Factors Associated with the Use of Gastric Cancer Screening Services in Korea: The Fourth Korea National Health and Nutrition Examination Survey 2008 (KNHANES IV)

  • Objective: Despite government efforts to increase participation in gastric cancer screening, the rate is still suboptimal in Korea. Therefore, we explored barriers to and predictors of gastric cancer screening participation among a nationally representative sample. Methods: We used the Health Interview Survey sub-dataset derived from the Fourth Korean National Health and Nutrition Examination Survey 2008 (KNHANES IV) to evaluate participation in gastric cancer screening and factors associated with attendance in individuals age ${\geq}40$ years. We enrolled 4,464 subjects who completed the questionnaire and were not previously diagnosed with gastric cancer. Four groups of factors were considered potential predictors of gastric cancer screening in a multivariate analysis: sociodemographic, health behavior, psychological and cognitive, and dietary factors. Results: Overall, 41.3% complied with the gastric cancer screening recommendations. Younger age, lower education level, living without a spouse, frequent binge drinker, and current smoker were significantly associated with less participation in gastric cancer screening. Conclusions: To improve participation in gastric cancer screening, more focused interventions should be directed to vulnerable populations, such as groups with low socioeconomic status or unhealthy behavior. In addition, there should be new promotional campaigns and health education to provide information targeting these vulnerable populations.

한국 국민의 국가암검진사업 인지와 수검의도간의 관련성 : 온라인 설문을 중심으로 (A Study on the Relation between the Awareness of National Cancer Screening Program by the Korean Public and Cancer Screening Intention : An Online Survey)

  • Objectives : The aim of this study was to investigate the degree of awareness of the national cancer screening program by the Korean public and define its relationship with cancer screening intention. Methods : The study collected data by conducting an online survey from April 19 to 24, 2016, and received a total 354 responses. Additionally, we performed frequency analysis, chi-square test, and logistic regression analysis. Results : Based on the results for the degree of awareness for the national cancer screening program, 151 (42.7%) people were aware of the program, while 203 (57.3%) were not aware the program. From the chi-square test and logistic regression analysis, marital status and awareness of the national cancer screening program were found to have a statistically significant influence on screening intention. Conclusions : An increase in the screening rate should be established with a systematic promotion plan for the national cancer screening program.

Cancer Screening in Korea, 2012: Results from the Korean National Cancer Screening Survey

  • We investigated the cancer screening rates for five types of cancer (stomach, liver, colorectal, breast, and cervix uteri) using data from the Korean National Cancer Screening Survey (KNCSS), which is a nationwide, annual cross-sectional survey. The eligible study population included cancer-free men 40 years of age and older and women 30 years of age and older. The lifetime screening rate and screening rate with recommendation were calculated. The lifetime screening rates for gastric, liver, colorectal, breast, and cervical cancers were 77.9%, 69.9%, 65.8%, 82.9%, and 77.1%, respectively. The screening rates with recommendation were 70.9%, 21.5%, 44.7%, 70.9%, and 67.9%, respectively. The most common reason for all types of cancer was "no symptoms", followed by "lack of time" and "fear of the examination procedure". Efforts to facilitate participation in liver and colorectal cancer screening among Korean men and women are needed.

보건소의 국가 암 검진사업 실태 (A Survey of the Implication of the Cancer Screening Program in Health District Centers)

  • Purpose: To evaluate every step of the cancer screening program. Method: 146 of 233 health centers participated in this study. Data were collected by mailing questionnaires between December 2002 and January 2003. The response rate was 65.5%. Result: The government cancer screening program was directed by a variety of departments of the health centers. 41.1% of persons in charges were nurses. 41.3% of the health centers received a list of the appropriate person from the National Health Insurance Cooperation within one or two months. 26.1% of health centers received ??? after five month from when the program started. All the health centers advertised their services, and most of them used mail and mass media 56.4%, used other institute's materials, and 72.2% of them used the government's materials. 76.7% of the heath centers recommended secondary health screening to the persons who had positive results at the first screening. 71.6% of the health centers ascertained the patients with cancer who were diagnosed at secondary screenings. 67.9% of the health centers had registered home-based cancer patients at the health center. 137 out of 146 health centers had a local institute for cancer screening; an average of 4.9 institutes had cancer screening. 80.1% of the persons in charge of the programs felt they needed this program, but 80.8% of them thought the program should be modified. Conclusion: Organized and standardized programs are needed to promote the efficiency of National Cancer Screening Program.

학교기반 학생 건강검진 참여 결정에 대한 연구 (Study on Opt-in of School-Based Student Health Screening)

  • 본 연구는 학교를 기반으로 하는 학생 건강 검진 참여를 결정하는 요인들이 무엇인지를 밝히는 연구이다. 연구대상은 미국 웨스트버지니아주의 건강 관리사 6명이며, 자료 수집은 전화 인터뷰를 활용하였다. 연구 결과는 다음과 같다. 첫째, 자녀 건강에 대한 관심과 외부적인 압박, 그리고 무료 검진에 의해 동의하는 것으로 나타났다. 둘째, 건강 검진에 대한 지식 부족, 가정 통신문에 대한 무관심, 그리고 자녀의 정보 노출에 대한 불안감, 피를 뽑아야 하는 두려움이 동의를 하지 않는 것으로 나타났다. 셋째, 건강 검진의 장점으로 모든 절차가 무료로 진행된다는 점이었다. 넷째, 학부모가 동의를 결정하는 주된 결정권자이지만 때로는 자녀에게 그 결정권이 존재하고 있었다. 다섯째, 학생 주변에 존재하는 교사, 동료학생, 학교 모두 참여를 결정지을 수 있음을 확인하였다. 그러므로 가정 통신문 개선, 학부모 및 교사 교육, 무료 검사가 필요할 것이다.

전라남도 거주 여성 중 사별군과 비사별군간의 건강행태 비교 (The Comparison of Health Behaviors Between Widowed Women and Married Women in Jeollanamdo Province, Korea)

  • Objectives: To compare the health behaviors of widowed women with those of currently married women. Methods: We randomly sampled the subjects from the Jeollanamdo Resident Registration Data and we then selected 2,331 widowed women and 4,775 married women. Well-trained examiners measured the height, weight, blood pressure and abdomen circumference, and the women were interviewed with using a questionnaire. Logistic regression analysis was used to estimate the odds ratios(OR) of the two groups. Results: The smoking rate (OR=2.46; 95% confidence interval [CI]1.65, 3.66) was significantly higher for the widowed women. On the contrary, the awareness rate of a smoking cessation campaign (OR=0.80; 95% CI=0.70, 0.92), a quit tobacco telephone line (OR=0.73; 95% CI =0.61, 0.88) and a quit smoking clinic (OR=0.74; 95% CI=0.62, 0.89) were lower for the widowed women. The rate of receiving a health exam (OR=0.80; 95% CI=0.70, 0.91), the rate of undergoing gastric cancer screening (OR=0.77; 95% CI=0.68, 0.88), breast cancer screening (OR=0.79; 95% CI=0.69, 0.89), cervix cancer screening in the last 2 years (OR=0.81; 95% CI=0.71, 0.92), colon cancer screening in the last 5 years (OR=0.74; 95% CI=0.63, 0.87) were significantly lower for the widowed women. Conclusions: This study revealed that the health behaviors are significantly different between the widowed women and the married women. To improve the health behaviors of the widowed women, further study and research that will investigate the socioeconomic and environmental factors that affect the health behaviors of widowed women will be needed.

A Multi-level Analysis of Factors Affecting Participation in Health Screenings in Korea: A Focus on Household and Regional Factors

  • Objectives: This study divided the factors that affect participation in health screenings into individual, household, and regional levels and conducted a multi-level analysis to identify the factors related to participation in health screenings. Methods: Participants from the 2017 Community Health Survey were classified into 2 groups (under 40 and 40 or older). A multi-level logistic regression analysis was conducted to identify the factors that affected participation in health screenings. Results: The screening rate of the participants was 69.7%, and it was higher among participants aged 40 and older (80.3%) than it was among participants younger than 40 (49.8%). At the individual level, the factors that influenced participation in health screenings included age, economic activity, smoking status, physician-diagnosed hypertension, and a moderate or high physical activity level. At the household level, the odds ratio of participation in health screenings was high for participants who lived in single-person households, lived with a spouse, earned a high monthly household income, and were not beneficiaries of national basic livelihood security. At the regional level, the odds ratio at the 95% confidence interval level of participation in health screenings was high for participants who had trust in the local community and lived in an area with a proportionally high social welfare budget. Conclusions: This study analyzed nationalwide data and confirmed that individual, household, and regional characteristics affected participation in health screenings. Therefore, policies that prioritize the improvement of regional level factors and especially household level factors are likely to be the most effective for improving the screening rate.

대장암 발생 고위험군의 예측모형 개발과 활용 (Developing the Predictive Model for the Group at High Risk for Colon Cancer)

  • Objectives: We developed the predictive model for the incidence of colon cancer by utilizing the health screening data of the National Health Insurance in Korea. We also explored the characteristics of the high risk group for colon cancer. Methods: The predictive model was used to determine those people who have a high risk for colon cancer within 2 years of their NHI health screening, and we excluded the people who had already been treated for cancer or who were cancer patient. The study population is the insured of the NHI, aged 40 or over and they had undergone health screening from the year 2000 to 2004, according to NHI health screening formula. We performed logistic regression analysis and used SAS Enterprise Miner 4.1. Results: This study shows that there exists a higher rate of colon cancer in males than females. Also, for the population in their 60s, the incidence rate of colon cancer is much higher by 5.36 times than that for those people in their 40s. Amongst the behavioral factors, heavy drinking is the most important determinant of the colon cancer incidence (7.39 times in males and 21.51 times in females). Conclusions: Our study confirms that the major influencing factors for the incidence of colon cancer are drinking, lack of exercise, a medical history of colon polypus and a family history of colon cancer. As a result, we can choose the group that is at a high risk for colon cancer and provide customized medical information and selective management services according to their characteristics.

종합 건강검진 과정에서 나타난 일부 중년기 여성의 건강문제 (Health Problem of the Middle-Aged Women)

  • This study was carried out to offer the basic data for more qualitative lives of the middle-aged women through their health maintenance and control. So, the researcher reviewed and analyzed the health problems of the middle-aged women, the differences of main symptoms in each subject, positive ratcs in screening tests, the difference of screening tests with age and the correlation of obesity and blood pressure with screening tests. All subjects were 218 women(40-59years) who had a check-up at a comprehensive health check-up center in K University Hospital in Seoul from July 1, 1998 to August 31. 1998. Questionaires were developed to get subjects, general characteristics. main symptoms, medical examination and diagnosed disease by the researcher referring through the questionaires of comprehensive health check-up center and the medical recorders were used as a tool of study. The researcher analyzed the data with SPSS PC+ ; the distribution of the subjects, general characteristics and main symptoms with percentage : the difference between main symptoms and screening tests with X$^2$-test, ANOVA. The results are as follows. 1) Among the age distribution, most were 40-45 years old and unemployed. 2) The most common complaints were indigestion in digestive system, cough in respiratory system, heart beat in cardiovascular system, general sweat in endocrine system. edema in urinary system, easy bruise in hematologic system, backache in musculoskeletal system, headache in psychoneurologic system, lochia in gynecology, itch in dermatology system, eye ball pain in visual system and tinnitus in auditory system. 3) In main symptoms according to age, the 40-45 age group had a higher rate of complaints in digestive system and gynecology than other age group ; the 46-49 age group in endocrine system. 4) The main symptoms according to diagnosis had no difference statistically. 5) The screening tests which showed high rates of abnormality were mammography(the highest), abdominal ultrasonography, upper gastrointestinal series/gastrofiberscopy, PFT. In all age group the highest rate of abnormality was seen in mammography. 6) In screening tests according to age. the 46-49 age group showed higher positive rates of pap smear, blood sugar test, urine test than any other age group ; the 50-55 age group showerd higher positive rates of obesity, BP, chest X-ray, abdominal ultrasonography, ESR, SGOT, CRP and urine micro. 7) In correlation of obesity with screening test, the obesity group showed higher positive rates of blood pressure, chest X-ray, abdominal ultrasonography, HCT and CRP than the normal weight group. 8) In correlation of blood pressure with screening test, the hypertension group showed higher positive rate of obesity, chest X-ray, abdominal ultrasonography, pap smear and blood sugar than the normal blood pressure group. In conculusion, the middle-aged women have various physical symptoms and affected by age. The obesity and BP have an important effect on the health of the middle aged women. Therefore, this study is considered significant as data for qualitative lives of the middle-aged women playing an important part of family health care by catching of their health problem complaints for prevention of disease and promotion of health.

유방암과 자궁경부암 수검자용 한국판 건강정보이해력 측정도구의 타당도와 신뢰도 검증 (Validity and Reliability of the Korean Version of Assessment of Health Literacy in Breast and Cervical Cancer Screening)

  • Purpose: Health literacy is a significant determinant of health and health behaviors such as cancer screening. Despite its significance, there are limited instruments available to assess health literacy targeting Koreans. The purpose of this study was to test the psychometric properties of Korean translation of a validated health literacy instrument in cancer screening-Korean version of assessment of health literacy in breast and cervical cancer screening (K-AHL-C). Methods: A total of 555 women aged 20~65 participated in the online survey study. Of 52 items addressing five domains included in the original version, we focused on 36 items addressing three key domains closely associated with cancer screening: familiarity, health navigation, and comprehension. Results: During content validation, two items from the health navigation domain were removed, yielding 34 items. Using Rasch analysis and confirmatory factor analysis, we found the evidence of construct validity of K-AHL-C. The Korean version was also significantly correlated with measures of Functional Health Literacy scale, cancer prevention behaviors, and subjective health status, suggesting convergent validities respectively. Finally, K-AHL-C had acceptable reliability coefficients (α) ranging from 0.71 to 0.92 for each domain and the total scale. Conclusion: These psychometric properties support the K-AHL-C is a valid and reliable instrument for measuring Koreans' health literacy in cancer screening. Also it is expected to use the instrument to detect breast and cervical cancer early and improve the screening rate, and ultimately to contribute to the promotion of women's health and women's health nursing practice.
