학교병원 유무에 따른 보건-의료계열 대학생의 임상실습 현황 및 만족도 조사 (A Study on Satisfaction Investigation and Current Status of Clinical Training Program for Healthcare-Affiliated Students Depends on University Hospital Availability)

  • The purpose of this study was to identify clinical practice (clinical training education) and satisfaction depend on university hospital availability according to a survey with 850 healthcare-affiliated students. The collected data were analyzed with the SPSS17.0 program. The result was shown statistically significant difference depends on gender, major departments and satisfaction of college life. In addition, there were significant differences in satisfaction of clinical training education in the environmental aspects of practice depending on whether affiliated hospital or not. There were no significant correlation found between degree of satisfaction of clinical training education and the need for affiliated hospital (r = .204, p <.01). As a result, Attached to the university hospital can be helpful to healthcare-affiliated students on environmental and educational aspects of clinical training education. Therefore, the effective and practical training programs using university-affiliated hospitals are surely needed to develop and to apply the active support.

서울지역으로의 원거리 의료 이용에 대한 영향 요인 -KTX 이용자를 중심으로- (Determinants of Bypass Healthcare Utilization for Hospitals in Seoul -The Case of KTX passengers-)

  • 이 연구는 2004년 KTX의 도입 이후 증가하고 있는 서울 지역 의료기관으로의 원거리 의료 이용 현상의 특성 및 그 영향 요인을 분석하였다. 설문조사를 통해 KTX로 서울지역 의료기관을 이용한 경험이 있는 203명에 대한 자료를 확보하여 이들을 대상으로 분석하였다. 분석 결과 최신 설비 및 의료장비의 보유 여부, 의료진의 수준 등이 원거리 의료 이용의 가장 중요한 이유로 나타났고, 질병 치료의 방문 목적, 40세 이상 연령, 전문대졸 이하 등의 요인이 서울지역 의료기관에서의 입원서비스 이용 가능성을 유의하게 높였다. 이는 지방 소재 상급종합병원의 중증질환 진료능력 제고와 이를 위한 정부 지원의 필요성, 그리고 의료 기관 인증 및 가감지급평가사업 결과 공시의 강화를 통한 합리적인 의료기관 선택 유도의 필요성을 시사한다.

싱가포르 보건의료체계의 약사 양성교육과 약사 직능에 대한 체계적 고찰 (A Systematic Review on the Vocational Pharmacy Education and Pharmacists' Role in the Singapore's Healthcare System)

  • Background: Singapore has the stable healthcare system with utilizing pharmacist manpower in proper positions by demand of populations' health among Asian countries. Objective: This study aims to systematically review (1) the pharmacists' role and (2) the pharmacy education system of Singapore in comparison with Korea. Method: We searched for information about academic, medical and governmental institutions related to professional pharmacists' practice in Singapore by primarily using database such as DBpia, KISS, Google Scholar and ProQuest and the official website of the Singapore Ministry of Health. We contacted and arranged the visit schedules with National University of Singapore, National Health Group's polyclinics, Agency for Integrated Care, National University Hospital, and community chain pharmacies. During onsite visits, we interviewed pharmacists working in each institution and obtained additional documents and materials relevant to this manuscript work. Results: To become a registered pharmacist in Singapore, the pharmacy curriculum requires four full-time academic years and six additional months allotted for pre-registration training. Pharm.D. course is offered for pharmacy graduate students with additional two full-time years of study. Team teaching and inter professional education program seem the most significant method in pharmacy education. Pharmacists working at hospitals, polyclinics, and community pharmacies in Singapore take broader roles and offer more cognitive services such as smoking cessation program and medication reconciliation. Especially, pharmacists in Agency for Integrated Care fill the role of primary care providers for the continuing care of the community through the governmental support toward the patients-centered integrated care. Conclusion: Singaporean pharmacists take significant and active roles in collaboration with other healthcare providers. Efforts such as interprofessional pharmacy education and governmental endorsement of the systematic and interactive care between pharmacists and other medical providers in Singapore are needed to be urgently applied to Korea healthcare system for the promotion of population health.

치과진료 시 소아고급생명구조술의 적용 (Application of a Pediatric Advanced Life Support in the Situation of a Dental Treatment)

  • In a dental treatment, a dentist has to know the possibility to happen all kinds of the emergency and to prepare for managing that situation. Especially, the cardiac arrest is the most serious emergent problem. If the accident were happened, most dentists got embarrassed. The American Heart Association (AHA) is offering the Basic Life Support (BLS), Advanced Cardiopulmonary Life Support (ACLS) and Pediatric Advanced Life Support (PALS) programs for the healthcare who need to prepare the life threatening situation. The PALS is specialized to someone who participate in pediatric health-care field. This program is composed of three major emergency problems, such as respiratory emergencies, shock and cardiac arrests. The main concepts of the PALS are early recognition and systemic team approach. The purpose of this study was to introduce about PALS and to prepare response system for emergencies in the dental environment.

Strengthening Human Immunodeficiency Virus and Tuberculosis Prevention Capacity among South African Healthcare Workers: A Mixed Methods Study of a Collaborative Occupational Health Program

  • Background: Insufficient training in infection control and occupational health among healthcare workers (HCWs) in countries with high human immunodeficiency virus (HIV) and tuberculosis (TB) burdens requires attention. We examined the effectiveness of a 1-year Certificate Program in Occupational Health and Infection Control conducted in Free State Province, South Africa in an international partnership to empower HCWs to become change agents to promote workplace-based HIV and TB prevention. Methods: Questionnaires assessing reactions to the program and Knowledge, Attitudes, Skills, and Practices were collected pre-, mid-, and postprogram. Individual interviews, group project evaluations, and participant observation were also conducted. Quantitative data were analyzed using Wilcoxon signed-rank test. Qualitative data were thematically coded and analyzed using the Kirkpatrick framework. Results: Participants recruited (n = 32) were mostly female (81%) and nurses (56%). Pre-to-post-program mean scores improved in knowledge (+12%, p = 0.002) and skills/practices (+14%, p = 0.002). Preprogram attitude scores were high but did not change. Participants felt empowered and demonstrated attitudinal improvements regarding HIV, TB, infection control, and occupational health. Successful projects were indeed implemented. However, participants encountered considerable difficulties in trying to sustain improvement, due largely to lack of pre-existing knowledge and experience, combined with inadequate staffing and insufficient management support. Conclusion: Training is essential to strengthen HCWs' occupational health and infection control knowledge, attitudes, skills, and practices, and workplace-based training programs such as this can yield impressive results. However, the considerable mentorship resources required for such programs and the substantial infrastructural supports needed for implementation and sustainability of improvements in settings without pre-existing experience in such endeavors should not be underestimated.

가정간호서비스를 받는 암환자 주부양자의 속박감 영향요인 (Factors Affecting the Entrapment of Primary Caregivers of Cancer Patients Receiving Home Healthcare Nursing Service)

  • Purpose: The purpose of this study was to examine the factors influencing the entrapment of primary caregivers of cancer patients. Methods: From 6 general hospitals, 146 primary caregivers of cancer patients who were receiving home healthcare nursing service were selected for the study. Regarding data collection, structured questionnaires were distributed to the caregivers for data collection. T-test, ANOVA and hierarchical regression were used for data analysis. Results: The significantly influential factors on their entrapment were caregiving time, taking turms to look after the patient, disease duration, home healthcare nursing period, quality of relationship, perceived health status, and social support. And the explanatory power was 55.1% Conclusion: To reduce primary caregivers' entrapment, it is necessary to perform comprehensive and continuous nursing intervention, and to develop a standardized home healthcare nursing intervention program, and to come up with a system for using resources available in local communities.

의료인을 위한 기본 심폐소생술 교육이 간호대학생의 심폐소생술에 대한 지식, 태도 및 자기효능감에 미치는 영향 (The effects of CPR Knowledge, Attitude, and Self-efficacy on the Basic Life Support course for healthcare provider Education in Nursing Students)

  • 본 연구는 의료인을 위한 기본심폐소생술 교육 후 간호대학생의 심폐소생술 지식, 태도 및 자기효능감에 미치는 효과를 파악하는 것을 목적으로 시도되었다. 단일군 사전-사후 설계로 J시에 소재 일개 대학의 간호학생 69명으로 2020년 7월 11일부터 2020년 8월 1일까지 자료를 수집하였다. 수집된 자료 분석은 SPSS 20.0을 이용하여 빈도, 백분율, 평균, 표준편차, paired t-test로 분석하였다. 그 결과 간호대학생의 심폐소생술에 대한 지식(t=-14.71, p<.001), 태도(t=-7.33, p<.001) 및 자기효능감(t=-6.63, p<.001)은 모두 통계적으로 유의하게 증가하였다. 본 연구 결과를 바탕으로 기본 심폐소생술 교육 프로그램은 지속되어야 하며, 다양한 교육방법 및 지속효과에 대한 후속연구가 필요하다.

Intervention Mapping을 적용한 지역사회기반의 골관절염 자기관리지지 프로그램 개발 (Using Intervention Mapping to Develop a Community-based Disease Self-management Support Program for Patients with Osteoarthritis)

  • Purpose: The purpose of this study was to apply intervention mapping (IM) to develop a community-based disease self-management support program for patients with osteoarthritis. Methods: IM was applied as follows: i) a needs assessment has been carried out by a literature review, survey and interview with osteoarthritis patients; ii) on the basis of the needs assessment, identification of expected outcomes and change objectives for the target population; iii) selection of theory-based methods and practical applications to influence self-management and the determinants of behavior; iv) design of the intervention by developing activities and materials such as osteoarthritis self-management guide and smart patient pocket book. The activities were integrated into an existing healthcare activities; v) implementation and evaluation plan has been developed. Results: The program is aimed at improving health status through activating patients by a patient-centered and tailored intervention for patients with osteoarthritis; consists of 8 sessions with coaching and cognitive emotional psychological skills; includes smart patient, communication, osteoarthritis, medication adherence, pain control, depressive mood control, physical activity and healthy diet. Conclusion: The IM is a systematic and feasible method for developing the program. The next step is to evaluate the impact of the intervention on activation, and health status.

결혼이주여성의 영유아 건강관리 교육 요구도 분석 (The Educational Needs Analysis on Child Health Care Education for Multicultural Mothers)

  • 본 연구의 목적은 결혼이주여성이 필요로 하는 영유아 건강관리의 세부 내용과 그 요구를 확인하여 교육프로그램 개발을 위한 기초자료를 마련하는 것이다. 연구대상은 B시 1개 다문화가정지원센터의 결혼이주여성 30명과 B시 전체 구군 보건소의 영유아 건강관리 업무 담당자 25명으로 하였으며, 2016년 12월부터 2017년 1월까지 일반적 특성, 영유아 건강관리교육 현황, 영유아 건강관리 교육 중요도 및 실행도, 교육요구도를 설문조사하였다. 자료분석은 SPSS 20.0을 사용하여 빈도분석, 교차분석, t-test, ${\chi}^2-test$하였다. 연구결과는 조사대상 결혼이주여성의 56.7%가 영유아 건강관리 교육을 받아 본 적이 없다고 하였고, 결혼이주여성의 영유아 건강관리 교육 중요도 및 실행도에서는 유의한 차이가 없었다. 결혼이주여성의 영유아 건강관리 교육 요구도 분석 중 Borich 분석에서는 '안전수칙/사고', '기저귀/대소변 훈련', '성장발달'을 상위 25%로 응답하였고, Locus for Focus에서는 HH영역으로 '안전수칙/사고', '목욕', '아기마사지', '기저귀/대소변 훈련', '이유식'을 응답하였다. 결혼이주여성과 영유아 건강관리 업무 담당자의 교육 중요도 비교에서는 영유아 건강관리 전 영역에서 영유아 건강관리 업무 담당자의 교육 중요도가 더 높았다. 본 연구결과를 토대로 정보 접금이 차단된 결혼이주여성을 포함하도록 조사대상을 확대한 조사와 이에 따른 영유아 건강관리 교육 프로그램 개발 및 실행을 제안한다.

재미한인간호사의 사회적 지지, 문화적응 스트레스가 우울에 미치는 영향 (Impacts of Social Support and Acculturative Stress on Depression among Korean Registered Nurses in the United States)

  • Purpose: The purpose of this study was to identify the impacts of social support and acculturative stress on depression among Korean registered nurses in the United States. Method: In total, 137 nurses were recruited through direct interviews and online surveys. The data were collected through a structured questionnaire from June 1, to September 16, 2012, and analyzed using descriptive statistics, t-test, ANOVA, Scheff$\acute{e}$'s test, Pearson correlation coefficient, and stepwise multiple regression with SPSS WIN 14.0. Results: Social support significantly differed by the length of one's nursing career in US, monthly income and perceived health status. Acculturative stress significantly differed by the length of one's nursing career in US and types of employment setting. Depression significantly differed by the total length of one's nursing career, type of employment setting, and perceived health status. Moreover, Depression was negatively correlated with social support; positively correlated with acculturative stress; social support was negatively correlated with acculturative stress. Factors influencing depression were acculturative stress, perceived health status, and social support, which explained about 23% of the total variance. Conclusion: These results suggest that an adaptation program that decreases acculturative stress and improves social support should be developed and implemented for Korean nurses to help them successfully integrate into the healthcare system of new country.
