A Study of the Assessing the High Risk Mothers and Influencing to Their Newborn Babies (고 위험 임신과 신생아 상태와의 관계에 관한 연구)

  • This study was undertaken to determine .the identify the relation between the high risk mother and their baby which then allows the nurse to assess and plen for the delivery of optimal health care to the high risk groups. This study was carried out between January through December 1978. This study sample consisted of 300 pregnant women who visited Ewha womens hospital during this time. The method used to for the collection of data was an“Antepartum High-risk pregnancy scoring form. The questionair included 4 categories: 1) reproductives history 2) Associated conditions 3) pre-sent pregnancy and 4) total risk score . The bind are as follows: 1. The frequency of high risk pregnancy women 149(49.7%) was highest. 2. In the investigation sample high risk factors were related to hypertension and toremia. 3. There was a difference in the high risk scores and newborn babys scores (r = 0.610). 4. Relationship between high risk pregnancy women and least of pregnant women was highest prenatal mortality. Implications of positive assessing of high risk factors by MCH nurse and community health nurses.

  • In spite of the great progress of the theory and skill of the Nursing Care & Medical area in relation to pregnancy, nurses in clinics face up to many challenges in maternity nursing care areas. The reason is that the mobility and mortality of mothers was sharply decreased and the unknown high-risk diseases of pregnancy woman in the past is made public. That's why it is difficult to meet the pregnancy woman in natural process from pregnancy to delivery in recently. Admission rooms are filled with high-risk pregnancy women. As a matter of fact, we have done nursing care into the surface symptoms and diseases of high-risk pregnancy women so far. We have been indifferent to a long period hospitalization, separation from family, and conflict of repeated examination. Therefore, it is widely spread to understand the emotional conflict experienced by high-risk pregnancy women and to need for nursing intervention to bring up about emotional support and the ability of perception in psychological crisis. Although the pregnancy woman judged in high-risk should carry out normal task of pregnancy, she have to be confronted with secondary risk situation. The health of self & fetus threatened by the risk situation could be decreased through care plan, but psychological stress increases. Therefore, the pregnancy brings into non-control state. It is important to ask that what the hospitalized pregnancy women in high-risk think of themselves status. Because misunderstanding or serious anxiety of themselves status put into mother and fetus in danger. And adaptation mode makes all the difference. I would like to consider how nurses could deal with this high-risk circumstances in the position of pregnancy woman on the basis of the above fact. This study uses phenomenological method to suggest the basis material for nurses to do nursing intervention in view of pregnancy woman. Because this method understands the nature of true life of pregnancy woman throughly. The phenomenological method is the sources to describe or explain affluently the process generated in confirmation areas and environment and is the application for readers to understand and recognize clinic reality and then apply this method to reasoning study place or other places. Specifically, the phenomenon study method, one of the phenomenological method, is applied. The use of that method is to describe and generalize the experience in environment exactly. The study of this study is as follows : Among 187 descriptive stamens from 8 study participants are classified into 42 theme cluster at the stage of the first analysis. Those theme is categorized into 8 sub-subjects such as anxiety of uncertainty, foreknowledge about risk circumstance, will power about overcome, unsettled feeling about hospital, relief, optimistic thought, family support, and indifferences. At the last stage of analysis, those things are categorized into 3 subjects. When high-risk pregnancy woman foretell the situation, they feel unsettlement about uncertainty and untrust feeling about hospital. But they are ease with family support and hospital support. On the other hand, they express indifferent 3-way structure response to the situation having will of overcome and exceeding optimistic thought. In those statements, the experience by pregnancy woman shows 3 respect subjects. 1. They are anxious of this situation and are in desperation and don't recognize their role to be carried out 2. They think of this situation as normal process of pregnancy and are not concerned that this can give themselves and fetus fatal damage. 3. The pregnancy women will never confront this situation. This study shows the pregnancy woman has anxiety and optimistic relief about the situation, and ignores and optimistic relief about the situation, and ignores many things. Therefore, nurses in clinic should give pregnancy woman knowledge and information about the high-risk and help them to deal with the situation spontaneously. High-risk pregnancy woman should have the care plan in respect of the right perception. And the nurse know that their support help out pregnancy woman overcome the crisis in this respect of the special nursing intervention.

Retrospective Analysis on Body Weight Changes in the Early Postpartum Period of Women of High-risk Pregnancy Experience and General Health after Korean Medicine Treatments. (산후 한의진료를 받은 고위험산모와 일반산모의 한의진료 체중변화에 대한 후향적 분석)

  • Objectives: This study was aimed to investigate the weight change of the high-risk group and the general maternal group and weight-relating factors in the early postpartum period. Methods: We retrospectively reviewed the medical records of those who received postpartum care from January 1, 2020, to December 31, 2020, in the postpartum care center affiliated with one Korean Medicine hospital. A total of 257 postpartum women's medical charts were included and divided into the high-risk group and the general maternal group. We investigated the weight changes and Body mass index (BMI) of the postpartum women and compared the difference between the two groups after taking the postpartum care. Finally, we used a Pearson correlation analysis to identify the weight-relating factors in the early postpartum period. Results: All the postpartum women showed the following results; 33.81±4.03 years old as the mean age; 22.23±3.28 as pre-pregnancy BMI; 58.21±9.18 kg of pre-pregnancy weight increased into 70.75±9.70 kg in the last month of pregnancy. Of the total 257 patients, 149 (58.0%) of high-risk pregnancy experience and 108 (42.0%) of general pregnancy were included. The edema index right after childbirth was significantly higher in the high-risk group than in the general maternal group (p<0.001), but there was no significant difference in BMI. After treatment with Korean medicine treatments, body weight, BMI, and edema index decreased significantly in both groups (p<0.01). As a result of correlation analysis, weight gain during pregnancy had a significant negative correlation with pre-pregnancy weight and pre-pregnancy BMI and a significant positive correlation with weight and BMI of the last month of pregnancy. In particular, pre-pregnancy BMI and body weight showed a significant negative correlation only in the high-risk group. Postpartum weight loss was significantly positively correlated with pre-pregnancy weight, pre-pregnancy BMI, weight & BMI of the pregnancy last month, weight gain during pregnancy, and decrease in edema (p<0.01) Conclusions: The weight during pregnancy of the high-risk group increased in inverse proportion to the pre-pregnancy BMI. The level of edema right after childbirth was significantly higher than that of the general maternal group, but showed a significant decrease after 2 weeks of Korean medicine treatments. Although it implicates the need for active Korean medicine treatments in the early postpartum period, further studies with controlled groups are needed.

Seropositivity of Toxoplasmosis in Pregnant Women by ELISA at Minia University Hospital, Egypt

  • Toxoplasmosis is considered as an important risk factor for bad obstetric history (BOH) and one of the major causes of congenitally acquired infections. The present study aimed to estimate the seropositivity of T. gondii infection and associated risk factors among the attendees of high risk pregnancy and low risk antenatal care clinic of Minia Maternity and Pediatric University Hospital, Minia, Egypt. The study was carried out from April 2013 to April 2014 through 2 phases, the first phase was case-control study, and the second phase was follow-up with intervention. A total of 120 high risk pregnant and 120 normal pregnant females were submitted to clinical examinations, serological screening for anti-Toxoplasma IgM and IgG antibodies by ELISA, and an interview questionnaire. Seropositive cases were subjected to spiramycin course treatment. The results showed that the seroprevalence of toxoplasmosis in high-risk pregnancy group was 50.8%, which was significantly different from that of normal pregnancy group (P<0.05). Analysis of seropositive women in relation to BOH showed that abortion was the commonest form of the pregnancy wastage (56.5%). The high prevalence of T. gondii seropositive cases was observed in the age group of 21-30 years. Post-delivery adverse outcome was observed in 80.3% of high-risk pregnancy group compared to 20% of normal pregnancy group. There was a statistically significant relationship between seropositivity and living in rural area, low socioeconomic level, and undercooked meat consumption (P<0.05). Serological screening for anti-Toxoplasma antibodies should be routine tests especially among high-risk pregnant women.

Awareness of Marriage, Childbirth, Fertility and Knowledge of High-risk Pregnancy among University Students (대학생의 결혼, 출산 및 가임력 관련 인식과 고위험 임신 관련 지식)

  • Purpose: This research aimed to study the awareness of marriage, childbirth, fertility and high-risk pregnancy among university students. Methods: 250 university students from Seoul and Gyeonggi-do completed questionnaires regarding their awareness on marriage, childbirth, fertility, and high-risk pregnancy. Results: The average age of participants was 22.5. The ideal marriage age was 29.8. 201 (80.4%) participants had an intention of getting married, and 160 (60.4%) answered they should have children. The ideal age of first childbirth was 30.4 and last childbirth was 35.1. There were 126 (50.4%) with prior education regarding high-risk pregnancy, subfertility or infertility. The score of needs for education associated with pregnancy was 7.6 out of 10. The average correct answer rate of awareness of fertility issues was 30.7%. There were significant differences between men and women, 25% of men and 38% of women overestimated the age at women are most fertile. Only 23.7% of men and 25.9% of women knew the correct age when there was marked decrease in women's fertility. Conclusion: Awareness of a lack of marriage, childbirth, fertility, and high-risk pregnancy may influence future family planning and health. These results can be used to plan programs or education for marriage, pregnancy, and childbirth.

Development of Pregnancy Risk Symptom Perception Scale (임신 위험 증상 지각 측정도구 개발)

  • Purpose: To develop Pregnancy Risk symptom Perception Scale (PRPS) and evaluate its validity and reliability. Methods: A preliminary 30-item version of PRPS was developed through literature review, in-depth interview, and Content Validity. Each item was scored on a four-point Likert scale. The preliminary scale was developed based on 301 pregnant women who visited a hospital. Date were analyzed using item analysis, factor analysis, confirmatory factor analysis, Pearson's correlation coefficients, and Cronbach's ${\alpha}$ (0.90 for total item, 0.80 to 0.88 for factors). Results: The PRPS consisted of 27 items. Three factors (physical, environmental, and emotional factors) explained 55% of the total variance. Cronbach's Criterion validity was supported by comparison with the Perception of Pregnancy Risk Questionnaire (r=0.34). In reliability test, the reliability coefficient of pregnancy risk symptom perception was high at 0.90. Conclusion: These results suggest that the pregnancy risk symptom perception scale developed in this study comprises items that can assess the level of pregnant women's pregnancy risk symptom perception in Korea. Its validity and reliability were proven. PRPS can be utilized to measure pregnant women's risk symptom perception during pregnancy. PRPS will contribute to the development of systematic prenatal care and effective risk management.

Health-care Needs of High-risk Pregnant Women Hospitalized in Maternal-Fetal Intensive Care Units: A Mixed-methods Design (산모 집중치료실에 입원한 고위험 임부의 건강관리 요구: 혼합적 연구방법 적용)

  • Purpose: To identify the characteristics and health-care needs of high-risk pregnant women in maternal-fetal intensive care units (MFICU). Methods: A mixed-methods design was adopted. Data were collected from 78 high-risk pregnant women admitted to the MFICU. Qualitative data included ten participants' experiences with hospitalization and childbirth, which were analyzed using mixed content analysis. Quantitative data were analyzed using at-test and one-way ANOVA testing. Results: The average score for pregnancy and childbirth health-care needs was 3.54 points. Average score by area was before-admission health care (3.70), health care of baby (3.67), health of childbirth (3.61), postpartum health (3.51), and pregnancy health care during hospitalization (3.48). Qualitative results showed diverse feelings and experiences of high-risk pregnant women and their need for health care, which was expressed in three themes and 11 sub-themes. Conclusion: Nurses should recognize high-risk mothers' feelings and needs for pregnancy and childbirth-focused health care to help patients accept their vulnerability and cope positively.

The Lifestyle and Awareness of Pregnancy of Female College Students (여자대학생의 생활습관과 임신에 대한 인식 조사)

  • Purpose: Along with the country's decreasing birth rate, many South Korean women are currently facing the issue of high risk pregnancy. Despite the increasing number of women who prefer to get married at an older age, fewer of them are aware of how to prepare for healthy pregnancy at such age. The following study has been conducted to check the level of awareness and educational demand regarding healthy pregnancy among female university students who are not majoring in medical fields. Methods: Google Forms was used to survey 245 female students attending a university located in the Province of Gyenongi. We asked about their attitudes to and knowledge of pregnancy, their lifestyle, and the demand for pregnancy education. Results: The average age of the participants was 20.4 years. Although the majority of the correspondents answered that they have received education about pregnancy before, many of them displayed a low level of knowledge regarding pregnancy. On contrast, the demand for education about pregnancy and high risk pregnancy was high. Conclusion: For healthier pregnancy and childbirth, more preparation is needed before entering pregnancy. Management programs to be given prior to pregnancy should be developed to promote the knowledge and awareness of healthy pregnancy.

Vaginal Microbiota Profiles of Native Korean Women and Associations with High-Risk Pregnancy

  • The vaginal microbiota may be important for pregnancy prognosis because vaginal dysbiosis during pregnancy appears to be related to preterm birth (PTB) or pregnancy loss. Previous reports have indicated that a Lactobacillus-poor microbial flora in the vagina and intrauterine infection by diverse anaerobes ascending from the vagina are associated with undesirable delivery outcomes. However, no research has involved the use of pyrosequencing analysis to examine vaginal microbiota profiles or their potential associations with high-risk pregnancy in Korean women. Vaginal swabs were collected from 500 Korean women for the identification of community state types (CSTs). Of these, 137 samples were further analyzed using a Roche/454 GS Junior pyrosequencer. Three distinct CSTs were identified based on the dominant vaginal microbes: CST I (Lactobacillus crispatus dominated), CST III (Lactobacillus iners dominated), and CST IV (with diverse species of anaerobes). Twelve of the 67 pregnant women had undesirable pregnancy outcomes (four miscarriages and eight PTBs). The dominant microbe in the vaginal microbiota of women who gave birth at full-term was L. crispatus. In contrast, L. iners was the dominant vaginal microbe in women who miscarried. Most (n = 6/8) vaginal microbiota profiles of women who experienced PTB could be classified as CST IV, with diverse bacteria, including anaerobic vaginal species. The present study provides valuable information regarding the characteristics of the vaginal microbiota of Korean women related to high-risk pregnancy. Investigation of the vaginal microbiotic structure in pregnant Korean women is necessary to enable better prediction of adverse pregnancy outcomes.
