뇌경색 후 발생한 자율신경 장애에 대해 심박변이도로 살펴본 분심기음의 효과 증례보고 1례 (A Case Report on the Use of Heart Rate Variability and the Treatment Effect of Bunshimgi-eum on Autonomic Dysfunction After Cerebral Infarction)

  • Background: The aim of this study is to investigate the use of heart rate variability on a stroke patient with autonomic dysfunction and the effectiveness of Bunshimgi-eum. Case report: The patient showed autonomic dysfunction, including chest discomfort, 煩燥 (sweating, anxiety, insomnia, nausea, lightheadedness), and abdomen discomfort (dyspepsia, heartburn) after ischemic stroke. She was treated with a herbal medicine (Bunshimgi-eum) through complex Korean medicine treatment. The effect was evaluated using heart rate variability (HRV), Mini-Mental State Examination-DS (MMSE-DS), mean pulse rate, and subjective percentage of symptoms. After treatment, the HRV score, MMSE-DS score, and subjective percentage of symptoms improved, and the pulse rate decreased from the upper boundary value. Discussion: The results suggest that HRV may be useful in evaluating stroke patients with autonomic dysfunction and that Bunshimgi-eum is suitable for treatment.

근위축성 측삭경화증 환자의 한방치료에 대한 국내 연구 문헌 고찰 (A Review of Korean Studies on Amyotrophic Lateral Sclerosis Treated with Traditional Korean Medicine)

  • ■ Objectives This study aimed to investigate the domestic research trend about Amyotrophic Lateral Sclerosis(ALS) treated with Traditional Korean Medicine(TKM). ■ Methods Searches of two major TKM databases were conducted for articles published up to July 2015. Searching keyword was "Lou Gehrig's disease" OR "amyotrophic lateral sclerosis". Studies dealt TKM for ALS were included. ■ Results 18 studies were met our inclusion criteria(two prospective studies, one retrospective study, one follow-up study, 14 case reports). Complex TKM intervention of herbal medication, acupuncture and pharmacopuncture was the most frequently used treatment. Individually, herbal medication was the most frequent followed by acupuncture. Short-term assessment showed improvement in ALS symptoms but long-term assessment showed progression of disease. Accessory symptoms including depression, anxiety and insomnia were improved in four studies. ■ Conclusion For more definite evidence, larger studies with long-term follow-up period are needed. Moreover, benefits that TKM can provide for ALS patients as a part of multi-disciplinary approach should be studied in depth.

LG-HMC의 미세먼지 유발 염증의 완화와 생체이물대사완화 효과 (Effect of LG Herbal Medicine Complex (LG-HMC) on Diesel Particulate Matter (DPM) Induced Skin Inflammation and Xenobiotic Response Activity In Vitro)

  • Diesel particulate matter (DPM)은 피부각질세포에서 염증을 일으킨다. 또 aryl hydrocarbon receptor (AhR)을 통한 xenobiotic response element (XRE) promoter activity에 영향을 주어 cytochromeP (CYP) family의 발현을 촉진시킨다. 이 연구에서는 LG-HMC의 미세먼지 유발 피부 염증완화 및 XRE 발현 조절기능에 대하여 연구를 진행하였다. 먼저, HaCaT에서 DPM이 XRE promoter를 과활성화 시켜주는 것을 확인하였고, 이를 LG-HMC가 완화 시켜주는 것을 확인하였으며 이들 원료 중, 상백피추출물, 황금추출물이 XRE promoter의 과활성화 억제에 관여하는 것을 확인하였다. 이어, HaCaT에서 DPM에 의해 발현이 증가하는 염증성 사이토카인에 대해 LG-HMC 및 상백피추출물, 황금추출물이 염증성 사이토카인의 발현을 줄여주는 것을 확인하였다. 추가적으로 상백피추출물과 황금추출물이 DPPH 라디컬을 효과적으로 줄여줌으로써 미세먼지에 의한 라디컬 손상도 효과적으로 방어할 수 있는 소재임을 확인하였다. 결과적으로 LG-HMC가 미세먼지에 의한 피부염증완화와 피부방어효과를 갖는 것을 확인하였고, LG-HMC를 구성하는 상백피추출물과 황금추출물이 미세먼지에 의한 손상방어에 중요한 역할을 하는 것을 확인하였다.

Management of Greying of Hairs (Sheeb) and Use of Hair Dyes (Khizaab) in Unani Medicine

  • Greying of hairs or Canities (Sheeb) is a hair disorder characterized by premature graying of scalp hair, beard, moustache etc. In Unani system of medicine (USM), the causes of premature greying of hairs, concept behind graying of hairs, principles of treatment, preparations that prevent premature greying of hairs, regimen, systemic and local treatment to stimulate pigmentation and the most interesting point is the use of different type of hair dyes (khizabat) is all illustrated. Classical texts described black, red, blond and white hair dye preparations with formulae and method of use. Initially for the hair graying management, utilizing simple and complex substances from plants, minerals, metals and mixture of these was the main method, which satisfied the desire to change the colour of the hair. With the advancement in chemical science, dyes formulaes, method of formation and application are changed. But due to the awareness about demerits of chemical dyes, people are looking back towards the natural ways to combat hair greying and herbal hair dye is an alternative. This paper is an overview of Unani drugs of local and internal use for hair greying with special attention towards herbal dyes. Most commonly used herbs in khizaab with their actions and constituents has been summarized. This is an effort to globalize the benefits of Unani herbs in hair greying problem. In short, International demand for hair dyes has been steadily growing and there is a wide scope for exploring different aspects of hair greying treatment and dyes in USM.

금은화(金銀花)와 황금(黃芩)이 배오(配伍)된 처방제(處方劑)의 항염증(抗炎症) 효과(效果) 연구(硏究) (Study on the Anti-inflammatory Effects of the Remedy Prescripted with Lonicerae Flos and Scutellariae Radix in U937 cells)

  • Inflammatory reaction is characterized by over-production of inflammatory mediators due to an up-regulation of inflammatory pathways, which produce pro-inflammatory mediators, such as interleukin-1beta (IL-$1{\beta}$), IL-6, tumour necrosis factor alpha (TNF-$\alpha$), prostaglantin $E_2$ ($PGE_2$), and nitric oxide (NO) in U937 cells. We investigate the anti-inflammatory effects of water extracts from Lonicerae Flos and Scutellariae Radix in lipopolysaccharide (LPS)-stimulated U937 cells. Each extract suppressed the production of inflammatory mediators (NO, IL-$1{\beta}$, TNF-$\alpha$, and $PGE_2$) and the expression of inducible NO synthase and cyclooxygenase-2 in LPS- stimulated U937 cells in a dose-dependent manner. These suppressive effects were synergistically increased by their combination. Their combination extract also inhibited NF-${\kappa}B$-DNA complex of NF-${\kappa}B$ binding activity and translocation of NF-${\kappa}B$ from cytosol to nucleus. These results suggest that the combination of water-extractable components of Lonicerae Flos and Scutellariae Radix may be useful for therapeutic drugs against inflammatory immune diseases, probably by suppressing the production of inflammatory mediators.

스트레스성 고혈압 환자에서 고혈압 약물 복용 없는 복합한의치료만의 강압 효과 - 증례보고 (A Blood Pressure Lowering Effect Only through Complex Korean Medical Treatment, without Antihypertensive Drugs, for Patients with Stress Induced Hypertension - A Case Report)

  • Objectives: This study considered the effectiveness of integrative Korean medical treatment for stress-induced hypertensive patients without the use of conventional medication. Methods: A 62-year-old female with stress-induced hypertension (HTN) was hospitalized for 33 days. Her pattern identification was ascendant hyperactivity of liver yang (Ganyangsanghang)-type HTN. Herbal medicine and acupuncture were used for the treatment: Cheonggansoyo-san for 33 days and Chunwangbosim-dan for 15 days, together with acupuncture for 20 minutes twice a day. Blood pressure was checked daily during hospitalization with a digital sphygmomanometer in the brachial artery. Results: After 33 days of treatment, blood pressure decreased. The Handicap Inventory (DHI) scale and Pittsburg Sleep Quality Index (PSQI) scores both decreased. The patient also reported fewer complaints. A five-month follow-up after discharge, with no further treatment, confirmed stable blood pressure. Symptom improvements continued with no significant side effects. Conclusions: This study indicates that Korean medical treatment is effective for stress-induced hypertensive patients.

아밀로이드증을 동반한 다발골수종 환자의 자가말초혈액조혈모세포 이식 전 유도항암화학요법 후 발생한 말초신경병증에 대한 복합 한의 치험 1례 (A Case Report of Peripheral Neuropathy after Induction Chemotherapy before Autologous Peripheral Blood Stem Cell Transplantation in a Patient with Multiple Myeloma with Amyloidosis Treated with Complex Korean Medicine Treatment)

  • Introduction: We present a case of multiple myeloma with amyloidosis, which has features of peripheral neuropathy after induction chemotherapy before autologous peripheral blood stem cell transplantation, in a 56-year-old woman with Korean medicine. Case Presentation: For 17 days of hospitalization, the patient with complaints of numbness and a tingling sensation in the hands and feet was treated with acupuncture, herbal medicine. To reduce the symptoms, we provided Korean medicine treatments, including herbal medicine, acupuncture, and moxibustion. The Visual Analog Scale (VAS) was used to evaluate the results of the treatment. Until discharge, the VAS scores decreased for both hands and the foot tingling sensation. Conclusion: According to these results, Korean medicine treatment may be considered an effective treatment for tingling sensations in a patient with multiple myeloma with amyloidosis. Prospective studies are needed in the future to confirm and expand these findings.

항암치료중 구안와사가 발생한 환자 3례에 대한 고찰 (Effect of Acupuncture Complex Therapy on Three Patients with Bell's Palsy During Chemotherapy)

  • Objectives : The aim of this study was to investigate the effect of oriental medical treatment on Bell's palsy patients receiving chemotherapy. Methods : A clinical study was done on 3 patients who were diagnosed and treated as Bell's palsy at the Dep. of Acupuncture and Moxibustion College of Oriental Medicine Daejeon University. The effects of combined treatment of acupuncture, herbal medicine and physiotherapy were evaluated with Yanagihara's unweighted grading tool. Results : 1. There was significant improvement on Yanagihara score after treating with Gunbiikqi(健脾益氣) method, which was based on pattern differentiation as Hur(虛證). 2. Three patients commonly complained heaviness, dyspepsia, anorexia after receiving chemotherapy and oriental medical treatment showed significant improvement on the symptoms. 3. All three cases received herbal medicine treatment and liver function test. Three cases showed normal range of liver function during and after the treatment. Conclusions : Oriental-medicine treatment did not damage in liver function and showed significant improvement on Bell's palsy in patients receiving chemotherapy. But it is considerd that we need to keep limits in getting enough patients.

한양방 치료를 병행한 삼출액을 주소로 하는 아토피피부염 치험 1례 (A Case of Atopic Dermatitis Patient with Exudate Using Korean Medicine Treatment and Western Treatment)

  • Objectives : This study is clinical report of one atopic dermatitis patient with exudate.Methods : We treated a 16-year old woman patient with acupuncture, pharmacopuncture, herbal medicine, herbal bath, aroma cream therapy and western medicine. The improvement of the patient was evaluated by SCORAD INDEX and photography.Results : The SCORAD Index score of admission day at hospital was 73.5, and the SCORAD INDEX score of discharge day was 11.4. Amount of exudate and the score of oozing were decreased.Consults : The complex treatments of korean medicine and western medicine was effective to improve of atopic dermatitis patient with exudate. And the influence of weather condition to prognosis of atopic dermatitis with exudate was observated. It is necessary to have more observations and case reports.

침도침 시술을 가미한 복합한방치료를 시행한 결절성 다발 동맥염의 치험례 (Case Report of Polyarteritis Nodosa Treated with Oriental Treatments Including Acupotomy)

  • Objectives : Polyarteritis nodosa is a progressive disease of connective tissue that is characterized by nodules along arteries; nodules may block the artery and result in inadequate circulation to the particular area. This report is intended to estimate the efficacy using oriental complex treatment on a patient with Polyarteritis nodosa. Materials and Methods : From 19th March, 2012 to 12th May, 2012, One male inpatient diagnosed with polyarteritis nodosa was treated with general oriental medicine therapy : needle-embedding therapy ; acupuncture ; pharmacopuncture ; acupotomy therapy and herbal medication. VAS(visual analogue scale) was used for evaluation of both leg pain. Other subjective symptoms including night sweat, tinnitus, upper heat were evaluated by percentage comparing the symtoms before and after treatment. Results : The patient showed a certain degree of improvement in both leg pain and other subjective symtoms. Conclusions : Oriental treatments such as needle-Embedding therapy, acupuncture and moxibustion therapy, pharmacopuncture therapy, acupotomy therapy and herbal medication can be effective for controlling pain and other accompanied symtoms due to polyarteritis nodosa.
