Prevalence of ischemic heart disease with respect to lifestyle changes in diagnostic patients of CAD

  • A majority of the peoples in Indian cities depend on high fat diet, smoking, ghutka chewing and improper sleep, these all are lifestyle changes, can cause ischemic heart disease. Globally, ischemic heart disease (IHD) is the leading killer. Unani System of medicine not only provides well-based medical cures for diseases, but its holistic approaches as it possess unique principles of diet, lifestyle and particularly therapeutics, to balance and enrich all aspects of physiology and psyche. All diseases are the result of poor management of the six governing (or essential) factors, beyond the ability of physics or Tabiat to maintain and restore homeostasis. In this context, lifestyle diseases can be prevented by conscious changes to the person's diet, behavior and environment. The holistic approach of Unani medicine is well placed to cover the two main pillars of lifestyle diseases, namely, prevention and treatment. In this paper, we report on the prevalence of CAD in patients with known diagnosis of CAD and try to find out its relationship with different lifestyle changes.

조선 후기 의안(醫案) 『경보신편(輕寶新編)』 연구 (Study of Gyeongbosinpyeon, a Late Joseon Medical Records)

  • Objectives : The objective of this paper is to review the healing processes employed in the traditional age and discover the unique features found in the Korean Medicine through categorizing and analyzing the distribution of patients, and the aspects and results of treatments as recorded in Gyeongbosinpyeon, a historical text thought to have been authored by a regional doctor active in Joseon during the mid- to late-19th century. Methods : A table is created to view all of the total of 141 medical records introduced in the Gyeongbosinpyeon, and 7 categories were created to each contain 2 to 3 medical records that have special images. The paper provides their translation texts along with the original texts, and analyzed their medical and social significances by comparing each medical record. Results : The clinical competence displayed by the doctor who had worked in Joseon during the 19th century was surprisingly high, and it seems its values are worthy of dissemination when compared with Yeogsimanpil that has been introduced to the world. There is a great significance in how the principle of holistic treatments, the fundamental aspect of Joseon's medical study, was adhered. Additionally, the parts that show the historical text's author's medical activities and their unique characteristics are also worthy of attention. Conclusions : Korean medicine possesses a remarkable text called Donguibogam, but clinical behaviors' successes are not guaranteed solely with textual knowledge. It can be witnessed that such texts of authority and such medical records that have recorded actual activities complement each other in order to improve the quality of Joseon's study of medicine.

Challenges and opportunities in integrating complementary and alternative medicine into mainstream of the Malaysian healthcare system

  • Complementary and alternative medicine (CAM) practice is still popular among the Malaysian population nowadays although western or allopathic medicine is the first line of treatment. Dissatisfaction with health services and therapeutic effects of western medicine or preference for holistic, integrative approach in treatment are common reasons favouring the increasing popularity of CAM practices. The efforts toward integration of CAM and western medicine in Malaysia were rather slow and in a piece-meal fashion. Strategic efforts in strengthening government and self-regulation among practitioners, formalizing education, promoting research, and cultivating national and international networks are necessary to achieve an integrative system. Regulations to restrict the practice and sale of CAM products to licensed practitioners, strict and mandatory registration of the practitioners, inclusion of CAM in essential medicines list, and pricing regulations must be comprehensively discussed. Development of curriculum, offers of scholarship and incentives, promotion of courses and seminars for professionals is necessary to increase the numbers of CAM experts. Malaysia should follow the efforts of other countries on the production and documentation of local CAM data, allocation of funding, and establishment of research centres to assess the efficacy of potentially useful local products. Local and international collaboration in research and continuous education is important for exchange of knowledge and skills. In conclusion more coordinated efforts in regulation of CAM practice and products, formalizing CAM training and education would significantly move the process forward and allow the public to enjoy more health benefits from CAM practice in Malaysia.

한의학(韓醫學)의 전망(展望)과 우리의 역할(役割) (A Personal Perspective and Our Role in Korean Oriental Medicine)

  • The development of Korean Oriental medicine is based upon the accumulation of experience and knowledge gathered over the centuries. The approaches taken are holistic and empirical. There is a need to understand their actions at molecular levels with more rational, objective and scientific studies. Today it appears that Chronic and age-associated diceases may be multifactorial and hence more complex. A different approach may be required. One claimed usage of Korean Oriental medicine is for the treatment and prevention of chronic and age-associated illnesses. Some of the botanical formulas used for this purpose were discovered thousands of years ago and continue to be used today. There are indications that these formulas may indeed be helpful in the treatment or prevention of chronic diseases. This multi-component medicine could not only be very useful meeting the unmet clinical needs but for defining a more synergistic therapy that supports and maintains the bodies natural curative abilities. The potential usefulness of Korean Oriental medicine embodies the belief of maintaining healthy homeostasis of the body through the proper balance of a mixture of chemical at different organs or tissues. This concept is different from western medicine and implies that multiple compounds may act on multiple mechanisms of action to maintain the balance of the complex web of biology. This is very important in view of sciences current direction to integrate fragmented information to develop future medicines. The western and eastern approaches to human health and disease are complementary to each other. The best approach in developing future medicines is to integrate both approaches.

Knowledge Based Recommender System for Disease Diagnostic and Treatment Using Adaptive Fuzzy-Blocks

  • Identifying clinical pathways for disease diagnosis and treatment process recommendations are seriously decision-intensive tasks for health care practitioners. It requires them to rely on their expertise and experience to analyze various categories of health parameters from a health record to arrive at a decision in order to provide an accurate diagnosis and treatment recommendations to the end user (patient). Technological adaptation in the area of medical diagnosis using AI is dispensable; using expert systems to assist health care practitioners in decision-making is becoming increasingly popular. Our work architects a novel knowledge-based recommender system model, an expert system that can bring adaptability and transparency in usage, provide in-depth analysis of a patient's medical record, and prescribe diagnostic results and treatment process recommendations to them. The proposed system uses a set of parallel discrete fuzzy rule-based classifier systems, with each of them providing recommended sub-outcomes of discrete medical conditions. A novel knowledge-based combiner unit extracts significant relationships between the sub-outcomes of discrete fuzzy rule-based classifier systems to provide holistic outcomes and solutions for clinical decision support. The work establishes a model to address disease diagnosis and treatment recommendations for primary lung disease issues. In this paper, we provide some samples to demonstrate the usage of the system, and the results from the system show excellent correlation with expert assessments.

프랑스에서 한의학의 보급과 제도화 과정 : 침술의 경우 (Diffusion and Process of the Oriental Medicine in France : In a case of the acupuncture)

  • The aim of this study is to observe the spread and implementation of acupuncture as an official medical treatment in France. This research is based on the study of the scientific magazine produced by the Association of Acupuncturist Doctors of France from 1945 to 2008 as well as medical and sociological studies. Western medicine was introduced and spread in Korea about one hundred years ago and is the foundation of modern Korean medicine. Nowadays it is more commonly employed than traditional Oriental medicine. Oriental medicine was introduced in France in the 17th century but only acupuncture gained notoriety. The French Faculty of Medicine officially chose to legalize acupuncture after WWII but only allowed doctors to study acupuncture. It takes three years to obtain an Inter-University Degree(DIU) in acupuncture therapy. Why would the Faculty of Medicine make a holistic treatment system official that is in opposition to the systematic treatment method of the Western medicine? The second question posed is, does Western Academia still recognize acupuncture as a viable treatment method? The final question addressed is, has acupuncture any real impact on medical treatment of the French Society?

교통사고로 발생한 두개뇌기종 환자 1례 증례보고 (The Clinical Study of Pneumocranium in Traffic Accident Patient : Reports of One Cases)

  • Objectives : The purpose of this study is to report the case with pneumocranium in a traffic accident patient by korean medicine. Methods : The subject is a patient with pneumocranium in a traffic accident. This patient was treated with korean medical treatments. Results : After the treatment, pneumocranium in right cerebral convexities in brain CT is removed. The symptoms is improved significantly Conclusions : This study shows that a patient with pneumocranium was diagnosed with stroke and cured without a recurrence through holistic treatment of Korean medicine.

  • Therapist have many troubles in the treatment of the patients who have a disorder in Centural Nerve System. It is the role of therapist that is to discriminate and control tone in the patients. In the case of hypertonus to be with hypotonus it is needed for therapist to inhibite the tonus properly without being influenced from the associated reaction. In this study the therapeutic control of the hypertonus and hypotonus through the Bobath approach is presented in detail. It is suggested that the holistic approach using the plasticity of patients base on the Bobath concept is to be used in the improvement of activities and functions of individual patients.

Improving shoulder function and pain in a paraplegic patient with massive irreparable rotator cuff tear using a subacromial balloon spacer

  • The subacromial balloon spacer is a promising treatment option for alleviating symptoms in patients with massive irreparable rotator cuff tears (MIRCTs). The balloon provides faster pain relief and easier rehabilitation than other options (such as arthroscopic partial rotator cuff repair). For a paraplegic MIRCT patient, the need for speedy recovery and rehabilitation is crucial, as these patients rely on their upper limbs for daily life activities and independence. In this report, we present a 60-year-old male paraplegic patient who presented to the clinic with an MIRCT of the right shoulder. After a holistic investigation and assessment of the patient, a subacromial balloon spacer with an upper border subscapularis repair was chosen as the treatment of choice. The patient had an uneventful recovery, and at the 1-year mark, had forward elevation of 170°, an American Shoulder and Elbow Surgeons score of 95, and a visual analog scale pain score of 0.

유방암 환자의 치료 전 경험 (The Patients' Experiences of the Diagnosis and Pre-Treatment Period of Breast Cancer)

  • Purpose: To date most research related to patients with breast cancer has discriminately investigated the status within or after the treatment although the patients demand holistic nursing care from the time of diagnosis. Thus, the purpose of this study was to investigate the trajectory of breast cancer diagnosis and patients' experiences in the pre-treatment period. Method: This qualitative study used qualitative thematic analysis. Nineteen Korean women who were diagnosed with breast cancer within the last 6 month participated in the study. Individualized interviews were conducted with each participant in a cancer center in K city. The interviews were tape-recorded, transcribed, and analyzed using the thematic analysis process. Results: The overriding theme was "the scattered life in an unforeseen swirl", which illustrates the participants' unexpected crisis with confusion and emotional distress. Two subthemes included "falling into an unavoidable journey", and "staggering in a muddle with urgency". The categories were "unexpected probability", "nagging nodularity", "ominous presentiment", "emotional upheaval", "bad thought intrusion", and "a sense of urgency". Conclusion: Patients in the pre-treatment period encountered utter emotional distress and a sense of urgency after being diagnosed breast cancer. Strategies to develop nursing care for patients in this period and nursing implications are discussed.
