한방 소아과에 내원한 환아 보호자 일반적인 인식도 조사와 한약치료후의 반응에 대한 후향적 연구 (A Survey on Parent's Recognition and Retrospective Study on Effect of Herbal Medication)

  • Objective : In this study we aimed to· investigate the parental recognition on Herbal Medication. The result showed that parents concerned about the effect of Herbal Medication on liver function and gaining weight. Therefore, we had a retrospective study on the effect of Herbal Medicaion on liver function and gaining weight. Method : In parent's recognition study, we surveyed 354 parents of children who make a first visit to Department of Oriental Pediatrics in ${\bigcirc}{\bigcirc}$ Medical Center from 3 October to 2 November 2005. In retrospective study, we examined medical record of 22 children who visited to Department of Oriental Pediatrics in Kyunghee Medical Center from 30 September 2005 to 1 January 2005. Height, weight, BMI, body fat mass, body fat ratio, AST and ALT were estimated at two points; before and after Herbal Medicaion treatment. Result: The mean of expectation of parents on the efficacy of Herbal Medication was 75.14 on the VAS scale, which was interpreted as significantly high. The result showed the effect of Herbal Medicaion on liver function(48%) and gaining weight(53%) took the largest portion. When we observe the effect of Herbal Medication on Liver function, after Herbal Medication treatment we have slightly increased AST and slightly decreased ALT, but they were not statistically significant. Also we observe the effect of Herbal Medication on getting obesity after Herbal Medication treatment. The result showed that we have slightly decreased BMI, body fat mass, body fat ratio, but they were not statistical1y significant. Conclusion : Our report shows parents of children have high expectation and stand favorable on the efficacy of the Herbal Medication. We found that parents concerned the most about the effect of Herbal Medicaion on liver function and gaining weight. Our retrospective study showed that Herbal Medicaion treatment on children has relationship with neither liver function nor the degree of obesity.

소아(小兒)의 한약 복용 순응도와 영향 요인 (Compliance of Herbal Medicine in Children and the Factors Associated with Compliance)

  • Objectives: Compliance of herbal medicine is extremely important in Korean oriental medicine, since herbal medicine is main approach to the most of outpatients in Korean pediatrics. The more compliant to the medication, the more effective care is given to children. Thus, we conducted this research to evaluate the compliance of herbal medicine, and factors associated with compliance in children. Methods: 61 children (men 28, women 33) were participated who visited the Korean Oriental Medicine Department in university hospital for two weeks. We used Morisky's self-reported questionnaire which is consisted of four questions. Also, we checked medical records and researched respondent to acquire more factors. We defined as a "full compliant" if answers were 'No' to all of the questions. We analyzed the compliance and associated factors with Pearson's $x^2$-test and Fisher's exact test, and Spearman correlation coefficient. Compliance and associated factors were analyzed with linear regression. Results: Compliance of herbal medicine was not related to respondent, and 34 children (55.7%) were non-compliant. The best compliant type of herbal medicine was granule medication (p=0.046). However, there was no relationship between compliance and age, sex, sibling, purpose of medication, period of disease, frequency of medication, and medication duration. The age, frequency of medication, medication duration, and purpose of medications(period of disease) had correlation(p<0.01). Also, older children tended to forget to take herbal medicine(p=0.004). Conclusions: Over an half of the total subjects (55.7%) were non-compliant, so we should improve compliance of herbal medicine. Since granule form of medication was shown to have the best compliance, we should try to use granule form of medication instead of liquid medications. Also, we can also improve compliance with giving attention to the patients, especially older children in order to improve their compliance.

  • Background : In western medicine, there has been much interest in medication teaching since the separation of dispensary from medical practice in 2000. On the other hand, few investigations have been carried out about medication teaching for herbal medicine. Objectives : The purpose of this study wasto investigate the current status of medication teaching of herbal medicine and develop a better guide. Methods : Pharmaceutical affairs law in Korea was searched and some medication teaching guides were compared and analyzed to develop a better guide suitable for herbal medicine. Results and Conclusions : The future guide should be based on scientific evidence and include the following: (1)the origin of each herbal formula (2)the constitution of each herbal formula and proportion of each herb included (3)the chief virtue of each herbal formula (4)the efficacy of each herbal formula (5)the safety of each herbal formula (6)combined treatment with herbal formulas and western drugs (7)the way of decocting each herbal formula (8)the way of safekeeping and period of circulation of each herbal formula (9)a summary and evaluation for each herbal formula (10)references of each herbal formula.

임신중 한약복용이 태아에 미치는 영향에 대한 실태분석연구 (A Clinical Study on the Effects of Herbal Medicine on the Fetus during Pregnancy)

  • Objectives: This study was conducted to find out whether herbal medication during pregnancy causes any deformity in the unborn fetus. Methods: Thirty-five gynecological outpatients of KyungHee Univ. Oriental Hospital (1996. 10 - 1999. 2) who had taken herbal medication during pregnancy were questioned about duration of pregnancy at the moment, chief complaints, kinds of herbal medicine taken and its dosage and they were asked to answer the questionnaire over the phone about duration of pregnancy at the time of delivery, the newborn’s weights, delivery methods and the condition of deformity. Results and conclusions: The results of this experiment in which 35 pregnant women with the experience of herbal medication during pregnancy confirm that herbal medication during pregnancy does not cause any deformity to the newborn. It is thought that more studies through clinical follow-up and experiments are needed in the future.

건선(乾癬)으로 3개월 이상 한약을 복용한 환자 82명의 간수치 변화 (A Report on Liver Function Tests of 82 Psoriasis Patients Taking Herbal Medication over 3 Months)

  • Objectives: There are some controversies over the safety of herbal medication in relation to liver function. This report, therefore, aimed to analyze the safety issue of herbal medications by scrutinizing the liver function tests (LFT) results of 82 psoriasis patients on herbal medications for 3 months at an oriental medical clinic. Methods: We tested psoriasis patients' liver function periodically with Reflotron Plus of Roche during the herbal medication. Then we statistically analyzed the LFT results to see if there was any negative change. Results: Statistical analysis on the LFT results of 82 psoriasis patients showed no meaningful change caused by herbal medication over 3 months. Conclusions: From this result we conclude that herbal medication for 3 months does no harm to patients' liver function.

Comparative Study on the Effects of Herbal Medicine and Western Medicine on the Liver Function Based on Cross-Sectional Design

  • Objective : The purpose of this study is to examine safety of herbal medicine on liver function and compare with western medicine. Method : 303 subjects of skin disease(vitiligo and psoriasis) were chosen at a local oriental medical clinic, and tested with a spot liver function test from Sept 1, 2006 to Dec 31, 2006. Subjects were grouped into untreated, herbal medicine, western medicine, and combination group by treatment record. Total bilirubin, AST, ALT and LDH was compared in each group, odds ratio and regression coefficient was calculated. Results : Subjects comprised of 116 individuals receiving western medication(38.28%), 54 receiving herbal medication(17.82%), 107 receiving combined forms of medication(35.31%), and 26 individuals without any types of medication(8.58%). With the mean age of 37.0 yr. 204 were male(67.3%), 99 were female(32.7%). Comparing variables of liver functions, there was no significant difference between the control and experiment groups. After adjusting potential confounders, monthly $\beta$(SE) of multiple regression -0.009~0.000 for the herbal group, -0.005~0.000 for the western group, and -0.001~0.001 for the combination group. No significant difference between the groups. OR of T-bilirubin were 1.02, 1.05, and 1.04. AST were 0.92, 0.94, and 0.98. ALT were 0.85, 0.99, and 0.97. LDH were 0.96, 1.06, 1.00 for the herbal, western, and combination group, respectively. Conclusion : Comparing with western medication, herbal medication did not cause special ill-effect on the liver function based on liver function tests.

침(鍼)-한약(韓藥)의 교차(交叉) 치료(治療)가 원발성(原發性) 월경통(月經痛)에 미치는 효과(效果)에 대한 연구(硏究) (A Clinical Study on the Effect of Crossing over Treatment of Acupuncture and Herbal Medication for Primary Dysmenorrhea)

  • Purpose : The purpose of this study is to identify the clinical effects of acupuncture and herbal medication in the treatment of primary dysmenorrhea during five or seven days before menstruation. Methods : This study was carried out to the 17 female students in kyungwon university without pelvic or other systemic disease and its history. We investigated general and menstrual characteristics through questionnaires and treated sample patients with crossover acupuncture treatment(Sojangjeonggyeok & $SP_6$) and herbal medication(Hyunburikyungtang-gagam) during five or seven days before menstruation two times. And the change of pain was measured by the method of VAS after menstration monthly two times. Results : The improvement frequency of pain after treatment according to VAS all got better after one, two months of treatment. Especially, the mean${\pm}$S.E. of VAS score after 2nd month treatment was decreased significantly compared with that of 1st month treatment(p<0.05). And Between acupuncture treatment and herbal medication groups had no significant differences on the efficacy of treatment. Conclusion : The acupuncture treatment or herbal medication during five or seven days before menstruation will be efficacious against primary dysmenorrhea.

중풍 후유증으로 인한 실어증 환자에 한방치료와 언어치료를 병행한 경험2례 (Two cases of Combination Therapy of Acupuncture, Herbal medication and Speech Therapy for Aphasic Stroke Patients)

  • Aphasia is frequent in stroke patients and most patients with aphasia exhibit spontaneous progressive improvement in language abilities over time, but few recover completely. Neurological variables, especially initial severity of aphasia and time post-onset, appear to have influence on improvement. Effect of speech therapy and pharmacotherapy has been studied and some drugs, like amphetamine are proved to be benefit for recovery of aphasia following stroke. But there has been few evidence to facilitate recovery from aphasia by acupuncture or herbal medication therapy. So we report two cases of aphasic stroke patients who treated by combination therapy of acupuncture, herbal medication(Cheongsinhaeo-tang) and speech therapy over 6 months and improved in language abilities. Further clinical studies will be needed to explore the effects of acupuncture and herbal medication therapy for aphasia. Researchers should examine the long term effect of these treatment, and whether it is more effective than speech therapy and western pharmacotherapy or not..

  • Objectives : The aim of this study is to analyze the prescription used for dysuria in herbal medication. Methods : Investigating the Oriental Medicine textbooks, I found the prescription used for dysuria. So I analyzed the herbal medication for frequency or compositon and classified it. Results : 1. The most used prescription on dysuria is Paljung-san. 2. The order of frequency of herb for treating dysuria is Moktong, Hwalseok, Gamcho, 3. The group of Yisusamseopyak is used for main prescription about 42%.

급성 경·요추 염좌로 진단된 교통사고 환자에서 귀비탕(歸脾湯)과 당귀수산(當歸鬚散)의 효과에 대한 임상 연구 (Clinical Study of the Effects of Gwibi-tang and Danggwisusan on Traffic Accident Patients with Acute Cervicolumbar Disorder)

  • Objectives This study was designed to compare the effects Gwibi-tang herbal-medication with Danggwisusan herbal-medication for patients with acute cervical and lumbar sprains due to a traffic accident. Methods This study was carried out on 119 acute cervical & lumbar sprain patients by traffic accident. We divided these patients into 2 groups. Group A was treated with Gwibi-tang and Group B was treated with Danggwisusan three times a day for three days. Each patient was treated with acupuncture, physical therapy and two types of herbal-medication. We measured the numerical rating scale (NRS) at two time points : pre-treatment (Tx). and post-Tx. Results 1. The NRS scores significantly decreased after treatment in all patients. 2. The changed NRS scores of nuchal pain showed a borderline significant difference between Group A and Group B (p=0.067). 3. In sub-group analysis, Gwibi-tang showed a greater effect on NRS scores of lumbago pain in female patients (p=0.068) and patients under 30s (p=0.054) compared to Danggwisusan. Conclusions In this study, both types of herbal-medication; Gwibi-tang and Danggwisusan were highly effective on acute cervical and lumbar sprain patients. Especially Gwibi-tang (Group A) was more effective than Danggwisusan (Group B) in reducing the initial nuchal pain. Also Gwibi-tang was more effective for lumbago pain in female and patients under 30s.
