지역사회 취약계층 아동의 건강행태와 건강관련 삶의 질 (Health Behaviors and Health-related Quality of Life among Vulnerable Children in a Community)

  • Purpose: The purpose of this study was to examine the association between health behaviors and health-related quality of life (HRQOL) among vulnerable children in a community. Methods: Using data from 'The Obesity Prevention Framework for Vulnerable Children', a secondary analysis was conducted for 165 children (ages 8~12 years) and their parents who were recruited from 16 K-gu Community Child Centers in Seoul. Six types of health behaviors related to eating and activity were assessed. Each behavior was categorized into the non-recommended vs. recommended levels. The scores of the recommended levels of the six health behaviors were summed up for the composite score of health behaviors. HRQOL was measured by KIDSCREEN-52. Results: The groups with a non-recommended level of fast food intake and sedentary behavior had a significantly lower total score of KIDSCREEN-52 than those with a recommended level. Moreover, the lower composite score of health behaviors was significantly associated with the lower total score of KIDSCREEN-52. Conclusion: Among the vulnerable children, the six recommended health behaviors and their composite score were in significant positive associations with the HRQOL levels. Therefore, nursing strategies for enhancing the recommended levels of health behaviors are needed for vulnerable children.

  • Objectives: This study aimed to assess the association between physical activity and health-related quality of life (HRQOL) in Koreans. Methods: A total of 9,689 adults(${\geq}19$ years, 4,036 men) from the Fourth National Health and Nutrition Examination Survey(2007-2008) were examined by the International Physical Activity Questionnaraire (IPAQ)-short form and Korean EuroQol-5 Dimension(KEQ-5D). Results: For the physical activity by IPAQ, 'Inactive group' was 75.3%, 'Minimally active group' was 23.5%, and 'Health-Enhancing Physical Activity(HEPA) group' was 1.2%. The mean HRQOL index was $0.89{\pm}0.13$. These relations of physical activity and HRQOL remained significant after adjustment for age, sex, smoking, subjective health status, stress, depression, and socioeconomic status variables (Adjusted $R^2$ = 0.334). Conclusions: Physical activity was associated with health-related quality of life. Physically active group had higher health-related quality of life than the inactive group.

청소년 자녀의 건강관련 삶의 질에 대한 부모와 자녀의 인식차이가 청소년 우울에 미치는 영향 (Correlates of Depression and Perception Gap of Adolescents' Health-Related Quality of Life between Parents and Children)

  • Objectives: The objective of this study is to evaluate the correlates of depression and perception gap of adolescents' Health-Related Quality of Life (HRQOL) between Parents and Children. Methods: Research targets are all students and their parents who are enrolled in from 1st~3rd grade of middle school to $1^{st}$ grade of high school which was selected intentionally. The survey was conducted from May $16^{th}$ to June $9^{th}$ in 2014 by research staffs equipped with structured questionnaire. $PedsQL^{TM}4.0$ Generic Core Scales and Korean form of the Kovacs' Children's Depression Inventory were the main tool for measuring the HRQOL and depression. Data of 985 respondents, both of parents and children, was used for analysis of this study. Results: In terms of gender difference of depression, 5.8% of adolescents, 3.2% of male and 8.5% of female (p<0.001), are shown depression symptom. Depression rate of the low level students of academic achievement was 11.0% which is the lowest rate to compare to 4.3% of middle level and 2.6% of high level students (p<0.001). The case that parents overestimated their children's overall HRQOL is 3.699 times more likely to have depression symptom than the case which is in concurrence between parents and children. Especially the overestimation in social, emotional, and academic area of HRQOL promote depression 3.918 times, 3.371 times, and 2.258 times more than the agreement occurred of each areas. In contrast underestimation in physical area of HRQOL reduce depression 0.266 times less than the agreement occurred of the area. Conclusion: The perception gap of adolescents' HRQOL between parents and children is the most influential factor to adolescents' depression among all variables which was evaluated in this study.

기혼 직장여성의 건강관련 삶의 질에 영향을 미치는 요인 (The Influencing Factors on Health Related Quality of Life(HRQOL) in Married Working Women)

  • Purpose: The purpose of this study is to identify the factors that influence HRQOL for married working women. Methods: The subjects of the study were 577 full-time working married women living in five province. The data were collected using a structured questionnaire, from April to September 2006. Results: As for the general characteristics, there was a statistically significant difference in the level of HRQOL(total) according to age, education, menopause, family income, leisure activities, residence, duration of marriage, number of children, firstborn age, household duties of husband, job, career, type of work, women's income, and purpose of working. The level of HRQOL showed a statistically significant difference according to BMI and behavioral factors except for caffeine intake and follow up health screening. The HRQOL were significantly correlated with depression, marital satisfaction and job satisfaction. The multiple regression analysis showed that the HRQOL were significantly predicted by depression, marital satisfaction, education, regular exercise, daily working hours and job satisfaction. These variables explained 48.5% of the variance of HRQOL. Depression was the main predictor of HRQOL. Conclusion: It could be concluded that nursing intervention to reduce depression should be developed to promote the HRQOL for married working women.

일부(一部) 생산직(生産職) 근로자(勤勞者)들의 건강관련(健康關聯) 삶의 질(質)과 양생수준(養生水準)과의 관계(關係) (The Relationship between Health-related Quality of Life and Yangseng Level among Blue-Collar Workers)

  • Objective : This research was done to discover whether or not workers' health related quality of life(HRQOL) depends on their own Yangseng(養生) level, which is also known as one's care of one's health, and also if their HRQOL is affected by the level, to what extent. The subjects of the study were blue-collar workers of a workplace. Method : Blue-collar workers of a workplace were asked to fill out their pre-organized questionaires given to them by their company as a process of health examination. The questionaires carried questions regarding their levels of Yangseng and their HRQOL. For the purpose of the research, a total of 961 data were selected from the questionaires filled out by the blue-collar workers and then analyzed. Result : Each workers's level of Yangseng becomes high or low by one or all of general characteristics. With regard to Yangseng level according to health-related lifestyle were found to have a relatively higher level of Yangseng. With reference to HRQOL according to general characteristics, those in the 20s were found to have a physical summary scale(PCS), but no significant difference was found in the other scales. As regards the quality of life(QOL) according to health-related lifestyle, those who exercise regularly were found to have a relatively higher PCS, while no smokers, those who do not drink alcoholic, and those who sleep at least seven hours a day were found to have a relatively higher mental summary scale(MCS). As to the relation between the level of Yangseng and HRQOL, the level of Yangseng was found to have an interrelation with PCS and MCS in terms of its degree. Statistics also show that each area of Yangseng level has a significant impact on the two qualities-PCS and MCS. Conclusion : One who has a higher level of Yangseng was found to be the one who enjoys a higher HRQOL. Specially, the three kinds of habits that are good for good health -non-smoking, no drinking and seeping seven hours a day- were found to be able to improve the QOL. In this respect, those who want to stay healthy are recommended to cultivate a healthier habit of living.

심장재활과 삶의 질 (Cardiac Rehabilitation and Quality of Life)

  • Recent shortening of hospitalization has partly led to the transition of health care services from hospitals to communities in cardiovascular (CVD) care. Home healthcare nursing is an alternative modality of care for chronically ill CVD patients. Cardiac rehabilitation (CR) has been redefined as a "secondary prevention center", consisting of: patient assessment; nutritional counseling; blood pressure and diabetes management; tobacco cessation; psychosocial management; and physical activity counseling. Improvement in health-related quality of life (HRQOL) is a major goal of the CR that integrates physical, psychological and social dimensions of care. The review of evidence on effects of CR on HRQOL may allow home healthcare nurses to provide better comprehensive care for CVD patients. There is evidence on beneficial effects of CR on HRQOL in patients with myocardial infarction (MI) as well as patients with chronic heart failure. Specifically, home-based CR, which is more cost-effective than hospital-based CR, has been reported to produce comparable improvements in HRQOL with hospital-based CR in MI patients. In conclusion, a newly-designed, home-based CR may be required to be applied to Korean home healthcare nursing system for improving HRQOL.

한국 노인의 구강보건행위가 삶의 질에 미치는 영향에서 스트레스의 매개효과 (The Mediating Effect of Stress on Relationship between Oral Health Behaviors and Health-related Quality of Life in the Korean Elderly)

  • 본 연구는 노인의 구강보건행위가 건강관련 삶의 질(Health-related Quality of Life, HRQOL)에 미치는 영향에서 스트레스의 매개 효과를 분석하기 위하여 국민건강영양조사(2010-2015)를 활용한 이차 자료분석 연구이다. 65세 이상 노인 8,148명을 대상으로 사회 경제상태, 건강 상태, 흡연 및 음주 습관, 스트레스 여부, 그리고 구강보건행위(칫솔질 횟수, 정기적인 치과 검진여부, 치간 세정도구 사용)를 조사하였으며 건강관련 삶의 질 측정을 위해 EuroQol-5 Dimension(EQ-5D) index를 사용하였다. 연구결과, 사회경제학적 수준이 낮거나 고혈압, 당뇨병을 가진 노인들에서 HRQOL이 낮았으며, 칫솔질 횟수가 적거나 정기적인 치과방문을 하지 않거나 치간 세정도구를 사용하지 않는다고 응답한 노인들에서 HRQOL이 낮았다. 그리고 스트레스는 노인의 삶의 질에 부적 상관을 보였으며 구강보건행위와 삶의 질의 관계에서 부분매개효과를 확인하였다. 이러한 결과는 노인의 구강보건행위와 삶의 질의 관계에서 스트레스 매개효과가 있음을 의미한다. 그러므로, 노인의 건강관련 삶의 질 증진을 위해 건강증진프로그램을 수행할 때 스트레스 해소 및 구강건강증진 프로그램을 포함한 융합적인 프로그램이 필요하며, 그러한 정책마련을 위한 기초자료로 제공하고자 한다.

노인 뇌졸중 환자의 후유증 유무에 따른 HRQOL 요인별 중요도 분석 (A Study on Health Status' Factors Affecting HRQOL in the Elderly Stroke Patients With or Without Sequela)

  • 목적 : 본 연구는 치료를 받고 있는 뇌졸중 환자 중 후유증의 유무에 따라 건강관련 삶의질(HRQOL)에 영향을 주는 건강상태의 중요도가 어떻게 달라지는지를 조사하여, 건강 관련 삶의 질 향상 방안을 위한 기초자료로 활용하는데 있다. 연구방법 : 뇌졸중 환자의 후유증 유무에 따라 HRQOL에 영향을 미치는 요인들의 중요도를 분석하기 위하여 국민건강영양조사 제5기의 자료를 활용하여 65세 이상 치료를 받고 있는 뇌졸중 환자 91명을 대상으로 2차 분석을 실시하였다. 결과 : 첫째, 후유증이 있는 뇌졸중 환자의 경우 없는 그룹보다 HRQOL과 주관적 건강상태가 유의하게 낮았으며(p<0.05), 자살 생각이 있을 위험도는 3.64였다. 둘째, 전체 그룹의 경우 주관적(${\beta}=0.39$), 정신적(${\beta}=0.29$), 의학적(${\beta}=0.23$) 건강상태가, 후유증이 있는 그룹의 경우 정신적(${\beta}=0.45$), 주관적(${\beta}=0.36$) 건강상태가, 후유증이 없는 경우 의학적(${\beta}=0.45$), 기능적(${\beta}=0.32$), 정신적(${\beta}=0.25$) 건강상태 순으로 HRQOL에 크게 영향을 주는 요인으로 추출되었다(p<0.05). 결론 : 종합하면 재활치료에 있어 뇌졸중 환자의 HRQOL 향상을 위해서는 후유증이 중증일 경우는 정신적 건강상태에 영향을 미치는 우울과 자살에 대한 조기 발견 및 조기 중재가 중요하며, 후유증이 경증이거나 없는 경우에는 기능적 회복에 초점을 맞추어 진행해야 하고, 후유증 유무와 상관없이 자신의 현 상태를 긍정적으로 받아들일 수 있도록 지속적인 지지를 해준다면 뇌졸중 환자의 기능적 회복과 더불어 HRQOL을 더욱 향상시켜 최종적으로 뇌졸중 환자의 독립적 자립에 긍정적인 영향을 줄 수 있을 것으로 사료된다.

일개 시지역 뇌졸중 환자의 삶의 질과 사회적 지지와의 관련성 (The Association of Social Support and Quality of Life of Stroke Patients in a City)

  • Objectives: The aim of this study was to investigate the association of social support and quality of life of stroke patients. Methods: Stroke patients were interviewed by a direct contact survey among 160 person. A structured questionnaire was configured about demographic features, social support and health related quality of life(SF12). Data were collected from July to August 2009. Results: Among social support, satisfaction of dependable when you need help is the highest score(5.3) and console you when you are very upset is the lowest(4.0). Vitality among health related quality of life(HRQOL) is the higest score(55.9) and general health score is the the lowest(17.4). Physical component score(PCS) is lower than mental component. The results of multiple regression analysis about the factors affecting the PCS and MCS score, disability grade and feel better variable gave effect HRQOL. Conclusions: For improvement of HRQOL, among stroke patients, community social support, specially, when patients feeling generally down-in-the dumps, the rely on to help feel better was needed.

Associations between Health Behaviors and Health-Related Quality of Life among Breast Cancer Survivors

  • Purpose: To examine the levels of physical activity (PA) and diet quality, socio-demographic and clinical correlates, and identify associations with health-related quality of life (HRQOL) among breast cancer survivors. Methods: The study used a cross-sectional study design. A total of 74 breast cancer survivors who had completed their primary cancer therapy were recruited from a comprehensive cancer center in Korea. Measurements used included the International Physical Activity Questionnaire, the Diet Quality Index, and the EORTC QLQ-C30. Results: Only twenty-six women (35.1%) met the American Cancer Society criteria of weekly PA, while most participants (93.2%) displayed good or excellent diet quality. Those less likely to meet the PA criteria were older women, women who had a lower economic level, and women not receiving anti-hormone therapy. However, there was no significant factor associated with diet quality. Women who met the criteria for PA displayed significantly better global QOL than women who did not meet the criteria. No significant differences were found in HRQOL between women who did and did not meet the diet quality criteria. Conclusion: Nurses should be aware of breast cancer survivors who are older and who have a low economic status when assessing and screening the level of PA to improve HRQOL.
