Purpose: This study is a descriptive research which is designed to investigate hospitalized patients' intent to use home care nursing according to the types of medical security. Method: This researcher surveyed 236 patients who were hospitalized at B medical center located in Busan,. Data were collected from Sep. 1 to Nov. 30, 2005 using a questionnaire survey, medical records, face-to-face interviews and observations. Collected data were analyzed in terms of frequency, percentage, mean and standard deviation through $x^2$-test and t-test under SPSS WIN 10.0 Program. Result: Out of the total subjects, 59.3% were medical aid clients and the remaining 40.7%, health insurance ones. The hospitalized period and frequency of the former group were 38.0 days and 4.0 times, respectively, while those of the latter, 37.7 and 3.4. When home care nursing clients were examined using a given classification device, it was found that out of the total 236 subjects, 205(86.9%) were needed to receive home care nursing, 121, medical aid and the other 84, health insurance. 24.0% of medical aid clients heard about home care nursing ever before, lower than 39.3% of health insurance clients. 43.8% of the former clients said cost for home care nursing was high while, 47.6% of the latter group responded expense for the nursing intervention was low. 30.6% of medical aid clients had intent to use home care nursing, lower than 47.6% of health insurance clients. 71.7% of those patients whose monthly income was 99 million won or below had no intent to use home care nursing, higher than 62.5% of those who were 100 million or over in monthly income(p<.05). 76.4% of those clients who had no nursing provider intented to use home care nursing, higher than those who had nursing provider(p<.05). Concerning contents of home care nursing, 85.1% of medical aid clients needed education, training and counseling while, 77.4% of health insurance aids wanted medication and injection. Conclusion: In conclusion, the use of home care nursing by medical aid clients should be promoted through improving conditions for home care nursing in terms of expense, family and residence and making public relations about activities and contents of the home care nursing.

Purpose: The aim of this study was to investigate the factors contributing to health-related quality of life (HRQOL) among patients with chronic hepatitis B (CHB). Methods: A cross-sectional, descriptive design was used. The sample included 114 patients in a gastroenterology outpatient department at one hospital located in G city. Data were collected using a structured questionnaire from June to December 2009. The collected data were analyzed using SPSS/WIN 15.0. Results: Health-related quality of life was most strongly related to age, sex, income, occupation, and the care provider. Old age, female gender, lower income, not having an occupation and having a non-medical person were associated with lower HRQOL ($R^2$=.03~.22, p=.041~<.001). Conclusion: CHB infection had a negative impact on HRQOL among the older, female gender, low socioeconomic status and not having medically knowledgeable care givers. Interventions beyond nursing care may be needed to improve the quality of health for patients with Chronic Hepatitis B.
Purpose: This paper aimed to examine the ethical considerations that are the basis for many functions in the healthcare field. The key ethical values in global health, as well as future considerations imperative to this area, were observed. Research design, data and methodology: The current study utilized the past literature studies that were examined in the field of global health. An overview of the role of ethics in the healthcare field, as well as important considerations that needed to be taken into account in order to provide advancements in this area, were investigated. Results: Ethics are an important set of principles that guides humankind into the right conduct or action to better society and each other. Ethical values are one of the pinnacle points for any healthcare provider, as healthcare is not only considered from the aspect of patient health and well-being, but also in its role of keeping ethical guidelines to achieve the best possible care for a patient. Conclusions: A comprehensive understanding of healthcare is needed in order to tackle next generational challenges in global health. These ethical considerations will inevitably play a significant role in harnessing the patient-healthcare professional relationships as well as care for the shortening of a global disparity on healthcare.
This study attempted to explore the experience of patients' having hypertension and diabetes participation in health care. 11 chronic patients who regularly visited community health centers for more than a year participated in this study. The data was collected through individual interviews and analyzed by thematic analysis method. As a result, 6 sub-themes and 3 themes derived from the data. The themes were 'Trying to utilize short consultation time efficiently', 'Answering within narrow limits to healthcare provider', 'Being lack of continuous motivation for health care'. The result showed the need for nurses to provide accurate and sufficient health information and motivate patients in supportive manner to encourage chronic patients' participation in health care.

Background: University health services have provided comprehensive medical care, counseling, health promotion, and public health services to their students and several other local institutions. To their faculty and staff, university health care centers have served occupational health services and employee assistant program. Purpose: We performed this study to review the health promotion services on two kinds of health care center with different style of university formate. Methods: We tried to collect the data by literature review and interview with executive and provider at health care center in University of California at Berkeley and San Jose State University. Results: Our results were as followed. First, students could use the medical services just as they would their regular doctor's office and urgent care center. Second, the health promotion unit offered programs and services for keeping students healthy and safe, including many opportunities for students to get involved in shaping the public health of the campus. Third, the health promotion recommendation offered from ACHA was useful guideline to improve health status of their member in university campus. Finally, the student satisfaction surveys were used for evaluation and quality improvement. Conclusions: The systematic approach to improve health status of students, faculty and staff can use to maintain a state of optimum health among the diverse student community in support of academic excellence. Coupled with health promotion and public health programs, university health service have to reach all segments of the healthy campus community. To achieve study goals in university, the health care center contributes to promote accountability and responsibility for the health and well being of the members in their campus.

The purpose of this study was to examine knowledge, attitude, and altruism on blood donation and influential factors in regard to its experience. Data were collected through questionnaires with a convenience sample of 311 healthcare providers. Subjects with blood donation experience accounted for 63.3%. In both nurses and laboratory technologist, the rate of blood donation in male group has higher than that of female group. There were differences in the knowledge of blood donation dependent upon the type of occupation of healthcare providers and the higher age and the longer period of career experience proved to be the significant factor for the level of altruism. The study suggests that more practical, concrete strategies should be identified to participate in blood donation. The further relevant variables need to be investigated since the most determinant motivator for blood donation among healthcare providers was the responsibility to help others.
This study was performed to explore the effect of customer satisfaction and switching barriers on customer royalties in medical services. 900 households, 1% sample were randomly selected from K city located in Kangwon province. Interview survey was performed with structured questionnaire for the entire people, 923 persons who experienced medical service utilization during one year before survey on time, september, 2002. In comparison of switching barriers by sociodemographic characteristics, lower income group and lower educated group showed the higher level of recognition level on availability of changing the service provider significantly. In terms of economic loss as one of switching barriers, the group of over aged 61, the lower income group and the lower educated group felt higher than other groups. Also, the time loss of switching barrier was recognized in 41-60 aged group and employee in service industry highly. For the perceived risk factor as another switching barrier, the high educated group showed the higher recognition level in performance risk and felt higher social risk than other groups. In analysis of customer satisfaction and loyalty levels by switching barrier components, the lower level of availability of changing the service provider and the higher level of recognition for economic and time loss, they showed the higher satisfaction and loyalty levels. In final step, multiple regression analysis showed the positive relationship between customer satisfaction, switching barrier and customer loyalties. Besides, the moderating effect of switching barrier in relationship between customer satisfaction and loyalties was significant and this results suggests that the influence of customer satisfaction to customer loyalties might be weakened in high level of recognition for switching barrier. In conclusion, perceived risk of economic and time loss as switching barrier is an important factor and should be considered in planning of marketing strategy carefully in terms of defensive marketing.

In this paper, We purpose the basic data for the success of effective CPR using mirror in order to increase the quality of chest compression during CPR. The subject of this study was an experimental study based on a randomized crossover design of 28 people who completed the BLS Health Care Provider, and collected data were analyzed by SPSS Ver. 23.0 for Win statistics program. As the research methods, depth, speed, compression to relaxation ratio, arm angle and easiness during the chest compression were measured. Taken together, the results of this study showed that using a mirror-based chest compression method for chest compressions in adult CPR could make chest compressions easier, in addition, the quality of breast compression was improved by improving the posture of the rescuers, such as the average depth of compression, compression to relaxation ratio, and arm angle. However, it is necessary to confirm the feasibility of clinical application through additional studies on various environmental factors and job groups for mirror-based chest compression method.

This study was conducted for effective CPR by comparing the quality of chest compressions when using a new chest compression method during pediatric CPR. As an experimental study by Randomized Crossover Design, the depth, speed, compression-to-relaxation ratio, ease, and stability were measured when performing chest compressions for 28 Emergency Medical Technicians who completed the BLS Health Care-Provider qualification. The collected data were analyzed by using the SPSS Ver. 23.0 for Win statistical program. To summarize the results of this study, if the new chest compression method was used for chest compression in pediatric CPR, the qualitative indicators of chest compressions were improved. It is good to stick to the existing method, but through the new chest compression method, it will be necessary to confirm the possibility of clinical use based on additional research on various age groups and environmental factors.

Objectives: This study aimed to explore the persistence and adherence to secondary preventive medication of stroke patients after discharge and to assess the reasons for persistence and nonadherence. Methods: Four hundred twenty-nine patients with stroke were surveyed to determine their behaviors from discharge. Reasons for stopping medications were ascertained. Persistence was defined as continuation of all secondary preventive medications prescribed at hospital discharge, and adherence as continuation of prescribed medications according to health care provider instructions. Results: Of the 429 patients, 86.5% were treatment persistent and 41.2%(non-intentional nonadherence=39.4%, intentional nonadherence=19.4%) were adherent. Independent predictors of persistence included having experience about health education. Independent predictors of non-intentional nonadherence were modified Rankin Scale(mRS) (Exp(B)=2.858, p=0.001) and health education experience (Exp(B)=0.472, p=0.032), and independent predictors of intentional nonadherence were mRS (Exp(B)=2.533, p=0.006), depressive symptoms (Exp(B)=1.113, p=0.016), beliefs about medications questionnaire(necessity, Exp(B)=0.879, p=0.011, concern, Exp(B)=1.098, p=0.019). Conclusions: Although up to one-ninth of stroke patients continued secondary prevention medications, nonadherence is common. Several potentially modifiable patient, provider, and system-level factors associated with persistence and adherence may be targets for future interventions. Specially, interventions to improve adherence should target patients' beliefs about their medication.
