Alcohol as a Risk Factor for Cancer: Existing Evidence in a Global Perspective

  • The purpose of the present review is to give an overview of the association between alcohol intake and the risk of developing cancer. Two large-scale expert reports; the World Cancer Research Fund (WCRF)/American Institute of Cancer Research (AICR) report from 2007, including its continuous update project, and the International Agency for Research of Cancer (IARC) monograph from 2012 have extensively reviewed this association in the last decade. We summarize and compare their findings, as well as relate these to the public health impact, with a particular focus on region-specific drinking patterns and disease tendencies. Our findings show that alcohol intake is strongly linked to the risk of developing cancers of the oral cavity, pharynx, larynx, oesophagus, colorectum (in men), and female breast. The two expert reports diverge on the evidence for an association with liver cancer and colorectal cancer in women, which the IARC grades as convincing, but the WCRF/AICR as probable. Despite these discrepancies, there does, however, not seem to be any doubt, that the Population Attributable Fraction of alcohol in relation to cancer is large. As alcohol intake varies largely worldwide, so does, however, also the Population Attributable Fractions, ranging from 10% in Europe to almost 0% in countries where alcohol use is banned. Given the World Health Organization's prediction, that alcohol intake is increasing, especially in low- and middle-income countries, and steadily high in high-income countries, the need for preventive efforts to curb the number of alcohol-related cancers seems growing, as well as the need for taking a region- and gender-specific approach in both future campaigns as well as future research. The review acknowledges the potential beneficial effects of small doses of alcohol in relation to ischaemic heart disease, but a discussion of this lies without the scope of the present study.

노인의 스트레스, 우울 및 건강위험 행위에 관한 연구 (Stress, Depression and Health Risk Behaviors in Korean Elders)

  • Purpose: The purpose of this study was to investigate stress, depression and health risk behaviors in elderly people. Methods: The subjects were 238 Korean elders who had visited the convalescent center. The level of stress was measured by the Korean version of BEPSI, which was developed by Bae, Jeung & Yoo (1992) and modified by Yim et al.(1996). CES-D was used to measure the level of depression. For health risk behaviors, we measured cigarette consumption and alcohol drinking during the last one month. Results: Of the subjects, 17.4% were excessive drinkers and 18.9% were currently smokers. The level of stress was between moderate and high, and 62.7% of the subjects were depressed. There were significant differences in stress and depression according to economic status and self-reported health. In addition, depression had positive correlations with stress and QF score. Conclusion: Stress and depression management programs for the elderly are needed. These findings indicate that nurses must consider stress and alcohol drinking when they take care of depressed elderly people.

성별에 따른 아파트 주민의 음주, 사회적 지지와 사회적 연결망과의 관계 (Relationships among Social Support, Social Networks and Drinking Behavior by Gender Differences in Residents of an Apartment Complex in Seoul)

  • This study explores the gender differences in the relationship between social support, social networks and drinking behavior and illustrates gender differences in drinking behavior in order to provide evidence in planning a community health promotion program. Data was collected from 444 surveys (Male=190, Female=244) from a total of 1,899 adult residents in 738 households in the "Y" Apartment Complex in the Gangnam area of Seoul, Korea. Results show significantly higher rates of drinking frequency and alcohol consumption volume in males. Women with high-risk drinking behavior have fewer social relationships than women with normal drinking behavior. Within social networks, friends were significantly associated with drinking behavior and alchol abuse. Drinkers had more friends than non-drinkers in both genders. However, in association with alchol abuse, while male abusers had less friends than normal male drinkers, women abusers had more friends, indicating women who have drinking problems have a stronger social network. This pattern suggests gender differences in the association between social networks and alchol abuse. Therefore in approaching drinking issues, social support and social networks act as a key factor. For men, alcohol prevention programs should be aimed at achieving healthy drinking behavior at the aggregate level including people in his social support and social network groups. For women, the priority is alchol abuse. More attention is required in revealing the context between female social networks and alchol abuse and in developing coping strategies other than drinking.

학교 내 손상에 미치는 영향 요인: 학교 폭력 경험을 중심으로 (Factors affecting Unintentional Injuries at School: Focused on Violence)

  • Purpose: The study aimed to identify factors affecting unintentional injuries at school focusing on violent experiences in adolescents. Methods: The study used the raw data of the Korea Youth Risk Behavior Web-based Survey (2019). A total of 60,040 students were included in the analysis. Descriptive statistics, χ2 test, and multiple logistic regression were performed, using SPSS version 25.0. General characteristics, gender, school, academic achievement, living with family, socioeconomic status, vigorous physical activity, perceived stress, sleep satisfaction, high caffeine drinking, risky alcohol drinking, smoking, sexual experience, drug use, and safety education, were adjusted for and the relationship between unintentional injuries and violence was analyzed. Results: The results showed 2.4% of the adolescents had experienced violence and 24.7% had suffered unintentional injuries. Violent experiences had a significant effect on unintentional injuries among adolescents. Injuries were 1.93 times higher among adolescents who had experienced violence. Gender, school levels, high-intensity physical activity, stress, sleep satisfaction, high caffeine drinks, dangerous drinking, drug use, and safety education were associated with unintentional injuries among adolescents. Conclusion: Adolescents at high risk of suffering violence and injuries should be identified and assessed preemptively. In addition, it is necessary to implement systematic school health-centered safety education programs in order to prevent violence and injuries.

남자 청소년의 약물사용 위험성에 관한 연구 (A Study on the Risk of Drug Use by Male Adolescents)

  • Purpose: The purpose was to identify the risk of drug use by adolescents. Method: The participants were 933 male students in the first grade of a high school in D city. The data were collected from Aug. 5th to Oct. 30th, 2004. The instrument was the High Risk Group Adolescent Drug User Screening Test(HIRIGADUST) developed by the Korea Adolescent Society(1996). The data were analyzed using SPSS. Results: For substance use, 64.5% of the students answered that they had drunk, 40.3% that they had smoked, and 2.0% that they had tried drug use. For scores on HIRIGADUST regarding socio-demographic characteristics, there were significant differences depending on school type, personality, academic performance, economic status, and ability to talk with parents. For scores on HIRIGADUST regarding drug using-related characteristics, there were significant differences depending on drinking experience, frequency of drinking, amount of alcohol intake, smoking experience and number of cigarettes smoked. Of the students 27.2% students were in the high risk group. Conclusion: In schools, systematic and intensive assessment of drug use should be done, and if needed, a service system connected to clinics specializing in drug addiction should be established. Prevention education should be carried out continuously.

청소년의 흡연 및 음주 행태와 사회경제적 수준과의 관계 (The Effects of Socio-Economic Status on Drinking and Smoking in Korean Adolescents)

  • There is growing evidence that substance use such as tobacco or alcohol consumption influences health disparity among adolescents. Previous research papers have shown an inconsistency in the relationship between socio-economic status (SES) and substance use in adolescents. However, little is known about socio-economic differences in unhealthy behaviors among Korean adolescents. The purpose of the present study is to explore associations between SES and substance use in Korean adolescents. The analysis was performed using data from the 2009 Korean Youth Risk Behavior Web-based Survey (YRBS), which included a nationally representative sample of middle and high school students. Drinking/heavy drinking and smoking/daily smoking behavior indices were used for dependent variables, and perceived economic status, family affluence score, parents' education were used for independent variables. Chi-square test were used to compare tobacco and alcohol consumption among 3 SES groups. Logistic regression models were used to identify statistically significant socio-economic factors after adjusting other covariates. Higher perceived economic status and higher family affluence were associated with higher rates of smoking, daily smoking, drinking, and heavy drinking, while lower level of parents' education was related to higher use of tobacco and alcohol. Socio-economic status significantly influences health behaviors in adolescents, and it may consequently affect health disparity in their adulthood. Therefore, there is a need of continuous monitoring and follow-up research of health disparity among adolescents.

성경험이 있는 여자 고등학생의 성매개감염 영향요인 (Predictors of Sexually Transmitted Infection among Adolescent Females in Korea)

  • Purpose: This study investigated the predictors of Sexually Transmitted Infections (STI) among female teenagers in Korea who have had intercourse at least once. Methods: This study is based on the 12th Korea Youth Behavior Web-based Survey conducted in April 2016. Data were collected from 798 middle schools and high schools nationwide in Korea and 65,528 students participated. Among the participants, the subjects of this study were 537 female high school students (Grade 10~12). The data were analyzed through complex samples multiple logistic regression using SPSS statistics 22. Results: Among the 537 female adolescents, 11.9% replied they had experienced STI. The predictors of STI among the subjects were 'age at first intercourse' and 'sexual intercourse after drinking'. The risk of STI was lower in the middle (OR=0.26, 95% CI=0.10~0.64) and high school (OR=0.11, 95% CI=0.04~0.27) age groups than those who had their first sexual intercourse at an elementary school age. The sexual intercourse after drinking group had a higher risk of STI, compared to the no sexual relation after drinking group (OR=2.54, 95% CI=1.28~5.07). Conclusion: Practical sex education programs should begin from the elementary stage in order to protect more female adolescents from STI. In addition, sex education including an alcohol prevention program should be considered to lower STI among female adolescents.

Changes of Plasma Cardiovascular Disease Risk Factors according to the Health Practice and Dietary Habits in Healthy Male University Studnets

  • This cross-sectional study was conducted to describe the changes of plasma cardiovascular disease(CVD) risk factors in Korea. Overnight fasting plasma levels of total cholesterol, high density lipoprotein(HDL)-cholesterol, triacylglycerol and glucose were analyzed. Blood pressure and anthropometric data were also measured. Health practice factors such as smoking status, alcohol consumption and frequency fo exercise were evaluated by a self-administered questionnaire. Questions regarding dietary habits and food preferences were also asked. Seventy eight percent of the subjects had more than one CVD risk factor. Plasma total cholesterol, triacylglycerol, and fasting blood glucose were significantly increased according to the subjects body mass index$(kg/m^2$, BMI), whereas HDL-cholesterol, low density lipoprotein(LDL)-cholesterol and blood pressure showed no significant differences with BMI. Current smokers had significantly high plasma total cholesterol, LDL-cholesterol and triacylglycerol levels. Alcohol consumption significantly increased plasma total cholesterol and fasting blood sugar, but regular exercise had no effects on the plasma CVD risk factors. Overeating and frequency of fast food consumption were positively correlated with the CVD risk score, whereas intake of grains, meats and vegetables were negatively correlated with that score. A stepwise multiple regression analysis was performed to examine the effects of specific dietary factors on plasma lipid levels. For plasma total cholesterol level, the frequency of fast food intake explained 8% of the variance, followed by habitual overeating, frequency of grain intake and high cholesterol food intake(Model $R^2$=22.4%). For plasma triacylglycerol level, preference of oily foods accounted for 7.5% of the variance, followed by eating breakfast, preference of fruit and frequency of grain intake(Model $R^2$=22.0%). The findings suggest that intervention programs to reduce the risk of CVD should focus on health practice through reducing BMI, smoking cessation and moderate or no alcohol drinking. Moreover, desirable dietary habits such as eating breakfast, not overeating and reduced intake of fast food may improve CVD risk.

경기도 일지역의 문제음주실태와 알코올 미충족욕구 조사연구 (Alcohol Problems and Related Service Needs in Urban Korean Community)

  • 연구목적 본 연구는 경기도 일지역의 음주실태, 위험음주와 알코올사용장애의 정도 및 지역사회 음주정책 요구도 조사를 위해 수행되었다. 방 법 연구대상자는 경기도 안산시 거주 19세 이상 65세 미만 성인 1,000명으로 2011년 6월부터 7월까지 두 달 간 조사하였다. 조사설문지는 연구목적에 의해 구성한 사회인구학적 특성 문항, Alcohol Use Disorder Identification Test(AUDIT)와 Cut down, Annoyed, Guilty, Eye-opener(CAGE) 척도, 지역사회 음주문제 해결에 대한 서비스 및 정책 요구도에 대한 문항으로 구성하였다. 자료분석은 SPSS version 18.0 통계프로그램을 이용하였다. 결 과 대상군의 평생음주율은 97.4%이고, 이들 중 21.9%가 주 2~4회 음주 빈도를 보였으며, 유해음주 빈도는 남성의 24.4%로 남성 4명 중 1명은 한 번의 좌석에서 소주 한 병 또는 맥주 4병 이상을 마시는 빈도가 주 1회 이상이었다. 음주시작 연령은 평균 20.3세이나 19세 이전에 음주를 시작하는 경우가 51.6%이었다. AUDIT 척도를 적용한 유해음주 및 알코올사용장애의 비율은 각각 14.9%, 3.3%, CAGE 척도 기준을 적용하였을 때 알코올사용장애자는 9.6%였다. 조사응답자들의 지역사회 음주문제에 대한 서비스 및 정책요구도는 예방교육(57.2%), 치료시설 연계(50.1%), 알코올상담과 개입(41.7%), 그리고 재활프로그램(39.8%) 순이었다. 결 론 본 연구결과, 경기도 도시 지역의 유해음주와 알코올사용장애 등 문제음주의 정도는 높은 비율로 나타나며, 지역주민들의 음주문제 관련 요구도와 음주시작연령에 대한 고려를 통해 효과적인 음주예방프로그램과 정책개발이 요구된다.

Perceptions About Alcohol Harm and Alcohol-control Strategies Among People With High Risk of Alcohol Consumption in Alberta, Canada and Queensland, Australia

  • Objectives: To explore alcohol perceptions and their association hazardous alcohol use in the populations of Alberta, Canada and Queensland, Australia. Methods: Data from 2500 participants of the 2013 Alberta Survey and the 2013 Queensland Social Survey was analyzed. Regression analyses were used to explore the association between alcohol perceptions and its association with hazardous alcohol use. Results: Greater hazardous alcohol use was found in Queenslanders than Albertans (p<0.001). Overall, people with hazardous alcohol were less likely to believe that alcohol use contributes to health problems (odds ratio [OR], 0.46; 95% confidence interval [CI], 0.27 to 0.78; p<0.01) and to a higher risk of injuries (OR, 0.54; 95% CI, 0.33 to 0.90; p<0.05). Albertans with hazardous alcohol use were less likely to believe that alcohol contributes to health problems (OR, 0.48; 95% CI, 0.26 to 0.92; p<0.05) and were also less likely to choose a highly effective strategy as the best way for the government to reduce alcohol problems (OR, 0.63; 95% CI, 0.43 to 0.91; p=0.01). Queenslanders with hazardous alcohol use were less likely to believe that alcohol was a major contributor to injury (OR, 0.39; 95% CI, 0.20 to 0.77; p<0.01). Conclusions: Our results suggest that people with hazardous alcohol use tend to underestimate the negative effect of alcohol consumption on health and its contribution to injuries. In addition, Albertans with hazardous alcohol use were less in favor of strategies considered highly effective to reduce alcohol harm, probably because they perceive them as a potential threat to their own alcohol consumption. These findings represent valuable sources of information for local health authorities and policymakers when designing suitable strategies to target alcohol-related problems.
