A Study on R&D program for the aging society (고령사회 대응 연구개발 지원방안 연구)

  • The purpose of this study is to explore the research and development(R&D) programs for effectively response of population aging. We analyzed of aging-related R&D support status by National Science and Technology Information Services(NTIS) and reviewed of foreign aging-related R&D programs. It was found that the overall lack of support systems, the R&D support status for Korea's aging society is very low performance and the difference appears in the annual support status, etc. It is very important to enhance the effectiveness of R&D programs in order to effectively prepare for aging society. I suggested support measures of the R&D for aging society by survey research. The result of this study will help in planning of the R&D for aging society and contribute to increasing of welfare for the aged.

  • The concerns over obesity and obesity-related health problems are increasing as many consumers relate these health problems with sugar. The demand for sugar reduction is also rising and regulatory movement by governments including Korea is driven to reflect such demand. For the past decades, there have been diverse development and marketing of various sweeteners to substitute sucrose and high fructose corn syrup. Low caloric alternative sweeteners can be divided into high intensity sweeteners that have greater sweetness potency compared to sucrose, and low intensity sweeteners such as polyols, oligosaccharides and rare sugars that have less sweetness potency. This paper discusses representative low caloric alternative sweeteners, their market and trend.

  • The convergence healthcare industry in Korea has been stalled due to conflicts between stakeholders as well as a supplier-centered industry structure. This situation is caused by the structural contradiction in which the Korean industry has a prolonged conflict structure among stakeholders due to a strong regulation and an institutional inertia from the viewpoint of the sociotechnical system. Therefore, it is necessary to identify new system management plan that enhances social acceptability such as laws, customs and ideas while reducing conflicts between stakeholders. In this study, mainly adapting the stakeholder-oriented autopoiesis and focusing on publicness of healthcare, we propose the rationale and direction for policy making to harmonize various systems within the convergence healthcare industry.

Korea National Health Insurance Service Financial Status and Prospect (건강보험 재정 현황과 전망)

  • Background: Korea National Health Insurance Service (NHIS) is operated as a social insurance system in which people pay a portion of their monthly income as insurance premiums and receive benefits when they experience illness or injury. Since 2005, the national health insurance remained surplus accumulating cumulative reserves each year. However, as of 2018, NHIS revenue recorded 62.11 trillion won and spending of 62.29 trillion won. The deterioration of NHIS finances is expected to accelerate with the aging population, income growth, new medical technology development, and enhanced security policies. Methods: To examine the financial health and sustainability of NHIS, we estimated the future revenue and spending until 2030 using the data from Korea Health Insurance Review and Assessment Service statistical yearbook. 2004-2018 average percentage change in NHIS revenue and spending was calculated. We estimated the future NHIS financial status using two methods. In the first method, we calculated the revenue and spending of the future NHIS by applying the 2004-2018 average percentage change to the subsequent years consecutively. In the second method, we estimated the future NHIS financial status after adjusting for the predicted demographic changes such as the aging population and declining birth rate in South Korea. Results: The estimates from this study suggest that the NHIS's cumulative reserves will run out by 2024. Conclusion: In terms of spending on current health insurance, there should be a search for ways of more efficient spending and funding options.

Workers' Compensation Insurance and Occupational Injuries

  • Objectives: Although compensation for occupational injuries and diseases is guaranteed in almost all nations, countries vary greatly with respect to how they organize workers' compensation systems. In this paper, we focus on three aspects of workers' compensation insurance in Organization for Economic Cooperation and Development (OECD) countries - types of systems, employers' funding mechanisms, and coverage for injured workers - and their impacts on the actual frequencies of occupational injuries and diseases. Methods: We estimated a panel data fixed effect model with cross-country OECD and International Labor Organization data. We controlled for country fixed effects, relevant aggregate variables, and dummy variables representing the occupational accidents data source. Results: First, the use of a private insurance system is found to lower the occupational accidents. Second, the use of risk-based pricing for the payment of employer raises the occupational injuries and diseases. Finally, the wider the coverage of injured workers is, the less frequent the workplace accidents are. Conclusion: Private insurance system, fixed flat rate employers' funding mechanism, and higher coverage of compensation scheme are significantly and positively correlated with lower level of occupational accidents compared with the public insurance system, risk-based funding system, and lower coverage of compensation scheme.

  • The conecpt of hospice care for the terminally ill is a development of relatively modern times, although it has its origins in antiquity. The hospice will be able to handle progressively more and more patient, to the limit of its resources. The purposes of this study were to review of demand and issues of the hospice care programes and to recommand the hospice care approaches in south Korea that using the book-review method. At this point, although at present there is a general unawareness of hospice throughout Korean society, the public will become gradually aware of hospice. This will thus aid the development of hospices. Awareness will come about because of the educational efforts of medical schools, nursing schools, allied health training programs, practicing hospice care giver, universities, and others interested in hospice education. At present, there are many issues of the hospice care program that are hospice resources problems of manpower, facilities, finances, and cost of hospice services, quality of care, and ethics. Additional studies are needed to determine the most efficacious organizational hospice models for varying conditions. They will insist upon reasonable regulatory agency regulations that will promote the hospice idea and. permit adequate remuneration for services provided. More research is needed on health professionals', patients', and others' attitudes towards death.

A comparison of antioxidant activity of Korean White and Red Ginsengs on H2O2-induced oxidative stress in HepG2 hepatoma cells

  • The aim of this study was to determine and compare the preventive effect of Korean White Ginseng and Red Ginseng on oxidative stress in $H_2O_2$-treated HepG2 cells. The roots of ginseng were extracted with 70% methanol and partitioned with butanol to obtain saponin fractions, which have been known as bioactive constituents of ginseng. 2',7'-Dichlorofluorescein diacetate (DCF-DA) assay and malondialdehyde (MDA) content were measured for evaluating intracellular reactive oxygen species (ROS) generation. Also, mRNA expressions and activities of antioxidant enzymes were analyzed to determine the antioxidant activity of saponin or non-saponin fractions of ginsengs. According to DCF-DA assay, $H_2O_2$-induced MDA release and ROS generation were significantly reduced by treatment with saponin fractions of white and red ginseng roots. Also, saponin fractions increased effectively intracellular antioxidant enzyme activities including catalase, glutathione peroxidase and superoxide dismutase in $H_2O_2$-treated HepG2 hepatoma cells. In general, red ginseng was more effective than white ginseng for reducing oxidative stress. These results indicate that administration of red ginseng may certainly contribute relatively stronger than white ginseng to prevent from damaging liver function by oxidative stress.

Development and validation of an instrument to measure the health status of healthy but unsatisfied people : Mibyeong index(未病 index) (이상증상 기반 미병 분류도구의 신뢰도 및 타당도 연구)

  • Objectives : The number of people in Mibyeong state that complain of physical and mental discomfort but without a clear medical diagnosis has rapidly increased, but the conventional medical system is insufficient to care for these people. By establishing an evaluation instrument for Mibyeong state, it will be possible to provide a research base for Mibyeong management system and expand the clinical area of integrative medicine. Methods : Mibyeong Index was designed to measure inconvenience and resilience of subject's complains including four physical symptoms (fatigue, pain, low sleep quality, indigestion) and mental distress include anxiety, anger, depression. A 21-item quality of healthy measure was developed and tested. We used results of a nation-wide stratified sampled adult data in Korea. Results and Conclusions : Mibyeong index had adequate internal consistency, Cronbach's alpha was 0.88 in general population(N=1,110). The correlation between establised quality of life questionnaires (including SF-12 and EQ-D5 VAS) and the Mibyeong index were from 0.468 to 0.493. The national promotion of advanced health for an aging society and original Mibyeong care technology based on traditional Korean medicine can be developed by a self-care system that enhancing health before suffering illness. We expect that this instrument could be contribute to health management of people in Mibyeong state.

Intervention Efficacy of Mother Training on Social Reciprocity for Children with Autism (자폐아동을 위한 어머니 훈련 프로그램이 가정에서의 사회적 상호작용에 미치는 효과)

  • Purpose: This study examined the efficacy of parent training interventions to facilitate social reciprocity and language development in children with autism. Methods: The social interaction behaviors of mothers and children over time were compared using single subject design experimentation methodology. five children who were diagnosed with autism and their mothers participated in the study. The participants were recruited from U city, Korea. The mothers were trained using training videotapes and demonstrations on how to facilitate social interaction with their children as well as promoting language development. following the training, data were collected three times per week by video taping mother-child interaction in their homes. Results: Four of the five mothers demonstrated increases in the use of imitation with animation and expectant waiting after the intervention compared to the baseline sessions; the children demonstrated noticeable increases in the use of initiation of interaction, vocalizations, and verbal production after their mothers received the training intervention. Conclusion : Results of this study demonstrate the efficacy of mother training to improve social interactions of children with autism. Additional important information can be gained by replicating this study with more participants and comparing intervention and control groups. Clearly, this intervention shows promise and has implications far clinical practice.

Research on the Application of Gamification in Fitness App Based on Kano Model

  • In recent years, public attention to health and wellness issues has increased. The integration of smart fitness hardware and innovative technologies have made the development of smart fitness a trend. The number of fitness applications in the market has surged, and demand for an optimal experience is increasingly high. This study selects Sweatcoin, Home Workout, Six Pack in 30 Days, and Fitness Coach & Diet as research subjects from the top ten global mobile health and fitness apps in 2022 based on download rankings. The research is based on eight gamification elements: motivation, challenge, achievement, relationships, sharing, reward, level, and competition, identified through preliminary studies. We distributed a total of 166 questionnaires to users and collected 163 valid responses for data analysis. The Kano Model was used to study the desires of fitness enthusiasts using fitness apps. To reduce the limitations of the research results, the Better-Worse Method was employed for satisfaction index analysis. Based on the final analysis, we propose suggestions for improvement for the four fitness apps to better meet user needs and create a more attractive and efficient application experience.
