Position Value for Relative Comparison of Healthcare Status of Korea among Organization for Economic Cooperation and Development Countries, 2015 (2015년 한국 보건의료의 상대적 위치와 추이: 경제협력개발기구 국가와 비교)

  • This study aims to evaluate the status of Korean healthcare among Organization for Economic Cooperation and Development countries and to monitor the trend of health care status since 2000. The position value for relative comparison (PARC) index was selected to gauge the level of healthcare status in demand, supply, accessibility, quality, and cost as per healthcare policy aspects. The Mann-Kendall test was conducted to allocate healthcare status of Korea since 2000. The PARC values indicate strength and weakness of Korean healthcare system by the mathematical comparisons. Korea positioned higher in demand, supply, accessibility, and quality. Yet, there are shortages in human resources and primary care. In conclusion, we suggest utilizing this study provides evidence to prioritize health care problems that can lead to establishing healthcare policy.

The Effect of Oral Environment and Self-care Behavior on Oral Health-related Quality of Life in the Elderly with Diabetes (당뇨병노인의 구강환경과 자가간호행위가 구강건강관련 삶의 질에 미치는 영향)

  • Purpose: The purpose of this study was to identify the influencing factors of oral environment and self-care behavior on the oral health-related quality of life in the elderly with diabetes. Methods: The participants were 110 elderly patients with diabetes and follow-up care on their outpatient clinic in D and G city. Data were collected September-December 2018, using questionnaires of the Oral Health Impact Profile (OHIP-49) and Kim's Self-care Behavior, measuring with the tooth-pick (Premiers Dental), Halitosis Checker (HC-212M), and Moisture Checker for Mucus (MCM) for oral environments. The data were analyzed the by independent t-test, oneway ANOVA, Pearson's correlation, and stepwise multiple regression analysis using an IBM SPSS Statistics 25.0 Program. Results: There was positive correlation between oral health-related quality of life and self-care behavior (r= .61, p< .001), negative correlations among halitosis (r = -.34, p< .001), gingival index (r = -.31, p= .001) and plaque index (r = -.32, p= .001). Self-care behavior (β= .46, p< .001), subjective health status (good) (β= .23, p= .002), halitosis (β= -.16, p= .030), and plaque index (β= -.15, p= .041) explained 46.0% of the variance in the oral health-related quality of life. Conclusion: It is necessary to develop nursing care for elderly patients with diabetes that can enhance the self-care behavior and subjective health status, and lower halitosis and plaque index, the factors influencing the oral health-related quality of life in elderly patients with diabetes.

Effects of an Individual Health Counseling Program for Community: Health One-stop Service Program (지역사회 대상의 개인별 건강상담서비스 프로그램 중재 효과: 건강원스톱서비스 사업)

  • Objectives: The aim of the study was to identify the effects of a community-level individual health counseling program for community. Methods: Data included baseline demographics, blood pressure, blood sugar, waist circumference, total cholesterol and health behavior index(body mass index, dietary practice guidelines score, physical activity, high-risk drinking) collected at public health centers in Chungnam province from January to September, 2011. Data obtained from the individual health counseling program in Chungnam province were analyzed using Wilcoxon Signed Rank Test and McNemar Test. Results: After the individual health counseling intervention, the results of health measurement index; systolic blood pressure, diastolic blood pressure, total cholesterol, waist circumference decreased in the health risk group, while total cholesterol and waist circumference decreased in the disease management group. Health behavior change in both groups. Body mass index, moderate physical activity, dietary practice guidelines scores were improved. Conclusions: These results indicate that the individual health counseling program for community was effective in improving health behaviors and status. The results demonstrate that step-by-step counseling program development and intervention studies are needed.

The Effects of a Tai Chi Exercise Program for Patients with Arthritis in Rural Areas (농촌지역 관절염대상자를 위한 타이치운동 프로그램 효과)

  • Purpose: The purpose of this study was to evaluate the effects of a Tai-Chi exercise program on subjective health and physiological function index. Methods: This study was designed as non-equivalent control group pretestposttest research. Eighteen women in each group completed the posttest with an attendance rate of 78.3 % after 12 weeks. The Tai Chi exercise program was provided two times a week over three months for the experimental group, but no program for the control group. A questionnaire was used for subjective health index, which consisted of Korean-WOMAC Index for joint stiffness, K-HAQ for physical disability, EQ-5D for quality of life and perceived health status, and CES-D for depression. To measure the effect on the physiological function index, femoral muscle strength, shoulder joint and back flexibility were measured. Results: All variables except left anterior femoral muscle strength had significant homogeneity between the two groups. There were statistically significant differences between the experimental group and the control group in joint stiffness (t=-2.165, p=.03), physical disability (t=-2.231, p=.038), EQ-5D index (t=3.783, p=.001), perceived health status (t=-2.349, p=.025) and femoral posterior muscle strength (t=2.487, p=.038). Conclusion: The Tai-Chi exercise program was beneficial for women with arthritis in rural communities.

A Study on Health Status and Health Related Quality of Life by Job Characteristics in Korean Adult Women (우리나라 성인 여성의 직업 특성별 건강 상태 및 건강관련 삶의 질)

  • Purpose: This study was aimed to identify the health related quality of life(HRQoL) by job characteristics and its predictors in Korean adult women. Methods: This study used data from cross-sectional surveys conducted for the 2005 National Health and Nutrition Survey, which was performed by using a face-to-face interview method. The data were analyzed by ANOVA, t-test and multiple regression using the SPSS program. Demographic characteristics, subject health state, EQ-5D were included in the study instruments. Results: The mean HRQoL index in the subjects was $0.792{\pm}0.102$. There were significant statistical differences on HRQoL among job characteristics. The health related quality of life of non-physical workers was the highest. The significant predictors of the HRQoL of all subjects were the subjective health status, the present health status compared to the previous year, education, age, marital status and income. The factors accounted for 22.1% of variance in the health related quality of life and the predictors of the HRQoL among job characteristics were different. Conclusion: These results may contribute to development of nursing strategy to improve female health related quality of life. The health related quality of life program for adult women by job characteristics is advised based on results of the study.

The Effect of Korean Elderly's Prosthetic Status on Health Related Quality by Using EuroQol-5 Dimension (한국 노인의 보철상태에 따른 건강관련 삶의 질)

  • The aim of this study was to evaluate the relationship between prosthetic status and health related quality of elderly by using EuroQol-5 dimension (EQ-5D) which is a standardized instrument used as a measure of health outcome. Data from 1,179 elderly subjects (over 65 years old) from the fifth Korea National Health and Nutrition Examination Survey were used in our analysis. Oral examination were conducted to obtain wearing prosthetic and needed prosthetic. EQ-5D is a widely used health condition measuring instrument. It consists of the five dimensions: Mobility, Self-care, Usual activities, Pain/discomfort and Anxiety/depression. EQ-5D index is a score calculated out of a weighting system. In this study, the mean score of the EQ-5D index among the elderly was 0.86. For EQ-5D index gender, female showed statistically significantly lower quality of life than male. The related factors EQ-5D index, when the lower demand for fixed prosthodontics and full denture. Wearers in partial and complete denture than non-wearers showed higher EQ-5D index. In order to improve EQ-5D index among the elderly who demand prosthetic status, which has lost its function through providing proper dental prostheses.

Nutritional status of Korean elderly by oral health level - based on 2009 national health and nutrition survey data (한국 노인의 구강건강수준에 따른 영양섭취 상태 - 2009년 국민건강영양조사 자료에 근거하여 -)

  • Objectives : The purpose of this study was investigation of quality and quantity of nutritional intake related oral health status among Korean elders. Methods : The nutrient intake and the dietary quality was evaluated on the basis of the Dietary Reference Intakes For Koreans(KDRIs). Chi-square test for Complex Samples was used to determine the relationship between oral health and inadequate nutrient intake in Korean elders. The complex samples general linear model was used to test difference of average value difference of nutrient intake percentage compared to dietary reference intake(DRI), energy intake rate from three major nutrients, average mean adequacy ratio(MAR) and index of nutritional quality(INQ) related oral health status. Age, sex and total energy intake was compensated for this analysis. PASW 18 was used for statistical analysis. Results : We could found the difference of the nutrient intake and the dietary quality related oral health status among Korean elders. Especially, Nutrient intake percentage and component ratio of protein among energy intake rate from three major nutrient was lower as oral health status became worse. The percentage of subjects with nutritional intakes under showed highest level in worst oral health status. As oral health status became worse, average mean adequacy ratio(MAR) was lower and the number of nutrient of which index of nutritional quality(INQ) was under 1 was more. Conclusions : From the result above, this study clearly shows the level of oral health affecting the inequalities of eating and the food for the people. And the various propose of oral health policies is needed for vulnerable groups who needs solution to solve the problem of inequality of food distribution where intensive distribution of nutrition problem occurred. Sufficient, safe, and a variety of healthy food intake is a fundamental right of our people. And also, to apply this policy in reality, institutional arrangements and organizations, and specific performing system will be needed.

Position Value for Relative Comparison of Healthcare Status of Korea in 2016 (2016년 한국 보건의료의 상대적 위치와 추이: 경제협력개발기구 국가와 비교)

  • This study investigated the healthcare status of South Korea and member states of the Organization for Economic Cooperation and Development (OECD). By employing the position value for relative comparison index, healthcare status was measured through the following components: demand, supply, accessibility, quality, and cost. Statistical analysis was conducted through the Mann-Kendall test from analyzing trends from 2000 onwards. Results showed that while Korea, on average, scores higher than the OECD average in most of the investigated components, it is below average in certain indexes including primary care and mental health care. Considering the various health issues that have been raised about these indexes, it is important these components be improved upon by policy-makers.

Factors Related to Depression Symptom and the Influence of Depression Symptom on Self-rated Health Status, Outpatient Health Service Utilization and Quality of Life (우울증상에 관련된 요인과 우울증상이 지각된 건강수준, 외래의료이용, 삶의 질에 미치는 영향)

  • Objectives: The purpose of this study is to evaluate the influence of depression symptom on the self-rated health status(SRHS), the outpatient health service utilization and quality of life(QOL) also the relationship depression symptom with socio-demographic and health related factors. Methods: We selected 9,550 participants without chronic diseases from a total of 18,104 in the '2009 community health survey in Gyeongnam. They were assessed by using a Korean version of the Center for Epidemiological Studies-Depression Scale(CES-D). Those with CES-D scores of 21 or greater were defined as having probable depression. Results: A probable depression were associated in bivariate analysis with gender, age, educational status, monthly household income, marital status, current smoking status, drinking habit, physical activities and body mass index. After adjustment for covariates, probable depression groups predicted a lower status in SRHS. Likewise probable depression groups predicted a higher utilization in outpatient health service. Also probable depression groups predicted a lower score in QOL. Conclusions: Probable depression influence SRHS, outpatient health service utilization and QOL even after adjusting for the socio-demographic, health related factors and chronic medical illness. Programs for prevention and management of depression will be helpful to promote health and QOL.

Association Between Tobacco Smoking and Dental Caries in the Indonesian Population: Results of a National Study in 2018

  • Objectives: The 2018 Basic Health Research (RISKESDAS), conducted by the Ministry of Health of the Republic of Indonesia showed a high prevalence of dental caries (88.8%) in Indonesia and suggested that smoking tobacco was associated with an increased risk of dental caries. This study analyzed the association between tobacco smoking and dental caries in the Indonesian population. Methods: This was a cross-sectional analysis of secondary data collected from RISKESDAS 2018. The study population included 35 391 Indonesians aged ≥10 years from all 34 provinces. The decayed, missing, and filled teeth (DMFT) index was used to measure dental caries. Smoking status was assessed qualitatively based on smoking activity, and the level of smoking exposure was assessed based on the Brinkman index. A multivariable logistic regression analysis was employed to examine the relationships of smoking status and smoking exposure levels with the DMFT index. Results: Of the population aged ≥10 years, 36% had a DMFT≥8 (females: 37.5%, males: 33.9%). Almost one-fourth (23.4%) were current smokers, and 4.1% were ex-smokers. Furthermore, 26.4% had a Brinkman index ≥400, indicating heavy smoking. According to the multivariate analysis, current smoking status was associated with the risk of DMFT≥8 in males (adjusted odds ratio [aOR], 1.40; 95% CI, 1.27 to 1.55; p<0.001) and overall (aOR, 1.07; 95% CI, 1.00 to 1.14; p=0.037). In females, ex-smoking was associated with a 41% higher risk of DMFT≥8 (aOR, 1.41; 95% CI, 1.07 to 1.84; p=0.014). Heavy smoking was associated with a higher risk of DMFT≥8 in males (aOR, 1.38; 95% CI, 1.25 to 1.52; p<0.001) and females (aOR, 1.24; 95% CI, 1.03 to 1.50; p=0.022). Conclusions: Tobacco smoking was associated with dental caries in the Indonesian population.
