The purpose of this research is to analyze the overall problems at the moment of October 2008, and then to find the improvements of home-help services of the Long-Term Care Insurance(LTCI), which has been revealed many problems since it was released in July 2008. The research uses the literature survey which analyzes 2nd-hand materials studied by other people already, and survey research was executed from active social workers in the area of LTCI. Based on the policy analysis framework of Gilbert and Specht, all the data are analyzed in the scopes of client·benefit(service)·finance·transferring system. This research has found the problems in each scope of home-help services of the LTCI. Firstly, the client system has some problems in mismatching between registered and service clients, estimating client number, and judging service levels. Secondly, the service system reveals deficiency in professionality of social workers, service quality lowering by loose qualification criteria on workers, non-reasonable limitation of service time available, and the same fare system applied to visiting-help service in spite of different levels. Thirdly, in financing system, clients need to pay additional money to get extra services such as meal, hair cutting, bathing etc., due to government financial support stopped, some organizations have to reduce services and replace full-time workers to part-time ones, which makes the service quality worse. Lastly, in the transferring system, the management system for service quality is not well prepared. There are too much competion because of allowing too many home-help service organizations and care worker academies. The suggestions that this research has found to improve the policy are as follows. ① It is desirable to make the registered clients the service ones as many as possible in the long term perspective. ② The LTCI organization requires more workers and higher professionality. ③ Many elderly people who are not eligible now require connection system to be more served. ④ Management system and service manual for care worker are to be developed. ⑤ Laws related to the service contents and process should be modified, the proportion of client charge needs to adjust. ⑥ Home-help service organization licensed by the LTCI needs to be financially supported publicly. ⑦ Monitoring system to home-help service organization needs to be strengthened. ⑧ Evaluation tools to home-help service organization and workers is required. ⑨ Specification to open the home-help service organization needs to be more strict.
The purpose of this study is to investigate the relationship between public care and family care. Public care for older adults began in 2008 with the implementation of the Long-Term Care insurance in South Korea. Although the expansion of public care has the purpose of reducing the care burden for the family, it is not easy to say whether the developments of public care system reduce the amount of family care for older family members. Theoretically, public care and family care are expected to have various relationships depending on the degree of the role and function(substitution, hierarchical compensatory, task specific, supplementation, complementarity). And literatures have showed inconsistent results depending on the country, data, and methods. In this study, we analyzed the relationship between two care types focusing on home care services for older persons. Analyses were based on data from the second(2008) to sixth(2016) waves of Korean Longitudinal Study of Ageing(KLoSA). To investigate elderly care dynamics in the households, we pooled the data for four changes between two periods(2008-2010, 2010-2012, 2012-2014, and 2014-2016). This study used an analytic sample of 262 older adults, who are aged 55 over and experienced public care at least one point of time. We used Fixed-Effects(FE) model to analyze the differences within the same individuals under the condition that time-invariant unobserved factors are controlled. This study distinguished the cases of entry into public care and other cases of exiting public care. The results showed that older people who are dependent on public care are less dependent on family care than before. In both entry and exit groups, negative relations were maintained, but in the entering stage of public care, the degree of negative relations was relatively small, whereas in the stage of maintaining or departing from public care, relatively negative relations were strong. At the beginning periods, even though public care increased, family care did not decrease significantly. On the other hand, at the time of ending public care and relying on family care, family care increased significantly. The results of this study show that the relationship between public care and family care is close to hierarchical compensatory model and varies according to the stage of caring transition. Also, it was found that the cases of transition from public care to family care have the biggest burden of elderly care than other groups.
The graying of populations is emerging as an international issue around the world, and this is a problem that is rapidly advancing in Korea as well, signaling the need for financial preparations for the aged. For this purpose, various retirement pension systems are being employed as preparatory measures for the nation's elderly. Using data from 1474 people in the 2007 panel study of National Security for the Retired, the present work attempts to look at satisfaction rates with regard to public pension receipts for the national pension and special occupational pensions according to general characteristics and factors related to the preparation for an aging society. Satisfaction with retirement pension receipts according to the type of pension was high for special occupation retirement pensions, individual retirement pensions and the national retirement pension, in that order. Looking at satisfaction rates based on the general characteristics of pension recipients, the study revealed that for the national pension, satisfaction was highest for groups with above-average physical and psychological health, groups who think appropriate living expenses for the elderly are lower, groups in which a partner also earns income, and groups who had amply prepared for their expected living expenses in later life. Regarding special occupation retirement pensions, satisfaction was high for groups over the age of 70, groups with good psychological health, and groups sufficiently prepared for their living expenses in later expenses, compared to groups for which these factors did not apply. In terms of the relative influences impacting retirement pension recipient satisfaction, satisfaction with the national pension was highest when the primary source to cover elderly living expenses was a resource other than income earned by the recipient and their partner and/or income received from children. Concerning special occupation retirement pensions, satisfaction was highest among those whose education terminated before middle school, and for those in good physical health. Based on the above results, it is vital that plans exist for preparing sufficiently for the living expenses of the elderly and for facilitating the physical and psychological health of pension recipients. Plans are also necessary to, ensure that citizens are provided with easily accessible educational programs and activities regarding general installment savings and deposits, stocks and bonds, real estate investments, individual retirement pensions, private insurance, severance pay pensions, and public pensions.

Purpose: The purpose of this study was to develop a new fall-prevention exercise intervention for the community-dwelling elderly and to examine the effects of program. Methods: A total of 7 older adults were participated in a 10-week fall-prevention exercise intervention (1 day per a week in community centers, 2 days per a week in a home). The exercise program consisted of warming-up, extension exercises, muscle strength exercises, balance exercises, and finishing exercise. Data were collected before the exercise and after 10 weeks of exercise. In this study, muscle strength of lower limbs was measured by 10 times of standing up and sitting on a chair. Static balance was measured by one-leg standing, and dynamic balance was measured by 6 meter walking. Results: After a 10-week exercise program, muscle strength of lower limbs (t=4.18, p<.05), statical balance were significantly improved (left leg, t=-3.11, p<.05; right leg, t=-4.56, p<.05). Seven of 11 items measuring fear of falls were significantly decreased (p<.05), and dynamic balance tended to be improved (p=.117). Conclusion: This result suggests that evidence-based, fall-prevention exercise program can improve muscle strength, static and dynamic balance, and fear of falling in the community living elderly.

This study examined the effect of the Mediterranean-DASH Intervention for Neurodegenerative Delay (MIND) diet of the Korean multi-domain dementia prevention program on the cognitive functions of the elderly with dementia risk factors. We developed the program including nutrition, exercise, cognitive training, vascular disease prevention, and motivation. One- hundred and fifty-three participants aged 60~79 years with at least 1 dementia risk factor were randomly assigned in a 1:1:1 ratio to the facility-based intervention (FMI), home-based intervention (HMI), and the control group. The nutrition education program consisted of 10 classes over 24 weeks: the FMI received 7 group sessions and three 1:1 sessions, the HMI received 4 group sessions and three 1:1 sessions with 3 homework sessions. The Nutrition Quotient for Elderly (NQ-E) and the Mini Nutritional Assessment (MNA) were used to evaluate nutritional status. The Repeatable Battery for the Assessment Neuropsychological Status (RBANS), Korean Mini-Mental State Examination (K-MMSE), and the Cognitive Complaint Interview (CCI) were used to evaluate cognitive functions. A total of 136 people completed the program with an 11.1% dropout rate. The NQ-E (P=0.009) and RBANS (P=0.001) scores significantly increased in the FMI (N=45) and HMI (N=49) groups compared to the control group (N=42) after the study. The changes in the score of MNA and CCI did not differ significantly between groups. In conclusion, the nutritional intervention which focused on the MIND diet as a part of a multi-domain intervention program had a positive effect on the improvement of healthy eating habits and cognitive function scores in the high-risk dementia group.

This study attempted to develop integrated family policy. For this purpose, a delphi survey was conducted twice among 15 professionals regarding the general contents of family policy, the goals and outcomes of president Roh's administration, the direction and plan of action for future family policy. The results of this study are as follows: Firstly, the main results of president Roh's administration were the creation of the Ministry of Women and Families, and the Healthy Families Law. Nevertheless, that government had no long-term vision or action programs. Secondly, there are two aspects of family policy which one is family members based and the other is family life cycle based. Accordingly the subjects of this study insist that family policy has to cover people's whole life, all families regardless of family structure, and has to integrate the plans and programs which have been served to individuals, for example, women, child, youth, and elderly. Thirdly, the orientations of future family policy are to be toward happiness, health, equality, coexistence, harmony, growth, generosity, diversity, self-control, and a sustainable society. So the concept of the integrated family policy has to cover all aspects of the family and the diversity of family life. Therefore, the service delivery system for family policy has to converge into the Healthy Family Center.
Background: This study examined the relationships of dementia, stroke, and combined multimorbidity with long-term care utilizations among older people in South Korea. Methods: A nationally representative sample of 10,130 older adults who used long-term care services in 2010 were analyzed. We used the 5% sample of aged 65 years or older linked with National Health Insurance Corporation registry data of long-term care insurance system. The sample was categorized into three groups: dementia only (47.6%), stroke only (36.3%), and both dementia and stroke (16.1%). We estimated the use of institutional care, home care, and total expenditure of long-term care services, adjusting for the severity of each function (such as daily life, behavior or cognitive change, nursing care needs, and rehabilitation care needs) and sociodemographic characteristics. Results: Having dementia symptoms was positively associated with the use of institutional care services, on the other hand, having stroke symptoms was positively related with the use of home care. The total long-term care cost was higher in the group of having both dementia and stroke. Conclusion: Older persons with dementia symptoms and stroke symptoms have different patterns of long-term care utilization, and the multimorbidity increased the overall expenditure of long-term care utilization. These findings imply a need for differentiated management strategy targeting physically and cognitively impaired older persons, and special concerning for persons with multimorbidity conditions for long-term care insurance program in Korea.
This study has been performed to identify meanings of good deaths among 350 old people aged 65 from September, 2010 until February, 2011. In this study, the subjects were asked about good meanings of death based on qualitative study of free format, and their statements were categorized into similar content areas. The results show that first, the subjects felt that the good death is a dignity death not weighing burdens to others, memorized as a good life, until th end of my life, a death receiving at home, comfortable death, a death like a sleep, with out pain, not long, a prepared process, a death after doing my best, receiving after doing my best, death after offering benevolence to others, within my beliefs, and finally a good death is after seeing good life of my descendent. These 16 free answers were also categorized into 6 meaningful areas such as considering others, at my home, comfortable scene, prepared, and death after living my wanted time.

In recent years, the population composition of rural area is changing due to the decreasing population growth of the aged with disability and people returning to farming. Since 1970's the rural community facilities have been spreaded out across most rural villages. Although the rural community facilities are the most widely used facilities, the existing facilities are need to be new built or remodeled according to demographic changes, usage of facilities, and environmental condition. Given this reality, the rural community facilities such as village assembly hall and nursing home for senior citizen are not only used for daily life place but also used like co-residence having meals and sleeping together. Therefore the rural community facilities have the potential to become welfare service space for rural elderly people. This research focuses on the current situation of universal design applied community facilities. According to the research, most of the village community facilities were built in the 1990s and 82% of the buildings were more than 15 years old. Furthermore, 45% of the village community facilities more than 15 years old most have a ground floor in masonry structure. The area of the building is 65% less than $100m^2$. The width of the access entrance, the height of the sink, gas safety valve and so forth were relatively well designed. However, the handrail of the entrance, space in front of the toilet bowl, the height difference between the entrance and floor, the installation of the width of the ramp and stair handrail was relatively incomplete. Village community facilities to be built in the future should be universal design fundamentally.
The main purpose of this paper is to investigate the factors which affect the alienation of the aged, and to study the relative importance of these factors. This study focused on the following research questions: 1) How do the demographic variables(sex, marital status, family structure, religion, economics, prior occupation, education level) affect the alienation of the aged? 2) How the alienation of the aged is affected by their involvement in social activities? 3) Does the degree of satisfaction with family relationship have a perceivable impact on the alienation of the aged? 4) How does the self-rated senescence affect the alienation of the aged? The research was practiced from the 18th Sept. 1999 to the 30th Sept. 1988, through individual interviews with 300 elderly people, all living in Seoul area. Among them only 285 were used in the analysis. The major findings of the study are as follows; 1) The sex of the respondents makes no difference in the alienation of the age . 2) Marital status is found to be relatively significant in determining the alienation. The degree of alienation of old people with spouse is substantially lower than that of those without spouse. 3) Family structure was found to have no significantly effect on the alienation of the aged. 4) Involvement in religion was found to significantly reduce the alienation of the aged. 5) Econimic factors seems to be critical in defermining the aged. The amount of pocket money avaliable for daily use has direct relationship with alienation. 6) Prior occupation was found to be another significant factor affecting alienation professionals experience far lower alienation than nonprofessionals. 7) The higher is education level, the lower is the alienation of the aged. 8) Active participation in social activities and good relationship with other family members were founded to contribute to lowering the alienation of the aged. Whereas, too much concern over one's senescence appears to hav negative effects on the alienation. The regression analysis shows that the satisfaction with the family relationship has the most important influence upon the alienation of the aged. Following are the factors of self-rated senescence, pocket money in a descending order. All these factors, which explain 42.65% of the total variance of alienation variable are statistically significant(p<.001)
