일 군 주민에서 사회적 지지의 수준에 따른 심박변이도의 차이 (Difference of the Heart Rate Variability According to the Social Support Level in a County)

  • 연구목적 : 본 연구는 경기도 소재 일 군의 '농촌기반 코호트 사업'의 일환으로서, 사회적 지지의 차이에 따른 심박변이도(heart rate variability, HRV)의 변화를 조사하기 위해 시행되었다. 방 법 : 건강증진 프로그램에 참여중인 일 지역 40세 이상의 1727명의 자료를 이용하였으며, 모든 연구 참여자에게 일대일 면담을 시행하여 사회 인구학적 정보에 대한 조사 및 신체 검진을 시행하였다. 또한 사회적 지지를 평가하기 위해 Medical Outcomes Study-Social Support Survey(MOS-SSS)를 작성하도록 하였으며, 자율신경계 기능을 평가하기 위하여 HRV를 측정하였다. 전체 대상 군 중 MOS-SSS 점수를 기준으로 상위 25%에 속한 432명을 high social support(HSS), 하위 25%에 속한 435명을 poor social support(PSS)로 구분하여 연구를 진행하였다. 결 과 : 두 군은 사회 인구학적 요인 중 평균 연령과 성별 구성에서 유의한 차이를 보였다(p<0.05). HRV 관련 지수 중 HSS 군이 PSS 군에 비하여 SDNN(F=4.938, p=0.027), TP(F=8.088, p=0.005), VLF(F=6.220, p=0.013) 및 LF(F=3.873, p=0.049) 값이 높았다. 결 론 : 연구 결과 PSS 군이 HSS 군에 비해 자율신경계의 기능 손상이 나타났다. 사회적 지지는 개인이 처한 위기를 극복하고 변화에 적응할 수 있도록 도우며 스트레스 상황에서는 완충 역할을 한다고 알려져 있는데, 이러한 사회적 지지의 낮은 수준이 자율신경계 기능에 영향을 주었다고 할 수 있다. 또한 HRV가 자율신경계 기능을 객관적으로 검사할 수 있는 면을 이용하여 사회적 지지에 대한 평가에 보조적으로 활용될 가능성이 있다는 것을 제시할 수 있었다.

지역사회 말기질환자 가족 부담감에 관한 연구 (A Study of Family Caregiver's Burden for the Terminally III Patients)

  • The purpose of this study was to describe the perceived burden of the terminally III patients's caregiver and to analyze relationship between the perceived burden and the various demographics, illness characteristics, family relationships, and economic factor of the family & patients. The sample of 132 caregivers who care for the terminally III patients Kyung-Gi province, Seoul, Korea. The period of this study was from August to September, 2002. The perceived burden of the family caregiver was measured by the burden scale(20 items, 4 point scale) developed by Montgomery et al. (1985). The Data was analyzed using SAS-program by t-test and ANOVA. The results were as follows; 1. The mean of the family caregiver's burden score was 3.02. The score showed that caregivers perceive severe the level of burden. The hight items of the family caregiver's burden were' I feel it is painful to watch patient's diseases'(3.77). 'I feel afraid for what the future holds for my patients'(3.66), 'I feel it reduced to amount of privacy time'(3.64). 2. The caregiver's burden was significantly related to patient's gender(F=3.17, p= 0.0020), patient's job(F=2.49, p=0.0476), caregiver's age(F=4.29, p=0.0030), and caregiver's job(F=2.49, p=0.0476). 3. The caregiver's burden according to illness characteristics showed no significant difference. 4. The caregiver's burden was significantly associated with patient's family relationship (F=4.05, p=0.0041), patient's care mean period in a day(F=47.18,

전북지역 일부 고등학생의 구강보건형태에 관한 연구 (A Study on the Oral Health Behavior of High School Students in Jeolla-bukdo)

  • 본 연구는 남학생의 구강보건행태에 관한 기초자료를 마련하고, 그 연관요인을 규명하여 구강건강증진을 도모하고자 2008년 6월 9일부터 20일까지 전라북도 남원 소재의 고등학교 남학생 388명을 대상으로 하였다. 설문지는 담임교사의 협조를 얻어 배포 회수하였고 자기기입식에 의한 설문법을 실시하였다. 자료 분석 방법으로는 ${\chi}^2$(Chi-square)검증과 기술통계를 시행하였으며 그 결과는 다음과 같다. 1. 1일 잇솔질 횟수로 61.1%의 학생들이 2회라고 응답하였고(p < 0.05), 69.1%가 아침식후에 가장 많이 잇솔질을 하는 것으로 나타났으며, 잇솔질 방법으로 위 아래로 닦는다는 응답이 36.6%로 가장 많았다. 보조 구강위생용품은 8.5%의 학생만이 사용하고 있었다(p < 0.01). 2. 구강진료기관 이용 경험률은 93.6%이었고, 마지막 구강진료 이용목적으로 82.9%의 학생들이 치료를 이유로 내원하였다. 1년 이내 이용 경험률은 47.7%이었고(p < 0.05), 77.1%의 학생들이 진료에 대해 만족하고 있었다(p < 0.05). 3. 흡연을 하는 학생들의 잇솔질 횟수는 흡연을 하지 않은 학생들에 비해 낮게 나타았고(p < 0.05), 음주를 하는 학생들은 하지 않은 학생들에 비해 잇솔질 횟수가 높게 나타났다(p < 0.05). 4. 구강질환으로 인한 활동제한 경험률은 16.0%이었고, 연간 결석 경험률은 5.2%이었으며, 학업수행 지장 경험률은 13.1%로 나타났다. 결과적으로 정기적인 구강검진 및 구강보건교육을 강화하고, 예방과 조기 치료 위주의 정책이 필요한 것으로 사료된다.

임부교실 운영효과 분석을 위한 일 연구 (A study on analyzing effectiveness of childbirth education)

  • The purpose of this study is to provide basic data regarding effective learning opportunities in childbirth education classes. Also analysis of the data indicates the optimum conditions for the welfare and improvements in the promotion of health in childbearing mothers. The results of this study are as follows; 1) The average age of the subjects in this study was 30.6 years and the total number of subjects was 58 pregnant women. The average number of children was one and 84.5% of the subjects were unemployed even though 63.8% of them held over bachelor's degrees. It was found that 22.4% of the subjects were living in an extended family. Also 61.5% of them were living with parents-in-law. The number of pregnancies were calssified as one, two, or three to nine times with the percentages of 58.7%, 22.4% and 18.9%, respectively. Further, 72.4% of the subjects had no abortion experience and 15.5% had one aborion experience. While 89.7% of the subjects planned to feed their babies with breastmilk, mixed feeding were used by only 22.4% of the sample. These data were collected at about 6 months after delivery. Thus one can see that a low rate of breastfeeding was common. 2) The length of one period of childbirth education is four weeks. It was found that 36.2% of the subjects participated in childbirth education only once, where as 13.8% participated four times and 19% of the subjects participated in this class more than four times. pregnant at least once. Further, 75.9% of the participants were participated in this education through their own will. Their motivation for participation developed through information, advertisement and posters which contained information on childbirth education. Those with unplanned pregnancies 92.9% participated after a suggestion by the nurses. The number of participants in terms of percentage according to the childbirth education contents can be classified as following. The most active participation was shown in preparation of delivery(77.6%), postpartrm management(56.9%) fetal development(37.6%) and physiology of pregnancy(17.2%). It was found that 75.9% of the subjects were willing to participate again if they were given a chance. The reason can be summarized as following: The content of the education is very helpful(47.7%). Scientific knowledge can be obtained through this program(20.5%). Participation helps in achieving psychological stability(9.1%). Participation enables one to establish a friendly relationship with other participants(6.8%) of the sample. 24.1% of the participants did not want to participate again. The reasons can be as following: They do not want another baby(42.9%). The first paricipation in childbirth education gave enough knowledge about childbirth(21.4%). Another reason for not want to participate again was because they had a cesarean birth(14.3%). Only 7.1% of them responded with a negative view. A response that they do not need childbirth education after their operation can be traced back to the general belief that childbirth education is the place where one prepares for natural birth through the Lamaze breathing technique. Of the subjects, 91.4% suggested that this program could be recommended to other childbearing mothers, because this program gave educational content along with psychological stability for childbearing women. Of the subjects 41.4% did not see any efforts towards the welfare of the baby, where as 88.2% did. Among the subjects 58.6% made some effort to eliminate the discomfort of labor by breathing and imagination and breathing and walking. Further 41.7% of the 24 subjects did not do anything toward the welfare of the baby, because they did have a cesarean section so that they didn't have a chance even though they had been educated about childbirth. Also 33.3% of the subjects did not do anything toward the welfare of the baby, because they lacked a willingness. After leaving the hospital, only 75.9% of the subjects did some exercises. The subjects who tried participate this program with their husband accounted for 20.7% of the sample. Interviewing with the subjects solved some of the uneasiness and. fear of delivery, increased self-confidence in parenting and active coping in the delivery process.

국내 25개 양돈장의 살모넬라 유병율 및 감염유형 (Prevalence and Infection Status of Salmonella in 25 Conventional Swine Farms in Korea)

  • 본 연구에서는 25개 양돈장의 사육돼지를 대상으로 혈청학적 및 미생물학적 모니터링을 실시하여 살모넬라의 유병율과 감염 양상을 분석함으로써 향후 한국 양돈장의 살모넬라 제어 프로그램 구축의 기초자료로 활용하고자 하였다. 25개 양돈장의 모돈과 사육단계별 돼지 혈청 총 848두에 대한 항체검사 결과, 전체 항체 보유율은 28.4%로 나타났으며, 사육단계별로는 모돈의 항체양성율이 83.1%로 가장 높았고, 20일령 이후 모체이행항체가 소실되고, 농장에서의 사육기간이 길어질수록 살모넬라에 노출되는 빈도가 높아지는 것으로 나타났다. 조사대상 25개 양돈장의 모돈군은 최소한 1두 이상의 항체 양성모돈을 보유하고 있어 향후 우리나라 살모넬라 방역에 있어 종돈장의 살모넬라 감염 통제 및 음성 후보돈 공급체계 확립이 가장 시급한 것으로 판단되었다. 25개 양돈장에서 채취한 사육단계별 돼지 분변에 대한 세균 분리 동정 결과, 혈청학적으로 감염유형인 16개 양돈장 중 3개 양돈장(12.0%)에서 살모넬라 균이 분리되었다. 이러한 결과는 양돈장 사육 돼지집단에 대한 살모넬라 감염상황을 정확하게 파악하기 위해서는 세균검사법만으로는 한계가 있으며, 돈군 내의 준임상감염축 또는 잠복감염축을 진단할 수 있는 혈청검사를 병행해야 함을 시사해준다. 또한 향후 우리나라의 양돈장 살모넬라 통제 프로그램을 구축하기 위하여 다양한 양돈장 모니터링 기법을 개발하고, 현장 효용성을 확인하기 위한 확대 시험이 필요하다.

서울시내 중 . 고등학교에서의 금연교육실태 (A Study on Anti - Smoking Education of Middle and High Schools in Seoul)

  • This study was performed during the period between March 22 and July 23, 1989 in other to examine status of aniti - smoking eduation in middle and high school in Seoul. The results were as follows; 1. The respondents of this study were 403 nurse teachers of whom 43.7% had implemented anti - smoking education and 56.3% had not implemented anti - smoking education. Classified by school, those schools implementing anti - smoking education were in order of frequency; boys' high schools (32.4%), coeducational high schools (18.3%) and boys' middle schools (13/6%) and those schools not implementing anti-smoking education were in order of frequency: girls' high schools (23.8%), girls' middle schools (22.5%) and coeducation middle schools(7.6%). 2. The number of times that anti - smoking education was done was once a year (60.8%) or twice a year (38.2%). For teaching materials, 49.4% of the schools reported possessing teaching materials and 50.6% reported not possessing teaching matenials. The content of the anti - smoking education material was classified by grade for 27.8% of the schools and not classified by grade for 72.2%. 3. In schools where anti - smoking education directed at giving up smoking was being used, several problems were presented; lack of reference data and education of data(38.1%) lack of equipment(29.0%), smoking by teachers(13.6%). On the other hand, in schools where anti - smoking education was not being used, the ploblems identified were, in the following order, lack of reference data and education of data(38.1%), lack of equipment(29.0%) lack of time (15.0%) and lack of information (15.4%). There was statistically significant difference the two types of schools. 4. In discussing the proper time for education on giving up smoking, the teachers in school with indicated anti - smoking education the following, in order of frequency, the first year of middle school(30.5%), the second year(27.6%), the last year of middle school (18.4%), primary school (11.9%). For those who did not have anti - smoking education, the result were similar, the first year of middle school (31.9%), the second year (23.5%), primary school (17.6%), and the last year of middle school (15.5%). There results were not statisically significant but they supports the idea of eaely education directed at giving up smoking. 5. The actual frequency of students smoking was as follows. In schools with anti - smoking education 33.5% of the students smoked a lot and seriously 33.0% smoked a little and not seriously. In schools without anti - smoking education, the majority of the teachers (50.4%) indicated that the students were 'few and not serious' followed by 'I don't know well'(19.8%), 'many but not serious'(15.6%), 'many and serious'(15.2%). This implies that the teachers in schools which have anti - smoking education think smoking is more seriously than those who are in schools which do not have anti - smoking education though it is influenced by the members of the school. The opinions on penalities for smoking were as follows; in schools with anti - smoking education, 'punish'(52.8%) and 'give advice'(27.8%). In schools with no anti-smoking education 'punish'(41.9%), 'give advice'(24.5%) and 'I don't know what the rules for punishment should be'(18.5%), and 'do not punish'(16.4). 6. For knowledge about smoking by nurse - teachers, in schools having anti - smoking education the average score was 30.40. There was not statistically significant differences in these scores. But, there is an indication of a need for a deeper and a moer systematic knowledge of smoking as shown by the problem points; lack of reference data and educational data. The reason for this is that education to give up smoking is not considered a part of routine life. But the majority(95.2%) of the respondents indicated that a systematic program in the schools would meet that need.

일부 성인의 교정치료 및 안면 성형 수술에 대한 실태 (State of Orthodontic Treatment and Facial Plastic Surgery among Adult Patients)

  • 본 연구는 서울 경기지역의 치과에 내원한 389명의 환자를 대상으로 교정치료 및 안면성형 수술에 대한 실태를 조사하고자 하였다. 자료수집 후 spss 통계프로그램을 이용하여 분석하였다. 교정치료경험이 있는 환자의 치열만족도가 높게 나타났고(p<0.01), 여자가 남자보다 301-400만원이 적당한 교정치료 비용이라고 높게 인식하고 있었으며(p<0.05) 교정치료 경험이 있는 환자가 19-24개월 이하를 적절한 교정치료 기간으로 인식하고 있었다(p<0.01). 또한 여자가 남자보다 교정치료 검진 필요성(p<0.001), 안면성형수술 경험(p<0.01)이 높게 나타났다. 안면성형수술 희망부위에서는 여자가 눈 수술이(p<0.01), 교정치료 경험이 있는 환자에서는 코 수술이 높게 나타났다(p<0.05). 한편, 여자가 안면 부위 중 눈을 가장 중요하게 인식하고 있었으며(p<0.001), 안면성형 부위 중 양악을 치과에서 해야 한다고 높게 인식하고 있었다(p<0.01). 결론적으로 임상에서 교정치료 및 안면성형을 희망하는 환자 상담 시 성별 및 개인의 요구를 파악하여 진료에 반영하고 특히 안면성형 중 양악수술은 구강의 기능을 고려해야 하므로 치과의료진의 참여가 필수적이라 사료된다.

간호대학생의 불안 및 지각된 스트레스 경험과 자아탄력성과의 관계 (The Relationship of Ego Resilience and involvement with the Experience of Anxiety and Perceived Stress in Nursing students)

  • 본 연구는 간호대학생의 자아탄력성, 불안, 주관적 스트레스의 정도를 파악하고 이들 간의 관계를 파악하고자 시도된 서술적 조사연구이다. K시 일개 간호 대학생을 대상으로 2013년 9월 9일에서 14일까지 자료를 수집하였으며 수집된 자료 526부 중 최종적으로 500부가 분석되었다. SPSS 18.0 통계 프로그램에 의해 t-test, ANOVA, Pearson correlation coefficients를 이용하여 대상자의 일반적 특성과 이에 따른 자아탄력성, 불안, 주관적 스트레스의 차이를 알아보고 세 변수의 관계를 파악하였다. 분석 결과 대상자의 자아 탄력성 평균점수는 3.36(0.50)점, 불안은 2.26(0.70)점, 스트레스는 2.55(0.55)점이었으며, 자아탄력성은 남학생이 여학생보다, 24세 이상 군에서 23세 이하의 군보다 유의하게(p<0.05) 높았다. 자아탄력성은 불안 (r=- .383, p=0.000) 및 지각된 스트레스와 (r=- .454, p=0.000) 음의 상관관계를 갖는 것으로 나타났으며 불안과 지각된 스트레스는 강한 양의 상관관계(r= .688, p=0.000)를 나타내었다. 이상의 연구결과를 바탕으로 자아탄력성이 갖는 긍정적 측면을 인식하고 이를 강화시킬 수 있는 프로그램이나 교과과정의 개발을 제언하는 바이다.

무용요법이 노인의 신체적.심리적 특성에 미치는 효과 (The Effect of Dance Therapy on Physical and Psychological Characteristics in The Elderly)

  • This study was performed to explore the effects of a dance therapy on physical and psychological characteristics in the elderly. The design of this study was a non-equivalent pre-post test experiment. The subjects consisted of elderly persons living in a facility located in Suweon and Bucheon. Fifty eight subjects, aged between 65 and 93 years who had normal cognition, sensory function, balance, and resting blood pressure. They underwent tests of balance, flexibility, muscle strength, depression, and anxiety as baseline data before dance therapy, and at 6th week and at the end of the 12nd week after following dance therapy. Twenty seven elderly persons were assigned to the experimental group and participated with the dance therapy between April and July, 1998. The dance therapy was developed by the author with the help of a dance therapist and a physiatrist. This therapy was based on the Marian Chace's dance therapy and Korean traditional dance with music. The dance therapy consists of 50 minutes session, 3 times a week for 12 weeks. One session was consisted of warming-up, expression, catharsis, sharing, and closing stage. The intensity of the dance therapy was at the 40 % of age-adjusted maximum heart rates. Data were analysed with mead standard deviation, Chi-square test, unpaired t-test, repeated measures ANOVA, and Bonferroni multiple regression using SAS program. 1. The results related to the physical characteristics were as follows : 1) The balance (standing on one leg, walking on the balancing bar), flexibility and muscle strength (knee extensor, knee flexor, ankle plantarflexor and dorsiflexor) of the experimental subjects significantly increased over time mere than that of the control subjects. 2) The experimental group had significantly higher score for balance, flexibility, muscle strength of knee extensor, and knee flexor than the control group at the 12nd week after dance therapy. 3) The experimental group had significantly higher score for muscle strength of ankle dorsiflexor and plantarflexor than the control group at the 6th week and the 12nd week after dance therapy. 2. The results related to psychological characteristics were as follows : 1) Scores of Geriatric Depression Scale, Hamilton Depression Rating Scale, and Zung's Self-rating Anxiety Scale of the experimental group were significantly decreased over time more than that of the control group. 2) The experimental group had significantly lower score for depression than the control group at the 12nd week after dance therapy. 3) The experimental group had significantly lower score for anxiety than the control group at the 6th week and the 12nd week after dance therapy. The findings showed that the dance therapy could be effective in improving the balances, flexibility, and muscle strength of lower limb, and effective in decreasing the depression and anxiety of the elderly. Additional merits of the dance therapy would be inexpensiveness, easy accessibility, and increasing interpersonal relationship. It can be suggested that the dance therapy is effective in the health promotion of the elderly.

저소득층의 평생교육 참여 실태와 요구 분석 (Analysis of Status and Demand of Participation in Lifelong Education of Low-Income Class)

  • 본 연구는 국민기초생활 보장법에 따라 저소득층으로 분류된 지역민 218명을 대상으로 평생교육 참여 실태와 요구를 분석하였다. 이에 따른 연구결과는 다음과 같다. 첫째, 평생교육의 참여 실태 중, 저소득층의 학습역량을 살펴보면, 외국어 사용 능력 95.3%, ICT 활용 역량 66.1%, 컴퓨터 활용 능력 75.7%로 사용 및 활용하지 않는 것으로 나타났다. 그리고 저소득층의 평생교육 참여율은 75.7%로 나타났으며 이를 수동적인 교육 참여로 보고 자발적 참여를 구별하여 분석한 결과, 실질적인 평생교육 참여율은 22.9%로 매우 저조하게 나타났다. 평생교육 참여여부에 따른 사회참여 및 인식은 참여경험이 있는 응답자가 참여경험이 없는 응답자보다 전반적으로 43.3% 높게 나타났으며, 특히 자발적인 참여자가 참여경험이 없는 응답자보다 73.3%나 높게 나타났다. 둘째, 저소득층의 평생교육에 대한 요구를 분석한 결과, 희망 교육프로그램이 있는지에 대해 응답자 중 74.8%가 없다고 응답하였다. 그리고 시간이 없어서 참여하지 못하는 경우가 50.0%로 가장 많았으며, 참여를 희망하는 프로그램에는 가정생활, 건강 및 의료강좌, 전문 자격 취득과정 등 실생활에 도움을 줄 수 있는 프로그램을 희망하였다. 이는 소득 수준이 평생교육 참여에 크게 작용하고 있음을 알 수 있었다. 이상의 본 연구 결과를 분석한바, 현재 한국의 평생교육이 강조되고 그에 따른 참여율도 급증하고 있지만, 저소득층의 평생교육 참여는 저조하게 나타나고 있다.
