t=10.57, p<.01) and the number of offspring(F=6.19, p<.01), there was a statistically significant difference in social support. 2) according to amount of Pocket money, there was a statistically significant difference in QOL(F=2.98, p<.05). 6. The relationship between the general characteristics of the elderly in institutions and the two variables were as follows ; 1) according to sex(t=6.24, p<.05), the number of offspring(F=6.16, p<.001) and religion (F=2.58, p<.05), there was a statistically significant difference in social support. 2) according to religion, there was a statistically significant difference in QOL(F=2.90, p<.05). In conclusion, it can be said that social support is an important variable related to QOL in the elderly and that social support levels are higher for the elderly residing at home. Therefore, more specific and objective approaches and efforts are needed to effectively use resources to maintain the elderly at home and to enhance social support available to the elderly in institutions and thereby increase QOL regardless of residence.

Actual conditions on the Living Environment of Small-scale Group-homes for the Elderly (고령자용 소규모 그룹홈의 생활환경 실태)

  • To investigate the actual conditions on the living environment of group-homes for the elderly and their living environment, 4 group-homes were surveyed. As the results, conditions of location for the group-home of the elderly looked positive, on the other hand, safety for criminal prevention, accessability and convenient facilities were not on level satisfied. The average area of each space in the group-home indicated that the bed room was $11.21\;m^2$, the living room $28.13\;m^2$, the kitchen $23.59\;m^2$, and the bath room had an integrated type of bath room and toilet $7.63\;m^2$. The physical environments of each space in the group-home were investigated, In bed room, door, windows and illumination were relatively good but safety was extremely inferior. Living room mostly looked good except the doorsill which needs refurbishment. In kitchen, cooking table, windows, illumination, ventilator were somewhat satisfactory but gas-warning device should have been installed. In addition, in bath room and toilet, location, door, window, illumination and finished material were satisfactory. But the stepped difference between the bath room and other space, safety-bar, emergency bell and floor heating system were seriously deteriorated and must be improved for the safety of the elderly. Conclusively, living environment of group-home for the elderly is mainly satisfactory but it was necessary to take a consideration for their safety.
