A Basic Study on a Magnetic Fluid Driven Artificial Heart (자성유체에 의해 구동되는 인공심장에 관한 기초연구)

  • A variety of actuators fur an implantable artificial heart have been studied. They, all, however, share the disadvantages of a complicated energy conversion mechanism and of the need to use bearings. A ferrofluidic actuator directly drives magnetic fluids by applying a magnetic field to these fluids; it does not require bearings. In this study, the feasibility of a ferrofluidic actuator for an implantable artificial heart was studied. An way of two Poles of ring solenoids was mounted near the acrylic tube $({\phi}\;7.4mm)$. A rubber sack (volume : $2m{\ell}$ was connected to both ends of the acrylic tube. The sack were encased in a rigid chamber that had inlet and outlet ports. The acrylic tube and the rubber sack were filled with water encased in a rigid chamber magnetic fluid and the iron cylinder were immersed in the water. Two experiment method was conducted. 1) distance between stoppers were 72mm and 2) distance between stoppers were 104mm. A stroke volume was stability and $0.96m{\ell}$ was obtained in the experiment 1 and $1.92m{\ell}$ in the experiment 2. The energy efficiency of Experiment method 2 is about five times than Experiment method 2. A magnetic fluid-driven blood pump could be feasible if the magnetic fluid with high magnetization (3 times yester than the current value) is developed.

MiRNA Molecular Profiles in Human Medical Conditions: Connecting Lung Cancer and Lung Development Phenomena

  • MiRNAs are endogenous, single stranded ~22-nucleotide non-coding RNAs (ncRNAs) which are transcribed by RNA polymerase II and mediate negative post-transcriptional gene regulation through binding to 3'untranslated regions (UTR), possibly open reading frames (ORFs) or 5'UTRs of target mRNAs. MiRNAs are involved in the normal physiology of eukaryotic cells, so dysregulation may be associated with diseases like cancer, and neurodegenerative, heart and other disorders. Among all cancers, lung cancer, with high incidence and mortality worldwide, is classified into two main groups: non-small cell lung cancer and small cell lung cancer. Recent promising studies suggest that gene expression profiles and miRNA signatures could be a useful step in a noninvasive, low-cost and repeatable screening process of lung cancer. Similarly, every stage of lung development during fetal life is associated with specific miRNAs. Since lung development and lung cancer phenomena share the same physiological, biological and molecular processes like cell proliferation, development and shared mRNA or expression regulation pathways, and according to data adopted from various studies, they may have partially shared miRNA signature. Thus, focusing on lung cancer in relation to lung development in miRNA studies might provide clues for lung cancer diagnosis and prognosis.

Cases of Exemplary Science Teachers' Professional Development Efforts

  • This qualitative research describes unique features of seven exemplary science teachers' professional development activities. Description of excellence in effective teachers' professional development efforts will provide some insights into required inservice initiatives and support systems for teachers' professional learning. Exemplary science teachers' professional development activities can be classified into four types: (1) practicing classroom supervision, (2) participating in voluntary communities of teachers, (3) playing instructional leader roles in teacher training programs, and (4) continuing one's studies at a graduate school. One of the common features of these exemplary teachers was that they can both articulate what makes teachers professionals and practice in accordance with their perspectives. These exemplary teachers not only improved their own classroom practices, but also participated actively in various professional communities to share their practical knowledge with their colleagues. The teachers have formed special-interest groups to investigate better ways of science teaching. They also took an active role in teachers' in-service education. Teachers' quality practices lie at the heart of classroom change. However, it's important to remember that there must be a support system that sustains and encourages teachers' initiatives. The implications for the support system to encourage teachers' professional development efforts are discussed.

Left Side Otalgia Caused by Acute Myocardial Infarction (급성 심근경색에 의해 발생한 좌측 이통 1예)

  • Otalgia consists of primary otalgia associated with disorders of the ear itself and referred otalgia due to disorders of organs that share the same innervation with the ear. Disorders of the oral cavity and laryngopharynx served by the glossopharyngeal and vagus nerves are common causes of referred otalgia. Chest pain from myocardial infarction spreads through the afferent pain pathway, especially through the sympathetic nerves in the cardiac plexus and the phrenic nerve, resulting in a typical chest pain or referred pain in the left upper extremity. However, pain spreading through the vagus nerve can theoretically cause referred otalgia. This association between the heart and ear has not been widely acknowledged, even though a referred otalgia can occasionally be the only symptom of the tragic disease, myocardial infarction, and should be taken seriously. We report a patient who complained of left otalgia as the only symptom of myocardial infarction.

Study of the Hermeneutical Phenomenon regarding the Meaning of the Driving Force of Life Experienced by the Elderly (노인이 경험하는 삶의 원동력 의미에 관한 해석학적 현상학 연구)

  • Purpose: The purpose of this study was to understand the meaning of the driving force of life experienced by the elderly and the realistic structure of their experience. Methods: The research question was "What is the essential meaning of the driving force experienced by the elderly?". Data were collected from individual in-depth interviews between March, 2017 and May, 2017 and analyzed using Van Manen's hermeneutic phenomenological methodology to identify the essential themes of their experience. Results: Through the analysis process, 22 themes and 14 essential themes were derived. The 14 essential themes of the driving force of the life of elderly were as follows: 'Memories from the past', 'Poverty to escape', 'Hand down family's heritage', 'A body bound by marriage', 'A body necessary for old age', 'A dignified body even if they grow older', 'A body that does not want to fall', 'A fruit of my life', 'Mature real love', 'Unchanging fraternity', 'Extended family', 'An old friend with whom share my heart', 'Satisfied with the situation', and 'Expanding the breadth of life'. Conclusion: Nursing programs that assess the driving force of life supporting the elderly and supports the driving force of such a life should be developed.

  • The author was a member of the Medical Terminology Committee of the Korean Society of Radiology and the Korean Medical Association Working Committee on Medical Terms for the publication of the sixth edition of the Korean Medical Association. The author participated in the revision of the sixth-edition medical terminology from July 2015 to January 2020. Based on that experience, this special article was written to share the changes in the Korean medical terminology and the name of the department of radiology.

Direction toward Economic Development of Southwest Area of Jeonnam Province through Construction of Airport and Port Supporting Complex (공항만배후단지 조성을 통한 전남 서남권 경제발전방향에 관한 연구)

  • As a regional development gap between Southwest and Southeast of Jeonnam Province are becoming more serious, it is highly time to come up with measures for the balanced development of the two areas. This paper suggests construction of the Airport and Port Supporting Complex in apparent efforts to reduce the gap between the two areas and facilitate the Southwest economy. The Supporting Complex aims to constitute a triangle industrial cluster which is classified into the following three categories; 1. Traditional competitive industries 2. New growth engine industries 3. Attraction of companies u-turning from abroad such as China etc. To provide one-stop service through set-up of R&D Business Center is the heart of this triangle core. This paper had LQ and Shift&Share Analysis in an attempt to search for competitive industries among the traditional industries in Mokpo-si(city) and Muan-gun(county) of Jeonnam Province. As a result of the analysis, metal manufacturing industry and metal-working process products industry have been found competitive in the two areas. Among the new growth engine industries, this paper suggests precision instruments industry and advanced parts industry for utmost strengths of airport and port while u-turning industries from abroad include higher value-added textile industry and fashion industry. It is expected that all of above-mentioned industries for the triangle industrial cluster will contribute to laying the groundwork to develop regional economy further and create more jobs in the Southwest area of Jeonnam Province.

End-of-Life Issues in the Era of the COVID-19 Pandemic

  • Purpose: The coronavirus disease 2019 (COVID-19) pandemic has brought the world to a standstill and has exposed the lack of preparedness of most nations' health care systems. Even in usual times, palliative care has not received its fair share of recognition as an important component of patient care; instead, the emphasis is often placed on aggressive patient management. Now, with the entire medical community and decision-making committees focussed on intensive patient care, end-of-life care has taken a backseat. Methods: This article is a brief communication. Results: COVID 19 infection has been shown to lead to greater mortality and morbidity in patients with pre-existing illnesses such as hypertension, diabetes, renal failure, and cancer. Patients typically in need of end-of-life care, such as those with late-stage cancer or heart failure, are therefore at a higher risk of both contracting COVID-19 and suffering a more severe disease course. The strict nationwide lockdowns being imposed in most countries have deterred patients from seeking medical attention or hospice care. Every day new research is coming to light regarding COVID 19. This has helped significantly in creating awareness and limiting the spread of disease. However, misinformation is also rampant, leading to discrimination and mistreatment of infected patients. Conclusion: This pandemic has been a terrifying ordeal for all and has exposed our entire population physically, psychologically, emotionally, and financially to unimaginable stresses. In the present scenario, EOL care is as much a necessity as intensive care and should be given at least a fraction of its importance.

Familial Atrial Myxoma with Carney's Complex - 1 Case - (Carney' Complex 소견을 보이는 가족성 심방 점액종 - 치험 1례-)

  • Myxomas are the most common form of intracardiac tumors and are found primarily in the left atrium. In rare cases, Carney and associates have described a syndrome called "the complex of myxoma" consisting of cardiac myxoma, which characteristically is familial, in assocation with two or more of the follow conditions: myxomatous masses (cardiac myxoma, cutaneous myxosma, and mammary myxoid fibroademoma), spotty pigmented lesions of the skin, and endocrine disorders. We report a case of familial atrial myxoma with Carney's complex in a 19-year old woman who has spotty pigmentations on her face, and left atrial myxomas, and myxoma on the right nipple. Her mother and sister share the left atrial myxoma. The myxomas originated in the septum of the left atrium and the anterior leaflet of the mitral valve were successfully excised. In conclusion, family members of affected patients should be screened periodically with echocardiography in an attempt to identify asymptomatic cardiac myxomas. Complete excision and postoperative follow up are necessary to rule out the muticentricity and high rate of recurrent lesions.

Women Caregivers' Experience of Sexual Behavior of Older Men in Nursing Homes (노인요양시설 남성노인의 성 행동에 대한 여성 요양보호사의 경험)

  • Purpose: This study aimed to shed light on the experiences of women caregivers' in nursing homes who provide care for older men; it focuses on sexual behavior and provides baseline data that can help improve quality of care. Methods: Colaizzi's phenomenological method was used for data analysis. Participants included 10 caregivers working at a nursing home. Face-to-face in-depth interviews were conducted for data collection between January and February, 2020. Results: Five theme clusters were identified based on women caregivers' experiences of sexual behavior when caring for older men: "The weight of charge and exhausted stamina", "Unwillingness to face awkwardness", "Unacceptable sexual desire by older men", "An incurable wounded heart", and "Hardship in its entirety becomes my share of responsibility". The analysis further yielded ten themes. The participants stated that was hard to accept the older men's sexual behavior through relationships with the older men. Conclusion: These results should be considered in planning caring interventions for older men with respect to sexual behavior in nursing homes. The findings support the need for educational strategies and programs to improve the ability of nursing home caregivers to adequately care for older men, with consideration for their sexual behavior and for developing relevant policy supports.
