Needs Analysis for Home Rehabilitation Services by Disabled Person in the Rural Areas (농촌 재가 장애인의 가정방문 재활서비스 욕구도 분석)

  • The purposes of this research were to investigate home care rehabilitation services in rural areas and to collect basic data about disabled persons necessary when for carrying out rehabilitation services. Respondents were selected from six of a total of eight townships (Myon) and one town (Eup) in the Wonju city area. Wonju is in Kangwon Province (Do). Of a total of 338 names provided by the Myon offices, 298 persons were located and included registered and non-registered persons. Conditions included stroke, spinal cord injury, and cerebral palsy in addition to disabilities classified as first, second or third degree, in the case of registered cases. Respondent demographic characteristics, medical characteristics, rehabilitation service needs, willingness to receive rehabilitation service and individual opinions regarding rehabilitation services were analysed by frequency and percentage. The results were as follows: 1) Rehabilitation services received by disabled persons living at home in the rural areas surrounding Wonju city were medical rehabilitation (41.7%), diagnosis (36.5%), rehabilitation assistive devices (7.6%), social assistance (7.1%), rehabilitation counseling (3.0%), vocational rehabilitation (1.8%), educational rehabilitation (1.6%) and housekeeping services (0.5%). The majority of rehabilitation services were medical rehabilitation provided at hospitals and oriental medicine hospitals. 2) Sixty point eight percent of respondents expressed their willingness to receive home care rehabilitation services. Needs expressed were highest for medical rehabilitation (27.0%), followed by social assistance (19.4%), medical examination (12.4%), physician-generated diagnosis in the home setting (11.6%), sociopsychological rehabilitation (9.3%), vocational rehabilitation (7.6%), rehabilitation engineering (6.0%), educational rehabilitation (3.3%), and housekeeping services (3.3%). 3) Rehabilitation service needs were analyzed by severity classification: 65.8% of first degree, 62.7% of second degree and 55.6% of third degree disability classification, and 62.7% of non-registered disabled individuals responded that rehabilitation service was necessary. 4) Rehabilitation service needs were also analyzed by diagnosis: 62.6% of stroke, 85.5% of amputation, 60.0% of spinal cord injury and 52.4% of traumatic brain injury respondents answered positively that they were willing to receive rehabilitation service if it were to be provided. Rehabilitation service utilization data of disabled individuals living at home in rural areas were investigated and their rehabilitation needs analyzed. This critical information can be used when community-based rehabilitation programs for disabled persons living at home are planned for provision out of a public health center or when community-based rehabilitation welfare policy is formulated.

  • Although the availability of home care rehabilitation services have been greatly increased since community-based rehabilitation was introduced to Korea, there is still a dearth of studies investigating the performance level of ADL (activities of daily living) for the physically disabled in the rural areas. The purposes of this study were to investigate the ADL performance level of disabled persons living at home in the rural areas of Wonju city, Korea, using FIM (functional independence measure) and to identify the specific areas of the ADL to be trained or evaluated by physical therapists or occupational therapists. 298 disabled people were interviewed by 10 physical therapy students. Analysis of the interviews indicated: 1) Forty seven point seven percent of the respondents were elderly-disabled persons whose ages were 61 years old or above, and 69.5% of respondents has the history of chronic disability period of 5 years or more. 2) FIM score of bathing, and stairs climbing showed severe dependent trend 3) FIM scores of self-feeding, urination, defecation, and comprehension ability were mildly dependent. These results revealed that functional evaluation/treatment for discharging from hospitals to the rural areas should be emphasized on the specific ADL performance areas such as bathing, and stairs climbing.

Need of Health Center-based Integrated Healthcare Services for the Elderly in Rural Area (농촌지역 보건소 중심의 노인 통합보건의료서비스 필요도)

  • Objectives: The challenge of an increasing elderly population has coupled with everpresent social concerns in Korea. A major problem in health center for the frail older people is that medical, healthcare, and welfare services are often fragmented in terms of providers and settings without appropriate coordination. The purpose of this study was to investigate the need of health center-based integrated healthcare services and its related factors for the elderly. Methods: A total of 110 elderly people who had visited at a county Health Center were interviewed using a self-administered questionnaire from November to December, 2005. The questionnaire consists of five domains according to the Program of All-inclusive Care for the Elderly. Results: Respondents had high need (total mean score with the 5-point Likert-type sacle: 3.67) of health center-based integrated healthcare services including home visiting service (mean: 4.08), chronic disease care service (mean: 4.06), and transportation service (mean 4.05). According to the results of hierarchical multiple regression analysis, among three regression models the magnititude of the variance of full model that is explained by the need of welfare-domain service was significantly larger than two reduced model. Income was a significant variable in increasing the need of health care and welfare services. Conclusions: This study suggests that the health center-based integrated healthcare services for the elderly must be continuously developed and provided for the health promotion and improved the quality of life of the elderly who live in rural area in Korea.

Development and Validation of the Scale of Care Competence for Adolescent (청소년 돌봄역량 측정도구의 개발 및 타당화)

  • The objective of this research is to conceptualize care competences for adolescents and to develop a valid measurement tool based on this conceptualization. The primary aim is to establish a solid foundation for effective communication in teaching care competences within the domain of home economics education. To achieve this, a comprehensive review of relevant literature and previous studies was conducted, and consultations with experts were undertaken to assess content and criterion-related validity. Furthermore, exploratory and confirmatory factor analyses were performed. The findings from these analyses resulted in a set of 34 items, which were categorized into five distinctive factors representing care competences in adolescents. These factors encompassed four items of care thinking, seven items of care emotion, six items of care behavior, seven items of self-care and ten items of community-care. Additionally, a reliability analysis was performed on the model, confirming the stability and consistency of the five factor structure and the 34 item scale, which assess care competences in adolescents.

Development and Application of an Education Program for Healthy Dietary Life for Elementary School Aftercare Class Children (초등학생 저학년 돌봄교실의 건강식생활 교육프로그램 개발 및 적용)

  • Objectives: This study aimed to develop a school-centered healthy eating environment for children in elementary care classrooms and prevent incorrect eating habits and obesity through the development and application of standardized healthy eating habit-forming educational materials. Methods: Ten schools in eight districts of Gyeonggi-do and 400 students from 19 care classes were selected. Based on the developed educational materials, the program was applied to students once in two weeks. 'Notices for Parents' forms were also sent to the students' home to educate their parents. Pre and post-surveys were conducted to evaluate the effectiveness of the education. The pre-education, education, and aftercare were conducted from September 28 to September 31, 2016, from October 3 to November 30, 2016, and from December 5 to December 9, 2016, respectively. Results: The healthy eating program for elementary care classes was designed to develop a school-centered healthy eating environment and provide standardized educational material for healthy eating habits. Twelve educational topics were developed: , , , , , , , , , , , and . Moreover, the materials were produced in four forms: for students, for after school caring teachers, for external specialists, and for parents. The effectiveness evaluation was conducted to confirm the application of the program. The average eating habits score was 3.3 ± 0.6, with no significant difference between before and after application. The score of overall satisfaction of the education was 3.9 ± 0.9. The most satisfying content was 'Did you get to know how to eat evenly?'. Significant increases were observed in two contents for parents regarding their children's knowledge changes after the education: 'Five nutrients needed for growing children' and 'Knowing sugar foods and sugar-containing foods'. On the other hand, their educational satisfaction was 3.6 ± 0.6, which was lower than the children's satisfaction. This might be because their education was conducted only through the 'Notices for Parents' form. Conclusions: In the long term, the healthy eating habit-formation education for lower elementary school children is expected to be beneficial. To prevent obesity and establish healthy eating habits of children, it is important to develop healthy eating education programs centered on elementary school aftercare classes, including the development of educational materials and an application system through connection with the home and community.

A Study on the Forecast of Bed Demand ofr Institutional Long-term Care in Taegu, Korea (대구광역시 노인복지시설 유형별 수요추정)

  • The purpose of this study was to estimate the forecast of bed demand for institutional long-term care for the elderly persons in Taegu Metropolitan City. The study subject was the total 1,877 elderly persons over age 65 living in Taegu. Among them 1,441 elderly persons were sampled from community and 436 were from the elderly admitted 5 general hospitals. Data collection was carried out by interview from 25 August to 25 December 1997. The measuring instrument of this study was the modified tool of CARE, MAI, PCTC, and ADL which were examined for validity and reliability. In order to forecast bed demand of Nursing Home, this study revised prediction techniques suggested by Robin. The results were as follows : 1. OLDi of Taegu City were 122,202 by the year 1998 and number of Low-Income Elderly Persons were 3,210. 2. The Level I : Senior Citizen Home $ADEMi=\frac{AQi * ASTAYi}{365 * AOCUi}$. AQi = OLDi * LADLi * NASi * ALONi * LIADLi * AUTILi. Predicted number of bed demand for Home Based. Elderly Persons were 4,210 and Low-Income Elderly Persons were 1,081 and Total Elderly Persons were 5,291 by the year 1998, 6,343 by the year 2000 and 8,351 by the 2005. 3. The Level II : Nursing Home $BDEMi=\frac{(BQ1i+BQ2i) * BSTAYi}{365 * BOCUi}$. BQ1i = OLDi * HADLi * ALONi * HIADLi BQ2i = OLDi * HADLi * FAMi * OBEDi Predicted number of demand for Total Elderly Persons were 668 by the year 1998, 802 by the year 2000 and 1,055 by the 2005. 4. The Level III : Nursing Home $CDEMi=\frac{COLDi * HDISi * CUTILi * CSTAYi}{365 * COCUi}+OQi/10$ Predicted number of demand for Total Elderly Persons were 1,899 by the year 1998, 2,311 by the year 2000 and 3,003 by the 2005. 5. Predicted number of bed demand of long-term care facilities in the year 1998 according to Levels were 4.3% among elderly persons in Taegu by Level I, 0.5% by Level II and 1.5% by Level III. Number of elderly persons in current long-term care facilities were 458 in LevelI I,284 in Level II. 6. Deficit number of bed demand of long-term care facilities were 4,833 in Level I, 384 in Level II, 1,899 in Level III for the elderly persons in Taegu Metropolitan City.

A Mixed Methods Study of the Successful Aging of Older Adults in Korea

  • The purpose of this study was to understand experiences of successful aging of community-dwelling elderly living in the Korean community using mixed methods. This study is a mixed methods research using convergent parallel design. 483 elderly people living in the 2 cities were selected as subjects to collect quantitative data. For qualitative data collection, 6 elderly people participated. The level of successful aging for elderly people living at home scored an average of 4.30 ± .59 out of 5 points (4.46 ± .64 for self-efficacy, 4.31 ± .65 for good self-control, 4.29 ± .70 for satisfaction with children's success, and 4.26 ± .61 for partnership with spouses). The main themes of successful aging derived by interview were as follows: 1) fulfill responsibilities of raising children as parents, 2) establish a stable life to overcome difficulties, 3) accept the body being different from the past, and 4) live in harmony with people around them. Based on these results, it is necessary to provide useful health care services that can be used in the local community and to provide service utilization information through various routes in order to achieve a successful aging.

The Study on Free Choice in Childcare Policy : Focusing on Public Childcare Service VS Home Childcare Allowance (아동양육정책의 자유선택 쟁점에 관한 연구 - 공보육서비스 vs 가정양육수당을 중심으로 -)

  • The issue of child care in family policy is one of the most important agendas. It is the primary task for a welfare state to form such conditions as socializing it to ensure labor rights and parental rights. The aim of this study is to review any controversies in the agenda of free choice in child care. But real 'free choice' comes true when all community members as individuals are able to enjoy their rights as both workers and parents without any restriction and sexual discrimination in the labor market. But in reality 'free choice' has not been realized in our society. This study is based on the analysis of these issues in free choice - the concepts of liberty, equity and equality, the role of the state, financial efficiency and the status of women in the labor market. Based on the analysis of the issues in accordance with the criteria, I suggest some policy recommendations: adequate and sufficient child care services, the introduction of univer sal child benefits and the expansion of public child care facilities.

Review of Planning on the Congregate Nursing Homes Based on the Case Study for the Local Friendly Elderly Welfare Facilities in Japan (일본의 지역친화형 노인복지시설 사례를 통해서 본 노인요양공동생활가정의 계획 방향)

  • The purpose of this research is to explore the best examples from Japan to derive spatial and institutional characteristics. This study used the field survey method for 6 elderly welfare facilities in Japan. The characteristics of local friendly facility are as follows; First of all, since elderly care facilities in South Korea are separated from local community and facility plans are large-scaled and unified which are disconnected from local area, this research set aging in community, multi-function, diversity, and intergeneration into the concept that is necessary for local friendly facilities. Secondly, residential environment plan's basic directions are 1) plan that minimizes facilities-like atmosphere, 2) spatial plan that focuses on the elderly dignity and privacy, 3) comfortable and enjoyable communal living space plan, 4) local friendly and communicating plan, 5)plan that minimizes staff's care giving burden. Thirdly, the rooms necessary for local friendly facility model are composed of bedroom, dining room, kitchen, living room, garden, toilet, laundry room, bathroom, corridor, and office based on the legal installation standards.

Knowledge, Attitude, and Practice towards Infection Control among Community-visiting Nurses (방문간호사의 감염관리에 대한 지식, 태도 및 수행)

  • Purpose: Purpose: This study aimed to identify whether infection control practice would correlate significantly with the knowledge and attitude of infection control in the pre-, mid-, and postvisiting rounds among community-visiting nurses. Methods: A descriptive study was conducted based on the knowledge, attitude, and practice (KAP) model by administrating questionnaires during September-October 2020. A total of 65 nurses working for 15 community health centers in Seoul, South Korea were included. The questionnaires were developed based on the epidemiologic triangle model and comprised of 28 items on practice, 18 items on knowledge, and 10 items on attitude. Results: The infection control practice showed a mean of 88.9 (range, 0-100). The infection control knowledge had 89.2% on the host domain, 80.0% on the environment domain, and 74.8% on the agent domain (range, 0-100). The infection control attitude showed a mean of 39.5 (range, 0-50). Higher scores on the infection control practice are significantly correlated with the higher scores on the infection control knowledge about the host domain (p= .004) at the pre-, mid-, and post-visiting rounds. Higher scores on the infection control practice are significantly correlated with the higher scores on the infection control attitude at the mid- (p= .018) and postvisiting rounds (p= .028). Conclusions: The infection control practice by community-visiting nurses may be enhanced with increased knowledge and attitude levels of infection control at the mid- and post-visiting rounds. The enhancement should be included in the on-the-job education for community-visiting nurses.
