고등학생의 건강증진 프로그램 참여의사에 영향을 미치는 요인 (The Factors Associated with the Participative Intention of Health Promotion Programs in High School)

  • Purpose : The principal objective of this study was to develop adequate health promotion programs satisfying the needs of high school students by exploring a variety of factors that affected their participation in these programs. This research was also designed to serve as preliminary data for the development of health promotion programs that will encourage active student participation. Methods : The study used a descriptive correlational design. The study participants were 658 coeducational high school students in Seoul, consisting of 317 9th graders and 341 10th graders. For this study, 599 structured questionnaires were selected. The survey was conducted for the period from December 12, 2005 to December 23, 2005. Results : Overall, students surveyed in this research showed relatively active participation in health-promoting behaviors. Among 5 dimensions, interpersonal support received the highest score while health responsibility ranked the lowest. Another finding was that posture correction program enjoyed the strongest student participation (75.5%) followed by stress management program(70.1%) and weight control program (56.6%). On the other hand, the participation rates for no-drinking(12.9%), smoking cessation program(17.0%), and influenza prevention program(22.9%) were at the low end. Multiple logistic regression results indicate that the higher interpersonal support score of health promoting behaviors gets, the higher student participation becomes. Conclusion : The study demonstrated that student participation was determined by the nature of health-promoting programs, demographic characteristics and health related physical fitness. It also showed that the sub-scales of the programs played a bigger role in student participation. Keeping this in mind, a wealth of health-enhancing programs of distinguishing qualities need to be tailored to meet various needs of students. In addition, serious efforts should be made to motivate them to seek relevant health promotion programs.

Associations of Ambient Air Pollutant Concentrations With Respiratory Symptoms and Perceived Health Status in Japanese Adults With and Without Chronic Respiratory Diseases: A Panel Study

  • Objectives: In recent years, transboundary air pollution from mainland East Asia has led to growing concerns about air pollution in Japan. Air pollution is reportedly associated with the exacerbation of respiratory diseases. In this study, we assessed the effects of air pollution on respiratory symptoms and the health status of participants with and without chronic respiratory diseases. Methods: Participants (n=2753) with and without chronic respiratory diseases who visited healthcare facilities in Japan during February from 2010 to 2015 filled out a self-report questionnaire regarding their symptoms and perceived health status. Participants were followed up during April-May and June-July. Results: Oxidant concentrations were associated with respiratory symptoms, overall health, and quality of life (QoL). Suspended particulate matter (SPM) and particulate matter <$2.5{\mu}m$ levels were associated with physical fitness; SPM was also associated with QoL. Recent experience of an Asian sand dust event had a significant effect on allergic symptoms, change in health, and QoL. Conclusions: Respiratory symptoms were more strongly affected by oxidants than by other pollutants. Significant associations of air pollutants were found with a comprehensive range of items related to perceived health status, including overall health and QoL. Although the effects of air pollutants on respiratory symptoms and health status were more apparent among patients with respiratory diseases, the adverse effects of air pollutants were significant even among participants without such conditions.

운동하는 몸에 대한 생태윤리학적 접근(1): 퇴계의 경을 중심으로 (Eco-ethical Approach to Sporting body: Focusing on Toegye's Gyeong)

  • 이 연구는 퇴계의 경(敬)을 중심으로 운동하는 몸에 대한 생태윤리학적 접근을 시도하였다. 인간과 자연의 갈등원인은 인간의 욕망에서 비롯된 잘못된 자연관을 들 수 있다. 퇴계는 욕망의 문제를 경을 통해 극복할 수 있다고 보았다. 퇴계 경사상의 생태윤리학적 접근 가능성은 다음과 같다. 첫째, 퇴계의 경은 자연 속에서 완성된 것이기에 가능하다. 둘째, 퇴계의 경은 자연에 대한 인간의 책임(배려)을 강조한다. 셋째, 경은 도덕 실천능력과 연관되기 때문이다. 한편, 퇴계는 '몸을 잘 보존하는 것'이 자기보존인 동시에 자연의 순리를 따르는 천리의 의미로 이해하였다. 이러한 관점에서 운동하는 몸은 신체건강과 마음건강의 의미가 존재하며 동시에 인간과 자연을 하나의 관계망으로 이해하여 자연 질서를 따르는 생태적 가치의 측면도 존재한다. 운동하는 몸을 통한 경의 획득은 '경신(敬身)'과 '활경(活敬)'의 실천을 제안한다. 자연과 함께하는 신체운동을 할 때 몸을 공경하는 '경신'의 단계에 머무르는 것뿐만 아니라 생태환경을 보전(活敬)하면서 자연을 통해 요산요수(樂山樂水)의 덕을 함양하는 단계로 나아가는 것이다.

탄력밴드 운동이 노인의 신체조성과 체력에 미치는 지속적 효과 (The Effect of Elastic Band Exercise Training and Detraining on Body Composition and Fitness in the Elder)

  • 노화가 진행됨에 따라 근육은 감소 현상(sarcopenia)을 나타낸다. 근육의 감소는 노인의 의료비 증가와 직접적인 관련성은 없어 보이지만, 근육의 감소가 근력의 감소로 연결되어 체력의 약화, 활동성의 감소, 낙상의 증가 등과 같은 독립적인 생활을 감소시킨다. 이는 신체장애로 연결되며, 또다시 당뇨, 비만, 고지혈증, 고혈압 등과 같은 만성퇴행성 질환으로 연결된다. 결국, 근감소증은 사망률을 상승시키는 잠재 위험요인이 된다. 본 연구는 탄력밴드 운동을 통한 근감소증 예방과 관련된 변인인 신체조성 및 체력의 향상된 변화와 더불어 운동 중단에 따른 운동효과의 상쇄(相殺) 정도를 살펴보는데 있다. 본 연구의 피검자는 S시 J구 J노인복지관 운동프로그램에 참가하는 60-70대의 노인 14명으로 선정하였다. 12주 동안의 탄력밴드 운동은 주2회의 빈도로 실시하였다. 측정시기는 12주간의 통제전, 통제후(운동전), 12주간의 운동 후(운동중단 전), 12주간의 운동중단 후로 신체조성 과 체력 변인을 측정하였다. 12주간의 통제전·후 신체조성과 체력 변인에는 통계적인 유의차가 나타나지 않았으나, 12주간의 운동전·후 신체조성의 체중(t=2.978, p=0.001), 체질량지수(t=3.502, p=0.004), 체지방율(t=2.216, p=0.045), 근육량(t=-3.837, p=0.002), 내장지방면적(t=5.186, p<0.001), 허리-엉덩이 둘레비(t=3.045, p=0.009) 모든 변인에서, 체력의 2분 제자리 걷기(t=-6.891 p<0.001), 덤벨들기(t=-4.702, p<0.001), 의자에서 일어섰다 앉기(t=-4.860, p<0.001), 의자앉아 앞으로 굽히기(t=-5.910, p<0.001), 등 뒤에서 손잡기(t=-3.835, p=0.002), 244cm 왕복 걷기(t=7.560, p<0.001)의 모든 변인에서 운동의 효과를 나타내었고, 그 효과가 12주간의 운동중단 후에도 신체조성의 체중(t=2.323, p=0.037), 체질량지수(t=2.503, p=0.026), 근육량(t=-3.137, p=0.008) 변인에서, 체력의 2분 제자리 걷기(t=-6.489 p<0.001), 의자에서 일어섰다 앉기(t=-4.694, p<0.001), 의자앉아 앞으로 굽히기(t=-3.690, p=0.003), 244cm 왕복 걷기(t=7.539, p<0.001)의 변인에서 그 유지가 지속되었다. 노인에게 있어서 탄력밴드 운동은 신체조성 및 체력에 긍정적인 영향을 나타내며, 그 운동의 효과가 12주가 경과되어도 유지되고 있음을 확인할 수 있었다.

Acupuncture in Sport Recovery: A Brief Review

  • Active and therapeutic sport recovery is becoming a popular and important component in approving performance for pro and recreational athletes alike. It is also helping in the realm of injury prevention. In the search of finding modalities that are widely effective, natural, and safe, acupuncture is a viable and cost-effective treatment for helping athletes achieve this goal. More direct related research is needed, but testimonials from pro athletes and the body of research that currently exists provides powerful evidence on acupunctures ability to help with enhancing recovery. Specializing in acupuncture and exercise science, Chris integrates acupuncture into musculoskeletal rehabilitation therapy or fitness training for pain modulation, speedy recovery, and enhanced performance. Clients can choose to focus on one-on-one corrective exercise therapy, manual and massage therapy, or acupuncture. However, for best results, Chris recommends all three. Other modalities that he uses in therapy are acu-taping, herbal therapy, nutrition supplementation, cupping, guasha, and stretching techniques. The corrective exercise component is one-on-one body balancing management, focusing on strength and conditioning, post physical rehab - exercise therapy, integrative sport specific exercise, weight loss, core strengthening, dynamic lumbar stabilization, active recovery techniques, and myo-fascial release techniques. The acupuncture component focuses on sport injuries, myofascial pain, peripheral neuropathy, arthritis, facial rejuvenation, stress, smoking cessation, addiction detoxification program, weight management, sport recovery and performance.

노인물리치료를 위한 저항트레이닝 처방에 대한 탐색 (An Exploration on Prescription of Resistance Training for Geriatric Physical Therapy)

  • There can be little dissension that the ultimate goal of all physical therapy interventions with the elderly is to restore or maintain the highest level of function possible for the individual. Whenever physical therapists take on this challenge, they assist elders in maintaining their identities as competent adults. Advancing age is associated with profound changes in body composition, including increased fat mass, decreased fat-free mass(particularly muscle), decreased total body water and decreased bone density. Along with these changes in body compositions, and perhaps as a direct result of them, elderly people have lower energy needs, reduced strength and functional capacity and a greatly increased risk for such diseases as noninsulin-dependent diabetes mellitus and osteoporosis. Resistance training is considered a promising intervention for reversing the loss of muscle function and the deterioration of muscle structure that is associated with advanced age. This reversal is thought to result in improvements in functional abilities and health status in the elderly by increasing muscle mass, strength and power and by increasing bone mineral density. In the past couple of decades, many studies have examined the effects of Resistance training on risk factors for age-related diseases or disabilities. We have explored the positive and negative aspects of older adults' participation in resistance training programs. The benefits to older adults are reported to be increased strength, endurance, muscle capacity, and flexibility; more energy; and improved self-image and confidence. The negative aspects include some pain or stiffness and other nonspecific problems. The positive and negative aspects of resistance training are therefore very similar to those in younger populations. Scientific investigations over the past 10 years have demonstrated that resistance training can be safely and successfully implemented in older populations. Even the frail and very sick elderly can benefit and improve their quality of life. Proper design and progression of a resistance training program for older adults is vital to optimal benefits from resistance exercise. The results of data provided by this research on resistance training for health shows that there is enough existing evidence to conclude that resistance training, particularly when incorporated into a comprehensive fitness program, can offer substantial health benefits which can be obtained by persons of all ages. These benefits, including improvements in functional capacity, translate into an improved quality of life.

일개 종합병원 입원환자의 낙상 실태 및 관련 요인 (Falls in the General Hospital Inpatients: Incidence, Associated Factors)

  • Background : To estimate fall incidence rate and associated factors in inpatients from a general hospital. Method : The data were collected from 104 fall incident reports developed by the patient safety committee in a general hospital in Seoul from 01 January 2007 to 31 December 2008. Information included general characteristics of patients, factors related to fall, types, places, circumstances and outcomes of fall. Result : The incidence rate of fall, which was 4.4 per 1,000 total discharged patients and 0.5 per 1,000 patient-days, was much lower than that of several hospitals in the United States. The difference may reflect the different incidence reporting system of each hospital. Fall-prone patients were, in general, $$\geq_-$$65 years of age, had an alert mental status, were ambulatory with some assistance, and were dependent on and ambulatory device. High incidence of falls was associated with patients with circulatory disease. The majority of fall events usually occurred in bed or at the bedside in the patient's room, and occurred more often during the night than during the day or evening. Risk factors of fall were use of drugs (antihypertensive or neuropsychiatric drugs) and environmental factors (e.g., overly high bed height, surrounding objects, inadequate fitness shoes and slippery floor). Physical injury occurred in 43.3% of fall events, which typically required diagnosis of injury and treatment such as suturing. Risk factors for repeated falls were use of a neuropsychiatric drug (odds ratio=13.9) and gait disturbance (odds ratio=91.2). Risk factors for fall-related injury were alert mental status (odds ratio=3.3 times more likely to fall than those who were drowsy or in a stupor) and general weakness(odds ratio=3.3 times more likely to fall than those who were not generally weak). Conclusion : Medical and nursing staff should be aware of the fall risk factors of hospitalized patients and should intensively pursue preventative strategies. Development of fall prevention education based on these results is recommended.

The Effects of Exercise-based Cardiac Rehabilitation Phase II on Pro- and Anti-inflammatory Markers in Patients with Acute Coronary Syndrome

  • The aim of this study was to evaluate the effect of exercise-based cardiac rehabilitation on pro- and anti-inflammatory markers in patients with acute coronary syndrome (ACS). ACS patients who underwent percutaneous coronary intervention (PCI) and took medicine during phase II of rehabilitation were recruited for study. Subjects were divided into two groups; exercise group (EX, n=21) and a non-exercise group (non-EX, n=13). Supervised exercise program in hospital consisted of treadmill and bicycle exercise was performed three times per week for 6 weeks. Patients of EX received individual counseling, including knowledge of heart disease, risk factor modification, and physical training. Cardiopulmonary fitness, body composition, and biochemical blood factors were analyzed before and after experiment. There was no significant difference in serum levels of hs-CRP and TGF-${\beta}1$ between groups, and between time intervals. But there was a significant decrease in serum levels of IL-18 (P<.001). And there was a significant increase in ratio of IL-18 to IL-10 (P<.01) and serum levels of IL-10 (P<.001). After cardiac rehabilitation, there was significant increase in exercise duration (P<.001), maximal oxygen uptake ($VO_{2peak}$; P<.001) and decrease in submaximal rate-pressure product (sRPP; P<.05) in EX. In conclusion, exercise-based cardiac rehabilitation during phase II in patients with ACS after PCI decreased serum IL-18 (pro-inflammatory) content and ratio of IL-18 to IL-10 in serum (highly related with disease recurrence), and increased serum IL-10 (anti-inflammatory) content. In addition, it led to improved cardiopulmonary fitness.

국교 5년생의 성장발달에 관한 조사연구 I . 전국 5학년 생의 체격분포 (A Study on Anthropometric Data of 5 th Grade Students I. The Distribution of Physical Conditions)

  • 우리나라 아동의 비만실태를 파악하기 위한 연구의 일환으로 전국의 국민학교 5학년 10.751명의 체격분포를 조사하였다. 건강기록부를 이용하요 1학년때 부터 학년까의 키, 몸무게를 분석하고 설문지를 이용하여 체격과 관련된 일반 현황 및 식품섭취 빈도를 조사하였다 1)조사대상아의 어머니 중 29%가 직업을 갖고 있었으며 출, 퇴근시간이 어린이의 영양관리를 제대로 할 수 없는 경우가 많았다. 조사대상아의 5.8%가 거의 매일 결식하였으며 건강을 위해 약제를 복용하는 경우가 26%였다. 2)키는 4학년까지는 남학생이 우세 하다가 5학년때는 남학생이 137.1cm, 여학생이 137.6cm 였다. 몸무게는 남녀 각각 31.5kg, 31.2kg였으며 키, 몸무게의 10, 25, 50, 70 ,90percentile 분포에 해당하는 값이 제시되었다. 고학년이 될수록 남학생의 키의 성장속도는 둔화된 반면 여학생은 오히려 증가였다. 몸무게는 학년이 높아지면서 남녀 모두 증가폭이 커졌으며 여학생의 증가폭이 더 컸다. 키와 몸무게를 이용하여 5년간 BMI, RI, RW 갑이 계산되었다. 3) 49% 아동이 우유를 매일 섭취 하였으며 과자, 계란, 아이스크림, 초코렛의 섭취빈도가 높은 반면 치즈, 햄, 두유의 섭취빈도는 매우 낮았다.

서킷 트레이닝 비만 중년 여성의 노화 관련 호르몬에 미치는 영향 (Effect of Circuit Training on Aging-related Hormones in Obese Middle-aged Women)

  • 노화 과정에 따라 중년 여성들은 폐경 이후 체력이 저하되기 시작하면서 체지방 증가와 제지방량의 감소로 비만으로 이어지기 쉽고, 이러한 현상은 체내 호르몬의 분비의 변화와 함께 발생된다. 본 연구는 서킷 트레이닝이 비만 중년 여성의 노화 관련 호르몬에 미치는 영향을 구명하기 위해 실시하였다. 대상자 체지방률 30% 이상인 비만 중년 여성 운동군 10명, 대조군 10명 총 20명을 실시하였다. 운동 시간은 준비운동 5분, 본 운동 20분, 정리운동 5분으로 총 30분 주 3회 실시하였으며, 운동 강도는 운동자각도를 이용하여 13~17(약간 힘들다-매우 힘들다)수준으로 실시하였고, 또한 POLAR 심박수 변화량을 측정하여 1~6주는 60~70%HRR (Heart rate reserve), 7~12주는 70~80%HRR로 설정하였다. IGF-1은 운동군이 유의하게 증가하였으며, GH는 운동군이 유의하게 증가하였고, DHEA-S는 운동군이 유의하게 증가하였다. 이상의 결과 서킷 트레이닝이 비만 중년 여성의 노화 관련 호르몬에 긍정적인 영향을 준 것으로 사료된다.
