Results of Endoscopic Surgery in Patients with Pituitary Adenomas : Association of Tumor Classification Grades with Resection, Remission, and Complication Rates

  • Objective : The endoscopic endonasal transsphenoidal approach is a widely-used method for the surgical treatment of pituitary adenomas. We aimed to evaluate the results of endoscopic surgery by comparing preoperative classification methods and investigating their relationship with postoperative resection and remission rates and complications. Methods : We retrospectively reviewed the medical records of 236 patients (118 males) who underwent surgery for pituitary adenomas. Preoperative Knosp classification, tumor size (TS), suprasellar extension (SSE), postoperative resection and remission rates, and complications were evaluated. Results : The follow-up period was 3 months to 6 years. The patients' ages ranged between 16 and 84 years. Endocrinologically, 114 patients (48.3%) had functional adenoma (FA), and 122 patients (51.7%) had non-functional adenoma (NFA). Among the FA group, 92 (80.7%) showed remission. A statistically significant difference was found between patients with and without remission in terms of the Knosp, TS, and SSE classifications (p<0.01). Knosp, TS, and SSE classification grades were found to be correlated with the resection rates (p<0.01). Meningitis was seen in seven patients (3.0%), diabetes insipidus in 16 (6.9%; permanently in two [0.9%]), and rhinorrhea in 19 (8.1%). Thirty-six patients (15.3%) developed pituitary insufficiency and received hormone replacement therapy. Conclusion : The resection categories and remission rates of FAs were directly proportional to the adenoma sizes and Knosp grades, while the degree of suprasellar growth further complicated resection and remission rates. Adenoma sizes less than 2 cm and SSEs less than 1 cm are associated with favorable remission and resection rates.

Study on Academic Stress and Athletes' Stress-related Health Status (학생들의 학업 및 운동스트레스와 건강상태의 관련성에 관한 연구)

  • The purpose of this study is to identify the effects of lifestyle, study stress and training stress on health status, and provide fundamental data for health management of university students majoring in physical education. In this study, 149 students participated and they were surveyed demographic characteristics, lifestyle, Maslach burnout inventory (student stress inventory), training stress inventory, and Mibyeong index. Height and weight were measured for calculating BMI. For statistical analysis, Student t-test, ANOVA test, chi-square test, correlation analysis, and multinominal logistic regression test has been used. There were differences between Mibyeong groups according to digestion status, smoking for female students and quality of sleeping for both male and female students. Study stress and training stress also affected to health status. Cynicism among study stress categories and all categories among training stress showed differences between sex. Correlation analysis and logistics regression analysis was used to estimate related factors of health status after adjusting for sex and age. Based on logistics regression analysis, quality of sleeping affected to Mibyeong 1 group and quality of sleeping, smoking and digestion status affected to Mibyeong 2 group. Among training stress category, dissatisfaction with game result and skills and lack of leisure time were affected to both Mibyeong 1 and 2 group. This study suggests that lifestyle, study stress and training stress might be significantly associated with university students majoring in physical education. Through managing those influence factors, health status of students could be improved.

Assessment of the Activities of General Physicians in Health Subcenters and a Scheme to Improve the Training Program (보건지소 공중보건 일반의사의 업무수행정도와 수련개선방안)

  • The activities of general physicians (GPs) in health subcenters and their competency for clinical skills were assessed to develop a scheme to improve the training program. One hundred-twenty GPs in southern four provinces (Kyungpook, Kyungnam, Chunpook, Chunnam) were randomly selected and 97 were interviewed between January 9 and February 10, 1984. Of the 97 GPs, 86 provided all the information we requested. Average number of patient visits per health subcenter in a day was 30-40 in the demonstration project area for the class II medical insurance whereas it was 3-4 visits in other area. The interviewees were asked to rate their competency in 63 clinical skills. The skills in which over 50% of the interviewees rated themselves competent were only 12 items including IM injection, IV injection, wound dressing, etc. Less than 10% of the interviewees rated themselves competent in such skills as maternal health care, emergency medical care, preventive and promotive health services. Most part of the training program of the NIH for the GPs were not applicable to their field work as the training contents were unrealistic. Clinical training at a local general hospital was of great help in 38.8% and the rest of training was not much helpful as the training was inadequate due to lack of trainer or indifference of the trainer. For more effective training of the GPs, the training program of the NIH should be modified to be more realistic and utilize competent field workers as the instructors. It may be more effective if the training is carried out at several local centers. Ideal length of the clinical training for the GPs is 4 months. A pocketbook should be developed that includes specific skills to master during the clinical training and require the trainer to confirm the achievement. The Ministry of Health and Social Affairs should provide the training hospitals with a training guideline and evaluate the training activities and make sure that the training hospital has specialist for each of the 4 major clinical departments. The Ministry of Health and Social Affairs should provide the GPs with a continuing education to assist the problem solving in the field and motivate them to actively carry out the health program. A province may be divided into several regions and a supervisory committee may be organized with specialists in each region. The committee may hold a meeting for the GPs periodically and respond to the specific questions of the GPs by mail.

A Study on Satisfaction Investigation and Current Status of Clinical Training Program for Healthcare-Affiliated Students Depends on University Hospital Availability (학교병원 유무에 따른 보건-의료계열 대학생의 임상실습 현황 및 만족도 조사)

  • The purpose of this study was to identify clinical practice (clinical training education) and satisfaction depend on university hospital availability according to a survey with 850 healthcare-affiliated students. The collected data were analyzed with the SPSS17.0 program. The result was shown statistically significant difference depends on gender, major departments and satisfaction of college life. In addition, there were significant differences in satisfaction of clinical training education in the environmental aspects of practice depending on whether affiliated hospital or not. There were no significant correlation found between degree of satisfaction of clinical training education and the need for affiliated hospital (r = .204, p <.01). As a result, Attached to the university hospital can be helpful to healthcare-affiliated students on environmental and educational aspects of clinical training education. Therefore, the effective and practical training programs using university-affiliated hospitals are surely needed to develop and to apply the active support.

A Study on the Methods of Activation for CIEO in Health Junior College - About the on-the-Spot-Training of Department Health Education - (보건전문대학의 산학협동 효율화 방안에 관한 연구 -보건계학과의 현장실습을 중심으로 -)

  • This study aimed at inquiring into the basic theory on CIEO(the Cooperation between Industry and Educational Organizations), grasping and analyzing the present situation of the health junior college, the present condition of CIEO and into problem and seeking to find a solution to activate the CIEO of the health junior colleges. This study suggested five methods as the method of consolidating the on-the-spot-training as below : 1. The execution of the basic research for the on-the-spot-training. And the reorganization of the existing curriculum into the curriculum the industrial organization requires. 2. The participation in solving commonly the problems of the industrial organization and in education. The establishing of the guidance system assuming exclusive charge of the on-the-spot-training. 3. The extension of the period of the on-the-spot-training and the reorganization of an educational system. The submission of the evaluation sheet for the on-the-spot-training. 4. The securing of the educational dost spent on the on-the-spot-training and the imposition of benefits on the industrial organization. 5. The persistent support for the on-the-spot-training at the governmental dimension. It is required that the CIEO law should be enacted as the legal and institutional solution and that government should lend systematic support to the business enterprise exeuting the industrial educational cooperation such as the institutional support for and the imposition of benifits on the cost of research. The cost of research and development, the cost of internal education and training, and the cost of experiment and practice.

  • This study aimed at inquiring into the basic theory on co-operative education, grasping and analyzing the present situation of the junior health college, the present condition of co-operative education and into problem. and seeking to find a solution to activate the co-operative edcation of the junior health colleges. For this purpose this study compared the co-operative education system in our country with that of foreign countries revolving around the related literature to co-operative education, analyzed it, and the pointed out its problem. And this study classified into the solution of establishing co-operative education system, the solution of consolidating the on-the-spot training system, the solution to activate the co-operative education of the junior health college. First this study suggested three methods as a part of establishing co-operative education system as below. 1. The need to set up the ideology concerning co-operative education. 2. The construction of the advisory committee for co-operative education. 3. The establishment of an agency for co-operative education. And this study suggested nine methods as the method of consolidating the on-the-spot training as below. 1. The execution of the basic research for the on-the-spot training. 2. The reorganization of the existing curriculum into the curriculum the industrial organization requires. 3. The establishing of the system in a closer cooperation with the industrial organization for the on-the-spot training. 4. The participation in solving commonly the problems of the industrial organization and in education. 5. Effect of a global health training program for nursing students on global citizenship and cultural competencies

  • This study was a quasi-experimental study with a single group pre-posttest design for evaluating the effects and meanings of a global health training program for nursing students. A mixed data collection method utilizing both qualitative and quantitative methods was applied. The qualitative method consisted of reflection journal entry analysis, while the quantitative study evaluated data collected from a self-reported questionnaire. After attending the program, cultural competencies of participants significantly increased than pre-test while global citizenship was not. Through an analysis of reflection journals written during the overseas training period, the meanings of experiences derived after the program were classified into five themes and 12 sub themes, which were, 'improvement of global health leadership', 'experience of barriers in reality', 'improvement of nursing professional value', 'inspiration of a perspective on health equity', and 'internal growth'. The results indicate that global health training program evaluated in this study was meaningful; it provided students with opportunities to experience the nurse's role in diverse global environments and attempted to measure the effects.

Analysis of Service Occupation Types and Training Programs of Services for the Aged in Busan (실버서비스를 위한 전문인력개발 프로그램의 분석)

  • Purpose: The purpose of this study was to analyze service occupation types and to develop training programs for the aged. Methods: This study used descriptive study design. The research process consisted of three stages: first, identified the demand of service manpower for the aged; second, investigated present jobs and education programs in Korea and Japan; and last, developed service jobs and education programs for the aged. Results: Potential users considered "health management" to be the most important area. They thought "providing job" as second most important. According to the result of analyzing Korean policies, there were 9 service occupations in 5domains. So, we derived 10 occupations such as 'daily living manager', 'care manager', 'care worker', 'health manager', 'education specialist', 'leisure manager', 'good manager', 'housing manager', 'financial specialist' and 'retirement consultant' in 5domains as healthcare, leisure, goods, housing and finance. Finally, we developed their tailored training programs. Conclusion: According to this study, there should be various occupations qualified by the government, and training programs should be settled. And healthcare providers must included in developing standardized training programs.

The Effects of Proprioceptor Training and Vestibular Organ Training on Balance Ability of Adults

  • The purpose of this study was to analysis of the effect of proprioceptor training and vestibular organ training for balance ability. The subjects was consist of two different subjects group, proprioceptor training group and vestibular organ training group. Proprioceptor training group consisted of 10 subjects and vestibular organ training group consisted of 10 subjects. Training was performed 3 times per week, 30 minutes per day, for 3 weeks. Balance ability analysis was performed using Romberg's one leg standing test and BT4 when opened eyes and closed eyes. The analysis results were as follows. There was no significant differences in balance after the training in both groups when they opened their eyes(p<.05). But there was significant differences in balance after the training in both groups when they closed their eyes(p<.05). And there was no significant difference in balance after the training between the proprioceptor training group and the vestibular organ training group when they closed their eyes(p<.05). Given the above results, proprioceptor training and vestibular organ training enhanced balance but there was no significant difference between the two methods.

  • The purpose of this study is to investigate the problem on safety and health training system in Occupational Safety and Health Act(OSHA) and to find countermeasures to improve it in the construction industry. The questionary survey was done on aiming at tracing the legal drawbacks of safety and health training system. The questionary was prepared to target on site workers, management supervisors, and safety supervisors in various construction sites. After analyzing the answers from the survey, realistic and optimal countermeasures on derived problems were proposed. These should be included on the next revision of OSHA.
