의치 장착 노인과 임플란트 장착 노인의 저작능력 및 삶의 질 비교 연구 (A study of comparative the mastication capability and life quality of elderly people using dentures or implants)

  • Objectives : The purpose of this study was to determine the effects of oral health on the quality of life of elderly patients with dentures or implants. It is our intention that through this analysis we can aid seniors in making the choice between dentures or implants, considering future masticatory function and the likely effects of this decision on the patients quality of life. Methods : This survey was conducted between November 1, 2010 and November 25, 2010, in Kyunggi-Do, whasung city. The research was carried out at one public health center, among elderly patients. 105 seniors were surveyed using the direct interviewing method, oral health related quality of life was measured by the Philadelphia Geriatric Center(PGC) Morale Scale. Collected data was examined using the SPSS 13.0 program, using frequency, mean, and standard deviation analysis, T-test and $x^2$ test. Results : The number of residual teeth for patients who have implants was $20.45{\pm}5.85$, while among those patients with dentures the number was considerably lower, $8.11{\pm}7.66$(p<0.05). The results of patients masticatory function, was $28.13{\pm}2.40$ for those with implants, and $25.35{\pm}4.15$ with dentures. The results were better for those with implants. Among implant patients overall satisfaction was rated $25.21{\pm}3.63$, higher than for those with dentures $20.20{\pm}6.79$(p<0.05). Quality of life was scored higher for patients with implants($12.76{\pm}2.61$) compared to those with dentures($10.47{\pm}3.52$)(p<0.05). The subjects masticatory function was highly related to the number of remaining teeth and greater masticatory function had a positive effect on patients quality of life and life satisfaction. Conclusion : Dental treatments such as dentures and implants will have a great influence on patients quality of life and life satisfaction. Implants can improve the oral health related quality of life and satisfaction and the general quality of life more than denture therapy.

$Renova^{(R)}$ 임플란트 식립 후 단기간의 생존율에 대한 다각적 분석 (Multilateral analysis of $Renova^{(R)}$ implant placement and its Survival rate)

  • Purpose: Given the predictability of dental implant procedure from the studies of successful osseointegration, implant dentistry is often the treatment of choice to replace missing teeth in edentulous patient instead of the fixed prosthesis or removable denture. The $Renova^{(R)}$ dental implant has a RBM(Resorbable Blast Media) surface, internal hex prosthetic connection and a tapered design. At this study gives the analysis of the implant and the short term survival rate of the implant. Material and Methods: In this study, a multilateral analysis was performed on the subjects undergoing placement with $Renova^{(R)}$ implant between August 2006 and February 2008 in Yonsei University dental hospital. 96 implants were placed in 56 patients and they were surveyed for cumulative survival rate. Among them 78 implants in 44 patients were surveyed for the rest analyses. Result: 1. The cumulative survival rate was 96.88% of 96 implants in 56 patients. 2. The mean marginal bone loss was 0.803mm and the marginal bone loss in augmentation group has higher value than the marginal bone loss in non augmentation group. 3. The health scale for the implants were 87% in success group, 9% in satisfactory survival group, 1% in compromised survival group, and 3% in failure group. 4. Two implants placed in poor bone posterior area by 2-stage failed during prosthetic procedure. Conclusion: $Renova^{(R)}$ dental implant showed high cumulative survival rate in installation on partial edentulous ridge and could be a predictable implant system.

Esthetic outcome for maxillary anterior single implants assessed by different dental specialists

  • PURPOSE. The aim of this study was to assess the esthetic outcome of maxillary anterior single implants by comparing the esthetic perception of dental professionals and patients. MATERIALS AND METHODS. Twenty-three patients with single implants in the esthetic zone were enrolled in this study. Dentists of four different dental specialties (Three orthodontists, three oral surgeons, three prosthodontists, and three periodontists) evaluated the pink esthetic score (PES)/white esthetic score (WES) for 23 implant-supported single restorations. The satisfactions of the patients on the esthetic outcome of the treatment have been evaluated according to the visual analog scale (VAS). RESULTS. The mean total PES/WES was $12.26{\pm}4.76$. The mean PES was $6.45{\pm}2.78$ and mean WES was $5.80{\pm}2.82$. There was a statistically significant difference among the different specialties for WES (P<.01) and Total PES/WES (P<.01). Prosthodontists were found to have assigned poorer ratings among the other specialties, while oral surgeons gave the higher ratings than periodontists, orthodontists, and prosthodontists. CONCLUSION. Prosthodontists seemed to be stricter when assessing aesthetic outcome among other specialties. Moreover, a clear correlation existed between dentists' and patients' esthetic perception, thereby providing rationales for involving patients in the treatment plan to achieve higher levels of patient satisfaction.

임플란트 환자의 치료 만족도 및 구강건강관련 삶의 질(OHIP-14)에 관한 연구 (A study on treatment satisfaction and oral health-related quality of life (OHIP-14) among implant patients)

  • Objectives: This study aimed to investigate factors affecting treatment satisfaction of patients with dental implants and quality of life. Methods: A questionnaire survey was conducted for adults who underwent dental implant treatment. at dental departments of general hospitals or dental clinics in Seoul and Gyeonggi-do. Data was analyzed to get descriptive statistics of variables, and was under independent t-test, one way ANOVA, and multiple regression analysis. Results: The mean of total satisfaction with implants was $3.73{\pm}0.41$ in the 5-point scale, and that of general satisfaction was $3.97{\pm}0.53$, followed by mean values of psychological satisfaction of $3.68{\pm}0.51$, aesthetic satisfaction of $3.67{\pm}0.49$, and masticatory function of $3.65{\pm}0.53$. Factors that affect implant treatment satisfaction among patients were monthly income (more than 4 million won), hesitation of the procedure (treatment period), regular checkup (yes) and concerns during treatment (work). Factors that affect the quality of life of patients who underwent implant treatments were satisfaction of aesthetic function and overall satisfaction. Conclusions: The above results showed that the higher the aesthetic satisfaction and overall satisfaction with the implant are, the higher the quality of life of patients get. Therefore, it is necessary for dental specialists to perform dental implants well to improve quality of life of patients.

Clinical and radiographic assessment of narrow-diameter and regular-diameter implants in the anterior and posterior jaw: 2 to 6 years of follow-up

  • Purpose: The present retrospective clinical study aimed to evaluate and compare the clinical and radiographic parameters, complications, and satisfaction in patients who received fixed prostheses supported by narrow-diameter implants (NDIs) in the anterior and posterior jaw. Methods: Patients aged ≥30 years who had NDI-supported fixed prostheses in the anterior or posterior region of either jaw for at least 2 years were included. Complications such as chipping of the crown; loosening or fracture of the screw, crown abutment, or implant; and loss of retention were recorded. Clinical peri-implant outcomes and crestal bone loss (CBL) were measured. A questionnaire was used to record responses regarding the aesthetics and function of the fixed restorations. Analysis of variance was used to assess the significance of between-group mean comparisons. The log-rank test was performed to analyze the influence of location and prosthesis type on technical complications. Results: Seventy-one patients (mean age: 39.6 years) provided informed consent with a mean follow-up duration of 53 months. Only bleeding on probing showed a statistically significant difference between NDIs in the anterior and posterior regions. The complication rate for NDIs in the posterior region was significantly higher than that for NDIs in the anterior region (P=0.041). For NDIs, CBL was significantly higher around splinted crowns than single crowns (P=0.022). Overall mean patient satisfaction was 10.34±3.65 on a visual analogue scale. Conclusions: NDIs in the anterior and posterior jaws functioned equally well in terms of periimplant soft and hard tissue health and offered acceptable patient satisfaction and reasonable complication rates.

임플란트 식립 후 불편감에 영향을 주는 요인 (The Factors Influencing to the Implant Patients Discomfort)

  • 본 연구는 최근 보철 영역에서 보편화되고 있는 임플란트 장착환자의 구강위생관리에 대한 인식과 불편, 불만감을 파악하고 임플란트 환자의 불편감을 줄이고 효율적인 구강위생관리를 위하여 부산, 울산, 대구지역의 20개소 임플란트 전문치과에서 임플란트 보철을 받은 환자 300명을 대상으로 3개월간 자기기입식 설문조사를 실시하여 분석한 결과는 다음과 같다. 1. 임플란트 보철 후 주요 관리로는 정기검진을 72.5%, 칫솔질방법이 17.3%이 많았으며, 남자에서 정기검진에 대한 인식이 더 높았다(p=0.027). 2. 하루 칫솔질 회수는 3회 56.7%, 2회 27.1%, 4회 12.3% 등의 순이었고 성별로 차이를 보였으며(p=0.005), 칫솔질 시기는 아침식사 후 79.6%, 저녁식사 후 71.7%, 점심식사 후 68.7%의 순으로 식후 칫솔질은 비교적 잘 이루어졌으나 취침 전 34.5%, 간식 후 3.9%는 칫솔질률이 낮았다. 아침식사 전은 남자가(p=0.062), 아침식사 후(p=0.041)와 저녁식사 후(p=0.036)는 여자가 각각 더 많이 하였다 3. 임플란트 보철 후의 불편 불만감은 음식물 끼임이 63.5%이 절대적으로 많았으며, 그 외에는 염증 출혈 3.6%, 이물감 2.8%, 저작능력 2.5% 등, 불편 불만감은 대체로 적었다. 4. 성, 연령, 동반질병 수 등을 보정하였을 때 불편 불만감은 보철수가 많을수록 증가하였고, 하루 칫솔질 횟수가 많을수록, 정기검진 빈도가 잦을수록, 보철 후 경과기간이 길수록 감소하였다. 5. 정기검진 빈도는 칫솔질 교육을 받은 경우, 보철수가 많은 경우, 불편 불만감이 높을수록 증가하였고, 보철 후 경과기간이 길수록 감소하였다. 이상의 결과를 통하여 임플란트 환자의 경우 불편감은 대부분이 음식물이 끼이는 것에 대한 내용을 미루어 생각해 볼 때 환자의 불편감을 줄이고 저작효율성을 높이기 위해서는 계속적인 구강관리교육과 더불어 체계적인 임플란트 환자 관리시스템을 갖추어 나가는 것이 필요하다.

완전 무치악 환자에서 하악 임플란트 지지 피개의치 사용 후의 환자만족도에 관한 전향적 연구 (A prospective study of patient satisfaction after implant supported mandibular overdenture treatment in fully edentulous patients)

  • 목적: 이 연구의 목적은 하악 임플란트 지지 피개의치와 전통적인 총의치의 환자 만족도를 비교하기 위함이다. 재료 및 방법: 하악 임플란트 지지 피개의치 치료가 계획된 40명의 상하악 완전 무치악 환자(51세 - 82세)를 대상으로 하였고, 모든 환자는 저작기능, 발음, 심미, 통증, 전반적인 만족도를 기존의 총의치 장착 시와 임플란트 지지 피개의치 장착 1주 후, 12주 후에 visual analog scale법을 이용하여 답하였다. 각 시기의 만족도 비교를 위해 반복측정 분산분석(repeated-measures ANOVA)을 시행하였다(P<.05). 결과: 모든 설문 항목에서 임플란트 지지 피개의치 장착 1주 후, 12주 후에 총의치를 장착했을 때보다 높은 만족도를 보였다(P<.05). 임플란트 지지 피개의치 장착 12주 후에 저작기능, 발음, 통증, 전반적인 만족도는 장착 1주 후에 비해 증가된 결과를 보였고, 심미에 대한 만족도와 전체적인 만족도는 감소함을 나타냈으나, 통계적인 유의성을 보이지는 않았다. 결론: 임플란트 지지 피개의치는 기존의 총의치에 만족하지 못하는 환자들의 만족도를 높임으로써 환자의 삶의 질을 개선시킬 수 있는 효과적인 치료 방법 중 하나가 될 수 있으리라 생각된다.
