A Secure Healthcare System Using Holochain in a Distributed Environment

  • We propose to design a Holochain-based security and privacy protection system for resource-constrained IoT healthcare systems. Through analysis and performance evaluation, the proposed system confirmed that these characteristics operate effectively in the IoT healthcare environment. The system proposed in this paper consists of four main layers aimed at secure collection, transmission, storage, and processing of important medical data in IoT healthcare environments. The first PERCEPTION layer consists of various IoT devices, such as wearable devices, sensors, and other medical devices. These devices collect patient health data and pass it on to the network layer. The second network connectivity layer assigns an IP address to the collected data and ensures that the data is transmitted reliably over the network. Transmission takes place via standardized protocols, which ensures data reliability and availability. The third distributed cloud layer is a distributed data storage based on Holochain that stores important medical information collected from resource-limited IoT devices. This layer manages data integrity and access control, and allows users to share data securely. Finally, the fourth application layer provides useful information and services to end users, patients and healthcare professionals. The structuring and presentation of data and interaction between applications are managed at this layer. This structure aims to provide security, privacy, and resource efficiency suitable for IoT healthcare systems, in contrast to traditional centralized or blockchain-based systems. We design and propose a Holochain-based security and privacy protection system through a better IoT healthcare system.

  • The purpose of this study is to evaluate on the fitting with human resource practice and strategic on 6 public hospital in the point of resource based view. At the result of this study, public strategy is not much fitted into HRM(human resource management) system, but it shows positive result of the fitting with efficiency strategy and HRM system. These results were associated with the integration of G Medical Center. Generally 6 public hospitals of the HRM system and the strategy had not been linked. They also did not achieve a great outcome. We suggest that the manager in charge of each hospital make strategies of the hospital, and then evaluates about fitting with HRM system. We hope, hereafter, it applied the analysis methods that evaluates it with financing result and individual non-financial result all together.

가정형 인공호흡기 사용 중인 재가 근위축성 측삭증후군 환자의 가정간호기반 호흡관리 프로그램이 미충족의료와 의료자원이용에 미치는 효과 (Effects of a Home Respiratory Management Program on Unmet Healthcare need and Healthcare resource utilizations for Patients applying Home Mechanical Ventilator with Amyotrophic Lateral Sclerosis)

  • 본 연구는 가정간호기반 호흡관리프로그램이 근위축성 측상경화증으로 가정에서 인공호흡기를 적용하고 있는 환자에게 미충족의료와 의료자원이용에 효과가 있는지를 확인하는 연구이다. 연구대상자는 실험군 19명, 대조군 21명으로 40명이다. Cox의 상호작용모델에 기반을 둔 가정간호기반 호흡관리프로그램은 인지적 동기로 교육, 전문간호, 사례관리, 내적 동기로 기도청결, 흉곽운동, 공기누적운동, 심리적 반응으로 명상과 적극적 경청이다. 이를 실험군은 12주 동안 제공하였고, 대조군은 통상적인 가정간호를 제공하였다. 중재관련변수는 사전, 사후 12주, 24주에 측정하였고, 의료자원이용은 24주에 측정하였다. 연구결과 호흡관리프로그램은 미충족의료정도에는 차이가 없었으나, 의료자원이용정도 중 입원횟수 감소에는 효과가 있었다. 이에 근위축성 측상경화증 환자 호흡관리프로그램은 호흡문제로 인한 의료기관 이용횟수를 감소시켜 환자의 호흡기능 향상에 기여할 수 있을 것이다.

The Effects of the Electronic Health Record System on Work Overload and Stress Moderation of Hospital Employees

  • Purpose - In endless competition, companies pursue cost reduction and work efficiency. So, entrepreneurs try to increase job intensity, which may lead to job stress and high turnovers because of job burnout. But, Information systems are acknowledged as a work support tool that secures work convenience and the productivity of employees. In this study, we aimed to confirm the effects of information systems in reduing the work overload of employees in a human resource intensive industry. Research design, data and methodology - This is based on the job demands-resources model, conducting an empirical analysis of surveys given to hospital employees working in a human resource intensive industry. Results - The research revealed that information systems reduced the work overload of employees in a human resource intensive industry. Conclusion - This study confirmed the effects of information systems as a job resource based on JD-R theory, and presentation of empirical results indicated that information systems alleviate employee job overload and increases job satisfaction in the medical services industry. In the medical services industry, using electronic health record system decreases in work overload, which results in employees gaining time for self-development and time management, reducing job stress, and leading to job satisfaction.

  • This study aims to identify the relationship between human resource management practices, work engagement, and proactive behavior of health workers in Gia Lai Province in Vietnam. Based on theoretical frameworks of human resource management, work engagement, and proactive behavior, this study developed a research model and tested its relationship between human resource management practices, work engagement, and the proactive behavior of healthcare workers in Gia Lai Province. This research has used a mixed research method with qualitative and quantitative research. The quantitative research was conducted by survey with 232 health workers. The hypotheses were tested by using structural equation modeling (SEM). The findings showed that human resource management practices have a positive and significant influence on proactive behavior, which directly influences healthcare workers' work engagement. Besides, work engagement plays a role in mediating the influence of human resource management practices on proactive behavior. This research implies that health care organizations should pay more attention to human resource management practices to improve work engagement that assists in increasing employee proactive behavior. Efficient human resource management practices help boost work engagement and, initiality, improve the quality of health care services and minimize errors in treatments.

IoT Healthcare Communication System for IEEE 11073 PHD and IHE PCD-01 Integration Using CoAP

  • With the proliferation of the Internet of Things (IoT) healthcare devices, significant interoperability issue arises where devices use proprietary data transfer protocols. The IHE PCD-01 standard has been suggested for the exchange of healthcare data in ISO/IEEE 11073 PHD data model. However, the PCD-01 is not efficient to be used in the IoT environment. This is because the use of SOAP for PCD-01 may be too complex to be implemented in the resource-constrained IoT healthcare devices. In this paper, we have designed a communication system to implement ISO/IEEE 11073 and IHE PCD-01 integration using the IETF CoAP. More specifically, we have designed the architecture and procedures, using CoAP, to seamlessly transmit the bio-signal from the tiny resource-constrained IoT healthcare devices to the server in a standardized way. We have also built the agent, gateway, and PCD-01 interface at the server, all of which are using the CoAP as a communication protocol. In order to evaluate the performance of the proposed system, we have used the PCD data to be transmitted over CoAP, MQTT, and HTTP. The evaluation of the system performance shows that the use of CoAP results in faster transaction and lesser cost than other protocols, with less battery power consumption.

  • 자동화된 건강관리 모니터링은 많은 시간이 소요되고, 많은 문제들이 발생할 수 있다. 왜냐하면 정시에 환자들의 건강 상태에 대한 모니터링 정보를 제공받기 어렵기 때문이다 이 논문에서는 임베디드된 건강 관리 서비스가 iSCSI 프로토콜에 의해 제안된다. 이 프로토콜은 자동화된 멀티에이전트와 제한된 자원의 디바이스들(예. PD4 스마트폰, 테블릿PC)의 협력체계에 기반을 두고 있다. 이 둘의 협력체계는 쌍방향 iSCSI 프로토콜에 의해 유지된다. 자동화된 건강관리 제어는 반복되는 방식에서 매우 유용할 수 있다. 완전 집중화된 제어에서는 이 방법이 적용될 수 없다.

  • 의료정보는 환자의 프라이버시 침해 뿐 아니라 환자의 생명에도 직결되기 때문에, 견고하고 유연한 보안모델에 대한 지속적인 연구가 필요하다. 특히, u-헬스케어 환경은 사용자 상황변화가 다양하기 때문에, 보다 더 유연하고 세밀한 접근 권한제어가 필요하다. 본 논문은 u-헬스케어 영역에서 상황타입(Context Type)과 상황제한(Context Constraint)을 정의하고, 의료정보의 비밀등급, 사용자의 권한등급, 사용자의 역할, 사용자 상황에 따른 권한 변경 등을 정의하였다. 또한, 사용자 역할을 기반하여 상황정보 변화에 적용할 수 있는 상황기반 접근제어 모델을 설계하였다. 상황기반 접근제어 모델은 K2BASE를 이용하여 자원과 권한의 관계성과 접근 포인트로 부터 도달 가능한 모든 자원에 대한 권한을 분석하였다. 상황기반 접근제어 모델은 u-헬스케어 영역에서 상황을 기반으로 권한변경과 역할을 유연하게 변경할 수 있고, 권한이 부여된 자원에 의미적으로 연결된 자원을 획득할 수 있어, 상황변화가 다양한 u-헬스케어 영역에서 유연하고 적응적인 접근제어 모델로서 적용 가능할 것이다.

델파이 조사법을 이용한 의료 자원 사용의 효율성 평가지표 개발 (Development of efficiency indicators for medical resources use using Delphi technique)

  • Because of the rising healthcare costs, there is a growing need for developing efficiency indicators for medical resources use and measuring efficiency of healthcare providers and healthcare systems using them. In this study, we aimed to develop efficiency indicators for medical resources use by means of Delphi technique. We systematically reviewed the existing measures of medical resource use. Thirty nine indicators were selected as a candidates across the six domains: medical personnel, medical equipment, medical facilities, ethical management, resource efficiency, and drug use. To develop efficiency indicators with professional consensus, a 2-round Delphi survey was conducted among 29 professional experts. The following indicators were selected based on the Delphi survey results: adjusted number of the patient per day and level of the nurse number medical personnel in medical personnel domain; the number of the scan a professional physician and the quality of the scan in medical equipment domain; bed utilization rate in medical facility domain; drug price reported pharmaceutical price by medical institutions, medical fee billing transparency, and medical care appropriateness in ethical management domain; costliness index in resource efficiency domain; and utilization of high cost drug and items per prescription in drug use domain. The efficiency indicators could provide valid information about efficiency of healthcare providers and healthcare systems with respect to their resources use and facilitate policies to improve their efficiency.

범부처 공공보건의료인력 관리조직의 필요성과 과제 (Creation of a central public healthcare human resources management department to coordinate existing agencies)

  • Purpose : The aim of this study is to review the current status of healthcare provision and its human resources administrative management and propose a coordinated human resource management plan for the more efficient operations of healthcare organizations. Methods : We reviewed the literature and held discussions with officials from the United States Department of Health and Human Services to survey United States Public Health Service Commissioned Corps operations. In addition, we surveyed the literature to analyze the current structure and responsibilities of governing bodies involved in public healthcare in Korea. Results : In Korea, there are several administrative offices involved in public health: the Ministry of Health and Welfare, the Ministry of Defense, the Environment Ministry and others. Since these diverse agencies don't integrate their operations, it is difficult to grasp their management of both public healthcare services and their personnel. A potential model is the United States Public Health Service Commissioned Corps, a sub-group of the Department of Health and Human Services and an elite team of highly qualified, public health professionals, which coordinates and manages the overall work and personnel of diverse healthcare organizations. Conclusion : We suggest the establishment of a federal level, public health administrative department of human resource management to centralize and coordinate the existing, disparate healthcare administrative agencies.
