Background: The human papilloma virus (HPV) is considered as the major risk factor for the development of cervical cancer. This virus is of different genotypes and generally can be classified into high and low risk types. Objective: To determine the rate of high risk HPV genotypes in women with vaginal discharge and lower abdominal pain in Kurdistan region, Iraq. Materials and Methods: Cervical swabs were taken from 104 women. DNA was extracted and the polymerase chain reaction (PCR) technique was used to determine the presence of high risk genotypes. Results: It was found that 13/104 (12.5%) of the samples were positive for high risk HPV genotypes. Amongst those who were positive, 4/13 (30.7%) were typed as genotype 16 and 7/13 (53.8%) showed mixed genotyping. On the other hand, genotypes 53 and 56 were found in only one sample each. Conclusions: High risk HPV genotypes are not uncommon and further community based study is needed to determine the prevalence of HPV and its genotypes and plan for prevention of infection.
Background: Gestational trophoblastic neoplasia (GTN) is a spectrum of disease with abnormal trophoblastic proliferation. Treatment is based on FIGO stage and WHO risk factor scores. Patients whose score is 12 or more are considered as at extremely high risk with a high likelihood of resistance to first line treatment. Optimal therapy is therefore controversial. Objective: This study was conducted in order to summarize the regimen used for extremely high risk or resistant GTN patients in our institution the in past 10 years. Materials and Methods: All the charts of GTN patients classified as extremely high risk, recurrent or resistant during 1 January 2002 to 31 December 2011 were reviewed. Criteria for diagnosis of GTN were also assessed to confirm the diagnosis. FIGO stage and WHO risk prognostic score were also re-calculated to ensure the accuracy of the information. Patient characteristics were reviewed in the aspects of age, weight, height, BMI, presenting symptoms, metastatic area, lesions, FIGO stage, WHO risk factor score, serum hCG level, treatment regimen, adjuvant treatments, side effects and response to treatment, including disease free survival. Results: Eight patients meeting the criteria of extremely high risk or resistant GTN were included in this review. Mean age was 33.6 years (SD=13.5, range 17-53). Of the total, 3 were stage III (37.5%) and 5 were stage IV (62.5%). Mean duration from previous pregnancies to GTN was 17.6 months (SD 9.9). Mean serum hCG level was 864,589 mIU/ml (SD 98,151). Presenting symptoms of the patients were various such as hemoptysis, abdominal pain, headache, heavy vaginal bleeding and stroke. The most commonly used first line chemotherapeutic regimen in our institution was the VAC regimen which was given to 4 of 8 patients in this study. The most common second line chemotherapy was EMACO. Adjuvant radiation was given to most of the patients who had brain metastasis. Most of the patients have to delay chemotherapy for 1-2 weeks due to grade 2-3 leukopenia and require G-CSF to rescue from neutropenia. Five form 8 patients were still survived. Mean of disease free survival was 20.4 months. Two patients died of the disease, while another one patient died from sepsis of pressure sore wound. None of surviving patients developed recurrence of disease after complete treatment. Conclusions: In extremely high risk GTN patients, main treatment is multi-agent chemotherapy. In our institution, we usually use VAC as a first line treatment of high risk GTN, but since resistance is quite common, this may not suitable for extremely high risk GTN patients. The most commonly used second line multi-agent chemotherapy in our institution is EMA-CO. Adjuvant brain radiation was administered to most of the patients with brain metastasis in our institution. The survival rate is comparable to previous reviews. Our treatment demonstrated differences from other institutions but the survival is comparable. The limitation of this review is the number of cases is small due to rarity of the disease. Further trials or multicenter analyses may be considered.

Purpose: The purpose of this study was to develop a tool to assess the severity of illness in high risk newborns. Method: The research design was a methodological study. The tool was developed in 4 stages: first, preliminary items were developed based on a questionnaire about the severity of illness index that was given to 8 health professionals in Neonatal Intensity Care Units (NICU) second, a panel of specialists reduced the preliminary items using 3 validity tests; third, final items were selected from the results of a pre-test. Finally, from July 2005 to May 2006, reliability and validity were tested with a sample of 160 high risk newborns admitted to the NICU. Results: The final tool to identify the severity of illness index in high risk newborns consisted 39 items and Cronbach's alpha coefficient for internal consistency was .922. Using factor analysis, 4 factors were extracted and these factors explained 54.451% of the total variance. Conclusion: The instrument for assessing the severity of illness in high risk newborns developed in this study was identified as a tool with a high degree of reliability and validity. In this sense, this tool can be effectively utilized for assessing and implementing care for high risk newborns.
This study investigated whether dietary factors are more influential factor than other health behavior such as drinking, smoking and exercise on abnormal serum cholesterol level inspite of Korean dietary pattern differences compared to Europeans and Americans. A double case control study model has been used for the study design. One model consisted of high blood cholesterol cases and control. the other model consisted of low blood cholesterol cased and controls. 5.398 sedentary male workers who had taken medical examinations at a university hospital were used as the study subjects. Out of the study subjects, 36individuals with high blood cholesterol cases and 30 individuals with low blood cholesterol cases were selected. For the 66 individual control selection, the individual control selection, the individuals matching method was adopted. The food frequency method was used to collect the data for assessment of the dietary factors. A standardized questionnaire was used to investigate other health behavior. logistic regression analysis was employed to measure the relative importance between the factors considered. There were no statistically significant differences observed in nutrients consumption or other health behavior among the low, normal and high blood cholesterol groups, An overmatching effect had been suspected as the cause of those findings. However, the results of logistic regression analysis to identify the factors influencing high serum cholesterol showed that odd ratios of dietary factors such as tocopherol(3.0) and saturated fatty acid(1.6) were higher than I. I of smoking and 1.2 of drinking. Similar results were also observed incases of low serum cholesterol. The above findings imply that although the dietary pattern is quite different from that of Europeans and America, the dietary factor is still a significant factor for abnormal blood cholesterol in Koreans. Therefore, the dietary risk factor identified in high fat consumption populations are still relevant for the relatively healthy Korean as guideline for preventive health practices. Purpose: The purpose of this study was to determine factors related to clinical nurses' nursing intention for high risk pathogen infected patient. Methods: The participants were 230 clinical nurses from four university hospitals which located in B and D metropolitan cities. Data were collected from September 14 to 25 in 2015 and were analyzed using a hierarchical regression. Results: The mean of nursing intention score for high risk pathogen infected patient for the participants was $110.62{\pm}12.68$ (range 36~180). The most influential factor for nursing intention was professional self concept of nurses (${\beta}=.321$), and organizational group culture (${\beta}=.166$) was the next influential factor. The level of education and working department were also statistically significant contributors. Conclusion: It is necessary to increase clinical nurses' nursing intention for high risk pathogen infected patient to improve professional self concept of nurses and organizational group culture.
1. Objectives The risk for cardiovascular diseases increases significantly when multiple risk factors exist, as in the form of metabolic syndrome, compared to a single risk factor. This study was to assess the prevalence rate of metabolic syndrome and evaluate the factors associated with metabolic syndrome and Sasang Constitutional Types. 2. Methods 666 subjects(280 men and 386 women), more 40 years old, in a rural community study were examined in point of Sasang Constitution and metabolic syndrome. 3. Results 1) Their Constitutional distribution were Taeeumin 436 persons (65.5%), Soyangin 83 persons (12.5%), Soeumin 147 persons (22.1%) and no Taeyangin diagnosed by PSSC. 2) Body weight, BMI, waist circumstance and hip circumstance were significantly high in Taeeumin group like previous study results. 3) Prevalence rate of metabolic syndrome was 32.0%, especially 27.1% in men and significantly higher as 35.5% in women than men. 4) In men, prevalence rate of metabolic syndrome was 1.4% in Soyangin, 2.9% in Soeumin and significantly high as 22.9% in Taeeumin. 5) In women, prevalence rate of metabolic syndrome was 12.5% in Soyangin, 3.6% in Soeumin and significantly high as 47.6% in Taeeumin. 6) Hazard ratios for metabolic syndrome were 1.14 for women to men, 2.34 for those at the age of 60s to 40s, 1.95 for Soyangin to Soeumin and 3.39 for Taeeumin to Soeumin. 4. Conclusions Sasang Constitutional Type may be an significant risk factor for metabolic syndrome and regimen according to Sasang Constitutional Type is thought to be needed to prevent metabolic syndrome.
The purpose of this study is to survey and classify the differences of the perceived fashion risk according to the apparels and accessories that consumers purchased. 243 ungraduate were separated into three groups and asked to rate 15 fashion risk concerns about each item on 5-point scale. The number of item was 103 in the total of the three group. Data were analyzed by using Mean, SO, ANOVA, Factor Analysis, Cluster Analysis, Cronbach $\alpha$ with SAS program. The result of this study was high perceived risk in leather Jacket, suit, long coat, sunglasses. The most important factor of the perceived risk structure in the fashion goods was about the perceived risk perception of others. The apparels and accessories which completes the dress were classified into the same cluster. Consumers don't perceive the fashion goods independently, but they make much of the combination of other items.
This study analyzed purchase behavior according to the risk perceptions when customers buy Cosmetics at Internet Shopping Malls. Participants were 232 women in their 20s and 30s with more than one buying experience at an internet fashion shopping mall. Data were analyzed using factor analysis, Cronbach's analysis, cluster analysis, one-way ANOVA and a Duncan test. Results were as follows. Firstly, five factors of risk perception were identified: These were the payment risk, service risk, quality risk, price risk and experience risk. Secondly, customers of internet shopping malls could be categorized into three groups: A low risk perception group, a payment risk perception group and a high risk perception group. Factor analysis showed significant differences between these groups( p<.001). Thirdly, purchase behavior based on the purchase standards, purchase items, information searching were investigated according to the different groups of risk perception of internet shopping malls and results again significant differences between groups (p<.05, p<.001).

Bayesian Networks are multivariate probabilistic factor graphs that are used to assess underlying factor relationships. From January 2005 to December 2018, the study examines how Dynamic Bayesian Networks can be utilized to estimate portfolio risk and return as well as determine inter-factor relationships among reversal profit-generating components in Pakistan's emerging market (PSX). The goal of this article is to uncover the factors that cause reversal profits in the Pakistani stock market. In visual form, Bayesian networks can generate causal and inferential probabilistic relationships. Investors might update their stock return values in the network simultaneously with fresh market information, resulting in a dynamic shift in portfolio risk distribution across the networks. The findings show that investments in low net profit margin, low investment, and high volatility-based designed portfolios yield the biggest dynamical reversal profits. The main triggering aspects related to generation reversal profits in the Pakistan market, in the long run, are net profit margin, market risk premium, investment, size, and volatility factor. Investors should invest in and build portfolios with small companies that have a low price-to-earnings ratio, small earnings per share, and minimal volatility, according to the most likely explanation.
Objective : The objective of this study was to examine how experts and the public perceived various environmental and technological hazards based on psychometric paradigm. Methods : We conducted a survey that included 30 hazards and 10 risk attributes. Subjects of this study were 214 people with three groups; (1) experts (55 people), (2) graduates( 78 people), (3) under graduates (81 people). Factor analysis was used to confirm the common risk attribute from 8 risk attributes. Also, multiple regression analysis was used to identify factors influencing on perceived risk and benefit of hazards. Results : This study revealed that experts tend to be more tolerant than graduates and under graduate students for the 30 hazards. Using factor analysis, two main factors were identified: factor 1, commonly called "Dread Risk", and factors 2, commonly called, "Unknown Risk" in the literature. We identified that environmental hormone concentration and global warming ranked high in both dread risk and unknown risk. Multiple regression models were used to test the association of perceived social risk and perceived social benefit with two main factors. Dread risk had significant explanatory power on perceived social risk and benefit. We identified that the experts were less likely to perceived dread risk and know more information about the hazards. Conclusions : There were differences of risk perception between experts and lay people. Especially, experts' perception of risk was commonly lower than other people's perception.
