심방세동 환자의 불확실감, 불확실감의 평가, 우울, 불안, 주관적 건강지각 간의 관계 (The Relationships among Uncertainty, Appraisal of Uncertainty, Depression, Anxiety and Perceived Health Status in Patients with Atrial Fibrillation)

  • Purpose: The purposes of this study were to explore the concept of uncertainty and to examine the relationships among uncertainty, appraisal of uncertainty, depression, anxiety, and perceived health status in patients with atrial fibrillation. Method: The study utilized a descriptive correlational survey design using a face to face interview method. A convenience sample of 49 subjects were recruited from K university hospital over 8 months. The data were analyzed by t-test, ANOVA, Pearson correlation and partial correlation analysis. Results: 1) Subjects perceived with moderately high uncertainty(M=65.98); moderate physical health(M=39.80), mental health(M=47.38), and general health(M=2.94); moderate anxiety(M= 44.78); and slightly low depression(M=15.33). 2) There were significant differences in uncertainty by gender and education. 3) Uncertainty and danger appraisal were significantly correlated(r=.32, p=.03) while the uncertainty was not associated with opportunity appraisal. 4) Uncertainty was significantly correlated with mental health(r=-.31, p=.04), anxiety(r=.38, p=.01), and depression(r=.37, p=.01). Conclusion: This study was the first trial to explore uncertainty and to examine the relationships among its associated factors in Korean patients with atrial fibrillation. Thus, based on the findings of this study, directions for nursing practice and further nursing research for patients with atrial fibrillation were suggested.

Influence of Illness Uncertainty on Health Behavior in Individuals with Coronary Artery Disease: A Path Analysis

  • Purpose: This study aimed to investigate the influence of uncertainty-related factors on the health behavior of individuals with coronary artery disease (CAD) based on Mishel's uncertainty in illness theory (UIT). Methods: We conducted a cross-sectional study and path analysis to investigate uncertainty and factors related to health behavior. The study participants were 228 CAD patients who visited the outpatient cardiology department between September 2020 and June 2021. We used SPSS 25.0 and AMOS 25.0 software to analyze the data. Results: The final model demonstrated a good fit with the data. Eleven of the twelve paths were significant. Uncertainty positively affected danger and negatively affected self-efficacy and opportunity. Danger had a positive effect on perceived risk. Opportunity positively affected social support, self-efficacy, perceived benefit and intention, whereas it negatively affected perceived risk. Social support, self-efficacy, perceived benefit and intention had a positive effect on health behavior. We found that perceived benefit and intention had the most significant direct effects, whereas self-efficacy indirectly affected the relationship between uncertainty and health behavior. Conclusion: The path model is suitable for predicting the health behavior of CAD patients who experience uncertainty. When patients experience uncertainty, interventions to increase their self-efficacy are required first. Additionally, we need to develop programs that quickly shift to appraisal uncertainty as an opportunity, increase perceived benefits of health behavior, and improve intentions.

폐암 환자의 불확실성과 디스트레스가 삶의 질에 미치는 영향: 극복력의 조절효과 (Relationships among Uncertainty, Distress, and Quality of Life in Lung Cancer Patients: Mediating effect of Resilience)

  • Purpose: The purposes of this study were to examine health-related quality of life and to identify the mediating effect of resilience on the relationship among uncertainty, distress, and health-related quality of life in lung cancer patients. Methods: A total of 149 lung cancer patients visiting the D hospital in B city completed a questionnaire, including demographic and disease-specific characteristics, uncertainty, distress, resilience, and health-related quality of life. Data were analyzed with descriptive analysis, t-tests, ANOVA, and multiple regression analyses via SPSS 24. Results: Health-related quality of life was $81.00{\pm}21.39$ (range 0~136) in lung cancer patients. In the results of hierarchical regression analyses, the health-related quality of life was associated with education, uncertainty, distress, and resilience. However, there was no mediating effect of resilience on the relationship among uncertainty, distress, and health-related quality of life. Conclusion: Lung cancer patients with high uncertainty and distress and low resilience could experience low health-related quality of life. In order to reduce uncertainty and distress, it is necessary to provide more detailed, systematic information and support, while reinforcing positive thinking.

  • This paper tests existence of precautionary saving motive under health uncertainty, using household level panel data from Korea. For this purpose, this paper considers a dynamic health capital model with health uncertainty and derives testable equations for changes in consumption and medical expenditures. Under this framework, households who face future health uncertainty will exhibit precautionary behavior by depressing consumption or increasing investment in health. To test this hypothesis, the paper uses the conditional variance of health as the direct measure of health uncertainty, obtained by estimating a multinomial logit model. Empirical results using the Korean Household Panel Study (KHPS, 1993 - 1997) suggest that Korean elderly households follow the precautionary behavior to insure against future health risk.

항암화학요법을 받는 암환자의 영적건강과 불확실성의 관계 (The Relationship between Spiritual Health and Uncertainty in Cancer Patients Having Chemotherapy)

  • Purpose: The purpose of this study was to provide the basic data for development of nursing intervention to decrease the uncertainty in cancer patients having chemotherapy through promoting their spiritual health and identifying the relationship between spiritual health and uncertainty. Method: 174 cancer patients who were admitted for chemotherapy at hospital were recruited from the 7th March to the 10th April, 2006. Two instruments have been used for this study. One was Highfield's Spiritual Health Inventory and the other was Mishel's MUIS(Mishel Uncertainty in Illness Scale). The collected data was analyzed by frequency, percentage, mean value, average mean, standard deviation, t-test, Scheffe's test, ANOVA and Pearson Correlation Coefficients with the SPSS PC 12.0 program. Results: The average mean value of the spiritual health $3.50{\pm}\;.48$ and the uncertainty was $1.91{\pm}\;.46$. It meant that the relationships between spiritual health and uncertainty were shown to have a moderately negative correlation(r=-.489, p= .000). This means that the greater is the spiritual health, the less is the patient's uncertainty. Conclusions: It was identified that cancer patients having chemotherapy had moderate spiritual health and uncertainty, and negative correlation between spiritual health and uncertainty. Therefore, it will be necessary for the development of spiritual nursing intervention strategies to relieve the uncertainty among cancer patients having chemotherapy.

허리 척추수술환자의 수술 후 불확실성에 미치는 영향요인 (Factors Affecting Post-operative Uncertainty of the Patients Undergone Lumbar Spinal Surgery)

  • Purpose: The purpose of this study is to identify main factors affecting patients' uncertainty following lumbar spinal surgery. Methods: A survey was conducted of a sample 155 post -operative patients in April to June 2010. Uncertainty and knowledge about self-care after spinal surgery and the back pain evaluation were measured. Data were analyzed with t-test, ANOVA, Pearson correlation, and multiple regressions. Results: The mean score of uncertainty level was $27.66{\pm}6.32$. Levels of uncertainty were different by age, gender, educational level, pain duration, and number of types of pain. Uncertainty was positively correlated with knowledge of post-op self-care, back pain, and mental health. The results of multiple regression indicated that knowledge of post-op self-care and mental health were significant predictors with 35.1% of the variance in uncertainty. Conclusion: A program including post-operative self-care education and mental health promotion is required to reduce uncertainty of patients with lumbar spinal surgery.

위절제술을 받은 위암환자의 불확실성, 대처, 건강증진행위 간의 관계 (Correlation of Uncertainty, Coping, and Health-Promoting Behavior in Patients with Gastric Cancer following Gastrectomy)

  • Purpose: This study was done to investigate the correlation of uncertainty, coping and health-promoting behavior in patients with gastric cancer who have undergone a gastrectomy. Methods: A descriptive correlational design was used and the participants were 120 gastric cancer patients from one general hospital. The structured questionnaire included Mishle's Uncertainty in Illness Scale, the Korean Cancer Coping Questionnaire, and the Health Promoting Lifestyle Profile. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson correlation coefficient, and multiple regression analysis. Results: There were significant negative correlations between uncertainty and intrapersonal coping (r=-.657, p<.001); between uncertainty and interpersonal coping (r=-.223, p=.014); and between uncertainty and health promoting behavior (r=-.594, p<.001). There were significant positive correlations between intrapersonal coping and health promoting behavior (r=.790, p<.001); and between interpersonal coping and health promoting behavior (r=.502, p<.001). Uncertainty, intrapersonal coping, and interpersonal coping explained 49% of health promoting behavior (F=21.312, p<.001). The factors that influenced health promoting behavior were intrapersonal coping (${\beta}=.582$, p<.001), and interpersonal coping (${\beta}=.246$, p<.001). Conclusion: The findings of this study indicate that intrapersonal coping and interpersonal coping were significant variables for health promoting behavior in patients with gastric cancer who had undergone a gastrectomy in the past six months.

뇌전증 환자의 불확실성 개념분석 (A Concept Analysis of Uncertainty in Epilepsy)

  • Purpose: This concept analysis was done to clarify 'uncertainty in epilepsy'. Methods: Walker and Avant's methodology guided the analysis. In addition, the concept was compared with uncertainty in other health problems. Results: 'Uncertainty in epilepsy' was defined as being in the condition as seen from the epilepsy experience where cues were difficult to understand because they changed, were in discord with past ones, or they had two or more contradictory values at the same time. Uncertainty in epilepsy is evolved from appraisal of the epilepsy experience. As a result, uncertainty leads epilepsy patients, their family or health care providers to impaired functioning and proactive/passive coping behavior. Conclusion: Epilepsy patients with uncertainty need to be supported by nursing strategies for proactive, rational coping behavior. This achievement has implications for interventions aimed at changing perception of epilepsy patients, their families or health care providers who must deal with uncertainty.

뇌종양 환자의 불확실성 개념분석 (Concept Analysis of Uncertainty in Brain Tumor Patients)

  • Purpose: This study aimed to clarify the concept of uncertainty in brain tumor patients. Methods: We used the Walker and Avant's concept analysis method. We searched RISS, MEDLINE, CINAHL, and EMBASE for published articles in Korean and English from January 2002 to December 2022. After applying the inclusion and exclusion criteria, 27 articles were selected for the final analysis. Result: "Uncertainty in brain tumor" was defined as a state in which related clues during the process of experiencing a disease after brain tumor diagnosis are unclear or difficult to understand, new experiences different from before, or a condition in which it is difficult to judge fragmentarily. Moreover, the empirical criteria for the attributes of uncertainty in brain tumor patients were ambiguity of the disease process, diversity of information, unpredictability of prognosis, and complexity of management. Conclusion: Brain tumor patients with uncertainty require strategic technology development so that brain tumor patients, their families, and health care providers can use reasonable coping methods.

  • Uncertainty and variability in Life Cycle Assessment(LCA) have been significant key issues in LCA methodology with techniques in other research area such as social and political science. Variability is understood as stemming from inherent variations in the real world, while uncertainty comes from inaccurate measurements, lack of data, model assumptions, etc. Related articles in this issues were reviewed for classification, distinguish and elaboration of probabilistic/stochastic health risk analysis application in LCA. Concept of focal zone, streamlining technique, scenario modelling and Monte Carlo/Latin Hypercube risk analysis were applied to the uncertainty/variability analysis of health risk in LCA. These results show that this general framework of multi-disciplinary methodology between probabilistic health risk assessment and LCA was of benefit to decision making process by suppling information about input/output data sensitivity, health effect priority and health risk distribution. There should be further research needs for case study using this methodology.
