Comparisons of Effects of Biman-tang according to Administration Period in Childhood Obesity (비만환아에서 복약기간에 따른 비만탕의 치료효과 비교연구)

  • Objectives The purpose of this study is to investigate the safety and effect of Biman-tang (BMT) on two different administration periods in the treatment of childhood overweight and obesity. Methods In retrospective study, 39 overweight and obese ($85^{th}$ percentile ${\leq}$ Body Mass Index (BMI)) children were treated with BMT from January 2006 to April 2013 at Korean Medical Clinic in Suwon, Korea. The primary outcomes were the changes in BMI and Obesity Index (OI) from baseline to the treatment groups. Secondary outcomes included the changes in height, weight and safety of the medicine. Comparisons of BMI, OI, height and weight between the short-period (SP) treatment group and the long-period (LP) treatment group were done by using ANCOVA. Results The change of the mean of BMI ($-0.5{\pm}0.6kg/m^2$ vs $-1.4{\pm}0.8kg/m^2$, respectively; p=0.003) and OI ($-3.6{\pm}3.9%$ vs $-9.7{\pm}4.7%$, respectively; p<0.001) showed significant reduction both in SP (n=16) and LP treatment group (n=23). The mean height showed no significant difference in both groups. The mean weight of LP treatment group showed significant reduction compared to SP treatment group ($-0.2{\pm}1.3kg$ vs $-1.6{\pm}1.6kg$, respectively; p=0.006). Most of the children were compliant to the medication and no serious adverse events were found in two groups. Conclusions These findings emphasize that BMT is effective in the treatment of childhood obesity and it requires at least 45 days of treatment for the best result.

The Study on Correlation between the KL-Grade and Improvement of Knee Pain Treated by Korean Medicine Therapy (KL-Grade의 정도와 슬관절 통증의 한의학적 치료 효과의 상관성 연구)

  • Objectives The purpose of this study is to compare the effects between the KL-Grade and improvement of knee pain treated by Korean Medicine therapy. Methods 114 patients who received inpatient treatment from July 2014 to May 2017 in the Daejeon Jaseng of Korean Medicine Hospital were divided into 5 groups by the KL-Grade. All patients received a combination of treatment including acupunture, pharmacopunture, herbal medication. They were compared and analyzed on the basis of improvement between measuring Numeric Rating Scale (NRS), Western Ontario and McMaster Universities Arthritis Index (WOMAC Index), EuroQol-5 Dimension Index (EQ-5D Index) as they were hospitalized and as they were discharged. The statistically significance was evaluated by SPSS 23.0 for windows. Results After treatment, KL-Grade 0 group's Numeric Rating Scale (NRS), Western Ontario and McMaster Universities Arthritis Index (WOMAC Index), EuroQol-5 Dimension Index (EQ-5D Index) improvement was $2.02{\pm}1.69$, $7.50{\pm}9.67$ and $0.11{\pm}0.15$ respectively. KL-Grade 1 group's improvement was $2.09{\pm}1.23$, $11.75{\pm}13.99$ and $0.12{\pm}0.13$ respectively. KL-Grade 2 group's improvement was $1.60{\pm}1.07$ and $14.70{\pm}14.19$ respectively. But In this group, EQ-5D Index has decreased by $0.01{\pm}0.10$. KL-Grade 3 group's improvement was $1.88{\pm}1.31$, $7.81{\pm}13.35$ and $0.13{\pm}0.20$ respectively (p<0.034). In the case of KL-Grade 4, the population was not statistically significant (N=2) and therefore excluded from statistical significance. And there was no statistically significance between 4 group's improvement after treatment (p>0.05). Conclusions The above study showed that Korean medicine treatments showed significant therapeutic effects on knee pain and degenerative knee joints, but there was no significant difference in the effectiveness of degenerative arthritis (KL-Grade).

A Case Study on Drug Prescription for Vascular Dementia in Western and Oriental Medicine (혈관성치매에 대한 한.양방 치료약물의 처방 사례연구)

  • Objectives : Vascular dementia is the second common cause of dementia after Alzheimer's disease. It assumed that the ratio of prescription drugs on vascular dementia are quite different from each hospital in Western medicine or Oriental medicine, respectively. The aim of this research is to collect and analyze the ratio of prescription drugs on vascular dementia in Western medicine or Oriental medicine in university hospitals. Methods : We collected and analyzed the data related to prescriptions on vascular dementia in the department of neurology in three university hospitals(A, B, C) and in the department of internal medicine in two Oriental medicine hospitals(Daegu Haany Oriental Hospital, Dong Eui Oriental Hospital). Results : In the department of neurology in A university hospital, donepezil(69.1%), memantine(14.0%), rivastigmine(12.3%), galantamine(4.5%) were prescribed in order. In B university hospital, galantamine(57.8%) donepezil(33.3%), rivastigmine(6.7%), donepezil with memantine(4.4%) were prescribed. In C university hospital, donepezil(62.0%), rivastigmine(25.0%), galantamine(7.0%) memantine(6.0%) were prescribed. The average frequencies of prescribed medication in the department of neurology in A, B, C university hospitals were donepezil(54.8%), galantamine(23.1%), rivastigmine(14.7%), memantine(7.4%). In Oriental medicine hospitals, various prescriptions have been used for vascular dementia. Among them, Ansincheongnoetang (安神淸腦湯) and Gamijihangeumja(加味地黃飮子) were often prescribed in Daegu Haany Oriental Hospital, and Bojungikgitang(補中益氣湯) in Dong Eui Oriental Hospital, too. Conclusions : Cholinesterase inhibitors such as donepezil, galantamine, rivastigmine and NMDA receptor inhibitor like memantine have been used as a drug of choice for vascular dementia in all surveyed university hospitals. In oriental hospitals, various prescriptions have been used for vascular dementia.

A Case Study on Effect of Oriental Medical Treatment to an Aged Female Obese Patient (한방비만치료의 여성노인환자 치험1례)

  • Objectives : This study was performed to evaluate the effect of oriental medical treatment on aged female obese patient. Methods : We experienced an aged female obese patient. We applied herbal medication(Choweseuncheng-tang), electrolipolysis, auricular acupuncture to her from 26th, March. 2007 to 25th, May. 2007. And we examined the changes of her weight, body composition, body size, skin resistance variability, visual analogue scale of knee pain. Results : 1. Her weight decreased from 73.5kg to 71.1kg and body mass index(BMI) decreased from $29.46kg/m^2$ to $28.11kg/m^2$. 2. Body fat mass(BFM) decreased from 25.7kg to 21.6kg but body muscle mass(BMM) increased from 45.1kg to 46.7kg. 3. Waist-hip ratio(WHR) decreased from 0.96 to 0.92. 4. Basal metabolic rate(BMR) increased from 1,273kcal to 1,314kcal. 5. The circumference of upper extremity decreased 1.03cm and lower extremity decreased 1.30cm but the circumference of the muscle of upper extremity increased 0.41cm and lower extremity increased 0.63cm. 6. The circumference of chest and hip are decreased. In addition to the circumference and fat of abdomen are decreased. 7. The visual analogue scale of knee pain is improved. Conclusions : According to above results, oriental medical treatment could be used safety to aged obese patients without loss of fat free mass. The study is more required to aged obese patient from now on.

Effects of Dandelion (Taraxacum coreanum) Extracts on the Mouse Liver with Acute Toxicated by Mercury Chloride (흰민들레(Taraxacum coreanum)추출물이 급성 수은 중독된 생쥐의 간에 미치는 효과)

  • Dandelion has been frequently used as a remedy for women's disease, inflammatory diseases and disorders of the liver and gallbladder. Dandelion extracts water extract, an herbal medication, may have an effect on the activity of hepatic antioxidant enzymes in diabetic rat. This study aims demonstrate the effect of dandelion extracts, one of the natural chelator, on the biochemical and enzyme activity changes in the mouse liver caused by $HgCl_2$. Mice approximately 30 gm in weight were grouped into the control, mercury chloride-treated, and the dandelion extracts-treated after mercury chloride groups. $HgCl_2$ (5 mg/kg) and dandelion extracts (3 g/kg) were delivered orally. Serum AST and ALT were measured, enzyme activity of liver were examined by spectrophotometer and ultrastructural alteration of liver were examined by light and electron microscopy. Dandelion extracts were decreased the increase of serum AST and ALT level induced by mercury. The catalase activity was decreased in the dandelion extracts group. The activity of SOD was dereased, but did not show significant differences. Mercury chloride-treated hepatic cell were irregular nucleus, enlarged and reduced number of mitochodria, enlarged rough endoplasmic reticulum, loss of ribosomes. Cells treated with dandelion extracts were similar to those of the control group. In conclusion, dandelion extracts may protect the mercury-induced toxicity on Liver.

The change of CBF(cerebral blood flow) of CVA patients by oriental medical treatment - Focused on Brain SPECT results - (뇌졸중환자(腦卒中患者)에 있어서 치료경과(治療經過)에 따른 뇌혈류(腦血流) 변화(變化) - Brain SPECT 결과(結果)를 중심(中心)으로 -)

  • Object : SPECT which can visualize the distribution of CBF was operated to find the basic evidence by objective and scientific inspection on the fact that how the oriental medical treatment for stroke patients can effect CBF. Method : This study made 18 cases an object of research. These patients came to the Dongshin Univ. Oriental Medical Hospical within 5 days from the first attack, were diagnosed as stroke from Brain CT and MAl and were in the hospital for over 4 weeks at least. They were taken acupunture treatment, negative technique and herbal medication. And this study operated SPECT at the instance of admission and at the 4th week after admission 2 times and compared the change of CBF. Results : I operated SPECT on 18 CVA patients and got a result on the change of blood supplies in brain as follows; In the 2nd SPECT compared with 1st, out of the 1st and 2nd SPECT, there are 10 cases(55.6%) of increase and 8 cases (44.4%) of decrease. According to L/Cb11 evauation standard, the 1 st and the 2nd changes of blood supplies in SPECT shows decrese from $0.830{\pm}0.071$ to $0.801{\pm}0.067$. On the other hand, according to L/C standard shows a minute increase from $0.894{\pm}0.079$ to $0.895{\pm}0.091$. But there is nothing meaningful. I treated patients' group with risk factors and the other patients' group without risk factors and observed the changes of blood supplies respectively. And therefore Patients' group without risk factors shows an meaningful increase of blood supplies from $0.835{\pm}0.076$ to $0.796{\pm}0.069$ L/Cbll and also an increase from $0.921{\pm}0.029$ to $0.939{\pm}0.029$ by L/C. But this result by L/C is not significant. Conclusion : As mentioned above, I treated CVA patients through oriental medical therapies using SPECT and researched the changes of blood supplies in their brains. But I could' t get any meaningful result. However, I think only after solving some problems in this treatment and quantifing the changes of blood supplies, this results can be the standard of scientific examination about oriental medical treatments.

The Inpatients' Awareness of Co-operative Treatment System on Western Hospital and Oriental Hospital (양방과 한방병원의 협진체계에 대한 입원환자의 인식도)

  • The purpose of this study is to provide the data for the co-operative treatment of western and oriental hospital. The studies were made a questionnaire to analyze inpatients' awareness on the systems of co-operative treatment and to observe the differences in medical service satisfaction between inpatients who had experienced the co-operative treatment of western and oriental hospital(Group 1) and those who did not (Group 2). The survey was conducted in February 1998, on 250 inpatients who were in a hospital which provided co-operative treatment of western and oriental medicine in Pusan. Korea. The results of this study were disclosed as follows: 54.2% of western hospital inpatients and 90.5% of oriental hospital inpatients suffered from diseases of the nervous system 88.9% of Group 1 and 72.2% of Group 2 believed that the co-operative treatment of western and oriental hospital was more effective in curing diseases of the nervous system. 33.5% or inpatiens in the western hospital and 87.4% of inpatients in the oriental hospital had received the co-operative treatment. In the case of the oriental hospital inpatients who had experienced western treatment, 36.8% received an examination radiologic, 30.7% received a laboratory test, 17.8% received physical therapy, and 14.1% received medication. Whereas, in case of the western hospital inpatients who had experienced oriental treatment, 71.8% received acupuncture, 23.9% received herbal medicine, and 2.8% received oriental medical tests. As to the opinion on the systems of co-operative treatment, 49.6% of Group 1 agreed that 'New medical institutions that adopt the merits of both western and oriental medicine are absolutely necessary.', and 48.9% of Group 1 agreed that 'Since there are strong points and weak points in both western and oriental medicine, partial and gradual introduction of the two systems would be better.' Whereas, 49.6% of Group 2 agreed that the partial and gradual introduction, and 35.7% of Group 2 agreed that the necessity of the new medical institutions. As to the motives for visiting the hospital, the most popular reason for all the inpatients was "others' advice". In the case of Group 1, however, the most popular reason was "the possibility of co-operative treatment". In regards to medical cost, the oriental hospital inpatients felt that their medical cost was too expensive. On the other hand, a smaller percentage of the western hospital inpatients felt that western hospital medical cost were too expensive. And between Group 1 and Group 2, a higher percentage of Group 1 felt that their medical cost was too expensive.

A Retrospective Clinical Analysis of 1,485 Patients Who Visited the Emergency Room of Korean Medical Hospital with Musculoskeletal Disorders (한방병원 응급실에 내원한 근골격계 질환 환자 1,485명에 대한 후향적 임상 분석)

  • Objectives : The purpose of this study is to review the characteristics of patients who visited the emergency room of a Korean medical hospital with musculoskeletal disorders during the last 5 years and to provide a potential basis for future studies and emergency medical practice. Methods : Patient visit records between May 1st, 2010 and February 28th, 2015 were reviewed and patients with musculoskeletal disorders were included in a retrospective analysis. Results : A total of 5,218 patient visit records were initially reviewed and 1,485 patients (28.46 %) were included in the analysis. S code group was the most frequently diagnosed coding group of Korean Standard Classification of Diseases. The male to female ratio was 1.05:1 and patients in their thirties represented the highest age group. When classified by time of visit, the distribution of patients was similar from 10:00 to 22:59. The most frequently visited day and month were Sunday and September. 879 patients (59.19 %) visited the emergency room within 24 hours from onset of symptoms. The most common symptom site was the low back (45.59 %), followed by ankle (13.94 %) and neck (13.80 %). The most frequently used treatment method was acupuncture (92.32 %), followed by infrared (45.45 %), TENS (30.03 %) and herbal medication (29.02 %). The follow-up and admission rates were 30.24 % and 13.00 % respectively. 343 patients (23.10 %) received Western medical treatment within a day before visiting the emergency room of the Korean medical hospital. Conclusions : This analysis provides the latest information on the characteristics of patients who visited the emergency room of a Korean medical hospital with musculoskeletal disorders. The results can provide the basis for further studies and for other attempts to improve the clinical setting of the emergency room.

The Steroid Sparing Effects of Cheongsangboha-tang in Asthmatic Patients (청상보하탕(淸上補下湯)의 기관지천식환자에 대한 스테로이드 절약효과)

  • Background : In recent years, the glucocorticoid hormone has become a fundamental medication for asthma. However, a long period of hormone administration will result. in general. side effects on many body parts as well as hormone dependence, which has become a serious problem for western physicians. Objectives : We aimed to identify the clinical effects of Cheongsanghoha-tang and the steroid sparing effects of Cheongsanghoha-tang on. Materials and Methods : A subject group consists of 36 asthmatics who had been treated with Cheongsanghoha-tang for four weeks. Cheongsanghoha-tang is a herbal decoction, which has been used of the traditional therapeutic agent of asthma. PFT, QLQAKA, blood eosinophil, serum IgE, Serum IL-4. IL-5, IFN-${\gamma}$ were checked before and 4weeks after the treatment. Results : The only FVC% in ICSG among asthmatic patients was increased significantly compared to NICSG. Treatment of Cheongsanghoha-tang for four weeks resulted in significant increase in QLQAKA. The NICSG treated with Cheongsangboha-tang for four weeks were no significant difference in the blood eosinophil, serum IgE, IL-4 and IL-5. The PFT and QLQAKA in NICSG were increased significantly after 4 weeks treatment. But the serum IFN-${\gamma}$ in NICSG was decreased significantly after 4 weeks treatment. Discontinuation of treatment with inhaled corticosteroid in ICSG resulted in insignificant changes in PFT, the blood eosinophil, the serum IgE, IL-4, IL-5 and IFN-${\gamma}$ and significant increase in QLQAKA. As a result. 8 of 13 cases were cured with hormones completely and the rest of ICSG reduced the dose of ICS. Conclusions : This study shows that Cheongsanghoha-tang has the effects on the improvement of pulmonary function and cures asthmatic patients. These findings demonstrate that Cheongsanghoha-tang has the steroid sparing effect. Some satisfactory therapeutic results have been obtained in treating hormone-dependent asthma by Cheongsanghoha-tang. However. the concept and mechanism of hormone-dependent asthma have not been fully defined yet, and the standard for judging therapeutic effects have not been established. Obviously further researches concerning all these are still necessary.

Moxibustion for Knee Osteoarthritis : A Protocol for a Pilot Randomized Controlled Trial (슬관절염에 대한 뜸 치료의 유효성 및 안전성 연구 : 무작위 대조 예비 임상연구 프로토콜)

  • Objectives : The purpose of this study is to evaluate the feasibility of massive clinical research and to make a basic analysis on the effectiveness and safety of moxibustion treatment on knee osteoarthritis compared to usual care. Methods and Results : This study is a protocol for a pilot randomized controlled trial. Forty participants are assigned to the moxibustion group (n=20) and usual care group (n=20). Participants assigned to the moxibustion group receive moxibustion treatment on the affected knee(s) at six standard acupuncture points (ST36, ST35, ST34, SP9, Ex-LE04 and SP10) three times per week for four weeks (total of 12 sessions). Participants in the usual care group don't receive moxibustion treatment during the study period and follow-up are made on the 5th, 9th and 13th weeks after random allocation. Both groups are allowed to use any kind of treatment, including surgery, conventional medication, physical treatment, acupuncture, herbal medicine, over-the-counter drugs and other active treatments. Education material that explains knee osteoarthritis and current management options and self-exercise is provided for each group. The pain scale of the Korean Western Ontario and McMaster Universities Questionnaire (K-WOMAC) is the primary outcome measurement used in this study. Other subscales of the K-WOMAC, the Short-Form 36 Health Survey (SF-36), Beck Depression Inventory (BDI), Physical Function test, Patient Global Assessment, and Pain Numeric Rating Scale (NRS) are used as outcome variables to evaluate the effectiveness of acupuncture. Safety is assessed at every visit. Conclusions : The result of this trial will provide a basis for the effectiveness and safety of acupuncture treatment for knee osteoarthritis.
