Effect and Safety of Calorie Restriction and Complex Herbal Medicine Short-term Treatment ('Oil-rescue' Program) for Weight Reduction of Obesity or Overweight Patients: A Practice Based Research (과체중 및 비만 환자의 체중감량을 위한 식사량 조절 및 복합 한약 단기 치료(5일 레스큐 프로그램)의 효과와 안전성: 진료기반 연구)

  • Objectives: This study is a practice based research conducted to investigate the effectiveness and safety of a 5-day short-term diet program ('Oil-rescue' program) designed to reduce the adverse events and initial dropout rate in obesity treatment. Methods: We retrospectively analyzed 4 Korean medicine clinic patient data who completed 'Oil-rescue' program which is consisting of Gambi-hwan, Bium-hwan, Butgiban-hwan and Jayoon Kyungokgo. The weight change before and after participating 'Oil-rescue' program was primary outcome of our study. Changes in body mass index, body fat mass, skeletal muscle mass, body water content, lean body mass, protein, and minerals were also measured. Results: A total of 35 patients who satisfied the eligible criteria were finally included. The body weight decreased from 69.45±11.86 kg to 67.43±11.58 kg, a total of 2.02±1.03 kg (P<0.001). Body fat mass decreased from 25.77±7.45 kg to 24.98±7.26 kg, a total of 0.78±1.21 kg (P<0.001). Body mass index decreased from 26.39±3.64 kg/㎡ to 25.64±3.49 kg/㎡, a total of 0.75±0.41 kg/㎡. (P<0.001). A total of 15 patients had side effects such as diarrhea, abdominal pain, and insomnia, but all were mild. 16 out of 35 people switched to long-term obesity treatment programs. Conclusions: Through this retrospective practice based research, it was found that the 'Oil-rescue' program effectively reduced body weight, body fat, and body mass index, and the other obesity related parameters. It was a relatively safe and effective short-term obesity treatment program.

A Case Report of Meralgia Paresthetica (Meralgia Paresthetica 치료(治療) 1례에(例) 대한 증례보고)

  • Objective : The purpose of this case is to report the improvement after treatment about a patient with meralgia paresthetica. Methods : We treated the patient with acupuncture therapy and Herbal medication from 11th October 2004 to 15th October 2004 by evaluating femoral function with VAS score and R.O.M of femur joint. Results : After 5 times of treatment, this patient achieved excellent outcome following the technique, showing that clinical symptom as like heating sense, edema and pain was disappeared, VAS changed from 10 to 1 and there was no limitation of ROM of femur. Conclusions : Meralgia paresthetica is a symptom complex that includes numbness, paresthesias, and pain in the anterolateral thigh, which may result from either an entrapment neuropathy or a neuroma of the lateral femoral cutaneous nerve(LFCN). Oriental medical treatment for meralgia paresthetica resulted in satisfactory results by diminishing the symptoms progressively during the five days of treatment. Differential diagnosis was based on careful physical examination. More research of meralgia paresthetica is needed.

  • Zinc has been considered as a vital constituent of proteins, including enzymes. Mobile reactive zinc ($Zn^{2+}$) is the key form of zinc involved in signal transductions, which are mainly driven by its binding to proteins or the release of zinc from proteins, possibly via a redox switch. There has been growing evidence of zinc's critical role in cell signaling, due to its flexible coordination geometry and rapid shifts in protein conformation to perform biological reactions. The importance and complexity of $Zn^{2+}$ activity has been presumed to parallel the degree of calcium's participation in cellular processes. Whole body and cellular $Zn^{2+}$ levels are largely regulated by metallothioneins (MTs), $Zn^{2+}$ importers (ZIPs), and $Zn^{2+}$ transporters (ZnTs). Numerous proteins involved in signaling pathways, mitochondrial metabolism, and ion channels that play a pivotal role in controlling cardiac contractility are common targets of $Zn^{2+}$. However, these regulatory actions of $Zn^{2+}$ are not limited to the function of the heart, but also extend to numerous other organ systems, such as the central nervous system, immune system, cardiovascular tissue, and secretory glands, such as the pancreas, prostate, and mammary glands. In this review, the regulation of cellular $Zn^{2+}$ levels, $Zn^{2+}$-mediated signal transduction, impacts of $Zn^{2+}$ on ion channels and mitochondrial metabolism, and finally, the implications of $Zn^{2+}$ in health and disease development were outlined to help widen the current understanding of the versatile and complex roles of $Zn^{2+}$.

  • This study was carried out to investigate the in vivo and in vitro inhibitory effect of a traditional herbal complex (HC) extract prepared from a mixture of four oriental herbs (Dioscorea Rhizoma, Glycine soja Sieb. et Zucc, Bombycis corpus, Fermented Glycine soja) that have been widely used for the treatment and prevention of diabetes mellitus on hyperglycemia. The water extract of HC showed potent inhibitory effect on $\alpha$-glucosidase with $IC_{50}$ value of 1.24 mg/mL. Additionally, the ethanol extract of HC was also found to exhibit significant inhibitory effect against protein tyrosine phosphatase $1{\beta}$ ($PTP1{\beta}$), which is known as a major regulator of both insulin and leptin signaling. In the $PTP1{\beta}$ inhibitory assay, the most active n-hexane fraction obtained from the ethanol extract of HC, was identified as a mixture of fatty acid derivatives by gas chromatography-mass spectrometry (GC-MS). In high-fat diet-low dose streptozotocin (STZ)-induced diabetic rat, the water extract of HC improved the oral glucose intolerance as compared with rosiglitazone. HC also caused a marked decrease of body weight and fasting blood glucose and a significant improvement on glucose tolerance in metabolic syndrome mice model. These findings support that this traditional HC may be useful in the control of blood glucose in diabetes mellitus and metabolic syndrome.

A Review of Marine Algae-derived Therapeutic Agents for Respiratory Disease Asthma (해조류 유래 호흡기 질환 천식 치료제 연구 동향)

  • Asthma is a complex inflammatory disease of the lung characterized by variable airflow obstruction, airway hyperresponsiveness, airway inflammation, and reduction of respiratory function. Its prevalence and incidence are increasing because of the effect of various environmental and lifestyle risk factors. Steroid inhalation, long-acting agonists, and other synthetic drugs are used for the treatment of this disease. However, they have some side effects and show unsatisfied result and response after treatment. Therefore, many researchers have focused on the development of natural product-related treatment for asthma to suppress the side effects and unsatisfied results. Seaweeds contain various bioactive compounds with anti-inflammatory, antibacterial, and anti-oxidant activities. Thus, we investigated the asthma treatment-related literature using marine algae via the Google scholar search engine. Consequently, the literature is rarely investigated, but is increasing steadily. The literature was performed as a comparison study with an ovalbumin-induced group or drug-treated group, and investigated the antiasthma activity of algae ethanol extract. Although many researchers have studied marine algae-derived therapeutic agents for asthma, the amount of literature is rare compared with those of herbal medicine-derived therapeutic agents. Conclusively, we suggest that many researchers should investigate and develop algae-derived therapeutic agents for asthma treatment.

Effects of Skin Anti-Aging Wellness Program on Factors Related to Wellness Index and Skin Health (피부건강 웰니스 프로그램이 웰니스지수 및 피부건강지표에 미치는 영향)

  • Purpose : Wellness tourism is defined as travel for the purpose of promoting health and well-being through physical, psychological, or spiritual activities. The development and verification of a comprehensive wellness program for health care for workers is needed. The purpose of this study is to investigate the wellness and skin health effects of skin health programs in order to develop high-value health care services. Methods : The subjects were 15 middle-aged women who understood the research and volunteered and participated in the two-day skin health wellness program. Participants were measured to determine their wellness index and skin health twice: before and after participating in the program. Wellness index measures include comprehensive wellness, physical wellness, mental wellness, emotional wellness, social/cultural wellness, financial wellness, and environmental wellness. Skin health measurement items consisted of skin oil, skin water, skin pore, skin elasticity, skin wrinkle, and skin pigment. The skin wellness program included aerobic exercise, endurance exercise, functional food, cosmetics, herbal tea, massages, spa treatments, meditation, and marine leisure sports. The t-test analysis was used to compare the difference between wellness index and skin health measurement items before and after the program. Results : Among the wellness index items, the comprehensive wellness index (p<.05), mental wellness index (p<.05) and environmental wellness index (p<.05) showed statistically significant differences. Among skin health items, skin oil (p<.05), skin elasticity (p<.01) and skin wrinkle (p<.01) all showed statistically significant differences. There was no statistically significant difference in physical wellness, emotional wellness, social/cultural wellness, financial wellness, skin water, skin pore and skin pigment. Conclusion : This study confirmed that the two-day complex wellness program is an effective program for some items of wellness index and skin health.

Review on the mechanism for the reduction of raphide-induced toxicity via processing of Pinelliae Tuber and Arisaematis Rhizoma (포제(炮製)에 의한 반하(半夏)와 천남성(天南星)의 침상결정 유발 독성 감소 기전 고찰)

  • Objectives : The processing of Pinelliae Tuber and Arisaematis Rhizoma is a crucial step to reduce the severe acrid irritation mainly due to the needle-like crystals (raphides). Ginger, alum and bile juice have been used as adjuvant materials for the processing. Methods : Bibliographic research on ancient processing and experimental processing was performed to investigate the toxicity reduction mechanisms of the processing with ginger, alum and bile juice. Results : Ginger has been a major adjuvant for the processing of Pinelliae Tuber, followed by alum and bile juice since Song (宋) and Myeong (明) dynasties, and Arisaematis Rhizoma has been mainly used as Damnamseong (膽南星). The raphides consisting of calcium oxalate, lectin, agglutinin and polysaccharides can induce acrid irritation and the inflammatory reactions. The lipophilic components in the ginger denatured the structure of raphides and 6-gingerol-contained ginger extract attenuated the inflammatory reaction. The calcium ion (Ca2+) of calcium oxalate was substituted to the aluminium ion (Al3+) of the alum, which damaged the calcium oxalate structure. Lectin attached to the surface of raphides was dissolved in alum solution and consequently its structure was denatured. The cholate in the bile juice formed the complex with the oxalate anion or the calcium cation. Moreover, the enzymes activated by Lactobacillus or Bifidobacterium during the fermentation promoted the fragmentation of oxalate. Conclusion : The adjuvant materials damaged the raphides by denaturing or degrading the calcium oxalate, resulting in the reduction of acrid irritation. Further experimental studies would support the toxicity reduction mechanism of the processing.

A Study on the Pyo-bon(標本) concept based on the verse "The Principal and secondary aspects must first be decided(標本須明後先)." in the Sanghan(傷寒) Chapter of "Yixuerumen(醫學入門)" ("의학입문.상한편(醫學入門.傷寒篇)"의 "표본수명후선(標本須明後先)" 조문(條文)에서 나타난 삼음삼양병(三陰三陽病)의 표본(標本) 개념에 대한 고찰)

  • "Yixuerumen" is a comprehensive medical text published in the Ming-dynasty by Li Chan(李梴). In this text, Sanghan(傷寒, cold damage) is categorized among external contraction(外感) with much emphasis. The subject of this study is the verse "The Principal and secondary aspects must first be decided." and its annotations in the in the Sanghan chapter of "Yixuerumen". The complex theoretical structure of this verse was firstly analyzed, together with the historical background of how and why Li Chan adopted this concept. The Pyo-Bon concept is the contrast between phenomena(標) and its underlying source of motivation(本). The methodology for this study was to compare and analyze this main verse with contents on Sanghan and Un-gi(運氣) within the text, while reviewing historical theories explaining the physiology and pathology of the human body in terms of the Pyo-bon(標本) concept. As a result, we discovered that the Pyo-bon(標本) concept used in the aforementioned verse of "Yixuerumen" matches the Three Eum Three Yang(三陰三陽)-標本中氣(pyo-bon-jung gi)-gi transformation(氣化) theory of Un-gi(運氣). Li Chan created the connecting link in understanding the Three Eum Three Yang diagnosis system through the viscera/bowels theory(臟腑論) by adopting the Three Eum Three Yang(三陰三陽)-標本中氣(pyo-bon-jung gi)-gi transformation(氣化) theory from Un-gi. Li's work lead to several changes in the field of Sanghan. First, Li understood the disease pattern of Sanghan by using the accumulated knowledge of the viscera/bowel theory during the Jin-Yuan dynasty, and developed a medical perspective that observes the disease pattern based on the body's essence gi(精氣). Second, he set the category of the Sanghan-Three Eum Three Yang disease pattern, establishing a separate guideline. Third, by adding knowledge of herbs to the accumulated knowledge of the viscera/bowel theory, the process of diagnosis and herbal application were made explicable. On the other hand, in the process of interpreting the 三陰三陽 diagnosis system with viscera/bowels theory, theoretical inconsistencies appeared, of which Li tried to mend by several means. The results of the research on "Yixuerumen(醫學入門) the Sanghan chapter(傷寒篇)" calls for further studies, as it has effected both "Dongeuibogam(東醫寶鑑) the Sanghan part(寒門)" and "Dongeuisoosebowon(東醫壽世保元)" as well.

The antimicrobial activity of Hwangheuk-san and synergy effect with oxacillin against methicillin-resistant Staphylococcus aureus (황흑산(黃黑散)의 메티실린 내성 황색포도상구균에 대한 항균 활성과 옥사실린과의 시너지 효과)

  • Objectives : Hwangheuk-san is a complex prescription composed of oriental traditional medicine and has been reported for antioxidant, antimicrobial and anticancer effects in the recent study. Methicillin-resistant Staphylococcus aureus (MRSA) is one of important causes of fatal infectious diseases such as septicemia, endocarditis, toxic shock syndrome, pneumonia, skin and soft tissue infections (SSTIs). S. aureus is reported as being for a variety of human diseases and its epidemiological relevance is mainly due to their ability of becoming highly resistant to common antimicrobials such as tetracycline, penicillin, cphalosporin and aminoglycoside. The objective of this study is to determine the antimicrobial effect of Hwangheuk-san ethanol extracts (HHS) and synergistic effects with antibiotics oxacillin against MRSA.Methods : The antimicrobial activity of HHS was measured by the disc diffusion method, broth microdilution method and the checkerboard dilution test, time-kill curve assay was performed to investigate synergistic effects with antibiotics oxacillin against MRSA.Results : HHS showed antimicrobial activity against MRSA with a MIC value of 125 ㎍/㎖. In the checkerboard test, the interaction of HHS with antibiotics oxacillin produced almost synergy or partial synergy against MRSA. This study showed that HHS reduced the MICs of oxacillin tested, and a remarkable antibacterial effect of HHS, with membrane permeability enhancers.Conclusions : These results suggest that HHS has the antimicrobial effect and synergistic effects with antibiotics oxacillin against MRSA. This study thus can be a valuable source for the development of a new drug with low MRSA resistance.

Anti-Proliferative Effects of Hesa-A on Human Cancer Cells with Different Metastatic Potential

  • Background: During the past few years, Hesa-A, a herbal-marine mixture, has been used to treat cancer as an alternative medicine in Iran. Based on a series of studies, it is speculated that Hesa-A possesses special cytotoxic effects on invasive tumors. To test this hypothesis, we investigated the selective anticancer effects of Hesa-A on several cancer cell lines with different metastatic potential. Materials and Methods: Hesa-A was prepared in normal saline as a stock solution of 10 mg/ml and further diluted to final concentrations of $100{\mu}/ml$, $200{\mu}g/ml$, $300{\mu}g/ml$ and $400{\mu}g/ml$. MTT-based cytotoxicity assays were performed with A549 (lung non small cancer), MCF-7 (breast adenocarcinoma), SKOV3 (ovarian cancer), and PC-3 (prostate adenocarcinoma) cells. Results: All treated cancer cells showed significant (P<0.01) or very significant (P<0.0001) differences in comparison to negative control at almost all of the tested doses ($100-400{\mu}g/ml$). At the lower dose ($100{\mu}g/ml$), Hesa-A reduced cell viability to 66%, 45.3%, 35.5%, 33.2% in SKOV3, A549, PC-3 and MCF-7 cells, respectively. Moreover, at the highest dose ($400{\mu}g/ml$), Hesa-A resulted in 88.5%, 86.6%, 84.9% and 79.3% growth inhibition in A549, MCF-7, PC-3 and SKOV3 cells, respectively. Conclusions: Hesa-A exert potent cytotoxic effects on different human cancer cells, especially those with a high metastatic potential.
