비만을 동반한 제 2형 당뇨병환자의 혈당 조절을 위한 운동 중재 : 체계적 문헌고찰 (Exercise Intervention on Blood Glucose Control of Type 2 Diabetes with Obesity : A Systematic Review)

  • PURPOSE: The aim of this study was to review the effects of exercise intervention on blood glucose control in obese type 2 diabetic patients. METHODS: The PubMed and KERISS search engines were used and 61 papers that met the key questions were selected. RESULTS: Exercise is an effective intervention for the control of blood glucose in type 2 diabetic patients because it does not impair glucose transport in the skeletal muscle induced by muscle contractions. Insulin resistance, which is characteristic of type 2 diabetes, is caused by decreased insulin sensitivity or insulin responsiveness. Acute exercise improves the glucose metabolism by increasing the insulin-independent signaling pathways and insulin sensitivity in the skeletal muscle, and regular long-term exercise improves the skeletal muscle insulin responsiveness and systemic glucose metabolism by increasing the mitochondrial and GLUT4 protein expression in the skeletal muscle. CONCLUSION: The improvement of the glucose metabolism through exercise shows a dose-response pattern, and if exercise consumes the same number of calories, high intensity exercise will be more effective for the glucose metabolism. On the other hand, it is practically difficult for a patient with obese type 2 diabetes to control their blood glucose with high intensity or long-term exercise. Therefore, it will be necessary to study safe adjuvants (cinnamic acid, lithium) that can produce similar effects to high-intensity and high-volume exercises in low-intensity and low-volume exercises.

감초탕 섭취가 고강도 저항성 운동 후 근부상, 혈중 코티졸, 테스토스테론 및 인슐린 민감도에 미치는 영향 (The Effects of Licorice Supplementation on Muscle Injury, Cortisol, Testosterone and Insulin Sensitivity After High Intensity Resistance Exercise)

  • Objectives : The aim of this study was to investigate the effects of licorice supplementation on muscle injury, plasma cortisol, testosterone and insulin sensitivity after high intensity resistance exercise. Methods : The fourteen health college male students were voluntarily participated in this study and were randomly divided into 2 groups: Control group (CON, n=7), Licorice group (LR, n=7). LR group ingested 2 g/time of licorice extract (mixed with 100 ml of water) two times/day for 10 days while the CON group ingested 100 ml of water. All subjects performed a high intensity resistance exercise (half-squat, 8 RM at 80% one-repetition maximum, 5 sets, 1min rest). Blood samples were collect before (-7) and after (0) licorice supplementation, and then 1 day, 2 day and 3 day post exercise. After 10 day treatment, plasma creatine kinase, cortisol, testosterone, glucose, insulin were measured. To determine the insulin sensitivity, HOMA-IR was calculated. Results : Plasma creatine kinase activities were significantly elevated after exercise, but there was not different between two groups. The plasma cortisol and testosterone levels were not significantly different between two groups. Plasma glucose levels were increased at 1 day and 2 day after exercise in the LR comparing with CON group (P<0.05) but plasma insulin levels were significantly lower in comparison with CON. HOMA-IR were significantly lower in the LR than CON group at 0 day to 3 day (P<0.05). Conclusions : The results of the current study suggest that licorice supplementation for 10 days might not attenuate the high-intensity exercise-induce muscle injury but may enhance the whole-body insulin sensitivity.

일시적 혈류 제한과 함께 수행한 저강도 운동 후 알파운동신경원의 변화 (Changes of α-Motor Neuron Excitability after Low-Intensity Exercise with Transient Restriction of Blood Flow)

  • PURPOSE: Low-intensity exercise with transient restriction of blood flow to muscle could be an alternative rehabilitation method which avoids the problems associated with conventional high-intensity exercise. However, the mechanism of low-intensity exercise with transient restriction of blood flow is not clearly known. Thus, the purpose of this study was to investigate the mechanism of improvement of muscular function after low-intensity exercise with transient restriction of blood flow using H-reflex analysis. METHODS: Twenty one healthy young adults with no medical history of neurological or musculoskeletal disorder voluntarily participated in this study. The ${\alpha}$-motor neuron excitability of the triceps surae was assessed using the H-reflex. The amplitude of the M-wave and H-reflex were measured across three conditions: rest, after low-intensity exercise without restriction of blood flow and after low-intensity exercise with restriction of blood flow. The subjects performed low-intensity ankle plantar flexion exercise at their own pace for one minute without or with transient restriction of blood flow achieved by a sphygmomanometer cuff on popliteal fossa at a pressure of 120mm of mercury(120 mmHg). RESULTS: No significant changes of the excitability of the ${\alpha}$-motor neuron were obtained across three different conditions. CONCLUSION: This study found that low-intensity exercise with transient restriction of blood flow did not influence to ${\alpha}$-motor neuron excitability of the triceps surae. From the results, I could come to the conclusion that further study will be required.

Effect of resistance training at different intensities on hippocampal neurotrophic factors and peripheral CCL11 levels in obese mice

  • We investigated the effect of moderate- and high-intensity resistance training on hippocampal neurotrophic factors and peripheral CCL11 levels in high-fat diet (HFD)-induced obese mice. C57/black male mice received a 4 weeks diet of normal (control, CON; n = 9) or a high-fat diet (HF; n = 27) to induce obesity. Thereafter, the HF group was subdivided equally into the HF, HF + moderate-intensity exercise (HFME), and HF + high-intensity exercise (HFHE) groups (n = 9, respectively), and mice were subjected to ladder-climbing exercise for 8 weeks. The hippocampal brain-derived neurotrophic factor (BDNF) and nerve growth factor (NGF) levels were significantly lower in the HF group than in the CON group (p < 0.05). In addition, in the HFME and HFHE groups were significantly higher than in the HF group (p < 0.05). The peripheral CCL11 levels were significantly higher in the HF group than in the CON group (p < 0.05). In addition, in the HFME and HFHE groups were significantly lower than in the HF group (p < 0.05). However, there was no significant difference according to the exercise intensity among the groups. Collectively, these results suggest that obesity can induce down-regulation of neurotrophic factors and inhibition of neurogenesis. In contrast, regardless of exercise intensity, resistance training may have a positive effect on improving brain function by inducing increased expression of neurotrophic factors.

  • 본 연구는 8주간 고강도 인터벌 운동이 비만중년여성의 성장호르몬과 피로물질에 미치는 영향을 알아보기 위한 것으로, 40대 비만 중년여성 20명을 임의로 선정하여 실험군 10명, 통제군 10명을 배정하였다. 실험군은 고강도 인터벌 운동을 8주간 4일 35분 운동을 실시하고 운동 전과 8주 후의 성장호르몬과 피로물질의 효과를 분석하였으며, 다음과 같은 결론을 얻었다. 첫째, 8주간 고강도 인터벌 운동에 따른 성장호르몬은 실험군에서 운동전 보다 8주 후에 유의한 증가를 보였다. 둘째, 8주간 고강도 인터벌 운동에 따른 피로물질은 실험군에서 운동전 보다 8주 후에 유의한 감소를 보였다. 결론적으로 8주간 고강도 인터벌 운동은 성장호르몬을 증가키고 피로물질을 감소시켜 피로예방, 관상동맥질환 및 심혈관질환 위험을 감소시키는데 긍정적인 영향을 미칠 수 있는 가능성이 있다고 생각된다.

Interaction of genetic background and exercise training intensity on endothelial function in mouse aorta

  • The purpose of this study was to characterize the genetic contribution to endothelial adaptation to exercise training. Vasoreactivity was assessed in aortas from four inbred mouse strains (129S1, B6, NON, and SJL) after 4 weeks of moderate intensity continuous exercise training (MOD), high intensity interval training (HIT) or in sedentary controls (SED). Intrinsic variations in endothelium-dependent vasorelaxation (EDR) to acetylcholine (ACh) as well as vasocontractile responses were observed across SED groups. For responses to exercise training, there was a significant interaction between mouse strain and training intensity on EDR. Exercise training had no effect on EDR in aortas from 129S1 and B6 mice. In NON, EDR was improved in aortas from MOD and HIT compared with respective SED, accompanied by diminished responses to PE in those groups. Interestingly, EDR was impaired in aorta from SJL HIT compared with SED. The transcriptional activation of endothelial genes was also influenced by the interaction between mouse strain and training intensity. The number of genes altered by HIT was greater than MOD, and there was little overlap between genes altered by HIT and MOD. HIT was associated with gene pathways for inflammatory responses. NON MOD genes showed enrichment for vessel growth pathways. These findings indicate that exercise training has non-uniform effects on endothelial function and transcriptional activation of endothelial genes depending on the interaction between genetic background and training intensity.

만성 발바닥 근막염 환자에 대한 물리치료적 접근법: 특이적 뻗침운동과 고부하 강화 운동의 효과 비교 (Physiotherapy Approach to Patients with Chronic Plantar Fasciitis : Comparison of the Effects of Specific Stretching Exercise and High-Load Strengthening Exercise)

  • Purpose : This study was to investigate the effectiveness of ESWT and plantar fascia-specific stretching exercise vs ESWT and high-load strengthening exercise in patients with chronic plantar fasciitis. Methods : The subjects were randomized to extracorporeal shock wave therapy ( ESWT, for 3 weeks) and daily plantar-specific stretching (Group I: Stretch group) or ESWT and high-load progressive strength (Group II: Strength group) performed every second day. The main outcome measures were ultrasound, visual analogue scale (VAS), and Korean Foot Function Index (KFFI). The ultrasound (plantar fascia thickness), pain intensity I, II (the most painful of the day?, the pain when you first step in the morning?) and KFFI (functional performance) were compared between the groups. Results : No significant difference was observed between the groups in the plantar fascia thickness but pain intensity I, II was significantly lower in Group 2 than in Group 1 at only 12weeks and functional performance was also significantly increased in Group 2 compared to Group 1 at only 12 weeks. Conclusion : The high-load strengthening exercise consisting of the progressive exercise protocol, resulted in superior after 12 weeks compared with plantar-specific stretching. High-load strength exercise may aid in a quicker reduction in pain and improvements in functional performance.

Comparisons of Increased Repetitions and Exercise Intensity of the Symmetric Upper Limbs between Men and Women

  • Background: Improperly conducted exercise may lead to worsening of musculoskeletal complications. Such may worsen due to increased repetition and intensity during exercise. In addition, different responses may show different needs for training program. Objectives: To compare kinematics of symmetric concentric and eccentric motions during increased repetitions and intensities for men and women. Design: Quasi-randomized trial. Methods: A total of ten men and eleven women participated in this study. Concentric and eccentric motions of the lateral raises were observed for initial positions of abduction and adduction. Low and high exercise intensities were applied, and 15 repetitions were conducted for both intensities. Initial, 3 inbetween repetitions, and last repetition were recorded for comparisons. Results: The concentric or abduction motions showed no significant differences for all comparisons. However, eccentric or adduction motions showed greater significant differences as the exercise intensity increased for both men and women. Such significant differences were most prevalent during the first and last repetitions with greatest differences during the initial repetitions. Conclusion: Kinematic difference between men and women during increased repetitions and intensity indicate the need for more individualized exercise intervention and consideration between men and women. Individualized interventions may prevent exercise-induced postural abnormality and corresponding musculoskeletal dysfunction.

단거리 달리기선수의 고강도 훈련 시 중탄산염 섭취가 혈액변인에 미치는 영향 (The Effects Sodium Bicarbonate Intake on Blood Variables During High Intensity Exercise of Sprinter)

  • 본 연구는 단거리 달리기선수의 고강도 훈련 시 중탄산염 섭취가 혈액변인에 미치는 영향 규명하는데 연구의 목적이 있다. S시 남·여 단거리달리기 선수로 등록된 운동경력 3년 이상의 엘리트 선수 30명을 대상으로 통제집단, 훈련집단, 중탄산염 투여와 훈련집단의 세 집단으로 설정 후, 고강도 운동프로그램은 통제집단과 중탄산염 투여와 훈련집단을 대상으로 80-90%HRmax 운동강도를 2-3주마다 증가시키며, 총 8주간, 주 5일, 1회 90분 실시하였다. 중탄산염 섭취는 중탄산염 투여와 훈련집단을 대상으로 체중 1kg당 300mg을 고강도 운동프로그램 시작 90분 전 1일 1회 8주간 섭취하였다. 연구결과, 단거리달리기 선수의 고강도 훈련 시 중탄산염의 섭취가 혈액변인에 미치는 효과는 중탄산염 투여와 훈련집단에서 수소이온농도의 상승과 젖산의 감소에 긍정적인 효과가 나타났으며, Ca2+, Na+, K+은 훈련집단과 중탄산염 투여와 훈련집단에서 유의한 증가가 나타났다. Mg2+은 세 집단 모두에서 유의한 변화를 나타내지 않았다. 고강도 훈련 시 중탄산염의 섭취가 혈액변인의 변화 중 수소이온농도의 증가와 젖산의 감소에 효과적인 변화가 나타나는 것을 확인 할 수 있다. 따라서 고강도 운동 시 중탄산염의 섭취가 운동능력의 개선에 효과적임을 알 수 있다.

Protective effect of low-intensity treadmill exercise against acetylcholine-calcium chloride-induced atrial fibrillation in mice

  • Atrial fibrillation (AF) is the most common supraventricular arrhythmia, and it corresponds highly with exercise intensity. Here, we induced AF in mice using acetylcholine (ACh)-CaCl2 for 7 days and aimed to determine the appropriate exercise intensity (no, low, moderate, high) to protect against AF by running the mice at different intensities for 4 weeks before the AF induction by ACh-CaCl2. We examined the AF-induced atrial remodeling using electrocardiogram, patch-clamp, and immunohistochemistry. After the AF induction, heart rate, % increase of heart rate, and heart weight/body weight ratio were significantly higher in all the four AF groups than in the normal control; highest in the high-ex AF and lowest in the low-ex (lower than the no-ex AF), which indicates that low-ex treated the AF. Consistent with these changes, G protein-gated inwardly rectifying K+ currents, which were induced by ACh, increased in an exercise intensity-dependent manner and were lower in the low-ex AF than the no-ex AF. The peak level of Ca2+ current (at 0 mV) increased also in an exercise intensity-dependent manner and the inactivation time constants were shorter in all AF groups except for the low-ex AF group, in which the time constant was similar to that of the control. Finally, action potential duration was shorter in all the four AF groups than in the normal control; shortest in the high-ex AF and longest in the low-ex AF. Taken together, we conclude that low-intensity exercise protects the heart from AF, whereas high-intensity exercise might exacerbate AF.
