생약 제제가 세포활성도에 미치는 효과 (Effects of some herbal drugs on gingival fibroblast and periodontal ligament cellular activity)

  • Healing of periodontal tissues require the migration and proliferation of gingival fibroblasts and periodontal ligament cells. There is many evidences that the some agents like cytokines and polypeptide growth factors are mediate these cellular events in wound healing. Recently someone is interested in herbal drugs on periodontal tissue healing processes. The purpose of this study was to examine the effects of 4 herbal drugs, Carthami Flis, Moutan Redias Cortex, Scirpi Rhisoma, Seed of Carthamus tinctorius L. on human gingival fibroblasts and periodontal ligament cells. Periodontal ligament cells and gingival fibroblasts were primarily cultured from extracted premolar with non-periodontal diseases. The powder from extracted. herbal drugs were prepared with distilled water. Cells were cultured with DMEM at $37^{\circ}C$, 5% $CO_2$, 100% humidity incubator, and treated with each herbal drugs with proper concentration for 1, 2, and 3 days. The cell activity was determined by ELISA reader using MTT assay. There was the most significant elevation in $10^{-3}g/ml$ of almost herbal drugs on cellular activities. The result of this study demonstrated that Carthami Flis, Moutan Radicis Cortex, Scirpi Rhisoma, Seed of Carthamus tinctorius L. appears to have beneficial effect on healing process after periodontal treatment.

현행 한국의 한약재 중금속기준개정 필요성 고찰 (Problems of Legal Permissible Limit in Metal Level of Herbal Drugs Using Current Korea)

  • Legal permissible limit of herbal drugs metal level in Korea is define 'less 30.0ppm in total(except mineral drugs)' including Pb Hg Cd As Cr Ni etc. This limit has a lot of problems in several factors, that is, not divide hazard and essential elements, not consider high and low toxic effect in each metal, not calculate dose-response relationship and average health behavior in Korean etc. As a that result, It has strong limits and weakness in the basic toxicology and Oriental medicine. To improve and correct these factors, We need to several new approach as like below. It must be radical study following problems in short and middle-long period in the future, toxic metal and essential element must be divide to basic toxicology and also be diverse toxic effect. But mineral drugs that included a amount of severe toxic metals is being used as active prescription drug until now. If toxic, safety and side-effect of metals will be considered, mineral drugs must prohibit instantly or use at least after examined toxic effect. But one of the most important things about herbal drugs contamination, all people and department (government, farmer and trader, oriental medicine doctors and association) will be participate cooperative and collection for preventive or the least contamination in herbal drugs.

한약・생약제제 품목허가신고심사 규정 변화에 관한 고찰 - 건국이후부터 2012년까지 - (A Study on the Changes in Regulations Regarding Approval, Notification and Review of Herbal Medicinal Preparations and Crude Drug Preparations - From 1948 to 2012 -)

  • Objectives : The purpose of this study was to analyze and identify the problems of the changes in regulations that are relevant to approval, notification, and review of herbal medicinal preparations and crude drug preparations. Methods : I collected the regulations of approval, notification, and review of medicinal products mostly from official gazettes, analyzed enactment and amendments regarding herbal medicinal preparations and crude drug preparations, and studied it from the view point of Korean medicine field. Results : Regulations in regards to approval, notification, and review of herbal medicinal preparations and crude drug preparations were first established in 1978. Herbal drugs started to be categorized as crude drug preparations in 1981 and the regulatory outlines were completed in 1999. From 2008 to 2012, the regulatory standards that let crude drug preparations be new drugs from natural products were established. Through those procedures, the followings became crude drug preparations: 1) wholly new prescriptions that are not recorded in Korean Medical Classics, 2) prescriptions that are recorded in Korean Medical classics but prepared with new standard, composition and efficacy, 3) prescriptions that are recorded in Korean Medical classics but prepared with new formulation, and 4) herbal drugs. In case of herbal medicinal preparations, however, only regulations that are related to 1) drugs prepared with new compositions that are not recorded in Korean Medical Classics, 2) drugs with same prescription and same formulation, and 3) drugs with new formulation were arranged. Conclusions : Actual circumstances on crude drug-oriented regulations regarding approval, notification and review and future forms of prescription and drug administration in Korean Medical Institutions can be expected due to expansion in range of herbal medicinal preparations and shrink in that of on-site preparations. Reasonable improvement in efficient usage of modernized herbal medicinal preparations in Korean medical institutions and prospective cooperation from related pharmaceutical industry are needed.

한약의 약성(사기)에 대한 현대적 검증 (I) -체온과의 상관성- (A Modern Aapproach to The Natures of Drugs(I) -Relation to The Rectal Temperature-)

  • The odor theory (기미론) has been defined as the Nature (기) and the Taste(미) of traditional herbal medicine to find the logic in treatment of various diseases by them. There is a strong possibility these Na1ures of the drug can be categorized according to yin (음) and yang (음). There is a neutral Nature which does not fit into one of these four categories. To understand the Natures of traditional herbal drugs in modern scientific approaches, changes in rectal temperature of rats have been measured at 0,30, 60, 90 min after a 10 g/Kg oral administration each of 34 different drugs. Following the classification of the four Natures of drugs, only the temperatures of warm group at 30, 60, and 90 min were elevated significantly from the control and the rest groups. Following a modified classification of 3 groups such as cold and cool, warm and hot, and neutral Nature, changes in temperatures after administration of Hot and Warm drugs were also increased significantly from the control (P<0.01). Thus, the measurement of rectal temperature can be a tool to define the Nature (기) of traditional herbal drugs, but the modified classification is another way to do.

Analgesic Effects of Toad Cake and Toad-cake-containing Herbal Drugs -Analgesic effects of toad cake-

  • Objectives: This study was conducted to clarify the analgesic effect of toad cake and toad-cake-containing herbal drugs. Methods: We counted the writhing response of mice after the intraperitoneal administration of acetic acid as a nociceptive pain model and the withdrawal response after the plantar surface stimulation of the hind paw induced by partial sciatic nerve ligation of the mice as a neuropathic pain model to investigate the analgesic effect of toad cake and toad-cake-containing herbal drugs. A co-treatment study with serotonin biosynthesis inhibitory drug 4-chloro-DL-phenylalanine methyl ester hydrochloride (PCPA), the catecholamine biosynthesis inhibitory drug ${\alpha}$-methyl-DL-tyrosine methyl ester hydrochloride (AMPT) or the opioid receptor antagonist naloxone hydrochloride was also conducted. Results: Analgesic effects in a mouse model of nociceptive pain and neuropathic pain were shown by oral administration of toad cake and toad-cake-containing herbal drugs. The effects of toad cake and toad-cake-containing herbal drugs disappeared upon co-treatment with PCPA, but not with AMPT or naloxone in the nociceptive pain model; the analgesic effect of toad-cake-containing herbal drugs also disappeared upon co-treatment with PCPA in the neuropathic pain model. Conclusion: Toad cake and toad-cake-containing herbal drugs have potential for the treatments of nociceptive pain and of neuropathic pain, such as post-herpetic neuralgia, trigeminal neuralgia, diabetic neuralgia, and postoperative or posttraumatic pain, by activation of the central serotonin nervous system.

Helicobacter pylori infection에 대한 한약과 합성의약품 병용투여 효과에 대한 메타분석 및 체계적 문헌고찰 : PubMED를 중심으로 (Meta-analysis and Systematic Review on the Effects of Herbal Medicine and Synthetic Drugs in Helicobacter pylori Infection: Focusing on PubMED)

  • Objectives : In clinical practice, there are many cases of co-administration of herbal medicine and synthetic drugs. This study tried to identify whether the combined administration of herbal medicine and synthetic drugs including amoxicillin increases Helcicobacter pylori eradication rate compared to the single administration of synthetic drugs or the combined administration of synthetic drugs and placebo herbal medicine through systematic review. Methods : Relevant randomized controlled trials were searched in PubMED database. The risk of biases was assessed through the Cochrane Risk of Bias criteria. Three reviewers were extracted the characteristics and outcomes of each study. Meta-analysis of eradication rate and adverse event was conducted. Results : Four RCTs were selected. In meta-analysis, the combination of herbal medicine and synthetic drugs showed eradication effect, but it was not statistically significant (Odds Ratio [OR] 0.46; 95% confidence interval [CI] 0.17 to 1.24; p=0.13; I2=56%) than administration of synthetic medicine alone. Combination of herbal medicine and synthetic medicine did not increased the incidence of adverse event(OR 1.07; 95% CI 0.72 to 1.59; p=0.68, I2=0%) compared to single administration of synthetic medicine. Conclusion : Although no significant difference was observed between the two groups in the eradication rate and the incidence rate of adverse events, it was difficult to draw a clear conclusion due to the heterogeneity between studies and the low quality of reporting. A number of studies that have overcome these limitations in the future will lead to definite conclusions.

Pharmacodynamic and pharmacokinetic interactions between herbs andwestern drugs

  • In recent years, the combined use of Herbal medicines and Western drugs has been increasing. Though certain problems may occur when both types of medicines are taken together, they havenot been adequately analyzed. It was reported that anticoagulation was enhanced in addition tobleeding when patients took long-term warfarin therapy in combination with Salvia miltiorrhiza(danshen), and laxative herbs accelerate intestinal transit and interfere with the absorption. Herbal constituents, curcumin, ginsenosides, piperine, catechins and silymarin were found to beinhibitors of P-glycoprotein. St John's wort induces the intestinal expression of P-glycoprotein. Anthraquinone, quercetin and coumarins were found to be a potent inhibitor of P-450. Glycyrrhizin or liquorice extracts, Garlic and St John's wort are a potent inducer of CYP3A4. This review provides a critical overview of interactions between herbal medicines and other drugs. Hence, it is necessary to study the pharmacodynamic and pharmacokinetic interactions of many herbal medicines between western drugs.

Review for Herbal Drug and Drug-Induced Liver Injury

  • Objectives: This study aimed to review the general features of drug induced liver injury (DILI) and the important factors in consideration of herbal drugs and DILI. Methods: We reviewed general aspects of DILI such as classification, inducible factors, diagnosis methods, prevention, and the status of herbal drug-associated DILI via literature. Results: Besides the drug itself, genetic and environmental factors affect hepatic toxicity. There is a lack of definitive diagnoses of DILI by drugs, including herbal remedies. The possibility of herbal drug-associated DILI is exaggerated, and majority of herbal drug-derived hepatic injury could be easily prevented if Oriental doctors pay attention to this issue. Conclusion: This study can provide Oriental doctors an overview and be helpful in minimizing the episodes of hepatotoxicity in use of herbal drugs.

PubMed 수록 논문을 활용한 한약 처방과 양약 상호작용에 관한 연구 동향 분석 (Analysis of Research Trends on Interactions between Herbal Formula and Conventional Drugs Using Papers from PubMed)

  • Objectives : Herbal formula consist of multiple herbs, which can potentially interact with conventional drugs. If these interactions are not properly understood, they may reduce treatment efficacy or cause unexpected side effects. Thus, this study collected and analyzed papers on herbal formula and conventional drug interactions from PubMed to analyze various research trends. Methods : To analyze research trends on herbal formula and drug interactions, we first created search queries using a dictionary of herbal formula terms and collected related papers from PubMed using the Entrez API. The PubTator API was applied to identify compound names in the abstracts, recognizing compounds registered in the DrugBank as conventional drugs. Sentences describing interactions between herbal formulas and drugs were extracted using pattern matching, and relevant papers were selected. Trends were then analyzed by year, country, major formulas, major drugs, and interaction networks. Results : Yearly analysis showed a gradual increase in paper counts with a significant rise after 2010. Country analysis revealed that China published the most papers (53), followed by Japan (19) and South Korea (8). formula analysis identified 'sosiho-tang' and 'siryung-tang' as the most frequently mentioned (7 times each). Drug analysis showed '5-fluorouracil', 'acetaminophen', 'entecavir', and 'streptozotocin' were the most frequently mentioned (4 times each). Network analysis revealed 'sosiho-tang and tolbutamide' and 'siryung-tang and prednisolone' as the most frequently, mentioned interactions (3 times each). Disease analysis indicated 'urogenital diseases' were the most discussed (32 mentions), Followed by 'pathological conditions, signs, and symptoms' and 'digestive system diseases' (25 mentions each). Conclusions : Analyzing research trends on herbal formula and conventional drug interactions provides basic data for subsequent research, aiming to reduce side effects and enhance treatment efficacy in clinical settings.

『뇌공포자론(雷公炮炙論)』·『포자대법(炮炙大法)』·『수사지남(修事指南)』에 나타난 증제법(蒸製法)에 관한 문헌적 고찰 (Studies on steaming processing method in the literatures 'Roegongpojaron', Pojadaebub', and 'Susajinam')

  • Objectives : Provides research basis on steaming processing method (SPM) as described in literatures 'Roegongpojaron', 'Pojadaebub', and 'Susajinam'. Methods : The following issues were considered for current investigation regarding herbal drugs: (i) categorizing SPM-applied drugs, (ii) protocols on the use of the supporting materials, (iii) kind of supporting materials, (iv) processing periods, (v) part of the herb plants, (vi) herbal efficacy, (vii) intrinsic properties and tastes of the herbal drugs, (viii) meridian tropism of the herbal drugs. Result : The number of herbal drugs was 70 species from 'Roegongpojaron', 73 from 'Pojadaebub', and 66 from 'Susajinam'. The abundance of supporting materials was in the order of alcohol, honey and tofu. The herbal parts of the roots, stem, fruit, and seed were most widely used. Based on herbal efficacy, a drug supplementing invigoration was used most, and a drug eliminating heat followed next. Based on four spirit features, herbs showing warm, cold, and mild features were used. In considering five tastes, herbs showing bitter and sweet tastes were used most. In considering meridian tropism, herbs converging to liver and kidneys were used, and drugs converging to 'Samcho, and gallbladder were not used. Conclusion : The present survey on SPM as described in 'Roegongpojaron', Pojadaebub', and 'Susajinam' indicates that there is a principal specialty on the use of supporting materials, steaming periods, steaming parts and efficacy.
