A Case of Hemorrhagic Gastritis due to Cow's Milk Allergy (우유 알레르기로 인한 출혈성 위염 1예)

  • Hematemesis is a rare condition in infants and can be a symptom of cow's milk-induced hemorrhagic gastritis. Other clinical manifestations of cow's milk allergy are vomiting, malnutrition and anemia. The criteria for the diagnosis of cow's milk allergy includes elimination of cow milk formula resulting in improvement of symptoms, specific endoscopic and histologic findings as well as exclusion of other causes. Cow's milk allergy should be considered in the etiologic differential diagnosis of hematemesis and gastritis in infancy. We have experienced a 1-month-old female infant with hematemesis due to cow's milk-induced hemorrhagic gastritis, and report the case with a review of previously published cases.

Inhibitory Activities of Ethanol Extracts from Saururus chinensis L. against Stress-Induced Hemorrhagic Gastritis (삼백초 에탄올 추출물의 위출혈성 스트레스 위염 억제 효과)

  • In this study, gastritis inhibitory substance was ethanol-extracted from Saururus chinensis in order to examine the effect as a part of natural bioactive substance research. In an oral administration experiment, S. chinensis was administered at doses of 0.25 g/kg B.W. to 2 g/kg B.W., resulting in stabilization at 1.5 g/kg B.W. and an $LD_{50}$ of 1.81 g/kg B.W. In a chronic toxicity experiment, 0.5 g/kg B.W. of S. chinensis was administered for 13 weeks, but toxicity was not observed. S. chinensis ethanol-extracts were administered at a concentration of 250 or 500 mg/kg B.W. before induction of gastritis. Gastrorrhagia, stomach edema, cytokine production, and cell damage were reduced in a concentration-dependent manner. Therefore, S. chinensis ethanol extracts inhibit cell damage by stress-induced hemorrhagic gastritis in a concentration-dependent manner via inhibition of cytokine expression.

  • Secondary peptic ulceration and gastritis have been known to be associated with stress, exogenous agents, drugs or infection. Salicylate (aspirin) ingestion has been known to be associated with increased incidence of gastric ulcer and more frequently as the cause of hemorrhagic gastric erosions and gastritis. Some medications such as tetracyclines and iron preparations have been associated with ulceration of the gastrointestinal tract. Chemotherapeutic agents including cytoxan and methotrexate also have been implicated in the development of mucosal and gastrointestinal ulcers. We have experienced two cases of hemorrhagic gastric ulcers due to creosote ingestion in a 13 month and a 5 year old boys. Creosote is a main component of one of the popular digestives, Jungrohwan in Japan and Korea.

  • We present a case in which a patient with acute hemorrhagic gastritis demonstrated abnormal gastrointestinal accumulation of radiotracer during $^{99m}Tc$-methylene diphosphonate(MDP) skeletal scintigraphy. A hemorrhagic gastritis was subsequently demonstrated by endoscopy. The mechanism for the intestinal localization of $^{99m}Tc$-MDP in this patient is not clear, but we guess that the extravasated blood containing the radiopharmaceutical cannot recirculate and stays at the bleeding site, so we can see the intestinal activity.

Mitigating Effect of Yukmijihwang-tang on Antral Gastritis and Duodenitis (생쥐의 Gastrin 조절을 통한 六味地黃湯의 날문위염과 십이지장염 완화효과)

  • Objectives This study investigated the mitigating effect of Yukmijihwang-tang on antral gastritis and duodenitis by regulating gastrin release in mice. Methods : The normal group comprised mice with no inflammation; the control group comprised mice with ethanol-induced gastroduodenal inflammation. The sample group included inflammation-induced mice treated with Yukmijihwang-tang. Results Based on morphology and histochemistry findings, many hemorrhagic erosions were observed in the control group, while significantly fewer erosions were seen in the sample group. Immunohistochemistry findings showed that the distributions of gastrin, substance P, HSP70, NF-κB p65, and iNOS were considerably lower in the Yukmijihwang-tang -treated group than in the control group. Conclusions The findings suggest that Yukmijihwang-tang has a potent mitigating effect on antral gastritis and duodenitis in mice.

  • Adrenal venous malformation is an uncommon disease. We report a case of an incidentally diagnosed adrenal cystic mass during an evaluation for gastritis in a child. A 14-year-old girl admitted to our pediatric department for epigastric pain. After the gastroscopy, she was diagnosed with hemorrhagic gastritis. A 5.5 cm-sized cystic mass was incidentally found adjacent to her left adrenal gland during an ultrasound examination for evaluating her abdominal symptoms. She underwent laparoscopic surgery for the diagnosis and treatment of this cystic mass, which was confirmed to be venous malformation at pathologic diagnosis.

Inhibitory Effects of ECQ on Indomethacin-Induced Gastric Damage in Rats

  • We investigated inhibitory effects of extract containing quercetin-3-O-${\beta}$-D-glucuronopyranoside (ECQ) extracted from Rumex Aquaticus Herba on indomethacin-induced gastric damage in Rats. Gastritis was induced in male Sprague-Dawley rats (200~220 g) by oral administration of indomethacin at a dose of 40 mg/kg. One hour before administration of indomethacin, animals were orally pretreated with ECQ at doses of 0.3, 1, 3 or 10 mg/kg. Six hours after indomethacin administration, the rats were sacrificed and the stomach was excised and opened along the greater curvature, and the surface area of gastric lesion was measured using optical microscope. Superoxide dismutase (SOD), catalase (CAT), myeloperoxidase (MPO) activities and malondialdehyde (MDA) levels were measured by ELISA. Western blot analysis was performed to detect protein expression of SOD-2. Linear hemorrhagic mucosal lesions were observed in the stomach 6 hours after oral administration of indomethacin. Pretreatment with ECQ significantly reduced the severity of the lesions in a dose-dependent manner. It also inhibited the reductions in SOD and CAT activities and SOD expression by the indomethacin-induced gastric damage. In addition, the pretreatment with ECQ significantly suppressed the elevation of the MPO activity and the MDA levels induced by indomethacin. These results suggest that ECQ has the inhibitory effects via antioxidative action against indomethacin-induced gastritis in rats.

A case of Peptic ulcer of the less recover from complicated erosive and hemorrhagic gastritis (십이지장 궤양, 출혈성 위염, 미란성 위염 병발 환자의 치험 1례)

  • A previous study suggested that there might be an association between use of herb medicine and peptic ulcer. The past history of patient with perforated duodenal ulcer was treated with herb medicine. We identified that the mucosa of stomach & duodenum were completed healed by herb medicine treatment on the gastro-endoscopy and the systom. Author report ths case with a brief review of literatures.

A Study of Clinical Manifestations of Gastrointestinal Symptoms in Children with Henoch-Schönlein Purpura (소아 Henoch-Schönlein Purpura 환아에서 위장관 증상 유무에 따른 임상 소견에 대한 연구)

  • Purpose: This study was undertaken to evaluate the factors correlated with the clinical course and outcome in patients of Henoch-Sch${\ddot{o}}$nlein Purpura. Methods: The medical records of 104 children diagnosed with Henoch-Sch${\ddot{o}}$nlein Purpura (HSP) from January 1996 to April 2006 were reviewed retrospectively. The patients were divided into two groups: patients with Gastrointestinal (GI) symptoms and those without GI symptoms. When there were joint, scrotum, and renal symptoms except for skin lesion in whole HSP, those patients were excluded. The history of acute infection, duration of admission, treatment requirement, recurrence of HSP, CBC, stool occult blood test, abdominal ultrasonographic findings and GI endoscopic findings were reviewed. Results: Among 104 patients, patients with GI symptoms included 66 cases (63.5%), those without GI symptoms accounted for 38 cases (36.5%). GI symptoms included: abdominal pain in 57 cases (54.8%), vomiting 21 cases (20.2%), GI bleeding 5 cases (4.8%), nausea 3 cases (2.9%) and diarrhea 3 case (2.9%). Positive GI symptoms and GI mucosal lesions on GI endoscopy had a statistically significant correlation with increased admission duration, treatment requirement, recurrence of HSP, and positive stool occult blood. Six cases with small intestinal wall thickening were noted on abdominal ultrasonography. Six cases of hemorrhagic gastritis and hemorrhagic duodenitis, 3 cases of duodenal ulcer, 3 cases of hemorrhagic gastritis and duodenal ulcer, 2 cases of hemorrhagic duodenitis and colitis, and 1 case of colitis were noted on GI endoscopy. Conclusion: These results suggest that GI endoscopic examination may be helpful for the diagnosis and treatment of children with HSP.

The Defensive Effect of Keuibi-tang on the Gastric Mucous Membrane of Mouse Injured by Stress and Ethanol (스트레스와 에탄올로 유발된 mouse의 위점막 손상에 대한 귀비탕의 예방효과)

  • This study was carried out to investigate the effect of Keuibi-tang (KBT) on the injury of gastric mucous membrane by stress and ethanol in mice. The normal group was non-inflammation elicited mice. The two control groups were mice with gastro-inflammation elicited by stress and ethanol. The two sample groups were mice administered KBT before gastro-inflammation elicitation. In the common morphology and histochemical change, the two control groups were observed with various injuries such as hemorrhagic erosion and ulcer, while the sample group was the same as the normal group. In the immunohistochemical change, the distributions of PNA and COX-1 treated with KBT noticeably increased over the control group (P<0.05). The distributions of $NF-{\kappa}B$ p50, COX-2 and TUNEL in the group treated with KBT were noticeably lower than in the control group (P<0.05). The distribution of KBT was the same as the normal group. According to the above results, it is supposed that KBT is applicable to gastritis and gastric ulcer due to stress and alcoholic drinks.
