Transdural Nerve Rootlet Entrapment in the Intervertebral Disc Space through Minimal Dural Tear : Report of 4 Cases

  • Four patients underwent lumbar surgery. In all four patients, the dura was minimally torn during the operation. However, none exhibited signs of postoperative cerebrospinal fluid leakage. In each case, a few days after the operation, the patient suddenly experienced severe recurring pain in the leg. Repeat magnetic resonance imaging showed transdural nerve rootlets entrapped in the intervertebral disc space. On exploration, ventral dural tears and transdural nerve rootlet entrapment were confirmed. Midline durotomy, herniated rootlet repositioning, and ventral dural tear repair were performed, and patients' symptoms improved after rootlet repositioning. Even with minimal dural tearing, nerve rootlets may become entrapped, resulting in severe recurring symptoms. Therefore, the dural tear must be identified and repaired during the first operation.

A Case of Intradiscal Oxygen-ozone Injection Therapy for Cervical Herniated Intervertebral Disc in a Dog (개에서 탈출된 경추디스크에 대한 디스크내 오존가스 주입 치료)

  • A 6-year-old castrated male Shih-tzu was presented because of a 1-month history of progressive tetraplegia. On the basis of the findings of neurological examination, radiographs and magnetic resonance imaging, herniated intervertebral disc of C4-C5, C5-C6 and C6-C7 compressed spinal cord. The oxygen-ozone was intra-operatively injected into the C4-C5 and C6-C7 intervertebral disc and a ventral decompression performed at C5-C6 intervertebral space. Immediately after the surgery, neck pain was disappeared and three weeks after surgery, there was complete resolution of the neurological deficits with normal gaits. On magnetic resonance images at 2-month after surgery, herniated disc materials of C5-C6 and C6-C7 were completely removed and that of C4-C5 remarkably reduced. Consequently decompression which was performed at all of the three lesions resulted in good clinical outcome without additional internal fixation for minimizing postoperative instability.

A Retrospective Statistical Analysis of Miniscalpel Needle Therapy for Herniated Intervertebral Disc or Spinal Stenosis

  • Background: This study examined the characteristics and prognosis of patients admitted to the Dunsan Korean medicine hospital for treatment of herniated intervertebral disc (HIVD) or spinal stenosis with Miniscalpel needle therapy (MSN). Methods: Patients were admitted to the Dunsan Korean medicine hospital from January 01, 2016 to September 30, 2017 for the treatment of HIVD or spinal stenosis with MSN. Crossover analysis, Independent sample t test, one-way ANOVA, multiple linear regression analysis, and binary logistic regression analysis were performed. Results: Crossover analysis showed statistically significant differences in treatment methods according to gender, current pain according to the disease duration, satisfaction of MSN according to disease duration, treatment methods, and intention of re-treatment with MSN according to treatment methods. Independent t test and one-way ANOVA showed that there was a difference in current Numeric Rating Scale (NRS) according to disease duration, and difference between discharge and current NRS, and number of MSN according to disease. Multiple linear regression analysis showed that age, disease duration, and number of MSN affect discharge NRS, disease duration, and number of MSN affect current NRS, and Western medical treatment after MSN, discharge NRS, and current NRS affect satisfaction of MSN. Binary logistic regression analysis showed that discharge NRS affects current pain, and gender, discharge NRS, and treatment methods affect intention of re-treatment with MSN. Conclusion: Characteristics, prognosis, satisfaction and variables affecting prognosis of MSN were statistically significant, indicating that more systematic studies are required to further examine the effects of MSN on HIVD or spinal stenosis.

Electrical Acupuncture Combined Bee Venom Therapy for Pain and Disability Induced Intervertebral Herniated Disc of L-spine;A Pilot Study

  • To examine the efficacy of electrical acupuncture combined bee venom in controlling low back with leg pain induced Herniated Intervertebral Disc (HIVD). 13 patients (men 8, women 5; age $41.30\;{\pm}\;17.27$) diagnosed as HIVD by CT or MRI had low back and leg pain as a chief complaint and diseases such as spinal tumor, infection and fracture were excluded. They were treated by electrical acupuncture (8 Hz) combined bee venom therapy three times a week in Wonkwang University Hospital, and pain intensity with VAS and functioning with Roland-Morris Disability Qestionnaire(RDQ) were measured at the baseline(B), and 4 weeks after treatment. The data were analyzed with Wilcoxon signed ranks test(SPSS12.0) and the level set for statistical significance was P<0.05. The pain was significantly reduced by two combined treatments (=0.002;$B7.69{\pm}2.06$,4Wks$4.70{\pm}2.72$). The function was also significantly improved by both treatments(=0.001;$B12.15{\pm}6.85$,4Wks$6.85{\pm}5.15$). The results suggest that a combination of acupuncture and bee venom therapy may be beneficial for low back pain from HIVD. In future, randomized clinical trials including follow-up are needed to examine the effects of these therapies.

Case Report of Syringomyelia with Herniated Intervertebral Disc of Cervical Spine Treated by Korean Medicine Therapy (척수공동증과 경추 추간판 탈출증을 진단받은 환자에 대한 한방치료의 치험 1례 보고)

  • Objectives : The purpose of this clinical case is to evaluate the efficacy of Korean Medicine treatment on Syringomyelia with Herniated Intervertebral Disc(HIVD) of C-SPINE. Methods : We used Acupunture, herbal acupuncture, herbal medicine for this patient. This patient received hospital treatment for 15 days. Results : Patient's chief complain were neck pain and trapezius pain. NRS of chief complaint decreased from 6 to 2 and Patient's NDI decreased from 46 to 14. Conclusions : We experienced improvement of symptoms in this case by acupuncture stimulation, herbal medicine. and we hope this study is helpful for the treatment of Syringomyelia with HIVD of C-SPINE.

The Case Report of Conservative Treatment on the Herniation of Intervertebral Disc Patient After the Fusion Surgery of a Lumbar Segment (요추부 한 분절 유합술 이후 발생한 추간판 탈출증환자에 대한 보존적 치료 치험 1례)

  • Objectives: The purpose of this study is the evaluation of conservative medical treatment on the herniation of intervertebral disc(HIVD) patient after the fusion surgery of a lumbar segment. Methods: We used acupuncture, herbal medication, and manipulation for this patient. And we measured of VNRS score and SLR test result to evaluate effect of conservative medical treatment. Results & Conclusions: Patient's low back and left leg pain VNRS is decreased, and the SLR test result is improved.

Effectiveness of ShinBaro Pharmacopuncture on Lumbar Spinal Herniated Intervertebral Disc : A Randomized Controlled Trial (요추추간판탈출증에 대한 신바로약침의 효과 : 무작위 대조군 시험)

  • Objective : The aim of this study is to investigate the effectiveness of ShinBaro Pharmacopuncture in the treatment of patients with Lumbar spine Herniated Nucleus Pulposus. Methods : We investigated 20 cases of patients with lumbar disc herniation and devided into two groups. Experimental group were treated with ShinBaro Pharmacopuncture with integrated package treatment and control group were treated the same therapies without ShinBaro Pharmacopuncture. To evaluate the treatment effects of two groups, we used numeric rating scale(NRS), oswestry disability index(ODI). Results : In NRS of lumbar and sciatica pain and ODI, that of experimental group was much more improved than control group, and decrement of NRS of lumbar pain showed statistical significance along with the duration of treatment. Conclusions : ShinBaro Pharmacopuncture was thought to be effective for relieving symptoms of lumbar spine herniated nucleus pulposus, although further study is needed.

Assesment of Bee-venom Acupuncture Effect on Herniated Disc Patients by Rating Scale (요추간판탈출증 환자 22례에 대한 봉독약침 치료의 요통 평가지수를 이용한 임상보고)

  • Herniation of intervertevral disc(HIVD) is the most common disease causing low back pain. Acupuncture, electroacupuncture and moxibustion has been used for treatment of HIVD. Bee venom (BV) also has been traditionally applied to relieve pain. Recently, it has been reported that BV is effective in treating HIVD. But more objective methods were required to assess the efficacy of BV on HlVD. So we assessed of BV-acupuncture effect on herniated disc patients by use of rating scale for low back pain. We researched 22 patients who were treated at Department of Acupuncture & Moxibustion, Kyung Hee Medical Center from January 1999 to June 1999. The patients were diagnosed by MRI as having a herniated disc. The results of treatment using rating scale for low back pain in HIVD showed that $43.77{\pm}14.39$ point of pre-treatment score and $84.5{\pm}3.97$ point post-treatment score. The improvement index showed $0.43{\pm}0.15$. Especially, clinical score of neurologic deficits showed most highest improvement index of $0.7{\pm}0.45$. Assessment of BV-acupuncture Effect by MRI finding were showed that protruded disc and extruded disc got more higher improvement index than bulging disc. Among 22 patients, satisfactory assessment of BV-acupuncture effect showed that 15(68.2%) were excellent, and 7(31.8%) were good. The result suggest that BV-acupuncture is good method for treatment of HIVD.

The Effect of Korean Medical Combination Treatment on 72 Cases of Herniated Intervertebral Lumbar Disc Patients: An Observational Study (요추 추간판탈출증 입원환자 72례에 대한 한의학적 복합치료 효과의 관찰 연구)

  • Objectives : This study was designed to assess the general distribution and clinical effectiveness of Korean medical treatment on lumbar disc herniation. Methods : This is an observational study. 72 patients admitted to Daejeon Jaseng Hospital of Korean Medicine with a diagnosis of herniated intervertebral disc(HIVD) by lumbar-CT of lumbar-MRI were observed from July, 2014 to April, 2015. They were analyzed according to sex, age, the period of disease, causal factors, symptoms on admission, admission day, disc herniation type and treatment efficacy. All patients received a combination of treatments during hospitalization, including acupuncture, Chuna, herbal medicines and physical therapy. A zero to ten numerating rating scale(NRS) assessing pain, Oswestry disability index(ODI) and EuroQol-5 dimension(EQ-5D) was used before and after treatments. Results : Average admission duration was $28.00{\pm}12.85$ days in lumbar disc patients. For lumbar patients, lower back pain NRS decreased from $5.89{\pm}2.00$ to $3.42{\pm}1.87$(p<0.001) and radiating pain from $5.96{\pm}2.12$ to $3.38{\pm}1.83$(p<0.001). ODI decreased from $46.69{\pm}19.25$ to $35.69{\pm}16.67$(p<0.001), and EQ-5D index increased from $0.63{\pm}0.26$ to $0.71{\pm}0.20$(p<0.05) after treatment in lumbar disc patients. Conclusions : Korean medical combination treatment might be effective in reducing pain and improving quality of life for patients with lumbar disc herniation. This study further confirmed the efficacy of Korean medical treatment on HIVD.

A Review of Acupuncture Treatment Methods for Lumbar Herniated Intervertebral Disc

  • The purpose of this study was to review clinical studies of lumbar herniated intervertebral disc (LHIVD) treatment using acupuncture. Online database (PubMed, COCHRANE Library, EMBASE, CNKI, KISS, NDSL, KoreaMed, KMbase, OASIS, and KISTI) searches were conducted in May 2018. Studies that used acupuncture, electroacupuncture or warm needle acupuncture were included, along with participants who had lower back pain and radiating pain of their lower limbs consistent with radiological findings. Animal studies and nonclinical data were excluded. Data on treatment methods, site, time, frequency, period, and scales used were analyzed. There were 69 studies including 38 randomized controlled trials, 14 retrospective observational studies, and 17 clinical case studies. There were 51 acupoints selected for acupuncture treatment of LHIVD. The most frequently treated acupoints were BL23, BL25, BL24, and BL40. The mean treatment time, frequency, and duration were $26.06{\pm}6.70$ mins, $6.29{\pm}1.70$ times/week, and $20.57{\pm}11.04$ days, respectively, in randomized controlled trials (RCT), and $18.62{\pm}4.60$ mins, $11.58{\pm}3.99$ times/week, and $34.43{\pm}17.62$ days, respectively, in case/retrospective studies ($mean{\pm}SD$). This review collates information about acupuncture treatment methods for LHIVD.
